3. The interplay between diabetes Mellitus and soft tissue infections in general surgical patients

Abstract

Abstract Background Diabetes mellitus (DM) is a worldwide pandemic affecting 500 million people. It is known to be associated with increased susceptibility to soft tissue infections (STI). Despite being a major public health burden, the literature relating the effects of DM and the presentation, severity and healing of STIs in general surgical patients remain limited. Method We conducted a retrospective review of all patients admitted with STI in a tertiary teaching hospital over a 12-month period. Patient demographics and surgical outcomes were collected and analysed. Results During the study period, 1059 patients were admitted for STIs (88% required surgery). DM was an independent risk factor for LOS. Diabetic patients presented with higher body-mass index (28 vs. 26), larger abscess size (24 vs. 14 cm2) and had a longer length of stay (4.4 days vs. 2.9 days). They also underwent a higher proportion of wide debridement and application of negative pressure wound therapy (42% vs. 35%). More diabetic patients underwent subsequent re-operation within the same sitting (8 vs. 4). Diabetic patients were two times more likely to present with carbuncles (p = 0.02). Conclusion The incidence of STIs among DM patients represent a significant disease burden, surgeons should consider intensive patient counselling and partnering with primary care providers in order to help reduce the incidence of future STI admissions based upon lifestyle modification and glucose control.

Abstract

Background: Septic shock is the progression of sepsis, defined as cardiovascular dysfunction during systemic infection, and it has a mortality rate of 40 %–80 %. Loss of vascular tone is an important pathophysiological feature of septic shock. Diastolic blood pressure (DBP) was reported to be associated with vascular tone. This study aimed to identify the associations of several hemodynamic indices, especially DBP, with outcome in pediatric septic shock to allow for timely interventions. Methods: Children with persistent catecholamine-resistant shock had a pulse index continuous cardiac output (PiCCO®) system implanted for invasive hemodynamic monitoring and were enrolled in the current study. Serial cardiac index, systemic vascular resistance index (SVRI), systolic blood pressure (SBP), mean arterial pressure (MAP), and DBP were recorded during the first 24 h following PiCCO® initiation. All hemodynamic parameters associated with 28-day mortality were further analyzed using receiver operating characteristic curve analysis. Results: Thirty-three children with persistent catecholamine-resistant shock were enrolled. The median age was 12 years and the youngest children were 5 years old. Univariate analysis noted that SVRI, SBP, MAP, and DBP were significantly higher, and shock index was significant lower, in survivors compared with non-survivors (p 0.75, indicating a good prediction for mortality. Conclusions: DBP correlated well with SVRI and it can serve as a predictor for mortality in pediatric septic shock. Furthermore, DBP was a superior discriminator of mortality when compared with SBP and MAP. A lower DBP was an independent hemodynamic factor associated with 28-day mortality.

Abstract

Abstract Objective Given the scarcity of studies analyzing the clinical predictors of pediatric septic cases that would progress to septic shock, this study aimed to determine strong predictors for pediatric emergency department (PED) patients with sepsis at risk for septic shock and mortality. Methods We conducted chart reviews of patients with ≥ 2 age-adjusted quick Sequential Organ Failure Assessment score (qSOFA) criteria to recognize patients with an infectious disease in two tertiary PEDs between January 1, 2021, and April 30, 2022. The age range of included patients was 1 month to 18 years. The primary outcome was development of septic shock within 48 h of PED attendance. The secondary outcome was sepsis-related 28-day mortality. Initial important variables in the PED and hemodynamics with the highest and lowest values during the first 24 h of admission were also analyzed. Results Overall, 417 patients were admitted because of sepsis and met the eligibility criteria for the study. Forty-nine cases progressed to septic shock within 48 h after admission and 368 were discharged without progression. General demographics, laboratory data, and hemodynamics were analyzed by multivariate analysis. Only the minimum diastolic blood pressure/systolic blood pressure ratio (D/S ratio) during the first 24 h after admission remained as an independent predictor of progression to septic shock and 28-day mortality. The best cutoff values of the D/S ratio for predicting septic shock and 28-day mortality were 0.52 and 0.47, respectively. Conclusions The D/S ratio is a practical bedside scoring system in the PED and had good discriminative ability in predicting the progression of septic shock and in-hospital mortality in PED patients. Further validation is essential in other settings.

