High Energy Physics - Experiment - Abstract
We report a measurement of muon-neutrino disappearance in the T2K experiment. The 295-km muon-neutrino beam from Tokai to Kamioka is the first implementation of the off-axis technique in a long-baseline neutrino oscillation experiment. With data corresponding to 1.43 10**20 protons on target, we observe 31 fully-contained single muon-like ring events in Super-Kamiokande, compared with an expectation of 104 +- 14 (syst) events without neutrino oscillations. The best-fit point for two-flavor nu_mu -> nu_tau oscillations is sin**2(2 theta_23) = 0.98 and |\Delta m**2_32| = 2.65 10**-3 eV**2. The boundary of the 90 % confidence region includes the points (sin**2(2 theta_23),|\Delta m**2_32|) = (1.0, 3.1 10**-3 eV**2), (0.84, 2.65 10**-3 eV**2) and (1.0, 2.2 10**-3 eV**2)., Comment: 7 pages, 4 figures

Physics - Instrumentation and Detectors - Abstract
Precise measurement of neutrino beam direction and intensity was achieved based on a new concept with modularized neutrino detectors. INGRID (Interactive Neutrino GRID) is an on-axis near detector for the T2K long baseline neutrino oscillation experiment. INGRID consists of 16 identical modules arranged in horizontal and vertical arrays around the beam center. The module has a sandwich structure of iron target plates and scintillator trackers. INGRID directly monitors the muon neutrino beam profile center and intensity using the number of observed neutrino events in each module. The neutrino beam direction is measured with accuracy better than 0.4 mrad from the measured profile center. The normalized event rate is measured with 4% precision., Comment: 32 pages, 27 figures, submitted to Nucl. Instr. and Meth. A

The T2K experiment is a long-baseline neutrino oscillation experiment. Its main goal is to measure the last unknown lepton sector mixing angle {\theta}_{13} by observing {\nu}_e appearance in a {\nu}_{\mu} beam. It also aims to make a precision measurement of the known oscillation parameters, {\Delta}m^{2}_{23} and sin^{2} 2{\theta}_{23}, via {\nu}_{\mu} disappearance studies. Other goals of the experiment include various neutrino cross section measurements and sterile neutrino searches. The experiment uses an intense proton beam generated by the J-PARC accelerator in Tokai, Japan, and is composed of a neutrino beamline, a near detector complex (ND280), and a far detector (Super-Kamiokande) located 295 km away from J-PARC. This paper provides a comprehensive review of the instrumentation aspect of the T2K experiment and a summary of the vital information for each subsystem., Comment: 33 pages, 32 figures, Submitted and accepted by NIM A. A search for sidereal modulation in the flux of atmospheric muon neutrinos in IceCube was performed. Such a signal could be an indication of Lorentz-violating physics. Neutrino oscillation models, derivable from extensions to the Standard Model, allow for neutrino oscillations that depend on the neutrino's direction of propagation. No such direction-dependent variation was found. A discrete Fourier transform method was used to constrain the Lorentz and CPT-violating coefficients in one of these models. Due to the unique high energy reach of IceCube, it was possible to improve constraints on certain Lorentz-violating oscillations by three orders of magnitude with respect to limits set by other experiments., Comment: 5 pages, 2 figures. Revision updates a reference and adds a comment on the 120 degree zenith restriction

A measurement of the atmospheric muon neutrino energy spectrum from 100 GeV to 400 TeV was performed using a data sample of about 18,000 up-going atmospheric muon neutrino events in IceCube. Boosted decision trees were used for event selection to reject mis-reconstructed atmospheric muons and obtain a sample of up-going muon neutrino events. Background contamination in the final event sample is less than one percent. This is the first measurement of atmospheric neutrinos up to 400 TeV, and is fundamental to understanding the impact of this neutrino background on astrophysical neutrino observations with IceCube. The measured spectrum is consistent with predictions for the atmospheric muon neutrino plus muon antineutrino flux., Comment: 19 pages, 24 figures, added background simulation to distribution plots, clarified a few points in the text

We report on the results of the search for extremely-high energy (EHE) neutrinos with energies above $10^7$ GeV obtained with the partially ($\sim$30%) constructed IceCube in 2007. From the absence of signal events in the sample of 242.1 days of effective livetime, we derive a 90% C.L. model independent differential upper limit based on the number of signal events per energy decade at $E^2 \phi_{\nu_e+\nu_\mu+\nu_\tau}\simeq 1.4 \times 10^{-6}$ GeV cm$^{-2}$ sec$^{-1}$ sr$^{-1}$ for neutrinos in the energy range from $3\times10^7$ to $3\times10^9$ GeV., Comment: 11 pages, 6 figures, 4 tables, accepted for publication in Physical Review D

