Abstract

Our aim was to study the feasibility of an intensified intravenous CMF (cyclophosphamide, methotrexate and 5-fluorouracil) schedule with the aim to escalate dose intensity (DI). Twenty-three premenopausal breast cancer patients received 6 cycles of adjuvant CMF intravenously on days 1 and 8 every 3 weeks and granulocyte colony-stimulating factor days 9–18. Endpoints were DI and toxicity. Twenty-one out of 23 patients (91%) received the projected total dose and reached ≥ 85% of the projected DI. Compared to ‘classical’ CMF, all patients reached ≥ 111% DI. Nine patients received the planned schedule without delay. Thirteen patients (57%) were treated for infection and four patients (17%) were hospitalized for febrile neutropenia. Twelve patients received red blood cell transfusions (52%). Radiation therapy (n= 6) had no adverse impact on dose intensity or haematological toxicity. This dose-intensified CMF schedule was accompanied by enhanced haematological toxicity with clinical sequelae, namely fever, intravenous antibiotics and red blood cell transfusions, but allows a high dose intensity in a majority of patients. © 2000 Cancer Research Campaign

Abstract

Eight patients with non-seminomatous testicular cancel relapsing after primary chemotherapy were treated with salvage chemotherapy consisting of high-dose methotrexate (12 g/m(2)), vincristine (1.2 mg/m(2) weekly for four weeks, followed after an interval of four weeks by 3 times 100 mg/m(2) cisplatin (50 mg/m(2) on day 1 and 2) every 10 days. This regimen resulted in 2 partial (PR) and 2 complete responses (CR). The two patients achieving CR remain disease-free for 43+ and 53+ months. Toxicity was mainly methotrexate-related and could be ameliorated to a large extent by leucovorin rescue. This small study shows that methotrexate, vincristine, followed by cisplatin is effective in the treatment of relapsed non-seminomatous testicular cancer at the cost of manageable toxicity.

Abstract

First-line intravenous chemotherapy (CT) following debulking surgery is associated with prolonged survival, in particular in patients who achieve a pathological complete remission (pCR) at second-look surgery but in whom a high rate of relapses still occurs. Between 1988 and 1997, 153 patients in pCR following platinum-based intravenous CT were randomized between four courses of intraperitoneal cisplatin (P) (90 mg/m2 every 3 weeks) or observation. Overall survival (OS) was the primary endpoint, while progression-free survival (PFS) was a secondary endpoint. This intent-to-treat analysis includes 16 patients who were not eligible and 17 patients who had protocol violations. The two groups were well balanced in terms of age (median = 55 years), performance status (78% P.S. O), FIGO stage (96% stage III), histology (serous in 66%), grade (2 or 3 in 80%), and residuum before intravenous CT (>1 cm in 40%). Intraperitoneal CT was delivered mainly through intraperitoneal catheters (Port-a-Cath 61% and Tenckhoff 25%). Side effects of intraperitoneal cisplatin included vomiting [> or =grade 2 (82%)], rise in serum creatinine [> or =grade 2 (14%)], abdominal pain [grade 1-2 (38%)], and neurotoxicity [grade 2-3 (15%)]. After a median follow-up of 8 years, 80 patients (52%) have progressed with no difference in the pattern of relapse between the two groups and 75 patients (49%) have died; the respective hazard ratios for PFS and OS with 95% CI are 0.89 (0.59-1.33) and 0.82 (0.52-1.29). These results are suggestive of a treatment benefit but do not support a change in clinical practice. Abstract

Breast cancer is the most common cause of cancer death among women worldwide. Therapeutic decisions in breast cancer are based on stage and specific tumour characteristics. In addition to conventional imaging and histopathological evaluation, potentially non-invasive molecular imaging of tumour metabolism (by means of the [18F] fluorodeoxyglucose (FDG)-positron emission tomography, FDG-PET) and known predictive biosensitivity, markers (oestrogen-and progesterone receptor, HER2) can be used for disease evaluation in the future. Molecular imaging provides a functional, dynamic aspect that might be useful for diagnostic purposes, treatment selection, and for monitoring treatment response at a molecular level. This is of particular interest in view of the dynamics of tumour metabolism and biomarker expression during progression and treatment of breast cancer. For staging of recurrent and metastatic breast cancer, FDG-PET imaging of tumour metabolism can be of value in selected cases, with its high sensitivity, but varying specificity. For response monitoring and prognostic evaluation, FDG-PET may be useful, but future studies are required to confirm this. Molecular imaging of tumour HER2 and the oestrogen receptor was shown to be feasible in metastatic breast cancer. Imaging of these biomarkers may allow a more tumour specific detection than with FDG-PET or conventional imaging, but its use in breast cancer staging-or treatment requires further evaluation. Future options for molecular imaging in breast cancer include monitoring of other significant biomarkers (such as the progesterone receptor), or direct treatment evaluation by radiolabelling targeted therapeutic drugs (such as the antivascular endothelial growth factor antibody bevacizumab). To establish molecular imaging in practical (breast) cancer care, more extensive research is needed, but clearly the possibilities are extensive. [ABSTRACT FROM AUTHOR]
