Abstract

Background: COVID-19 patients were often transferred to other intensive care units (ICUs) to prevent that ICUs would reach their maximum capacity. However, transferring ICU patients is not free of risk. We aim to compare the characteristics and outcomes of transferred versus non-transferred COVID-19 ICU patients in the Netherlands. Methods: We included adult COVID-19 patients admitted to Dutch ICUs between March 1, 2020 and July 1, 2021. We compared the patient characteristics and outcomes of non-transferred and transferred patients and used a Directed Acyclic Graph to identify potential confounders in the relationship between transfer and mortality. We used these confounders in a Cox regression model with left truncation at the day of transfer to analyze the effect of transfers on mortality during the 180 days after ICU admission. Results: We included 10,209 patients: 7395 non-transferred and 2814 (27.6%) transferred patients. In both groups, the median age was 64 years. Transferred patients were mostly ventilated at ICU admission (83.7% vs. 56.2%) and included a larger proportion of low-risk patients (70.3% vs. 66.5% with mortality risk

Abstract

Introduction: The NICE registry contains data from individual intensive care admissions concerning severity of illness and outcome. This study was undertaken to analyze the trends in time related to crude mortality and severity of illness corrected mortality. Methods: Data on all patients admitted from January 1 1999 to January 1 2008 were used. Patients who did not fulfil the inclusion criteria for the SAPS II prediction model were excluded. To analyze trends in time for individual ICUs, this study uses data from ICUs that participated in the NICE registry for a minimum of 5 years between January 1, 2001 and January 1, 2008. Repeated cross-sectional data analysis was performed for Standardized Mortality Ratio (SMR), and longitudinal data analysis using Variable Live Adjusted Displays (VLAD both on individual and national level were used. Results: Overall, 243 182 patients were included in the NICE registry. Of these, 144 696 met the SAPS II inclusion criteria and were included in this study. In the non-cardiac surgery population, the SAPS II model was customized on 9227 admissions from 13 ICUs between 1999 and 2000 to obtain a baseline Dutch SMR of 1.0 for comparison of subsequent years. In individual ICUs, the SAPS II model was customized on 15 863 admissions from 20 ICUs that participated in NICE for a minimum of five years between 2001 and 2002 to obtain a baseline SMR of 1.0 for comparison of subsequent years. From 2001 to 2007 in the non-cardiac surgery population, a significant decrease in SMR was shown from 1.0 tot 0.83 (p=0.023). This decrease was most significant in patients after elective surgery (p=0.01). Seven (25%) individual ICUs showed a significant decrease in their SMR. The VLAD curve for individual ICUs showed a highly variable course. Conclusion Over time, the NICE registry showed a significant improvement in SMR for all patient groups nationwide, and for 7 individual ICUs. Longitudinal analysis for individual ICUs by VLAD curve shows a variable course between ICUs and within an individual ICU over time. This information can be used for quality improvement initiatives both locally and nationwide.

Abstract

As oversedation is still common and significant variability between and within critically ill patients makes empiric dosing difficult, the population pharmacokinetics and pharmacodynamics of propofol upon long-term use are characterized, particularly focused on the varying disease state as determinant of the effect. Twenty-six critically ill patients were evaluated during 0.7–9.5 days (median 1.9 days) using the Ramsay scale and the bispectral index as pharmacodynamic end points. NONMEM V was applied for population pharmacokinetic and pharmacodynamic modeling. Propofol pharmacokinetics was described by a two-compartment model, in which cardiac patients had a 38% lower clearance. Severity of illness, expressed as a Sequential Organ Failure Assessment (SOFA) score, particularly influenced the pharmacodynamics and to a minor degree the pharmacokinetics. Deeper levels of sedation were found with an increasing SOFA score. With severe illness, critically ill patients will need downward titration of propofol. Abstract

Background: Selective digestive tract decontamination (SDD) and selective oropharyngeal decontamination (SOD) are infection-prevention measures used in the treatment of some patients in intensive care, but reported effects on patient outcome are conflicting., Methods: We evaluated the effectiveness of SDD and SOD in a crossover study using cluster randomization in 13 intensive care units (ICUs), all in The Netherlands. Patients with an expected duration of intubation of more than 48 hours or an expected ICU stay of more than 72 hours were eligible. In each ICU, three regimens (SDD, SOD, and standard care) were applied in random order over the course of 6 months. Mortality at day 28 was the primary end point. SDD consisted of 4 days of intravenous cefotaxime and topical application of tobramycin, colistin, and amphotericin B in the oropharynx and stomach. SOD consisted of oropharyngeal application only of the same antibiotics. Monthly point-prevalence studies were performed to analyze antibiotic resistance., Results: A total of 5939 patients were enrolled in the study, with 1990 assigned to standard care, 1904 to SOD, and 2045 to SDD; crude mortality in the groups at day 28 was 27.5%, 26.6%, and 26.9%, respectively. In a random-effects logistic-regression model with age, sex, Acute Physiology and Chronic Health Evaluation (APACHE II) score, intubation status, and medical specialty used as covariates, odds ratios for death at day 28 in the SOD and SDD groups, as compared with the standard-care group, were 0.86 (95% confidence interval [CI], 0.74 to 0.99) and 0.83 (95% CI, 0.72 to 0.97), respectively., Conclusions: In an ICU population in which the mortality rate associated with standard care was 27.5% at day 28, the rate was reduced by an estimated 3.5 percentage points with SDD and by 2.9 percentage points with SOD. Abstract

Objective: To determine survival rates of patients treated for more than 30 days in an intensive care unit (ICU)., Design: Retrospective, descriptive., Setting: Intensive care unit of the St. Antonius Hospital in Nieuwegein, the Netherlands., Methods: All patients who required more than 30 consecutive days ICU treatment between January 1985 and January 1992 were included. With the aid of a computerised data base the medical records of all patients were analysed. If discharged, their family doctor was contacted for information about survival and quality of life. Kaplan-Meier survival curves were calculated., Results: Among a total of 18,126 ICU admissions, 223 patients required more than 30 days ICU treatment; 25% died in the ICU; 14% died after discharge from the ICU, but still in the hospital; 31% of the patients were discharged to another hospital or nursing home. Of all patients 50% eventually reached home. Two months after ICU discharge 75% were alive, after 1 year 50%. Mean survival time was 36 months (SD: 3). Patients under 60 years of age and those who were discharged directly home had the best prognosis. 30% of the protracted IC patients could ultimately function independently at home., Conclusions: Patients who needed more than 30 days ICU treatment had a high ICU mortality; 2 months after discharge 75% were alive.