Astrophysics - Instrumentation and Methods for Astrophysics - Abstract
Over 5,000 PMTs are being deployed at the South Pole to compose the IceCube neutrino observatory. Many are placed deep in the ice to detect Cherenkov light emitted by the products of high-energy neutrino interactions, and others are frozen into tanks on the surface to detect particles from atmospheric cosmic ray showers. IceCube is using the 10-inch diameter R7081-02 made by Hamamatsu Photonics. This paper describes the laboratory characterization and calibration of these PMTs before deployment. PMTs were illuminated with pulses ranging from single photons to saturation level. Parameterizations are given for the single photoelectron charge spectrum and the saturation behavior. Time resolution, late pulses and afterpulses are characterized. Because the PMTs are relatively large, the cathode sensitivity uniformity was measured. The absolute photon detection efficiency was calibrated using Rayleigh-scattered photons from a nitrogen laser. Measured characteristics are discussed in the context of their relevance to IceCube event reconstruction and simulation efforts., Comment: 40 pages, 12 figures

Astrophysics - Cosmology and Extragalactic Astrophysics - Abstract
Neutrino-telescopes like Super-Kamiokande and IceCube have started to explore the neutrino fluxes from WIMP annihilations in the Sun. The non-observation of a signal can put constraints on the WIMP properties. We here focus on the neutrino signal from WIMP annihilation in the Sun and show that under reasonable assumptions, the non-observation of a signal from IceCube puts a much tighter constraint on the spin-dependent WIMP-proton scattering cross-section than current direct detection experiments like COUPP and KIMS. For the spin-independent scattering cross-section, the limits from IceCube and current direct detection experiments like XENON10 and CDMS place similar constraints. We here go through the assumptions being made and the uncertainties that arise in converting from limits on the muon flux from the Sun to limits on the WIMP-proton cross-section, and present our results as easy to use conversion factors., Comment: 10 pages, 6 figures. Trends in annual HF incidence rate and prevalence, risk of all-cause and cardiovascular disease (CVD)-related 1-year mortality and use of diagnostic tests 6 months before and after first HF diagnosis (cohort 2) were assessed. Results: Baseline demographic and clinical characteristics were similar for cohort 1 (N=174,537) and 2 (N=8,702), with mean ages of 77.4 and 76.6 years, respectively; almost 30% of patients were aged ≥85 years. From 2010 to 2014, age-adjusted annual incidence rate of HF/1,000 inhabitants decreased (from 3.20 to 2.91, cohort 1; from 4.34 to 3.33, cohort 2), while age-adjusted prevalence increased (from 1.61% to 1.72% and from 2.15% to 2.18%, respectively). Age-adjusted 1-year all-cause and CVD-related mortality was higher in men than in women among patients in cohort 1 (all-cause mortality hazard ratio [HR] men vs women 1.07 [95% CI 1.06–1.09] and CVD-related mortality subdistribution HR for men vs women 1.04 [95% CI 1.02–1.07], respectively). While 83.5% of patients underwent N-terminal pro-B-type natriuretic peptide testing, only 36.4% of patients had an echocardiogram at the time of diagnosis, although this increased overtime. In the national prevalent HF population (patients with a diagnosis in 1997–2004 who survived into the analysis period; N=273,999), death from ischemic heart disease and myocardial infarction declined between 2005 and 2013, while death from HF and atrial fibrillation/flutter increased (P