Abstract Purpose Acute kidney injury (AKI) is commonly seen in neonatal intensive care units (NICUs) and is potentially associated with adverse prognoses in later stages of life. Our study evaluated the impact of sustained AKI (SAKI) on both neurodevelopmental impairment (NDI) and early growth restriction (EGR) in neonates. Methods This case-control study retrospectively analyzed the medical records of neonates diagnosed with SAKI in the NICU of a tertiary medical center during the period from January 2007 to December 2020. Cases without subsequent follow-up and those resulting in death were excluded. We analyzed demographic, biochemical, and clinical outcome data. Results Of the 93 neonates with SAKI, 51 cases (54.8%) were included in this study, while 42 cases (45.2%) were excluded due to a lack of follow-up or death. An age-matched control group comprised 103 neonates, who had never experienced AKI or SAKI, were selected at random. In total, 59 (38.3%) cases were identified as NDI and 43 (27.9%) as EGR. Abstract

Abstract Background SARS-CoV-2 posed a threat to children during the early phase of Omicron wave because many patients presented with febrile seizures. The study aimed to investigate predicting factors for acute encephalopathy of children infected by SARS-CoV-2 Omicron variant presenting with febrile seizures. Methods The retrospective study analyzed data from pediatric patients who visited the emergency department of Chang Gung Memorial Hospital in Taiwan between April and July 2022. We specifically focused on children with COVID-19 who presented with febrile seizures, collecting demographic, clinical, and laboratory data at the pediatric emergency department, as well as final discharge diagnoses. Subsequently, we conducted a comparative analysis of the clinical and laboratory characteristics between patients diagnosed with acute encephalopathy and those with other causes of febrile seizures. Results Overall, 10,878 children were included, of which 260 patients presented with febrile seizures. Among them, 116 individuals tested positive for SARS-CoV-2 and of them, 14 subsequently developed acute encephalopathy (12%). Those with acute encephalopathy displayed distinctive features, including older age (5.1 vs. 2.6 years old), longer fever duration preceding the first seizure (1.6 vs. 0.9 days), cluster seizure (50% vs. 16.7%), status epilepticus (50% vs. 13.7%) and occurrences of bradycardia (26.8% vs. 0%) and hypotension (14.3% vs. 0%) in the encephalopathy group. Besides, the laboratory findings in the encephalopathy group are characterized by hyperglycemia (mean (95% CI) 146 mg/dL (95% CI 109–157) vs. 108 mg/dL (95% CI 103–114) and metabolic acidosis (mean (95% CI) pH 7.29(95% CI 7.22–7.36) vs. 7.39 (95%CI 7.37–7.41)). Conclusions In pediatric patients with COVID-19-related febrile seizures, the occurrence of seizures beyond the first day of fever, bradycardia, clustered seizures, status epilepticus, hyperglycemia, and metabolic acidosis should raise concerns about acute encephalitis/encephalopathy. However, the highest body temperature and the severity of leukocytosis or C-reactive protein levels were not associated with poor outcomes.

Abstract

TAR DNA-binding protein-43 (TDP-43) proteinopathies are accompanied by the pathological hallmark of cytoplasmic inclusions in the neurodegenerative diseases, including frontal temporal lobar degeneration-TDP and amyotrophic lateral sclerosis. We found that transthyretin accumulates with TDP-43 cytoplasmic inclusions in frontal temporal lobar degeneration-TDP human patients and transgenic mice, in which transthyretin exhibits dramatic expression decline in elderly mice. The upregulation of transthyretin expression was demonstrated to facilitate the clearance of cytoplasmic TDP-43 inclusions through autophagy, in which transthyretin induces autophagy upregulation via ATF4. Of interest, transthyretin upregulated ATF4 expression and promoted ATF4 nuclear import, presenting physical interaction. Neuronal expression of transthyretin in frontal temporal lobar degeneration-TDP mice restored autophagy function and facilitated early soluble TDP-43 aggregates for autophagosome targeting, ameliorating neuropathology and behavioural deficits. Thus, transthyretin conducted two-way regulations by either inducing autophagy activation or escorting TDP-43 aggregates targeted autophagosomes, suggesting that transthyretin is a potential modulator therapy for neurological disorders caused by TDP-43 proteinopathy.

