Item does not contain fulltext, BACKGROUND: Balancing the risks of recurrent ischaemic stroke and intracranial haemorrhage is important for patients treated with antithrombotic therapy after ischaemic stroke or transient ischaemic attack. However, existing predictive models offer insufficient performance, particularly for assessing the risk of intracranial haemorrhage. We aimed to develop new risk scores incorporating clinical variables and cerebral microbleeds, an MRI biomarker of intracranial haemorrhage and ischaemic stroke risk. METHODS: We did a pooled analysis of individual-patient data from the Microbleeds International Collaborative Network (MICON), which includes 38 hospital-based prospective cohort studies from 18 countries. All studies recruited participants with previous ischaemic stroke or transient ischaemic attack, acquired baseline MRI allowing quantification of cerebral microbleeds, and followed-up participants for ischaemic stroke and intracranial haemorrhage. Participants not taking antithrombotic drugs were excluded. We developed Cox regression models to predict the 5-year risks of intracranial haemorrhage and ischaemic stroke, selecting candidate predictors on biological relevance and simplifying models using backward elimination. We derived integer risk scores for clinical use. We assessed model performance in internal validation, adjusted for optimism using bootstrapping. The study is registered on PROSPERO, CRD42016036602. FINDINGS: The included studies recruited participants between Aug 28, 2001, and Feb 4, 2018. 15 766 participants had follow-up for intracranial haemorrhage, and 15 784 for ischaemic stroke. Over a median follow-up of 2 years, 184 intracranial haemorrhages and 1048 ischaemic strokes were reported. 29. Geotechnical characterisation of coal tailings down the beach and constant rate of loading consolidation in a slurry consolidometer

Abstract

The conventional disposal of slurried tailings in a tailings storage facility (TSF) leads to the formation of a beach, on which the tailings sort hydraulically, settle, consolidate, and desiccate upon exposure. Basic geotechnical testing using samples from a TSF at an Eastern Australian coal mine (such as specific gravity, Atterberg limits, and particle size distribution), found that there was no particular trend of changes in the tailings properties down the beach. The settled sediment found in a settling test was slurry-like and unsuitable for conventional oedometer testing. Constant rate of loading (CRL) consolidation testing is more representative of the continuous disposal of tailings slurry than conventional step loading. Consolidation testing in a slurry consolidometer was conducted and captured from a slurry-like to soil-like state. A slow rate of loading increased the test time, while a too quick rate of loading built up very high excess pore water pressure (PWP). Abstract

This study brings to light a neglected topic of particular importance--offender gambling issues within the context of re-entry into the community. Fifteen correctional professionals from Nevada (high gambling availability) and Utah (no legalized gambling) participated in semi-structured interviews to provide insights into how gambling may impact the offender re-entry process in their respective locations. While gambling in prisons and jails may be common, offender gambling issues upon re-entry reportedly are associated with substance abuse and the commission of crime. Participants in both Utah and Nevada reported a lack of resources for dealing with offender gambling issues. Implications for offender re-entry are discussed. Abstract

Antiphospholipid syndrome (APS), an acquired autoimmune thrombophilia, is characterised by thrombosis and/or pregnancy morbidity in association with persistent antiphospholipid antibodies. The 16th International Congress on Antiphospholipid Antibodies Task Force on APS Treatment Trends reviewed the current status with regard to existing and novel treatment trends for APS, which is the focus of this Task Force report. The report addresses current treatments and developments since the last report, on the use of direct oral anticoagulants in patients with APS, antiplatelet agents, adjunctive therapies (hydroxychloroquine, statins and vitamin D), targeted treatment including rituximab, belimumab, and anti-TNF agents, complement inhibition and drugs based on peptides of beta-2-glycoprotein I. In addition, the report summarises potential new players, including coenzyme Q10, adenosine receptor agonists and adenosine potentiation. In each case, the report provides recommendations for clinicians, based on the current state of the art, and suggests a clinical research agenda. The initiation and development of appropriate clinical studies requires a focus on devising suitable outcome measures, including a disease activity index, an optimal damage index, and a specific quality of life index. © The Author(s) 2020.