Background In a Phase I study treatment with the serum amyloid P component (SAP) depleter miridesap followed by monoclonal antibody to SAP (dezamizumab) showed removal of amyloid from liver, spleen and kidney in patients with systemic amyloidosis. We report results from a Phase 2 study and concurrent immuno-positron emission tomography (PET) study assessing efficacy, pharmacodynamics, pharmacokinetics, safety and cardiac uptake (of dezamizumab) following the same intervention in patients with cardiac amyloidosis. Methods Both were uncontrolled open-label studies. After SAP depletion with miridesap, patients received ≤ 6 monthly doses of dezamizumab in the Phase 2 trial (n = 7), ≤ 2 doses of non-radiolabelled dezamizumab plus [89Zr]Zr-dezamizumab (total mass dose of 80 mg at session 1 and 500 mg at session 2) in the immuno-PET study (n = 2). Primary endpoints of the Phase 2 study were changed from baseline to follow-up (at 8 weeks) in left ventricular mass (LVM) by cardiac magnetic resonance imaging and safety. Primary endpoint of the immuno-PET study was [89Zr]Zr-dezamizumab cardiac uptake assessed via PET. Results Dezamizumab produced no appreciable or consistent reduction in LVM nor improvement in cardiac function in the Phase 2 study. In the immuno-PET study, measurable cardiac uptake of [89Zr]Zr-dezamizumab, although seen in both patients, was moderate to low. Uptake was notably lower in the patient with higher LVM. Treatment-associated rash with cutaneous small-vessel vasculitis was observed in both studies. Abdominal large-vessel vasculitis after initial dezamizumab dosing (300 mg) occurred in the first patient with immunoglobulin light chain amyloidosis enrolled in the Phase 2 study. Symptom resolution was nearly complete within 24 h of intravenous methylprednisolone and dezamizumab discontinuation; abdominal computed tomography imaging showed vasculitis resolution by 8 weeks. Conclusions Unlike previous observations of visceral amyloid reduction, there was no appreciable evidence of amyloid removal in patients with cardiac amyloidosis in this Phase 2 trial, potentially related to limited cardiac uptake of dezamizumab as demonstrated in the immuno-PET study. The benefit-risk assessment for dezamizumab in cardiac amyloidosis was considered unfavourable after the incidence of large-vessel vasculitis and development for this indication was terminated. Trial registration NCT03044353 (2 February 2017) and NCT03417830 (25 January 2018).

- Abstract
Hexa-X will pave the way to the next generation of wireless networks (Hexa) by explorative research (X). The Hexa-X vision is to connect human, physical, and digital worlds with a fabric of sixth generation (6G) key enablers. The vision is driven by the ambition to contribute to objectives of growth, global sustainability, trustworthiness, and digital inclusion. Key 6G value indicators and use cases are defined against the background of technology push, society and industry pull as well as objectives of technology sovereignty. Key areas of research have been formulated accordingly to include connecting intelligence, network of networks, sustainability, global service coverage, extreme experience, and trustworthiness. Critical technology enablers for 6G are developed in the project including, sub-THz transceiver technologies, accurate stand-alone positioning and radio-based imaging, improved radio performance, artificial intelligence (AI) / machine learning (ML) inspired radio access network (RAN) technologies, future network architectures and special purpose solutions including future ultra-reliable low-latency communication (URLLC) schemes. Besides technology enablers, early trials will be carried out to help assess viability and performance aspects of the key technology enablers. The 6G Hexa-X project is integral part of European and global research effort to help define the best possible next generation of networks.

- Abstract
While 5G is being deployed and the economy and society begin to reap the associated benefits, the research and development community starts to focus on the next, 6 th Generation (6G) of wireless communications. Although there are papers available in the literature on visions, requirements and technical enablers for 6G from various academic perspectives, there is a lack of joint industry and academic work towards 6G. In this paper a consolidated view on vision, values, use cases and key enabling technologies from leading industry stakeholders and academia is presented. The authors represent the mobile communications ecosystem with competences spanning hardware, link layer and networking aspects, as well as standardization and regulation. The second contribution of the paper is revisiting and analyzing the key concurrent initiatives on 6G. A third contribution of the paper is the identification and justification of six key 6G research challenges: (i) “connecting”, in the sense of empowering, exploiting and governing, intelligence; (ii) realizing a network of networks, i.e., leveraging on existing networks and investments, while reinventing roles and protocols where needed; (iii) delivering extreme experiences, when/where needed; (iv) (environmental, economic, social) sustainability to address the major challenges of current societies; (v) trustworthiness as an ingrained fundamental design principle; (vi) supporting cost-effective global service coverage. A fourth contribution is a comprehensive specification of a concrete first-set of industry and academia jointly defined use cases for 6G, e.g., massive twinning, cooperative robots, immersive telepresence, and others. Finally, the anticipated evolutions in the radio, network and management/orchestration domains are discussed.

