BackgroundCoronavirus disease 2019 (COVID-19)-related short-term mortality is high in dialysis patients, but longer-term outcomes are largely unknown. We therefore assessed patient recovery in a large cohort of dialysis patients 3 months after their COVID-19 diagnosis.MethodsWe analyzed data on dialysis patients diagnosed with COVID-19 from 1 February 2020 to 31 March 2021 from the European Renal Association COVID-19 Database (ERACODA). The outcomes studied were patient survival, residence and functional and mental health status (estimated by their treating physician) 3 months after COVID-19 diagnosis. Complete follow-up data were available for 854 surviving patients. Patient characteristics associated with recovery were analyzed using logistic regression.ResultsIn 2449 hemodialysis patients (mean ± SD age 67.5 ± 14.4 years, 62% male), survival probabilities at 3 months after COVID-19 diagnosis were 90% for nonhospitalized patients (n = 1087), 73% for patients admitted to the hospital but not to an intensive care unit (ICU) (n = 1165) and 40% for those admitted to an ICU (n = 197). Patient survival hardly decreased between 28 days and 3 months after COVID-19 diagnosis. At 3 months, 87% functioned at their pre-existent functional and 94% at their pre-existent mental level. Only few of the surviving patients were still admitted to the hospital (0.8-6.3%) or a nursing home (∼5%). A higher age and frailty score at presentation and ICU admission were associated with worse functional outcome.ConclusionsMortality between 28 days and 3 months after COVID-19 diagnosis was low and the majority of patients who survived COVID-19 recovered to their pre-existent functional and mental health level at 3 months after diagnosis.

Background In the general population with coronavirus disease 2019 (COVID-19), obesity is associated with an increased risk of mortality. Given the typically observed obesity paradox among patients on kidney function replacement therapy (KFRT), especially dialysis patients, we examined the association of obesity with mortality among dialysis patients or living with a kidney transplant with COVID-19. Methods Data from the European Renal Association COVID-19 Database (ERACODA) were used. KFRT patients diagnosed with COVID-19 between 1 February 2020 and 31 January 2021 were included. The association of Quetelet's body mass index (BMI) (kg/m2), divided into: Results In 3160 patients on KFRT (mean age: 65 years, male: 61%), 99 patients were lean, 1151 normal weight (reference), 1160 overweight, 525 obese I and 225 obese II/III. During follow-up of 3 months, 28, 20, 21, 23 and 27% of patients died in these categories, respectively. In the fully adjusted model, the hazard ratios (HRs) for 3-month mortality were 1.65 [95% confidence interval (CI): 1.10, 2.47], 1 (ref.), 1.07 (95% CI: 0.89, 1.28), 1.17 (95% CI: 0.93, 1.46) and 1.71 (95% CI: 1.27, 2.30), respectively. Results were similar among dialysis patients (N = 2343) and among those living with a kidney transplant (N = 817) (Pinteraction = 0.99), but differed by sex (Pinteraction = 0.019). In males, the HRs for the association of aforementioned BMI categories with 3-month mortality were 2.07 (95% CI: 1.22, 3.52), 1 (ref.), 0.97 (95% CI: 0.78. 1.21), 0.99 (95% CI: 0.74, 1.33) and 1.22 (95% CI: 0.78, 1.91), respectively, and in females corresponding HRs were 1.34 (95% CI: 0.70, 2.57), 1 (ref.), 1.31 (95% CI: 0.94, 1.85), 1.54 (95% CI: 1.05, 2.26) and 2.49 (95% CI: 1.62, 3.84), respectively. Conclusion In KFRT patients with COVID-19, on dialysis or a kidney transplant, obesity is associated with an increased risk of mortality at 3 months. This is in contrast to the obesity paradox generally observed in dialysis patients. Additional studies are required to corroborate the sex difference in the association of obesity with mortality.

