11. Multimodal imaging with nanoGd reveals spatiotemporal features of neuroinflammation after experimental stroke

Abstract

The purpose of this study is to propose and validate a preclinical in vivo magnetic resonance imaging (MRI) tool to monitor neuroinflammation following ischemic stroke, based on injection of a novel multimodal nanoprobe, NanoGd, specifically designed for internalization by phagocytic cells. First, it is verified that NanoGd is efficiently internalized by microglia in vitro. In vivo MRI coupled with intravenous injection of NanoGd in a permanent middle cerebral artery occlusion mouse model results in hypointense signals in the ischemic lesion. In these mice, longitudinal two-photon intravital microscopy shows NanoGd internalization by activated CX3CR1-GFP/+ cells. Ex vivo analysis, including phase contrast imaging with synchrotron X-ray, histochemistry, and transmission electron microscopy corroborate NanoGd accumulation within the ischemic lesion and uptake by immune phagocytic cells. Taken together, these results confirm the potential of NanoGd-enhanced MRI as an imaging biomarker of neuroinflammation at the subacute stage of ischemic stroke. As far as it is known, this work is the first to decipher the working mechanism of MR signals induced by a nanoparticle passively targeted at phagocytic cells by performing intravital microscopy back-to-back with MRI. Furthermore, using a gadolinium-based rather than an iron-based contrast agent raises future perspectives for the development of molecular imaging with emerging computed tomography technologies.

Abstract

High variability in infarct size is common in experimental stroke models and affects statistical power and validity of neuroprotection trials. The aim of this study was to explore cerebral collateral flow as a stratification factor for the prediction of ischemic outcome. Transient intraluminal occlusion of the middle cerebral artery was induced for 90 min in 18 Wistar rats. Cerebral collateral flow was assessed intra-procedurally using multi-site laser Doppler flowmetry monitoring in both the lateral middle cerebral artery territory and the borderzone territory between middle cerebral artery and anterior cerebral artery. Multi-modal magnetic resonance imaging was used to assess acute ischemic lesion (diffusion-weighted imaging, DWI), acute perfusion deficit (time-to-peak, TTP), and final ischemic lesion at 24 h. Infarct volumes and typology at 24 h (large hemispheric versus basal ganglia infarcts) were predicted by both intra-ischemic collateral perfusion and acute DWI lesion volume. Collateral flow assessed by multi-site laser Doppler flowmetry correlated with the corresponding acute perfusion deficit using TTP maps. Multi-site laser Doppler flowmetry monitoring was able to predict ischemic outcome and perfusion deficit in good agreement with acute MRI. Our results support the additional value of cerebral collateral flow monitoring for outcome prediction in experimental ischemic stroke, especially when acute MRI facilities are not available.

Abstract

International audience; Introduction The grey mouse lemur is an interesting alternative to transgenic models for the study of normal and pathological ageing [1]: aged animals (>5 years old) spontaneously and progressively develop sensori-motor deficits, alterations of biological rhythms, of thermal regulation, of metabolism, as well as cognitive decline and cerebral atrophy [2]. We here report for the first time aged-related vascular abnormalities, as detected by ultrasound imaging (US) performed at different locations. Methods Twelve females of 1 to 8 years old were studied by ultrasound imaging (Vevo 770, Visual Sonic, with a 20-60Hz mechanical probe) in search for morphological and functional abnormalities of arterial walls at the level of the aortic arch and the common carotids. Total thickness (TT), intima-media thickness (IMT) and extra-media thickness (EMT, [3]) were measured by a blinded, experienced investigator. Pulsatility was expressed as the percentage of variation between the diastolic and systolic diameters. Nine animals underwent blood sampling for LDL subfraction analysis using the Lipoprint system (Quantimetrix). Heart, aorta and carotids of three animals were kept in formol and processed for histology. Results Focal thickenings of carotid walls were observed on B-mode images in 5 out of 6 old animals (Fig. 1). Systematic proximal-wall measurements on M-mode images showed significantly higher carotid TT, IMT and EMT in aged vs young animals, along with decreased pulsatility (Fig. 2). LDL analysis and histology are in progress. Conclusions Using different US indexes of vessel wall functionality [3], we report in vivo evidence of atherogenesis in aged mouse lemurs. Therefore, this species holds promise as a new primate model of atherosclerosis.

