Objective To investigate the prevalence of mild cognitive impairment (MCI) among the elderly in rural communities in Hangzhou, Zhejiang, and to explore the screening accuracy of Screening Scale for Mild Cognitive Impairment (sMCI) in the elderly with low education. Methods From April 2010 to September 2010, 360 elderly people in Sijiqing street, Jianggan district (now Shangcheng district), Hangzhou, Zhejiang were recruited. Dementia and MCI were judged by Mini⁃Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA), sMCI and Clinical Dementia Rating Scale (CDR). Draw receiver operating characteristic (ROC) curve and calculate the area under the curve (AUC), and compare the accuracy of sMCI, MoCA and CDR scores in screening MCI. Results Finally, 171 cases completed all investigations. 1) 55 cases (32.16%) were diagnosed as MCI, including 25 cases (14.62%) of amnestic MCI (aMCI) and 30 cases of non⁃aMCI, 11 cases (6.43%) of dementia, 10 cases (5.85%) of depression, 4 cases (2.34%) of anxiety disorder, one case (0.58%) of bipolar disorder, one case (0.58%) of schizophrenia and one case (0.58%) of mental retardation. Among 154 patients with cognitive impairment, 25 cases (16.23%) were screened for dementia by MMSE, 8 cases (5.19%) were screened for dementia by CDR, and 11 cases (7.14%) were clinically confirmed; 138 cases (89.61%) of MCI were screened by MoCA, 117 cases (75.97%) by sMCI, 70 cases (45.45%) by CDR, and 55 cases (32.16%) were clinically confirmed. 2) Taking clinical diagnosis as reference standard, the ROC curve showed CDR score had the highest accuracy in screening MCI, and the AUC was 0.90 ± 0.03 (95%CI: 0.844-0.957, P=0.000); the AUC of MoCA score was 0.53 ± 0.05 (95%CI: 0.430-0.621, P=0.603); when the cut⁃off value of sMCI score was 23, the AUC was 1.00 ± 0.00 (95%CI: 1.000-1.000, P=0.000). The cut⁃off value of subjects with education level of 0-3 years was adjusted to 22, and the AUC was 0.67 ± 0.05 (95%CI: 0.578-0.756, P=0.001). 3) According to education level, they were divided into 0-3 years group (113 cases) and 4-6 years group (47 cases). Taking CDR score as the reference standard, ROC curve showed the AUC of MoCA score in screening MCI in 4-6 years group was 0.49 ± 0.17 (95%CI: 0.157-0.824, P=0.955), the cut⁃off value of sMCI score was 23, the AUC of sMCI score was 0.50 ± 0.17 (95%CI: 0.161-0.839, P=1.000); the AUC of MoCA score in the 0-3 years group was 0.51 ± 0.06 (95%CI: 0.402-0.617, P=0.858), and the cut⁃off value of sMCI score was adjusted to 22, and the AUC was 0.64 ± 0.05 (95%CI: 0.535-0.744, P=0.011). Conclusions It is more common for the elderly in the rural communities with low education to have MCI, the accuracy of sMCI in screening MCI is higher than MoCA, and the cut⁃off value is 23 (education level 4-6 years) and 22 (education level 0-3 years), which is worthy of clinical application.

Cognitive impairment has become one of the most important diseases threatening the health of the elderly population. How to identify and evaluate it early is the key to the early diagnosis and treatment for this kind of disease. This review compares the cognitive assessment scales commonly used at home and abroad so as to provide a reference for researchers to choose a suitable scale. It also introduces the recent progress of cognitive impairment screening scales in China in order to develop a cognitive impairment screening scale for Chinese population.

In order to meet the diverse and multilevel health service needs of the people, following the implementation of the “multi-point practice” of registered physicians by the government in 2009, many pilot projects on the legality of “shared nurses” have been carried out, but there are many difficulties in the process. Through reviewing and researching the relevant literature inland and abroad, this study explores the application of family-community-hospital integrated medical and nursing management model in order to provide a reference for the development and management of “shared nurses” in China.