Abstract

In the net-zero era, the burgeoning demands within engineering applications require materials that are not only lighter and stronger but also ductile. A robust strain hardening, crucial for achieving high strength and ductility, is challenging due to limited dislocation density evolution. This study discovers a cooperative strain-hardening strategy in a newly designed high-entropy steel (HES) with a density of 6.82 g/cm3. In this lightweight HES, the duplex microstructure of compositionally complex austenite and D03 intermetallic compounds facilitates the interplay between geometrically necessary dislocations (GNDs) and deformation twins (DTs) during plastic deformation. It generates a strain-hardening resilience during deformation and yields a very high dislocation density of 6.62 × 1015 m-2, contributing to strain hardening of over 900 MPa and a large elongation of 47 %. The resilient strain hardening achieved by the GND-DT cooperative strategy can be applied to various heterostructured alloys, offering a pathway for strong and ductile lightweight materials.

Abstract Ferroelectric semiconductor α‐In2Se3 has gained significant attention due to its favorable physical characteristics, including an appropriate bandgap (≈1.4 eV) for semiconductor devices, intercorrelated out‐of‐plane and in‐plane polarization, and high Curie temperature (>200 °C). Combining its semiconducting and ferroelectric properties, α‐In2Se3 holds promise for developing many innovative applications. However, the large‐scale synthesis of uniform layered α‐In2Se3 for practical use and a comprehensive understanding of its phase transition during synthesis are lacking. In this study, layered α‐In2Se3 on amorphous SiO2 substrates at a cm2‐scale is successfully synthesized and explored its phase transition during synthesis, by using a 2D solid‐phase crystallization (2DSPC) method with a SiO2 encapsulation. The formation of highly crystalline 2D layered α‐In2Se3 is observed through the real‐time β‐phase to α‐phase transition at room temperature. The electrical (field‐effect mobility µFE ≈ 1 cm2 V−1s−1), optical, and ferroelectric properties of the synthesized α‐In2Se3 thin films are further investigated. This study contributes to the understanding and control of stoichiometry and phases of In2Se3 and provides an efficient approach for synthesizing large‐area 2D layered α‐In2Se3.

Abstract

Self-assembled monolayers (SAMs) with short organic chains terminated with specific functional groups are attractive for modifying surface properties for a variety of applications, including being a copper diffusion barrier layer on Si wafer. However, SAM formation is process-sensitive and ideal SAM is difficult to obtain, both of which need to carefully deal with before practical use. Herein we developed a system to achieve seamless electroless deposited copper (ELD Cu) film on an amino-terminated silane SAM modified Si wafer with the help of a bifunctional polyvinyl alcohol-capped Pd nanocluster (PVA-Pd) catalyst. The PVA-Pd acts as a catalyst for ELD Cu as well as a healing agent to fix defects of the amino silane SAM. The sheet resistance, cross-sectional morphology analysis and adhesion of ELD Cu on the Si wafer are extensively studied. By controlling the abundancy of PVA in PVA-Pd, the diffusion barrier property of a flawed amino-terminated silane SAM remains reliably effective even after a 500 °C rapid thermal annealing (RTA), evidencing the superior healing function of PVA. The whole thickness of the PVA-Pd/amino-terminated silane SAM before and after RTA is 〜6 nm and 〜2 nm, respectively, which is applicable for advanced microelectronic manufacturing.