- Abstract
Levosimendan was first approved for clinical use in 2000, when authorization was granted by Swedish regulatory authorities for the hemodynamic stabilization of patients with acutely decompensated chronic heart failure (HF). In the ensuing 20 years, this distinctive inodilator, which enhances cardiac contractility through calcium sensitization and promotes vasodilatation through the opening of adenosine triphosphate-dependent potassium channels on vascular smooth muscle cells, has been approved in more than 60 jurisdictions, including most of the countries of the European Union and Latin America. Areas of clinical application have expanded considerably and now include cardiogenic shock, takotsubo cardiomyopathy, advanced HF, right ventricular failure, pulmonary hypertension, cardiac surgery, critical care, and emergency medicine. Levosimendan is currently in active clinical evaluation in the United States. Levosimendan in IV formulation is being used as a research tool in the exploration of a wide range of cardiac and noncardiac disease states. A levosimendan oral form is at present under evaluation in the management of amyotrophic lateral sclerosis. To mark the 20 years since the advent of levosimendan in clinical use, 51 experts from 23 European countries (Austria, Belgium, Croatia, Cyprus, Czech Republic, Estonia, Finland, France, Germany, Greece, Hungary, Italy, the Netherlands, Norway, Poland, Portugal, Russia, Slovenia, Spain, Sweden, Switzerland, the United Kingdom, and Ukraine) contributed to this essay, which evaluates one of the relatively few drugs to have been successfully introduced into the acute HF arena in recent times and charts a possible development trajectory for the next 20 years. © 2020 Lippincott Williams and Wilkins. - Abstract
Amphibians are among the most threatened vertebrate taxa due to anthropogenic habitat change and emerging pathogens. The fungus Batrachochytrium dendrobatidis (Bd) may cause trade‐offs between the immune responses and other important functions, such as mobility. The pool frog (Pelophylax lessonae) is red‐listed in Sweden, and an earlier study conducted in our study area did not detect the fungus. In this study, 67 Swedish pool frogs were tested for Bd. Of these, 28 adults were provided with external radio transmitters and tracked during the summer and autumn to compare movement among infected and non‐infected individuals. Additionally, we tested some individuals for Bd twice during the summer to study intra‐seasonal variation in individual infections. Our results suggest that Bd has recently invaded these pool frog populations. During autumn, movement distances of infected pool frogs were shorter compared to uninfected individuals, and summer movements within ponds were reduced by increasing Bd load. We also found that the frogs can clear (or reduce) Bd infection during their active season. The results from this study increase understanding of the implications of Bd infections in an anuran fringe population, as well as in amphibian metapopulations, and can further guide amphibian conservation planning and management.

- Abstract
Aim: The aim of this study was to assess PCWP with passive leg-lifting (PLL) and exercise, in two groups of patients presenting with normal left ventricular ejection fraction (LVEF); one group with elevated NT-proBNP (eBNP), and one with normal NT-proBNP (nBNP) plasma concentration. Methods and results: Fifty-one patients with eBNP (NT-proBNP ≥ 125 ng/l) and LVEF > 50%, were investigated and compared with 34 patients with nBNP (NT-proBNP < 125 ng/l) and LVEF > 50%. Both groups underwent right heart catheterization (RHC) at rest, PLL and exercise. From RHC, mean pulmonary arterial pressure (mPAP), cardiac output (CO), and PCWP were measured. All nBNP patients had PCWP < 15 mmHg at rest, and a PCWP of < 25 mmHg with PLL and during exercise. Patients with eBNP had higher (p < 0.01) resting mPAP, PCWP, and mPAP/CO. These values increased with exercise; however, CO increased less in comparison with nBNP patients (p = 0.001). 20% of patients with eBNP had a PCWP > 15 mmHg at rest, this percentage increased to 47% with PLL and 41% had a PCWP > 25 mmHg during exercise. Of those with PCWP > 25 mmHg during exercise, 91% had a PCWP > 15 mmHg with PLL. A PCWP > 15 mmHg on PLL had a 91% sensitivity and 92% specificity in predicting exercise-induced PCWP of > 25 mmHg. Conclusion: In patients presenting with eBNP, PLL can predict which patients will develop elevated PCWP with exercise. These findings highlight the role of stress assessment., Originally included in thesis in manuscript form with title: "Passive leg lifting in heart failure patients predicts exercise induced rise in left ventricular filling pressures"