BackgroundA vaccine against coronavirus disease 2019 (COVID-19) with highly effective protection is urgently needed. The anti-severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) antibody response and duration after vaccination are crucial predictive indicators.ObjectivesTo evaluate the response and duration for 5 subsets of anti-SARS-CoV-2 antibodies after vaccination and their predictive value for protection.MethodsWe determined the response and duration for 5 subsets of anti-SARS-CoV-2 antibodies (neutralizing antibody, anti-RBD total antibody, anti-Spike IgG, anti-Spike IgM, and anti-Spike IgA) in 61 volunteers within 160 days after the CoronaVac vaccine. A logistic regression model was used to determine the predictors of the persistence of neutralizing antibody persistence.ResultsThe seropositivity rates of neutralizing antibody, anti-RBD total antibody, anti-Spike IgG, anti-Spike IgM, and anti-Spike IgA were only 4.92%, 27.87%, 21.31%, 3.28% and 0.00%, respectively, at the end of the first dose (28 days). After the second dose, the seropositivity rates reached peaks of 95.08%, 100.00%, 100.00%, 59.02% and 31.15% in two weeks (42 days). Their decay was obvious and the seropositivity rate remained at 19.67%, 54.10%, 50.82%, 3.28% and 0.00% on day 160, respectively. The level of neutralizing antibody reached a peak of 149.40 (101.00–244.60) IU/mL two weeks after the second dose (42 days) and dropped to 14.23 (7.62–30.73) IU/mL at 160 days, with a half-life of 35.61(95% CI, 32.68 to 39.12) days. Banana streak virus (BSV) belongs to the members of the genus Badnavirus, family Caulimoviridae. At present, BSV contains nine species in the International Committee on Taxonomy of Viruses (ICTV) classification report (2018b release). Previous study indicated that the viral particles of Banana streak virus Acuminata Yunnan (BSV-Acum) were purified from banana (Cavendish Musa AAA group) leaves in Yunnan Province, China, and its complete genome was obtained. To further determine whether this sample infecting with Banana streak GF virus (BSGFV), the polymerase chain reaction (PCR) cloning and complete genome analysis of the Banana streak GF virus Yunnan isolate (BSGFV-YN) isolate were carried out in this study. The result showed that BSGFV-YN infecting Cavendish Musa AAA group was co-infecting this sample. Its genome contains a total of 7,325 bp in length with 42% GC content. This complete genome sequence was deposited in GenBank under accession number MN296502. Sequence analysis showed that the complete genome of BSGFV-YN was 98.14% sequence similarity to BSGFV Goldfinger, while it was 49.10–57.09% to other BSV species. Two phylogenetic trees based on the complete genome and ORFIII polyprotein indicated that BSGFV-YN and other BSV species clustered into a group, while it was the highest homology with BSGFV Goldfinger. Although BSGFV-YN and BSGFV Goldfinger were highly homologous, their cultivating bananas are different. The former cultivating banana was from Cavendish Musa AAA group, while the latter cultivating banana was from Goldfinger Musa AAAB group. Compared with BSGFV Goldfinger, the genome of BSGFV-YN has an extra multiple repetitive sequences in the intergenetic region between ORFIII and ORFI, suggesting that this region might be related to host selection. In summary, a BSGFV-YN distant from BSV-Acum was identified from the same sample, and its complete genome sequence was determined and analyzed. The study extends the polymorphism of BSVs in China and provides scientific clue for the evolutionary relationship with host selection of badnaviruses.

The genovariation of endothelin receptor type B (EDNRB) was identified in a Chinese family with Waardenburg syndrome type I (WS1) in the present study. WS1 was diagnosed in a 19-year-old young man, his older sister and aunt according to WS consortium criteria. After extracting genomic DNA from the peripheral blood samples, the coding exons and intronic regions of EDNRB were sequenced. A missense heterozygous mutation was found in the coding region of exon 2 in the EDNRB gene on chormosome 13q22.3 of the proband. The same mutation was detected in the proband’s afflicted paternal aunt and first older sister. Subsequent polyphen analysis and three-dimensional modeling confirmed that the c.469A>G heterozygous mutation in EDNRB was possibly pathogenic. This is the first report of EDNRB mutation as a potential disease-causing mutation in Chinese patients with WS1.

A grasp planning method based on the volume and flattening of a generalized force ellipsoid is proposed to improve the grasping ability of a dexterous robotic hand. First, according to the general solution of joint torques for a dexterous robotic hand, a grasping indicator for the dexterous hand—the maximum volume of a generalized external force ellipsoid and the minimum volume of a generalized contact internal force ellipsoid during accepted flattening—is proposed. Second, an optimal grasp planning method based on a task is established using the grasping indicator as an objective function. Finally, a simulation analysis and grasping experiment are performed. Results show that when the grasping experiment is conducted with the grasping configuration and positions of contact points optimized using the proposed grasping indicator, the root-mean-square values of the joint torques and contact internal forces of the dexterous hand are at a minimum. The effectiveness of the proposed grasping planning method is thus demonstrated.

AIM: To evaluate the clinical therapeutic efficacy of intravitreal ranibizumab injection combined grid laser photocoagulation for macular edema secondary to branch retinal vein occlusion(BRVO). METHODS: Forty-two confirmed cases(42 eyes)with macular edema secondary to BRVO were randomized into 3 groups, each group contained 14 eyes. The ranibizumab group was received intravitreal injection of ranibizumab(0.05mL), the laser group was received grid laser photocoagulation, and the combined group was received a second therapy of grid laser photocoagulation after 1wk of the intravitreal injection of ranibizumab. Recorded the best-corrected visual acuity(BCVA)and the central macular thickness(CMT)preoperative and at 1, 3, 6mo after therapy. RESULTS: The BCVA and the CMT had no differences among three groups pretherapy(P>0.05). While BCVA was much better and CMT was reduced significantly posttherapy than pretherapy in all three groups(PPP>0.05). While the BCVA was better and the CMT was thinner in the combined group than ranibizumab group and laser group at every time point(PPCONCLUSION: The intravitreal ranibizumab injection combined grid laser photocoagulation is an effective treatment method for the macular edema secondary to BRVO, it is more effective in improving BCVA than intravitreal ranibizumab or grid laser photocoagulation alone.