13. A study on thermal-induced phase transformation behavior and deterioration mechanism of 310S stainless steel

Abstract

In this study, cold drawing 310 S stainless steel was selected as the raw material and employed two heat treatment methods, isothermal treatment (900°C for 12 hours) and thermal cycling processes (900°C-1 h↹room temperature-1 min-12 cycles and 900°C-1 min↹room temperature-1 min-100 cycles), to investigate the effects of heat treatment on the microstructural characteristics and mechanical properties. The results indicate that after isothermal treatment (900°C for 12 hours), the microstructure of AISI 310 S stainless steel transforms into a single-phase equiaxed grain structure. The strength decreases while the ductility increases. After thermal cycling treatment, the grain size is refined, resulting in increased strength but decreased ductility. Through FIB (Focused Ion Beam), WDS (Wavelength Dispersive Spectroscopy), and EPMA (Electron Probe Microanalysis) analyses, it was revealed that in a high-temperature, long-term environment, silicon (Si) tends to diffuse to the surface and aggregate with carbon (C) and oxygen (O) to form eutectic SiCO phase. These eutectic SiCO phase, upon melting at high temperatures and subsequent solidification after the experiment, result in the formation of shrinkage cavities in subsurface. Therefore, leads to the deterioration of tensile properties. On the other hand, after thermal cycling tests (900°C-1 min↹room temperature-1 min-100 cycles), due to thermal expansion and contraction inducing shear-induced defects in the lattice, the material exhibits recrystallization behavior, resulting in grain refinement and an increase in tensile mechanical properties. Additionally, conducting tensile strain analysis on the specimens after thermal cycling (two strain levels: 16%, 32%), it was observed that tensile cracks continue to propagate and grow along the surface cracks generated during the original thermal cycling, confirming the failure mechanism of thermal cycling.

Abstract Hydrogen embrittlement reduces the durability of the structural steels required for the hydrogen economy. Understanding how hydrogen interacts with the materials plays a crucial role in managing the embrittlement problems. Theoretical models have indicated that carbon vacancies in metal carbide precipitates are effective hydrogen traps in steels. Increasing the number of carbon vacancies in individual metal carbides is important since the overall hydrogen trapping capacity can be leveraged by introducing abundant metal carbides in steels. To verify this concept, we compare a reference steel containing titanium carbides (TiCs), which lack carbon vacancies, with an experimental steel added with molybdenum (Mo), which form Ti-Mo carbides comprising more carbon vacancies than TiCs. We employ theoretical and experimental techniques to examine the hydrogen trapping behavior of the carbides, demonstrating adding Mo alters the hydrogen trapping mechanism, enabling hydrogen to access carbon vacancy traps within the carbides, leading to an increase in trapping capacity.

Abstract

Titanium-based alloys exhibit superior resistance to corrosion and biocompatibility in comparison to other alloys utilized for implant applications, such as stainless steel and cobalt-based alloys. Nonetheless, no completely inert metal is suitable for implantation in the human body, as aseptic loosening of the implant or the release of ions in the vicinity of the implant can result in inflammation and implant failure. Therefore, the primary objective of this study was to employ hydrogenated titanium 6-aluminum-4-vanadium (H–Ti6Al4V) alloy to reduce reactive oxygen species (ROS) and thereby achieve the function of regulating inflammatory signaling when employed as an implant.The Ti6Al4V alloy was successfully hydrogenated using a cathodic hydrogen-charging method. The H–Ti6Al4V alloy displayed a surface Young's modulus 33.97 ± 9.21 GPa similar to that of trabecular bone (∼22.3 GPa) and notably lower hardness (3.32 ± 0.62 GPa) than the Ti6Al4V alloy (21.79 ± 2.42 GPa). In vitro and in vivo investigations indicated that the H–Ti6Al4V alloy did not negatively affect bone-related cells and triggered minimal inflammatory responses in Sprague–Dawley rats. Moreover, reduced ROS production was observed around the H–Ti6Al4V alloys, confirming that hydrogenation could effectively counteract the ROS generated during cell metabolism and indirectly impede the inflammatory response around the prosthesis, thereby rendering it a promising metallic implant for the treatment of bone defects.