In the general population with COVID-19, the male sex is an established risk factor for mortality, in part due to a more robust immune response to COVID-19 in women. Because patients on kidney function replacement therapy (KFRT) have an impaired immune response, especially kidney transplant recipients due to their use of immunosuppressants, we examined whether the male sex is still a risk factor for mortality among patients on KFRT with COVID-19. From the European Renal Association COVID-19 Database (ERACODA), we examined patients on KFRT with COVID-19 who presented between February 1st, 2020, and April 30th, 2021. 1204 kidney transplant recipients (male 62.0%, mean age 56.4 years) and 3206 dialysis patients (male 61.8%, mean age 67.7 years) were examined. Three-month mortality in kidney transplant recipients was 16.9% in males and 18.6% in females (p = 0.31) and in dialysis patients 27.1% in males and 21.9% in females (p = 0.001). The adjusted HR for the risk of 3-month mortality in males (vs females) was 0.89 (95% CI 65, 1.23, p = 0.49) in kidney transplant recipients and 1.33 (95% CI 1.13, 1.56, p = 0.001) in dialysis patients (pinteraction = 0.02). In a fully adjusted model, the aHR for the risk of 3-month mortality in kidney transplant recipients (vs. dialysis patients) was 1.39 (95% CI 1.02, 1.89, p = 0.04) in males and 2.04 (95% CI 1.40, 2.97, p interaction = 0.02). In patients on KFRT with COVID-19, the male sex is not a risk factor for mortality among kidney transplant recipients but remains a risk factor among dialysis patients. The use of immunosuppressants in kidney transplant recipients, among other factors, may have narrowed the difference in the immune response to COVID-19 between men and women, and therefore reduced the sex difference in COVID-19 mortality risk.

BACKGROUND: Kidney transplant patients are at high risk for coronavirus disease 2019 (COVID-19)-related mortality. However, limited data are available on longer-term clinical, functional, and mental health outcomes in patients who survive COVID-19.METHODS: We analyzed data from adult kidney transplant patients in the European Renal Association COVID-19 Database who presented with COVID-19 between February 1, 2020, and January 31, 2021.RESULTS: We included 912 patients with a mean age of 56.7 (±13.7) y. 26.4% were not hospitalized, 57.5% were hospitalized without need for intensive care unit (ICU) admission, and 16.1% were hospitalized and admitted to the ICU. At 3 mo follow-up survival was 82.3% overall, and 98.8%, 84.2%, and 49.0%, respectively, in each group. At 3 mo follow-up biopsy-proven acute rejection, need for renal replacement therapy, and graft failure occurred in the overall group in 0.8%, 2.6%, and 1.8% respectively, and in 2.1%, 10.6%, and 10.6% of ICU-admitted patients, respectively. Of the surviving patients, 83.3% and 94.4% reached their pre-COVID-19 physician-reported functional and mental health status, respectively, within 3 mo. Of patients who had not yet reached their prior functional and mental health status, their treating physicians expected that 79.6% and 80.0%, respectively, still would do so within the coming year. ICU admission was independently associated with a low likelihood to reach prior functional and mental health status.CONCLUSIONS: In kidney transplant recipients alive at 3-mo follow-up, clinical, physician-reported functional, and mental health recovery was good for both nonhospitalized and hospitalized patients. Recovery was, however, less favorable for patients who had been admitted to the ICU.

Due to its principles of voluntariness and openness and because of its diverse offer of subject-related educational opportunities, open youth work has a high potential to support young people in their development and self-education. These learning and educational opportunities are holistic and comprehensive and often go far beyond the acquisition of skills and competences. How can these educational processes be described empirically and what form of education can be spoken of here at all? These questions are addressed by the qualitative-reconstructive study "Educational Experiences in Non-formal Settings", which is conducted at the Centre for Childhood and Youth Research (CCY) at the University of Luxembourg. The study examines the subjective educational experiences of young people in open youth work from various perspectives: on the basis of participant observations in youth centres, in problem-centred interviews and group discussions with young people, as well as via a complementary quantitative survey of young people.

The detection of Reactor Coolant System (RCS) leakage inside containment with sampling radiation monitors located outside containment is reviewed. The target for the radiation monitor response is the clear indication of a one gpm increase in RCS leakage within one hour. The limitations of present methods using beta sensitive detectors are discussed. The limitations are often due to the detection of interfering radioactive materials not related to RCS leakage. Modeling of radioactive material buildup from leakage is used to predict the response of gamma sensitive detectors operated in spectral modes. This would allow isolating isotopes responsive from leak detection considerations from isotopes unresponsive and from isotopes unrelated to RCS leakage.