Abstract

International audience; Purpose: MRI coupled with the injection of ultrasmall particles of iron oxide (US-PIO) has been successfully applied for pre-clinical and clinical studies of cerebral inflammation following stroke. Current limitations of this approach are represented by the difficulty of interpreting MR signal changes in terms of exact USPIO localisation. To overcome this limitation, we introduce Synchrotron Radiation phase micro-computerized tomography (SR-PCT) as a new method to visualise USPIO distribution into the whole brains of mice with cerebral ischaemia.

Abstract

International audience; The broad aim underlying the present research was to investigate the distribution and homing of bone marrow-derived macrophages in a rodent model of transient middle cerebral artery occlusion using MRI and ultrasmall superparamagnetic iron oxide (USPIO) to magnetically label bone marrow-derived macrophages. The specific aim was to assess the intra-carotid infusion route for bone marrow-derived macrophage delivery at reperfusion. Fifteen Sprague-Dawley rats sustained 1 h of middle cerebral artery occlusion. USPIO-labeled bone marrow-derived macrophages were slowly injected for 5 min immediately after reperfusion in ischemic animals (n=7), 1 h after the end of surgery in sham animals (n=5) and very shortly after anesthesia in healthy animals (n=3). Multiparametric MRI was performed at day 0, just after cell administration, and repeated at day 1. Immunohistological analysis included Prussian blue for iron detection and rat endothelial cell antigen-1 for endothelium visualization. Intra-carotid cell delivery brought a large number of cells to the ipsilateral hemisphere of the brain, as seen on both MRI and immunohistology. However, it was associated with high mortality (50%). The study of sham animals demonstrated that intra-carotid cell delivery could induce ischemic lesions and may thus favor additional brain damage. The present study highlights severe drawbacks to the intra-carotid delivery of macrophages at the time of reperfusion in this rodent model of transient cerebral ischemia. Multiparametric MRI appears to be a method of choice to monitor longitudinally the effects of cell infusion, allowing the assessment of both cell fate with the help of magnetic labeling and of potential tissue damage.

Abstract

Effective collateral blood flow seem to be an important factor associated with a small infarct volume and a good clinical outcome. We aimed to assess leptomeningeal collateral blood flow on source perfusion-weighted images in patients with acute stroke.29 patients with proximal middle cerebral artery occlusion (MCA alone, n=17; MCA + internal carotid artery [ICA] occlusion, n=12) were evaluated with MRI at baseline before thrombolytic therapy, and at day 60. Clinical evaluation was performed at days 0 and 60 with the National Institutes of Health Stroke Scale (NIHSS) score, and at day 60 with the modified Rankin score. We assessed (on source images of the dynamic contrast-enhanced T2*-weighted perfusion [PWI] sequence) the presence of a hypointensity consistent with delayed contrast arrival within the global perfusion deficit (delayed perfusion sign). We analyzed the extent of the area demonstrating such delayed perfusion (DP area) on source images of the PWI sequence, and compared it with the global perfusion (GP) abnormality shown by time-to-peak maps. We calculated the Spearman rank correlation coefficient between the DP/GP ratio and: 1. age; 2. clinical scores; 3. site of occlusion [MCA alone versus ICA+MCA occlusion]; 4. DWI lesion size at day 0, and T2WI lesion size at day 60; 5. PWI-derived parameters (time-to-peak [TTP], relative cerebral blood volume [rCBV], relative cerebral blood flow [rCBF], and peak height). All tests were bilateral and a p value0.05 was considered as significant.Delayed perfusion areas of various size were found within the global perfusion deficit in all patients. High DP/GP ratio values were significantly correlated with: 1. better clinical scores at day 0 and day 60 (all p=0.04); 2. smaller lesions at day 0 DWI and at day 60 T2WI (all p=0.004); 3. ICA patency (r=0.49, p=0.01); 4. lower TTP delays, and higher values of rCBV, rCBF, and peak height.These preliminary data suggest that a delayed contrast filling observed on native perfusion-weighted images may be a marker of leptomeningeal collateral blood flow, and may lead to better clinical and morphological outcomes in acute ischemic stroke.