Abstract

Abstract Due to extraordinary electronic and optoelectronic properties, large‐scale single‐crystal two‐dimensional (2D) semiconducting transition metal dichalcogenide (TMD) monolayers have gained significant interest in the development of profit‐making cutting‐edge nano and atomic‐scale devices. To explore the remarkable properties of single‐crystal 2D monolayers, many strategies are proposed to achieve ultra‐thin functional devices. Despite substantial attempts, the controllable growth of high‐quality single‐crystal 2D monolayer still needs to be improved. The quality of the 2D monolayer strongly depends on the underlying substrates primarily responsible for the formation of grain boundaries during the growth process. To restrain the grain boundaries, the epitaxial growth process plays a crucial role and becomes ideal if an appropriate single crystal substrate is selected. Therefore, this perspective focuses on the latest advances in the growth of large‐scale single‐crystal 2D TMD monolayers in the light of enhancing their industrial applicability. In the end, recent progress and challenges of 2D TMD materials for various potential applications are highlighted.

30. Efficacy of derinat as a treatment for murine and androgenetic alopecia (AGA) patients

Abstract

Abstract Background Androgenetic alopecia (AGA), one of the most common types of hair loss, is associated with oxidative stress, inflammation and aging. Derinat, a transient receptor potential canonical channels (TRPCs) inhibitor, restrains TRPCs-mediated increase intracellular Ca2+ signaling, which initiates the skin aging process with intracellular reactive oxygen species (ROS) accumulation. This study investigated whether Derinat protected skin from oxidative stress-induced damage and aging, thus inhibiting AGA pathogenesis. Methods The lifespan of Caenorhabditis elegans was measured to examine the capacity of Derinat to oppose the oxidative stress induced-aging process, which drives the hair cycle from anagen to catagen phase. The experiments that used BALB/c-nu and C57BL/6 mice determined the effects of Derinat on hair cycle and oxidative stress in skin. To further apply Derinat to clinical study, the resulting relationship between AGA pathogenesis and TRPCs-regulated oxidative stress was confirmed using the bioinformatics approach. We consequently used the parameters of hair density, hair diameter, hair recovery and quality of life index to evaluate the effect of Derinat treatment on AGA subjects. Results Derinat restrained the oxidative stress induced-aging process sufficiently to extend the lifespan of worms. Derinat also changed the hair growth patterns of mice by maintenance of the hair cycle at the anagen phase. This efficacy was due to reduction of TRPCs-mediated ROS accumulation. Because the bioinformatics analysis found that AGA pathogenesis is associated with TRPCs-regulated oxidative stress and inflammation, treatment with Derinat in AGA subjects increased positive outcomes of oral medication while mitigating the impairment of AGA subjects’ quality of life. Conclusions Derinat restrains AGA pathogenesis and may provide a new therapeutic approach for treating AGA. 31. BNT162b2 immunization-related myocarditis in adolescents and consequent hospitalization: Report from a medical center

Abstract

Background: To investigate Pfizer-BioNTech 162b2 mRNA COVID-19 vaccine (BNT162b2) immunization-related myocarditis and describe the risk factors for consequent hospitalization in the pediatric intensive care unit (PICU) in children between 12 and 18 years. Methods: Children and adolescents 12 years of age and older who presented with discomfort after BNT162b2 immunization (BNTI) and visited pediatric emergency room (PER) at Chang Gung Memorial Hospital from September 22, 2021 to March 21, 2022, were included for analysis. Results: 681 children presented with discomfort after BNTI and visited our PER. The mean age was 15.1 ± 1.7 years. Three hundred and ninety-four (57.9%) and 287 (42.1%) events were after 1st and 2nd dose, respectively. 58.4% (n = 398) were male. The most common complaints were chest pain (46.7%) and chest tightness (27.0%). The median (interquartile range [IQR]) interval of discomfort after BNTI was 3.0 (1.0−12.0) days. BNTI-related pericarditis, myocarditis and myopericarditis were diagnosed in 15 (2.2%), 12 (1.8%) and 2 (0.3%) patients, respectively. Eleven (1.6%) needed hospitalization in PICU. The median (IQR) hospital stay was 4.0 (3.0−6.0) days. There was no mortality. More patients were diagnosed myocarditis (p = 0.004) after 2nd dose BNTI. PICU admission occurred more commonly after 2nd dose BNTI (p = 0.007). Risk factors associated with hospitalization in PICU were abnormal EKG findings (p = 0.047) and abnormal serum troponin levels (p = 0.003) at PER. Conclusion: Myocarditis in children aged 12−18 years occurred more commonly following 2nd dose BNTI. Most cases were of mild or intermediate severity without death. Factors predicting BNTI-related myocarditis and consequent hospitalization in PICU were abnormal EKG findings and abnormal serum troponin levels at PER in this study.