Objective Pre‐eclampsia (PE) is associated with an increased risk of cardiovascular disease later in life. In cases with PE there is a substantial increase in levels of the antiangiogenic factor soluble fms‐like tyrosine kinase‐1 (sFlt‐1) and decreased levels of the proangiogenic factor placental growth factor (PlGF). Elevated levels of sFlt‐1 are also found in individuals with cardiovascular disease. The aims of this study were to assess levels of sFlt‐1, PlGF and the sFlt‐1/PlGF ratio and their correlation with signs of arterial aging by measuring the common carotid artery (CCA) intima and media thicknesses and their ratio (I/M ratio) in women with and without PE. Methods Serum sFlt‐1 and PlGF levels were measured using commercially available enzyme‐linked immunosorbent assay kits, and CCA intima and media thicknesses were estimated using high‐frequency (22‐MHz) ultrasonography in 55 women at PE diagnosis and in 64 women with normal pregnancy at a similar gestational age, with reassessment at 1 year postpartum. Results During pregnancy, higher levels of sFlt‐1, lower levels of PlGF, a thicker intima, a thinner media and a higher I/M ratio of the CCA were found in women with PE vs controls (all P

- Abstract
BACKGROUND Serelaxin is a recombinant form of human relaxin-2, a vasodilator hormone that contributes to cardiovascular and renal adaptations during pregnancy. Previous studies have suggested that treatment with serelaxin may result in relief of symptoms and in better outcomes in patients with acute heart failure. METHODS In this multicenter, double-blind, placebo-controlled, event-driven trial, we enrolled patients who were hospitalized for acute heart failure and had dyspnea, vascular congestion on chest radiography, increased plasma concentrations of natriuretic peptides, mild-to-moderate renal insufficiency, and a systolic blood pressure of at least 125 mm Hg, and we randomly assigned them within 16 hours after presentation to receive either a 48-hour intravenous infusion of serelaxin (30 μg per kilogram of body weight per day) or placebo, in addition to standard care. The two primary end points were death from cardiovascular causes at 180 days and worsening heart failure at 5 days. RESULTS A total of 6545 patients were included in the intention-to-treat analysis. At day 180, death from cardiovascular causes had occurred in 285 of the 3274 patients (8.7%) in the serelaxin group and in 290 of the 3271 patients (8.9%) in the placebo group (hazard ratio, 0.98; 95% confidence interval [CI], 0.83 to 1.15; P=0.77). At day 5, worsening heart failure had occurred in 227 patients (6.9%) in the serelaxin group and in 252 (7.7%) in the placebo group (hazard ratio, 0.89; 95% CI, 0.75 to 1.07; P=0.19). There were no significant differences between the groups in the incidence of death from any cause at 180 days, the incidence of death from cardiovascular causes or rehospitalization for heart failure or renal failure at 180 days, or the length of the index hospital stay. The incidence of adverse events was similar in the two groups. CONCLUSIONS In this trial involving patients who were hospitalized for acute heart failure, an infusion of serelaxin did not result in a lower incidence of death from cardiovascular causes at 180 days or worsening heart failure at 5 days than placebo. Patients in the latest stages of heart failure are severely compromised, with poor quality of life and frequent hospitalizations. Heart transplantation and left ventricular assist device implantation are viable options only for a minority, and intermittent or continuous infusions of positive inotropes may be needed as a bridge therapy or as a symptomatic approach. In these settings, levosimendan has potential advantages over conventional inotropes (catecholamines and phosphodiesterase inhibitors), such as sustained effects after initial infusion, synergy with beta-blockers, and no increase in oxygen consumption. Levosimendan has been suggested as a treatment that reduces re-hospitalization and improves quality of life. However, previous clinical studies of intermittent infusions of levosimendan were not powered to show statistical significance on key outcome parameters. A panel of 45 expert clinicians from 12 European countries met in Rome on November 24-25, 2016 to review the literature and envision an appropriately designed clinical trial addressing these needs. In the earlier FIGHT trial (daily subcutaneous injection of liraglutide in heart failure patients with reduced ejection fraction) a composite Global Rank Score was used as primary end-point where death, re-hospitalization, and change in N-terminal-prohormone-brain natriuretic peptide level were considered in a hierarchical order. In the present study, we tested the same end-point post hoc in the PERSIST and LEVOREP trials on oral and repeated i.v. levosimendan, respectively, and demonstrated superiority of levosimendan treatment vs placebo. The use of the same composite end-point in a properly powered study on repetitive levosimendan in advanced heart failure is strongly advocated.
