Abstract

Objetivos: A presente pesquisa buscou avaliar as condições clínicas de entrada e evolução em casos de hematoma subdural crônico (HSDC), a prevalência de sua recidiva e possíveis associações e correlações entre os parâmetros abordados no estudo e a recidiva do HSDC. Metodologia: Estudo observacional e retrospectivo baseado em 114 casos de HSDC, analisando parâmetros identificados durante internamento hospitalar, posteriormente analisados estatisticamente. Resultados: A prevalência da recidiva do HSDC constatada na amostra foi de 26,32%. 25. Eficácia do topiramato como terapia adicional em pacientes com estado epiléptico refratário: uma breve revisão sistemática.

Abstract

Objective: To identify current evidence on the use of topiramate for refractory status epilepticus. Methods: We reviewed the literature to investigate the efficacy of topiramate in the treatment of refractory status epilepticus. The search terms used were "status epilepticus", "refractory", "treatment" and "topiramate". No restrictions were used. Results: The search yielded 487 articles that reported using topiramate as a treatment for refractory status epilepticus and its outcomes. Case reports, review articles, and animal experiments were excluded. After excluding duplicates and applying inclusion and exclusion criteria, nine studies were included for analyses. Descriptive and qualitative analyses were performed, and the results were as follows: response rates (defined as termination in-hospital until 72 hours after the administration of topiramate) varied from 27% to 100%. The mortality rate varied from 5.9% to 68%. Positive functional long-term outcomes, defined as discharge, back to baseline or rehabilitation, were documented by seven studies, and the rates ranged between 4% and 55%. Most studies reported no or mild adverse effects. Conclusion: Topiramate was effective in terminating refractory status epilepticus, presented relatively low mortality and was well tolerated. Therefore, topiramate could be a good option as a third-line therapy for refractory status epilepticus, but further studies are necessary. [ABSTRACT FROM AUTHOR]

28. The Effectiveness of Reality Orientation Therapy in the Treatment of Parkinson Disease Dementia.

Abstract

Patients with Parkinson disease dementia (PDD) have deficits resulting mainly from frontostriatal dysfunction. The aim of this study was to assess the effectiveness of reality orientation therapy (ROT) combined with drug therapy (acetylcholinesterase inhibitors) in PDD treatment and compare it with that of drug therapy alone. Patients (n = 12) with a diagnosis of PDD were divided into 2 groups: group A--drug therapy and ROT; group B--drug therapy alone. Reality orientation therapy was applied weekly for 6 months, and patients were assessed during the same period. Significant improvements in frontostriatal deficits were observed in the group that received the combined therapy, as shown mainly by the scores in verbal fluency in the Consortium to Establish a Registry for Alzheimer's Disease (CERAD) battery (P = .02) and in attention in Scales for outcomes of Parkinson's Disease-Cognition (P = .021) and Clock Drawing Test (P = .037). Patients who received only medication had worse results in constructional praxis recall in the CERAD battery (P = .037). Our results indicate that ROT may help in the treatment of frontostriatal cognitive deficits and can potentially be used to complement drug therapy. [ABSTRACT FROM AUTHOR]
