The exploratory study assessed trends in the abundance of CTX-M-type extended spectrum beta-lactamase (ESBL) and vancomycin-resistance genes vanA and vanB in the stool samples of German soldiers and police officers returning from predominantly tropical deployments next to the common diarrheagenic Escherichia (E.) coli pathovars enteropathogenic Escherichia coli (EPEC), enterotoxigenic E. coli (ETEC) and enteroaggregative E. coli (EAEC)) as well as rarely imported Vibrio spp. between 2006 and 2024. Surveillance was performed applying real-time polymerase chain reaction and results were stratified by World Health Organization region of deployment as well as by deployment period. For the latter, the study interval was divided into three pre-COVID-19-pandemic periods, the COVID-19-pandemic period and the post-COVID-19-pandemic period. Averaged prevalences were used as references. In stool samples of 1817 deployed German soldiers and 117 police officers, averaged prevalences were 47.9% and 24.8% for the ESBL-type beta-lactamase bla
CTX-M , 30.2% and 14.5% for vanB , 9.0% and 17.9% for EPEC, 3.4% and 12.8% for ETEC, 4.0% and 3.4% for EAEC as well as 2.0% and 3.4% for Vibrio spp., respectively. While resistance genes peaked during early deployments, maximum prevalences for enteropathogens were seen later. The assessment suggested time- and region-dependence of the assessed parameters. Comment in Does HIV-1 virulence matter in the ART era? Lewitus E, Rolland M. Med (N Y). 2022 Apr 8;3(4):217-219. doi: 10.1016/j.medj.2022.03.003. PMID: 35590149; International audience; We discovered a highly virulent variant of subtype-B HIV-1 in the Netherlands. One hundred nine individuals with this variant had a 0.54 to 0.74 log 10 increase (i.e., a ~3.5-fold to 5.5-fold increase) in viral load compared with, and exhibited CD4 cell decline twice as fast as, 6604 individuals with other subtype-B strains. Without treatment, advanced HIV—CD4 cell counts below 350 cells per cubic millimeter, with long-term clinical consequences—is expected to be reached, on average, 9 months after diagnosis for individuals in their thirties with this variant. Age, sex, suspected mode of transmission, and place of birth for the aforementioned 109 individuals were typical for HIV-positive people in the Netherlands, which suggests that the increased virulence is attributable to the viral strain. Genetic sequence analysis suggests that this variant arose in the 1990s from de novo mutation, not recombination, with increased transmissibility and an unfamiliar molecular mechanism of virulence.

10. A Case of a Complicated Malaria Tropica in a German Soldier on Deployment in Mali

Der Sanitätsdienst der Bundeswehr sieht heutzutage bei weltweiten Einsatzszenarien aufgrund wirksamer Prävention, aber auch beeinflusst durch die Art der aktuellen Einsätze, im Vergleich zu Partnernationen wenige Malariafälle innerhalb der Streitkräfte. Dabei muss trotzdem die Malaria zwingend frühzeitig als Differentialdiagnose in Betracht gezogen werden, um die Identifizierung der Krankheit nicht zu verzögern und schwere Verläufe zu verhindern. Der folgende Fall einer komplizierten Malaria tropica mit verzögerter Diagnosestellung eines deutschen Soldaten in Mali dient beispielhaft für die Komplexität dieser Problematik. Dieser Fall dient einer intellektuellen Auseinandersetzung mit der Komplexität der Malaria im Rahmen eines militärischen Einsatzes und der Notwendigkeit einer unmittelbaren Diagnose und Therapie. Als Konsequenz lässt sich die individuelle Verbesserung der tropenmedizinischen Qualifikation im Sanitätsdienst der Bundeswehr ableiten. Genauso scheint eine vorbereitende Weichenstellung hinsichtlich personellem und materiellem Ausbau des bereits teilweise vorhandenen Kompetenznetzwerks für aktuelle und zukünftige Einsatz- und Bündnisszenarien essenziell., Compared to partner nations today, the Medical Service of the Bundeswehr faces only a few cases of Malaria within the armed forces in worldwide deployment scenarios. This is mainly a consequence of effective preventive measurements but is also influenced by the nature of the current missions themselves. Nevertheless, considering Malaria as early as possible as a potential differential diagnosis has the highest priority in order to avoid catastrophic outcomes. The following case of a complicated Malaria tropica with delayed diagnosis of a German soldier in Mali serves as an example of the complexity of this problem. It´s not a goal of this case report to criticize the involved personnel, aim is anintellectual evaluation of the disease with the need of timely testing and treating. In conclusion, the individual qualification regarding tropical medicine in the Bundeswehr Medical Service should be improved and conceptual measures like the expansion of a currently only partly existing network of excellence for current and future missions should be taken into consideration.
