Abstract

Narratives posted on the internet by patients contain a vast amount of information about various concerns. This study aimed to extract multiple concerns from interviews with breast cancer patients using the natural language processing (NLP) model bidirectional encoder representations from transformers (BERT). A total of 508 interview transcriptions of breast cancer patients written in Japanese were labeled with five types of concern labels: "treatment," "physical," "psychological," "work/financial," and "family/friends." The labeled texts were used to create a multi-label classifier by fine-tuning a pre-trained BERT model. Prior to fine-tuning, we also created several classifiers with domain adaptation using (1) breast cancer patients' blog articles and (2) breast cancer patients' interview transcriptions. The performance of the classifiers was evaluated in terms of precision through 5-fold cross-validation. The multi-label classifiers with only fine-tuning had precision values of over 0.80 for "physical" and "work/financial" out of the five concerns. On the other hand, precision for "treatment" was low at approximately 0.25. However, for the classifiers using domain adaptation, the precision of this label took a range of 0.40–0.51, with some cases improving by more than 0.2. Abstract

The joint JAXA/NASA ASTRO-H mission is the sixth in a series of highly successful X-ray missions developed by the Institute of Space and Astronautical Science (ISAS), with a planned launch in 2015. The ASTRO-H mission is equipped with a suite of sensitive instruments with the highest energy resolution ever achieved at E > 3 keV and a wide energy range spanning four decades in energy from soft X-rays to gamma-rays. The simultaneous broad band pass, coupled with the high spectral resolution of Delta E < 7 eV of the micro-calorimeter, will enable a wide variety of important science themes to be pursued. Background:Patients with breast cancer have a variety of worries and need multifaceted information support. Their accumulated posts on social media contain rich descriptions of their daily worries concerning issues such as treatment, family, and finances. It is important to identify these issues to help patients with breast cancer to resolve their worries and obtain reliable information.Objective:This study aimed to extract and classify multiple worries from text generated by patients with breast cancer using Bidirectional Encoder Representations From Transformers (BERT), a context-aware natural language processing model.Methods:A total of 2272 blog posts by patients with breast cancer in Japan were collected. Five worry labels, “treatment,” “physical,” “psychological,” “work/financial,” and “family/friends,” were defined and assigned to each post. Multiple labels were allowed. To assess the label criteria, 50 blog posts were randomly selected and annotated by two researchers with medical knowledge. After the interannotator agreement had been assessed by means of Cohen kappa, one researcher annotated all the blogs. A multilabel classifier that simultaneously predicts five worries in a text was developed using BERT. This classifier was fine-tuned by using the posts as input and adding a classification layer to the pretrained BERT. The performance was evaluated for precision using the average of 5-fold cross-validation results.Results:Among the blog posts, 477 included “treatment,” 1138 included “physical,” 673 included “psychological,” 312 included “work/financial,” and 283 included “family/friends.” The interannotator agreement values were 0.67 for “treatment,” 0.76 for “physical,” 0.56 for “psychological,” 0.73 for “work/financial,” and 0.73 for “family/friends,” indicating a high degree of agreement. Abstract

Early detection and management of adverse drug reactions (ADRs) is crucial for improving patients’ quality of life. Hand-foot syndrome (HFS) is one of the most problematic ADRs for cancer patients. Recently, an increasing number of patients post their daily experiences to internet community, for example in blogs, where potential ADR signals not captured through routine clinic visits can be described. Therefore, this study aimed to identify patients with potential ADRs, focusing on HFS, from internet blogs by using natural language processing (NLP) deep-learning methods. From 10,646 blog posts, written in Japanese by cancer patients, 149 HFS-positive sentences were extracted after pre-processing, annotation and scrutiny by a certified oncology pharmacist. The HFS-positive sentences described not only HFS typical expressions like “pain" or “spoon nail”, but also patient-derived unique expressions like onomatopoeic ones. The dataset was divided at a 4 to 1 ratio and used to train and evaluate three NLP deep-learning models: long short-term memory (LSTM), bidirectional LSTM and bidirectional encoder representations from transformers (BERT). The BERT model gave the best performance with precision 0.63, recall 0.82 and f1 score 0.71 in the HFS user identification task. Our results demonstrate that this NLP deep-learning model can successfully identify patients with potential HFS from blog posts, where patients’ real wordings on symptoms or impacts on their daily lives are described. Thus, it should be feasible to utilize patient-generated text data to improve ADR management for individual patients.

Abstract

The calorimeter array of the JAXA Astro-H (renamed Hitomi) soft x-ray spectrometer (SXS) was designed to provide unprecedented spectral resolution of spatially extended cosmic x-ray sources and of all cosmic x-ray sources in the Fe-K band around 6 keV. The properties that made the SXS array a powerful x-ray spectrometer also made it sensitive to photons from the entire electromagnetic band as well as particles. If characterized as a bolometer, it would have had a noise equivalent power of <4 × 10(exp −18) W/((Hz)(exp 0.5)). Thus, it was imperative to shield the detector from thermal radiation from the instrument and optical and UV photons from the sky. In addition, it was necessary to shield the coldest stages of the instrument from the thermal radiation emanating from the warmer stages. These needs were addressed by a series of five thin-film radiation-blocking filters, anchored to the nested temperature stages, that blocked long-wavelength radiation while minimizing x-ray attenuation. The aperture assembly was a system of barriers, baffles, filter carriers, and filter mounts that supported the filters and inhibited their potential contamination. The three outer filters also had been equipped with thermometers and heaters for decontamination. We present the requirements, design, implementation, and performance of the SXS aperture assembly and blocking filters.