The report addresses current treatments and developments since the last report, on the use of direct oral anticoagulants in patients with APS, antiplatelet agents, adjunctive therapies (hydroxychloroquine, statins and vitamin D), targeted treatment including rituximab, belimumab, and anti-TNF agents, complement inhibition and drugs based on peptides of beta-2-glycoprotein I. In addition, the report summarises potential new players, including coenzyme Q10, adenosine receptor agonists and adenosine potentiation. In each case, the report provides recommendations for clinicians, based on the current state of the art, and suggests a clinical research agenda. The initiation and development of appropriate clinical studies requires a focus on devising suitable outcome measures, including a disease activity index, an optimal damage index, and a specific quality of life index.

Abstract

The ELectron Spectrometer (ELS) from the Analyzer of Space Plasmas and Energetic Atoms (ASPERA-3) flown on the Mars Express spacecraft has an 8% energy resolution, combined with the capability to oversample the martian electron distribution. This makes possible the resolution and identification of electrons generated as a result of the He 304 [Angstrom] ionization of C[O.sub.2] at the martian exobase on the dayside of the planet. Ionospheric photoelectrons were observed during almost every pass into the ionosphere and C[O.sub.2] photoelectron peaks were identified near the terminator. Atmospherically generated C[O.sub.2] photoelectrons are also observed at 10,000 km altitude in the martian tail near the inner magnetospheric boundary. Observations over a wide range of spacecraft orbits showed a consistent presence of photoelectrons at locations along the inner magnetospheric boundary and in the ionosphere, from an altitude of 250 to 10,000 km. Abstract

The Analyzer of Space Plasmas and Energetic Atoms (ASPERA-3) experiment flown on the Mars Express (MEX) spacecraft includes the Electron Spectrometer (ELS) as part of its complement. The ELS instrument measures the differential electron flux spectrum in a 128-level logarithmic energy sweep within a time period of 4 s. The orbital path of MEX traverses the martian sheath, cusps, and tail where ELS recorded periodic electron intensity oscillations. These oscillations comprised periodic variations of up to an order of magnitude (peak to valley) in energy flux, with the largest amplitudes in the tens to hundreds of eV range. The observed oscillations displayed periods ranging from minutes down to the instrument sweep resolution of 4 s. In the cases analyzed here, the frequency of the integrated electron energy flux typically peaked between 0.01 and 0.02 Hz. This frequency range is nearly the same as the typical [O.sup.+] gyrofrequency in the magnetosheath, calculated using magnetometer data from Mars Global Surveyor. Due to the motion of the spacecraft, it is unclear if the wave structures observed were permanent standing waves or rather constituted waves propagating past the spacecraft. Abstract

Because of significant recent budget cuts across the U.S., various correctional services have been targeted for possible elimination (Polson, 2002). Correctional recreation (CR) often appears to be viewed by policy makers, correctional administrators and staff as an offender privilege or perhaps even a luxury, and such programs frequently struggle to maintain their existence (Finn, 1996). In this article, we explore why CR may be viewed as being expendable, the potential substantial contributions CR may be capable of making to psychotherapeutic rehabilitation efforts based on a multidisciplinary literature review, and suggestions for how such potential contributions might be realized. Our conclusion is that the rehabilitative potential of CR needs to be empirically determined, but that it is premature for policy makers and corrections administrators to give CR the death penalty. Abstract

The Magnetospheric Imaging Instrument (MIMI) onboard the Cassini spacecraft observed the saturnian magnetosphere from January 2004 until Saturn orbit insertion (SOI) on 1 July 2004. The MIMI sensors observed frequent energetic particle activity in interplanetary space for several months before SOI. When the imaging sensor was switched to its energetic neutral atom (ENA) operating mode on 20 February 2004, at ~[10.sup.3] times Saturn's radius [R.sub.S] (0.43 astronomical units), a weak but persistent signal was observed from the magnetosphere. About 10 days before SOI, the magnetosphere exhibited a day-night asymmetry that varied with an ~11-hour periodicity. Once Cassini entered the magnetosphere, in situ measurements showed high concentrations of [H.sup.+], [H.sub.2.sup.+], [O.sup.+], O[H.sup.+], and [H.sub.2][O.sup.+] and low concentrations of [N.sup.+]. The radial dependence of ion intensity profiles implies neutral gas densities sufficient to produce high loss rates of trapped ions from the middle and inner magnetosphere. ENA imaging has revealed a radiation belt that resides inward of the D ring and is probably the result of double charge exchange between the main radiation belt and the upper layers of Saturn's exosphere., The magnetosphere of Saturn was discovered by Pioneer 11 in 1979 (1, 2) and was investigated in detail by Voyager 1 in 1980 (3) and Voyager 2 in 1981 (4).