- Abstract
Background. Optimal phosphate control is an unmet need in chronic kidney disease (CKD). High serum phosphate increases calcification burden and is associated with mortality and cardiovascular disease in CKD. Nicotinamide (NA) alone or in combination with calcium-free phosphate binders might be a strategy to reduce phosphate levels and calcification and thus impact cardiovascular disease in CKD.Methods. We studied the effect of NA alone and in combination with magnesium carbonate (MgCO3) as a potential novel treatment strategy. CKD was induced in dilute brown non-agouti/2 mice by subtotal nephrectomy followed by a high-phosphate diet (HP) and 7 weeks of treatment with NA, MgCO3 or their combination. Control mice underwent subtotal nephrectomy and received an HP or underwent sham surgery and received standard chow plus NA.Results. CKD mice showed increased serum fibroblast growth factor 23 and calcium-phosphate product that was normalized by all treatment regimes. NA alone increased soft tissue and vascular calcification, whereas any treatment with MgCO3 significantly reduced calcification severity in CKD. While MgCO3 supplementation alone resulted in decreased calcification severity, it resulted in increased intestinal expression of the phosphate transporters type II sodium-dependent phosphate transporter 1 (Pit-1). Combined therapy of MgCO3 and NA reduced tissue calcification and normalized expression levels of intestinal phosphate transporter proteins.Conclusions. In conclusion, the data indicate that NA increases while MgCO3 reduces ectopic calcification severity. Augmented expression of intestinal phosphate transporters by MgCO3 treatment was abolished by the addition of NA. However, the clinical relevance of the latter remains to be explored. Importantly, the data suggest no benefit of NA regarding treatment of calcification in addition to MgCO3.

Aims: The focus of the presentation is the specific situation of young people with disabilities during their transition period into adulthood. Methods: The presentation is based on 51 guideline-based qualitative interviews with adolescents and young adults with a disability. The study was conducted within the framework of the Luxembourg Youth Report 2015 (Willems, 2015). Results: The aspirations and ideas of young people with disabilities about being and becoming an adult are little different from those of their able-bodied peers. For them, too, being an adult primarily means financial, emotional and social independence and autonomy. Nevertheless, it is obvious that for them the transition to self-reliant, independent adult life is more difficult: only one in seven of those questioned had found their first job on the labour market and one in two were still living with their parents. Conclusion: Our conclusion is that for adolescents and young adults with a disability, the transition to adulthood presents a disproportionately tough challenge compared with their able-bodied peers. Sociological and psychological theories (Hurrelmann & Quenzel, 2012; Havighurst, 1972) consider the successful accomplishment of certain developmental tasks as a master challenge of youth. Young people with disabilities have to cope with the same developmental tasks as young people without disabilities. However, achieving them is much more difficult, not only because of their physical, mental or psychiatric impairment but also because of the lack of respect and the stigmatisation that they experience in daily life.

Der vorliegende Bericht stellt die Ergebnisse der Zwischenevaluation über die Durchführung und die Wirkung des Programms Erasmus+ in Luxemburg vor. Mit der Durchführung der Evaluation und der Berichterstellung wurde die Jugendforschung der Universität Luxemburg unter Leitung von Prof. Dr. Helmut Willems vom zuständigen Ministerium (Ministère de l'Éducation nationale, de l'Enfance et de la Jeunesse) beauftragt. Der vorliegende Evaluationsbericht ergänzt die im Auftrag der Europäischen Kommission parallel erfolgende externe Evaluierung und rückt die nationale luxemburgische Perspektive in den Fokus. Der Schwerpunkt der Evaluation liegt daher auf den Erfahrungen und Rückmeldungen der relevanten Akteure (Ministerium, Agenturen, Projektverantwortliche, Projektteilnehmer) bezüglich der Implementierung, Durchführung und Auswirkungen des Programms in Luxemburg. Deren Einschätzungen werden im Bericht dargestellt.

Durch die typologische Einordnung auf Basis der individuellen Erfahrungen und Deutungen können Erfolgsfaktoren, Gelingensbedingungen und Hemmnisse für den Übergang ins Erwerbsleben identifiziert werden. Dabei zeigt sich, dass der Übergang in Arbeit zwar maßgeblich die gesellschaftliche Integration junger Menschen beeinflusst, das subjektive Gefühl des Erwachsenseins davon aber durchaus unabhängig sein kann.