Abstract

Objectives: This study sought to evaluate whether the therapeutic effects of an anti-inflammatory drug such as minocycline could be monitored by serial ultrasmall superparamagnetic particles of iron oxide (USPIO)-enhanced MRI in experimental stroke.Methods: Mice received a three-dose minocycline treatment (n = 12) or vehicle (n = 12) after permanent middle cerebral artery occlusion. USPIOs were administered 5 h post-surgery. MRI was performed before, 24 h and 48 h post-USPIO administration. MRI endpoints were the extent of signal abnormalities on R2 maps (=1/T2) and quantitative R2 changes over time (∆R2). Post-mortem brains were prepared either for immunohistology (n = 16) or for iron dosage (n = 8).Results: As expected, treatment with minocycline significantly reduced infarct size, blood-brain barrier permeability and F4/80 immunostaining for microglia/macrophages. Areas of R2 maps > 35 ms(-1) also appeared significantly decreased in minocycline-treated mice (ANOVA for repeated measures, P = 0.017). There was a fair correlation between these areas and the amount of iron in the brain (R(2) = 0.69, P = 0.010), but no significant difference in ∆R2 was found between the two groups.Conclusions: This study showed that the extent of signal abnormalities on R2 maps can be used as a surrogate marker to detect minocycline effects in a murine experimental model of stroke. Abstract

International audience; Introduction In this paper, we introduce a fluorescent magnetic hybrid nanoprobe (5011), based on gadolinium fluoride nanoparticles functionalized with luminescent conjugated polythiophenes moieties (Fig 1). The aim of this study was to investigate its potential for molecular imaging in experimental stroke. Specifically, we aimed to test the hypothesis that 5011 may cross the disrupted blood-brain barrier (BBB) in the ischemic lesion. Methods Transient middle cerebral artery occlusion (tMCAO) was performed in 7 rats using the intraluminal model. In this model, mild BBB disruption was consistently shown at 3 days post-surgery (1). Therefore, dynamic MRI of the brain was performed 3 days post-tMCAO, before, during and after bolus injection of 5011 (N=6) or Gd-DOTA (N=1) both at 150 µmol Gd/kg (7T, TE/TR: 10.5/800-ms, flip angle 30°, 40 repetitions, one-hour acquisition). We used a model of focal BBB opening by heat injury (N=6) to ascertain that the MR sequence used was sensitive enough to detect contrast enhancement due to 5011 leakage through a severely disrupted BBB. Brains and livers of tMCAO rats were prepared either for biphotonic microscopy (N=3) or inductively coupled plasma-mass spectroscopy (ICP-MS, N=3). Results As expected in tMCAO rats, Gd-DOTA injection resulted in a significant enhancement of the ischemic lesion (22% compared to pre-contrast signal). In contrast, there was no signal enhancement difference between the ischemic lesion (-3±1%) and the contralateral region (-2±1%) in rats injected with 5011 (Fig 2A). Biphotonic microscopy showed sparse agglomerates of 5011 in the parenchyma of the ischemic lesion (Fig 2, insert). In the model of focal BBB opening, 5011 injection resulted in a significant enhancement of the lesion (13±3%) (Fig 2B). ICP-MS analyses are in progress. Conclusions Transport of 5011 across the disrupted BBB could be detected in tMCAO rats using a sensitive two-photon microscope but its accumulation was too low to be detected with MRI. Based on these results, we aim to functionalize the hybrid nanoprobe to target vascular adhesion molecules that are highly expressed after experimental stroke, rather than intraparenchymal (extravascular) targets.