Abstract

The calorimeter array of the JAXA Astro-H (renamed Hitomi) soft x-ray spectrometer (SXS) was designed to provide unprecedented spectral resolution of spatially extended cosmic x-ray sources and of all cosmic x-ray sources in the Fe-K band around 6 keV, enabling essential plasma diagnostics. The SXS had a square array of 36 x-ray calorimeters at the focal plane. These calorimeters consisted of ion-implanted silicon thermistors and HgTe thermalizing x-ray absorbers. These devices demonstrated a resolution of better than 4.5 eV at 6 keV when operated at a heat-sink temperature of 50 mK. We will discuss the basic physical parameters of this array, including the array layout, thermal conductance of the link to the heat sink, resistance function, absorber details, and means of attaching the absorber to the thermistor-bearing element. We will also present the thermal characterization of the whole array, including thermal conductance and crosstalk measurements and the results of pulsing the frame temperature via alpha particles, heat pulses, and the environmental background. A silicon ionization detector was located behind the calorimeter array and served to reject events due to cosmic rays. We will briefly describe this anticoincidence detector and its performance.

Abstract

We summarize all of the in-orbit operations of the soft x-ray spectrometer (SXS) onboard the ASTRO-H (Hitomi) satellite. The satellite was launched on February 17, 2016, and the communication with the satellite ceased on March 26, 2016. The SXS was still in the commissioning phase, in which the set-ups were progressively changed. This paper is intended to serve as a concise reference of the events in orbit in order to properly interpret the SXS data taken during its short lifetime and as a test case for planning the in-orbit operation for future microcalorimeter missions.

Abstract

Early detection and management of adverse drug reactions (ADRs) is crucial for improving patients’ quality of life. Hand-foot syndrome (HFS) is one of the most problematic ADRs for cancer patients. Recently, an increasing number of patients post their daily experiences to internet community, for example in blogs, where potential ADR signals not captured through routine clinic visits can be described. Therefore, this study aimed to identify patients with potential ADRs, focusing on HFS, from internet blogs by using natural language processing (NLP) deep-learning methods. From 10,646 blog posts, written in Japanese by cancer patients, 149 HFS-positive sentences were extracted after pre-processing, annotation and scrutiny by a certified oncology pharmacist. The HFS-positive sentences described not only HFS typical expressions like “pain" or “spoon nail”, but also patient-derived unique expressions like onomatopoeic ones. The dataset was divided at a 4 to 1 ratio and used to train and evaluate three NLP deep-learning models: long short-term memory (LSTM), bidirectional LSTM and bidirectional encoder representations from transformers (BERT). The BERT model gave the best performance with precision 0.63, recall 0.82 and f1 score 0.71 in the HFS user identification task. Our results demonstrate that this NLP deep-learning model can successfully identify patients with potential HFS from blog posts, where patients’ real wordings on symptoms or impacts on their daily lives are described. Abstract

The High Resolution Mid-Infrared Spectrometer (HIRMES) instrument will fly onboard NASA's airborne Stratospheric Observatory for Infrared Astronomy (SOFIA) in 2019. HIRMES will provide astronomers with a unique observing window (25 122 µm) for exploring the evolution of protoplanetary disks into young solar systems, and the composition of our Solar System. There are two focal plane detector arrays for the instrument: a high-resolution (/ = 100,000) 8x16 detector array with a target noise-equivalent power, NEP 3x10-18 W/Hz; and a low-resolution (/ = 2,000 19,000) 16x64 detector array with a target NEP 2x10-17 W/Hz. The detectors for both of these arrays are superconducting Mo/Au bilayer transition edge sensor (TES) bolometers on thin suspended single-crystal silicon membranes. Here we present our characterization results for the detectors in both arrays, including measurements of thermal conductance with comparison to phonon transport models, saturation power, noise, and array uniformity.

Abstract

The Hitomi (ASTRO-H) mission is the sixth Japanese X-ray astronomy satellite developed by a large international collaboration, including Japan, USA, Canada, and Europe. The mission aimed to provide the highest energy resolution ever achieved at E greater than 2 keV, using a microcalorimeter instrument, and to cover a wide energy range spanning four decades in energy from soft X-rays to gamma-rays. After a successful launch on 2016 February 17, the spacecraft lost its function on 2016 March 26, but the commissioning phase for about a month provided valuable information on the on-board instruments and the spacecraft system, including astrophysical results obtained from first light observations. The paper describes the Hitomi (ASTRO-H) mission, its capabilities, the initial operation, and the instruments/spacecraft performances confirmed during the commissioning operations for about a month.

