1. The 'Little AVM': A New Entity in High-flow versus Low-flow Vascular Malformations

Abstract

Background: Arteriovenous malformations (AVMs) are high-flow lesions with abnormal connections between arteries and veins without an intervening capillary bed. Infrequently, the radiographic diagnosis of a vascular lesion will not support the clinical diagnosis of an AVM. These “discrepant” lesions are not adequately captured within the current classification system and represent a treatment dilemma. The purpose of this study is to review our center’s experience with vascular malformations where incongruity in a patient’s clinical and radiographic presentation produces a diagnostic and therapeutic challenge. Methods: A retrospective chart review of patients with atypical AVM pre sen ta tions was performed. Parameters reviewed included patient history and demogra phics, clinical presentation, radiological imaging, and treatment modalities. Results: Over a 15-year period, we identified 7 cases of vascular malformations with discrepant clinical and radiological findings concerning flow characteristics. All patients were treated based on their radiological diagnosis and most were managed with sclerotherapy. No lesions evolved into a high-flow process, and there was no recurrence at a minimum of 24 months of follow-up. Conclusions: We have identified and described a unique subcategory of vascular malformations that have clinical features of high-flow malformations but radiological features of low-flow malformations. These lesions behave like low-flow malformations and should be treated as such. We propose that complex vascular malformations are best evaluated by both clinical and specialized diagnostic radiological means; the radiologic diagnoses should supplant what is found clinically, and ultimately treatment should be preferentially based on a radiological diagnosis.

Abstract

The newborn hearing screening programme (NHSP) exists to identify permanent childhood hearing impairment (PCHI) in babies born in the UK. Early identification of hearing loss facilitates the provision of hearing aids or a prompt referral for cochlear implantation to ensure children optimise speech, language and communication skills, with the hope that this translates to the best possible educational, mental health, social and quality of life outcomes. Diagnosis of hearing impairment allows aetiological investigations to be offered to look for a potential cause of the PCHI, and given the recent advances in genomic testing, this can facilitate testing for additional family members and future children if a genetic cause is identified. In this article, the authors describe the structure of the NHSP and illustrate referral pathways into the programme from primary care for the few babies that may have been missed or fallen outside the remit of the programme.

Abstract

The National Lung Screening Trial (NLST) is a randomized multicenter study comparing low-dose helical computed tomography (CT) with chest radiography in the screening of older current and former heavy smokers for early detection of lung cancer, which is the leading cause of cancer-related death in the United States. Five-year survival rates approach 70% with surgical resection of stage IA disease; however, more than 75% of individuals have incurable locally advanced or metastatic disease, the latter having a 5-year survival of less than 5%. It is plausible that treatment should be more effective and the likelihood of death decreased if asymptomatic lung cancer is detected through screening early enough in its preclinical phase. For these reasons, there is intense interest and intuitive appeal in lung cancer screening with low-dose CT. The use of survival as the determinant of screening effectiveness is, however, confounded by the well-described biases of lead time, length, and overdiagnosis. Despite previous attempts, no test has been shown to reduce lung cancer mortality, an endpoint that circumvents screening biases and provides a definitive measure of benefit when assessed in a randomized controlled trial that enables comparison of mortality rates between screened individuals and a control group that does not undergo the screening intervention of interest. The NLST is such a trial. The rationale for and design of the NLST are presented.

Abstract

The diversity of Adult and Community Learning (ACL) provision and the particular nature of its workforce are important elements in any deliberation around the planning, access to and take-up of professional development of ACL teaching staff. One of the key goals of the LONCETT (London Centre for Excellence in Teacher Training) ACL project has been to present evidence-based ways forward in relation to the Initial Teacher Training (ITT) and Continuing Professional Development (CPD) needs of tutors. The LONCETT research has been conceived as an opportunity to record the perspectives of key players in the professional development of ACL tutors. This paper identifies the key barriers to professionalising staff in this part of the Learning and Skills Sector (LSS). It explores the potential of collaborative approaches between teacher training providers and learning providers to ensure appropriate and accessible professional development for the sector’s workforce.

Abstract

9589 Background: The cancer journey for an AYA is often complicated by diagnostic delays, poor prognosis and negative physical and psycho-social consequences. Distress of young patients and survivors is often overlooked and the real incidence of psychopathology remaining hidden. We aimed to understand the psychological impact and identify which factors most affect the patient experience. Methods: 172 patients on the Cambridge AYA register were invited to take part. Patients were asked to complete 2 questionnaires, SCL90 and RSQ, measuring psychological symptoms and response styles respectively, and to take part in a semi-structured interview. Parents were asked to complete the York Survey and be interviewed. Results: 33 patients and 45 parents/others were recruited. Questionnaires completed: 33 SCL90 and RSQ; 42 York Survey. Interview data from: 23 patients, 26 parents, 3 others. Patient sample: 16 male, 17 female; mean age (recruitment) = 20.76 (16-26); age (diagnosis) = 17.26 (12-24). Diagnoses evenly distributed across solid and haematological malignancies. All patients had experienced significant levels of psychological distress at some point in their cancer journey. 51% reached caseness on the SCL90 (showed a symptom profile consistent with a high risk of a disorder). Caseness increased with age and was correlated with higher rumination scores. Positive influences included family, friend and peer support, good communication and support from the AYA service and specialist nurses. Negative influences included delayed diagnosis, poor communication styles, anxiety about long term effects and lack of appropriate psychological support. Conclusions: We have shown evidence of elevated psychological symptoms in this population. Patients and their families have prioritized clear needs; early diagnosis, high quality of information and communication, access to psychological help at each stage and post treatment help in gaining independence and planning for a positive future. We should work towards meeting these needs for this vulnerable group.