Die Phase des Übergangs vom Jugend- zum Erwachsenenalter wird von gesellschaftlichen Normvorstellungen und entsprechenden Erwartungen begleitet. Aus entwicklungspsychologischer Perspektive werden den Jugendlichen bestimmte Entwicklungsaufgaben zugeschrieben, die in dieser Phase bewältigt werden sollen (Havighurst 1972). Darunter werden Anforderungen an das Individuum verstanden, deren Bewältigung zu einer erfolgreichen gesellschaftlichen Integration notwendig ist. Zu den Wesentlichen gehören die Ablösung vom Elternhaus, die Gründung einer eigenen Familie, die Aufnahme einer Erwerbstätigkeit und damit zusammenhängend eine ökonomische Unabhängigkeit (Hurrelmann 2009, Kerckhoff 2000, Merkens 2012). Im Widerspruch zu diesen Normen stehen die zu beobachtenden Tendenzen einer immer weniger normierten Transitionsgestaltung: Übergänge werden zunehmend individueller und verlaufen weniger einheitlich (Konietzka 2010). Diese Entwicklung führte in den vergangenen Jahren und Jahrzehnten zu einer Verlängerung der Jugendphase (Zinnecker 2003), einer Verlagerung von Transitionsmarkern auf einen späteren Zeitpunkt, einer Entkopplung von Transitionsereignissen sowie einer Diversität bzw. De-Standardisierung von Verläufen (Stauber/Walther 2004). Damit einhergehend ist die Phase des Übergangs mit geringerer Sicherheit und Planbarkeit sowie höherem Risiko für das Individuum verbunden. Dies betrifft insbesondere den Übergang junger Menschen in den Arbeitsmarkt, der im Fokus des Vortrags steht. Vorgestellt wird eine Typologie von Transitionsverläufen und Bewältigungsmustern junger Menschen in Luxemburg sowie deren subjektive Perspektiven auf das Erwachsenwerden und Erwachsensein. Durch die typologische Einordnung auf Basis der individuellen Erfahrungen und Deutungen können Erfolgsfaktoren, Gelingensbedingungen und Hemmnisse für den Übergang ins Erwerbsleben identifiziert werden. Dabei zeigt sich, dass Transitionsprobleme zu Maßnahmenkarrieren, Armut, sozialer Exklusion und gesundheitlichen bzw. psychischen Beeinträchtigungen führen können. Sie erschweren aber nicht nur die gesellschaftliche Integration, sondern haben darüber hinaus auch Einfluss auf das subjektive Gefühl des Erwachsenseins. Die Präsentation basiert auf den Ergebnissen einer qualitativen Befragung von 77 Jugendlichen und jungen Erwachsenen im Rahmen des luxemburgischen Jugendberichts 2015.

Research issue: Sociological and psychological theories (Hurrelmann & Quenzel, 2012; Havighurst, 1972) consider a successfully accomplishing of certain developmental task as master challenge of youth. Regarding the economic situation, the transition into work is nowadays a big issue for young people. As a result of social liberalisation and individualisation, developmental tasks and social roles can be more individually chosen and shaped, offering both advantages (more personal freedom) and disadvantages (greater personal responsibility, less strong orientation). How do the Luxembourgish youth of today handle the transition into work in the light of the above and what are the crucial components in successfully mastering the transition into work? Research questions: Our interests are related to the speed, the time and the degree of success of the transition and the coping strategies behind. In what way transition into work is influenced by gender, nationality/migration status and level of education? What are the varying processes, rationales and patterns for accomplishing the transition into work? And what is the role of support services for young people facing problems during the transition? Context and data: The research is based on various data sources. A secondary data analyses was used to a systematic description of young people differentiated by age, level of education and nationality/migration status with regard to transition markers. We devote particular attention to the participation of the young people themselves as experts in our research methods. 77 guideline-based interviews with young people were used to collect subjective information about their coping strategies to accomplish the transition into work. A standardized survey of participants in support services focused on their experiences and their learning outcomes was filled out by 1162 young persons. Experts from academia, professional practice, administration and politics together with representatives of young people participated in 11 focus group discussions. The triangulation of different methodical approaches and different data sources resulted in a broad and multi-perspective representation of young people’s transition into adulthood. Main results: 1) There is a big influence of the educational qualifications, the migration status and family support. For adolescents with average or high educational qualifications the transition into adulthood is easier (less frequently unemployed, find jobs matching their qualifications, more permanent employment contracts). Young people with poor school leaving qualifications face considerable difficulty with the transition into work. An analysis differentiating levels of education by migration background and nationality shows large differences. The data also demonstrates in particular the important supporting role played by the family (financial or emotional, acting as a role model, providing information or informal knowledge, or access to networks). 2) Furthermore our study identified widely varying processes, rationales and patterns for accomplishing the transition. Four types of transition could be identified (direct transition, alternative transition, transition requiring support and failed transition), distinguished by the different strategies for action, values, perspectives and attitudes. 3) The results of the survey indicate some subjective benefit of support service participation, but also potential for improvement.