Background: Early in the COVID-19 pandemic, there was a global shortage of masks. Although mask reprocessing was practiced, no clinical study has assessed systematically the impact of repeated cycles of wear and decontamination on the integrity of N95 filtering facepiece respirators (FFRs). Methods: We evaluated mask fit assessed by qualitative respirator fit test (QRFT) after each cycle of wear and decontamination, as well as four measures of mask integrity—bacterial filtration efficacy, particle filtration efficacy, differential pressure, and splash resistance through five cycles of wear and decontamination using one of the four modalities (moist heat, steam, ultraviolet-C irradiation, and hydrogen peroxide plasma). Results: A total of 60.6% (hydrogen peroxide plasma) to 77.5% (moist heat) of the FFRs passed five cycles of wear and decontamination, as assessed by the wearers passing QRFT all five times. Moist heat-decontaminated FFRs retained all technical measures of integrity through all five cycles. Conclusions: This is the first large-scale study to assess systematically the impact (clinically and quantitatively) on N95 FFR integrity of repeated cycles of wearing followed by decontamination.Our results suggest that moist heat is a promising method for decontaminating N95 FFRs. Performing QRFT after every cycle of wear and decontamination ensures wearer safety. Although there is currently no mask shortage, reprocessing may reduce medical waste and improve sustainability.

Abstract

IntroductionBrain metastases commonly occur in patients with non-small cell lung cancer (NSCLC). Standard first-line treatment for NSCLC, without an EGFR, ALK or ROS1 mutation, is either chemoimmunotherapy or anti-PD-1 monotherapy. Traditionally, patients with symptomatic or untreated brain metastases were excluded from the pivotal clinical trials that established first-line treatment recommendations. The intracranial effectiveness of these treatment protocols has only recently been elucidated in small-scale prospective trials.MethodsPatients with NSCLC and brain metastases, treated with first-line chemoimmunotherapy or anti-PD-1 monotherapy were selected from the Australian Registry and biObank of thoracic cancers (AURORA) clinical database covering seven institutions. The primary outcome was a composite time-to-event (TTE) outcome, including extracranial and intracranial progression, death, or need for local intracranial therapy, which served as a surrogate for disease progression. The secondary outcome included overall survival (OS), intracranial objective response rate (iORR) and objective response rate (ORR).Results116 patients were included. 63% received combination chemoimmunotherapy and 37% received anti-PD-1 monotherapy. 69% of patients received upfront local therapy either with surgery, radiotherapy or both. The median TTE was 7.1 months (95% CI 5 - 9) with extracranial progression being the most common progression event. Neither type of systemic therapy or upfront local therapy were predictive of TTE in a multivariate analysis. The median OS was 17 months (95% CI 13-27). Treatment with chemoimmunotherapy was predictive of longer OS in multivariate analysis (HR 0.35; 95% CI 0.14 – 0.86; p=0.01). The iORR was 46.6%. The iORR was higher in patients treated with chemoimmunotherapy compared to immunotherapy (58% versus 31%, p=0.01). The use of chemoimmunotherapy being predictive of iORR in a multivariate analysis (OR 2.88; 95% CI 1.68 - 9.98; p=0.04).ConclusionThe results of this study of real-world data demonstrate the promising intracranial efficacy of chemoimmunotherapy in the first-line setting, potentially surpassing that of immunotherapy alone. No demonstrable difference in survival or TTE was seen between receipt of upfront local therapy. Prospective studies are required to assist clinical decision making regarding optimal sequencing of local and systemic therapies.