Abstract

The SXS instrument was launched aboard the Astro-H observatory on February 17, 2016. The SXS spectrometer is based on a high sensitivity x-ray calorimeter detector system that has been successfully deployed in many ground and sub-orbital spectrometers. The instrument was to provide essential diagnostics for nearly every class of x-ray emitting objects from the atmosphere of Jupiter to the outskirts of galaxy clusters, without degradation for spatially extended objects. The SXS detector system consisted of a 36-pixel cryogenic microcalorimeter array operated at a heat sink temperature of 50 mK. In pre-flight testing, the detector system demonstrated a resolving power of better than 1300 at 6 keV with a simultaneous band-pass from below 0.3 keV to above 12 keV with a timing precision better than 100 microsecond. In addition, a solid-state anti-coincidence detector was placed directly behind the detector array for background suppression. The detector error budget included the measured interference from the SXS cooling system and the spacecraft. Additional margin for on-orbit gain-stability, and on-orbit spacecraft interference were also included predicting an on-orbit performance that meets or exceeds the 7 eV FWHM at 6 keV requirement. The actual on-orbit spectral resolution was better than 5 eV FWHM at 6 keV, easily satisfying the instrument requirement. Here we discuss the actual on-orbit performance of the SXS detector system and compare this to performance in pre-flight testing and the on-orbit predictions. We will also discuss the on-orbit gain stability, additional on-orbit interference, and measurements of the on-orbit background.

Abstract

The calorimeter array of the JAXA Astro-H (renamed Hitomi) Soft X-ray Spectrometer (SXS) was designed to provide unprecedented spectral resolution of spatially extended cosmic x-ray sources and of all cosmic x-ray sources in the Fe-K band around 6 keV, enabling essential plasma diagnostics. The SXS has a square array of 36 microcalorimeters at the focal plane. These calorimeters consist of ion-implanted silicon thermistors and HgTe thermalizing x-ray absorbers. These devices have demonstrated a resolution of better than 4.5 eV at 6 keV when operated at a heat-sink temperature of 50 mK. We will discuss the basic physical parameters of this array, including the array layout, thermal conductance of the link to the heat sink, resistance function, absorber details, and means of attaching the absorber to the thermistor-bearing element. We will also present the thermal characterization of the whole array, including thermal conductance and crosstalk measurements and the results of pulsing the frame temperature via alpha particles, heat pulses, and the environmental background. A silicon ionization detector is located behind the calorimeter array and serves to reject events due to cosmic rays. We will briefly describe this anti-coincidence detector and its performance.

We report a new method developed for the determination of stable carbon isotopic composition of homologous [alpha], [omega]-dicarboxylic acids and phthalic acid isolated from environmental samples such as atmospheric aerosols and snow. Dicarboxylic acids are derivatized with B[F.sub.3]/1-butanol to dibutyl esters, which are analyzed for the stable carbon isotopic composition using a capillary GC interfaced to on-line combustion isotope ratio mass spectrometer. The [delta][sup.13]C values for individual dicarboxylic acid are then calculated from [delta][sup.13]C of 1-butanol and butyl ester derivative using a mass balance equation. The accuracy of the [delta][sup.13]C measurement for [C.sub.2]-[C.sub.10] diacids is within 0.8 [per thousandth]. We report a few examples of the [delta][sup.13]C ratios of saturated [C.sub.2]-[C.sub.9] [alpha], [omega]-dicarboxylic acids, unsaturated (maleic, phthalic) diacids, and oxocarboxylic acids in the aerosol and snow samples.

Abstract

Mice lacking the stress-signaling kinase SEK1 die from embryonic day 10.5 (E10.5) to E12.5. Although a defect in liver formation is accompanied with the embryonic lethality of sek[1.sup.-/-] mice, the mechanism of the liver defect has remained unknown. In the present study, we first produced a monoclonal antibody specifically recognizing murine hepatoblasts for the analysis of liver development and further investigated genetic interaction of sek1 with tumor necrosis factor-[alpha] receptor 1 gene (tnfr1) and protooncogene c-jun, which are also responsible for liver formation and cell apoptosis. The defective liver formation in sek[1.sup.-/-] embryos was not protected by additional tnfr1 mutation, which rescues the embryonic lethality of mice lacking NF-[kappa]B signaling components. There was a progressive increase in the hepatoblast cell numbers of wild-type embryos from E10.5 to E12.5. Instead, impaired hepatoblast proliferation was observed in sek[1.sup.-/-] livers from E10.5, though fetal liver-specific gene expression was normal. The impaired phenotype in sek[1.sup.-/-] livers was more severe than in [c-jun.sup.-/-] embryos, and sek[1.sup.-/-] [c-jun.sup.-/-] embryos died more rapidly before E8.5. The hepatoblast proliferation required no hematopoiesis, since liver development was not impaired in AML[1.sup.-/-] mice that lack hematopoietic functions. Stimulation of stress-activated protein kinase/c-Jun N-terminal kinase by hepatocyte growth factor was attenuated in sek[1.sup.-/-] livers. Thus, SEK1 appears to play a crucial role in hepatoblast proliferation and survival in a manner apparently different from NF-[kappa]B or c-Jun.
