Abstract

Multimodal nonlinear endoscopes have been a topic of intense research over the past two decades, enabling sub-cellular and label-free imaging in areas not reachable with table-top microscopes. They are sophisticated systems that can be implemented on an optical table in a lab environment, but they cannot be easily moved within or out of the lab. We present here a multimodal and flexible nonlinear endoscope system able to perform two photon excited fluorescence and second harmonic generation imaging with a stand-alone and moveable kart integrating a compact ultrashort laser source. In addition, the system can perform three photon excited fluorescence and third harmonic generation thanks to a delivery optical fiber used to deliver ultrashort pulses from massive and not movable laser systems into the stand-alone kart. The endoscopic fiber probes and delivery optical fibers are based on functionalized negative curvature hollow core fibers. The endoscope distal head has a diameter <2.2mm and can perform nonlinear imaging at max 10 frames/s over a field of view up to 600 $\mu$m with a ~1 $\mu$m spatial resolution.

Abstract

Background and Objectives COVID-19–related inflammation, endothelial dysfunction, and coagulopathy may increase the bleeding risk and lower the efficacy of revascularization treatments in patients with acute ischemic stroke (AIS). We aimed to evaluate the safety and outcomes of revascularization treatments in patients with AIS and COVID-19. Methods This was a retrospective multicenter cohort study of consecutive patients with AIS receiving intravenous thrombolysis (IVT) and/or endovascular treatment (EVT) between March 2020 and June 2021 tested for severe acute respiratory syndrome coronavirus 2 infection. With a doubly robust model combining propensity score weighting and multivariate regression, we studied the association of COVID-19 with intracranial bleeding complications and clinical outcomes. Subgroup analyses were performed according to treatment groups (IVT-only and EVT). Results Of a total of 15,128 included patients from 105 centers, 853 (5.6%) were diagnosed with COVID-19; of those, 5,848 (38.7%) patients received IVT-only and 9,280 (61.3%) EVT (with or without IVT). Patients with COVID-19 had a higher rate of symptomatic intracerebral hemorrhage (SICH) (adjusted OR 1.53; 95% CI 1.16–2.01), symptomatic subarachnoid hemorrhage (SSAH) (OR 1.80; 95% CI 1.20–2.69), SICH and/or SSAH combined (OR 1.56; 95% CI 1.23–1.99), 24-hour mortality (OR 2.47; 95% CI 1.58–3.86), and 3-month mortality (OR 1.88; 95% CI 1.52–2.33). Patients with COVID-19 also had an unfavorable shift in the distribution of the modified Rankin score at 3 months (OR 1.42; 95% CI 1.26–1.60). Discussion Patients with AIS and COVID-19 showed higher rates of intracranial bleeding complications and worse clinical outcomes after revascularization treatments than contemporaneous non–COVID-19 patients receiving treatment. © 2014 Elsevier Ltd. All rights reserved. A new method of consideration and reduced analysis of long-time series is suggested. The method is based on the scaling (fractal) properties of the beta-distribution function that allows finding the stable parameters and reducing the long-time series containing 5 × 105-106 data points to analysis of 10-20 stable parameters. The new procedure of clusterization with the usage of the generalized Pearson correlation function allows to take into account the influence of external factor and combine/separate different parameters into a cluster with respect to the influence of the qualitative external parameter considered. The method is rather flexible and can be applied to wide set of large data. As an example the triple pendulum data are considered. This method opens new possibilities in creation of the reduced database (specific fingerprints) of different long-time series for their comparison and subsequent analysis.

Abstract

Numerical and experimental studies of a triple physical pendulum are performed. The experimental setup of the triple pendulum with the first body externally excited by the square-shape function of time is build and described. The mathematical model of the real system is presented and the real rig parameters are estimated. Then, numerical analysis of the model is performed obtaining a good agreement with the results obtained from the experiment. 40. Emergency Room (ER) Utilization Rate Among Patients with HPV related Oropharynx Cancer Undergoing Radiation De-Escalation.