Abstract

Abstract The scaling of transistors with thinner channel thicknesses has led to a surge in research on two-dimensional (2D) and quasi-2D semiconductors. However, modulating the threshold voltage (V T) in ultrathin transistors is challenging, as traditional doping methods are not readily applicable. In this work, we introduce a optical-thermal method, combining ultraviolet (UV) illumination and oxygen annealing, to achieve broad-range V T tunability in ultrathin In2O3. This method can achieve both positive and negative V T tuning and is reversible. The modulation of sheet carrier density, which corresponds to V T shift, is comparable to that obtained using other doping and capacitive charging techniques in other ultrathin transistors, including 2D semiconductors. With the controllability of V T, we successfully demonstrate the realization of depletion-load inverter and multi-state logic devices, as well as wafer-scale V T modulation via an automated laser system, showcasing its potential for low-power circuit design and non-von Neumann computing applications.

Abstract We investigated the unique passive behaviour of Hybrid steel in de-aerated sulfuric acid and aqueous sodium chloride solutions through corrosion tests, surface analysis, and thermochemical modelling. Electrochemical measurements confirmed that Hybrid steel possesses stainless steel characteristics, including passivity, breakdown, and pitting, akin to low-alloyed stainless steel. Synchrotron hard X-ray photoelectron spectroscopy revealed a dynamically protective nanoscale passive film composed of Fe, Cr, Ni, and Al oxides, contributing to its stainless nature. The presence of Al and Ni enhances Cr’s role in forming a spontaneously passive and protective surface, resulting in exceptional corrosion resistance in acidic and chloride-containing solutions. Hybrid steel’s surface oxides remain robust even beyond the Cr(III)-to-Cr(VI) redox potential, distinguishing it from other stainless steels. This work demonstrates the potential for designing sustainable stainless steel with high-strength properties without requiring the conventional Cr threshold concentration of 10.5 per cent.

Abstract Background Early childhood is a critical stage for the prevention of dental caries. The prevalence of caries in preschool children is still high in Taiwan, where National Health Insurance covers 99% of the population. The effort to improve the oral health of preschool children should be based on conceptual model that encompasses more than individual-level factors. This study input nationwide survey data in a conceptual model to evaluate the effects of comprehensive factors related to the high prevalence of caries in preschool children. Methods This observation study examined factors related to the oral health of preschool children by employing a comprehensive multilevel model to analyse nationally representative data from the Taiwan Oral Health Survey of Preschool Children (TOHPC) 2017–2018. Individual-level, family-level and community-level contextual effects were evaluated through multilevel analysis in this study. The proportional change in variance (PCV) was used to compare the multilevel model with the null model and individual-level, family-level, and community-level context effects. Results The estimated deft index for preschool children was 1.34 (1.22–1.47) at age 3, 2.20 (2.08–2.32) at age 4, and 3.05 (2.93–3.18) at age 5. The overall prevalence of caries in preschool children in Taiwan was 34.27% (30.76%, 37.78%) at age 3, 51.67% (48.99%, 54.35%) at age 4, and 62.05% (59.66%, 64.44%) at age 5. The model that included the individual-, family-, and community-context levels exhibited the highest reduction of variance (PCV = 53.98%). The PCV was further reduced to 35.61% when only the level of accessibility to dental services for individuals, families, and the community was considered. For the model in which no community-context cofactors were considered and the model considering only the individual level, the PCVs were 20.37% and 5.52%, respectively. Conclusions Our findings indicate the key components that affect oral health in preschool children and can serve as a reference for policy makers. The most notable finding of this study is that to improve the oral health of preschool children, community-level factors should be targeted. To rely solely on dentists for leading oral health education programs for children is impractical and inefficient. Training more professional oral health educators to provide additional community-based oral health promotion campaigns is critical.