Abstract

HPV related oropharynx cancer (HPV+ OPC) is increasing in incidence in the US. The most common standard non-surgical treatment of loco-regionally advanced HPV+ OPC is concurrent cisplatin chemotherapy with radiation to 70Gy. This treatment is associated with substantial morbidities such as dehydration, weight loss, and acute kidney injury (AKI), resulting in subsequent increased healthcare utilization including emergency room (ER) visits. Prospective trials at our center explored the role of tumor hypoxia to direct radiation dose de-escalation to 30Gy with chemotherapy while preserving treatment efficacy of HPV+ OPC. We examined the ER utilization rates comparing patients who received radiation to 30Gy vs 70Gy on trial. Patients with HPV+ OPC were enrolled and underwent 18F-fluoromisonidazole PET scan which was used to measure tumor hypoxia and guide radiation to either 30Gy or 70Gy (oncologic results reported elsewhere). Patient visits at the institutions and outside ERs were recorded from the start of radiation to 8 weeks after the completion of chemoradiation treatment. Rates of ER utilization, defined as the percentage of patients who presented to ER were compared between those who received radiation to 30Gy vs those treated to 70Gy. We analyzed 305 patients on trial, of whom 234 received 30Gy radiation (76.7%). Of the total cohort, 63 had at least one ER visit in the specified timeframe (20.7%). However, upon stratification by RT dose, only 39 of 234 patients (16.7%) who received 30Gy had ER visits, compared with 24 of 71 patients (33.8%) who received 70Gy (chi2 P = 0.002). 2 of 234 (0.85%) patients who received 30Gy had 2 ER visits, while 5 of 71 (7%) patients who received 70Gy had 2 ER visits (chi2 P = 0.002). No patients had more than 2 ER visits. The most common reasons for ER visits were fever, syncope, dehydration, nausea and vomiting, and AKI. Among the patients who had the ER visits, 17 (7.3%) patients who received 30Gy were hospitalized, whereas 13 (18.3%) patients who received 70Gy were hospitalized (chi2 P = 0.006). In this cohort of patients undergoing definitive chemoradiation for HPV+ OPC, 30Gy was associated with decreased rate of ER utilization by ∼50% compared with 70Gy when followed from the start of radiation to 8 weeks after treatment. Patients who received 70Gy had higher rates of > 1 ER visits versus the 30Gy cohort. These findings suggest that in addition to lower morbidity with 30Gy, the direct consequence is decreased ER healthcare utilization compared to the standard 70Gy cohort, underscoring the importance of exploring dose de-escalation strategies in HPV+ OPC to decrease financial burden and healthcare expenditure. Abstract

Introduction: Mechanical Cardiac Support and Left Ventricular Assist Devices (LVAD) have been demonstrated to prolong Heart Failure patients' survival and improve their quality of life. LVAD implantation has a considerable effect on patients' body image. Patients find it hard to accept the device as an extension of their body, especially the driveline. The study aimed to examine the relationships between anxiety and depressive symptoms, acceptance of illness, beliefs about pain control, and quality of life with body image among LVAD-implanted patients., Methods: The cross-sectional study included 54 conveniently recruited patients who completed the Body Image Scale, SF-12, Acceptance of Illness Scale, Beliefs about Pain Control Questionnaire, NRS, HADS, and demographic and clinical data. Multiple regression analyses examined the associations between the research variable., Results: The mean Age of the participants was 59.64 (SD=9.63), and 96.3% were men. The mean scores were: Body Image Scale - 28.33 (SD=5,91); Acceptance of Illness Scale - 25.51 (SD=5.92); beliefs about pain control: internal factors - 15.85 (SD=4.96), the influence of physicians - 17.57 (SD=3.15), random events - 14.37(SD=3.32), NRS=2.80(SD=1.86), HADS anxiety - 5.33 (SD=4.12), HADS depression - 4.66 (SD=3.10), SF12MCS-45.49 (SD=6.48), SF-12PCS-41,33 (SD=6.48). Abstract

Background: Cutaneous metastases (CMs) are a manifestation of advanced cancer and can be treated with oncolytic immunotherapy. Laboratory studies suggest radiotherapy (RT) may facilitate response to immunotherapy. We hypothesized that oncolytic immunotherapy with talimogene lapherparepvec (T-VEC, an oncolytic immunotherapy that expresses granulocyte-macrophage colony stimulating factor) and RT would produce response in non-targeted metastases., Methods: A randomized phase 2 trial of T-VEC+/-RT was conducted. Eligible patients had ≥1 CM from a solid tumor amenable to T-VEC and RT and another measurable metastasis. Tumor and overall response was assessed using modified World Health Organization (mWHO) criteria. Adverse events (AEs) and quality of life (QOL) were characterized using CTCAE v4.0 and Skindex-16, respectively. Correlative analyses of tumor genomics and the immune system were performed., Results: 19 patients were randomized to receive T-VEC (n = 9) or T-VEC+RT (n = 10). One patient in each arm demonstrated complete response in the largest non-targeted metastasis. The trial was closed after the first stage of enrollment because of no overall mWHO responses, slow accrual and the COVID-19 pandemic. AEs were consistent with prior reports of T-VEC. Skin related QOL was poor before and after treatment. Median progression free survival was 1.2 and 2.5 months in the T-VEC and T-VEC+RT arms; median overall survival was 4.9 and 17.3 months in the T-VEC and T-VEC+RT arms. Analyses of peripheral blood cells and cytokines demonstrated responders exhibited several outlying lymphocyte and cytokine parameters., Conclusions: Low overall response rate, slow accrual, and the COVID-19 pandemic led to closure of this trial.
