Abstract

Universal developmental surveillance is considered best practice for early identification of autism. We analysed data from 175 New Zealand Well-Child/Tamariki Ora nurses who attended a 1-day training in developmental surveillance for autism using the social attention and communication surveillance-revised (SACS-R) tool. We used a survey to measure nurses' knowledge of typical development, knowledge of early signs of autism, general autism knowledge, and confidence in identifying and discussing early signs, prior to the workshop, after the workshop, and at follow-up. We measured perceived acceptability of the SACS-R after the workshop and at follow-up. Nurses showed improvements on all measures from pre-workshop to post-workshop and pre-workshop to follow-up. Implementation of the SACS-R across different contexts appears feasible and acceptable.

26. Sexuality Education for Primary School Students with Disability in Fiji

Abstract

Background: People with disability have the same sexual and reproductive health needs and rights as people without disability, yet experience considerable barriers in accessing information and services. The Reproductive and Family Health Association of Fiji, Family Planning Australia, and disability stakeholders implemented a comprehensive sexuality education (CSE) disability inclusion project to increase access for students with disability to age-appropriate sexuality education. Method: A train the trainer model was utilised, whereby staff and disability stakeholders undertook competency-based training on delivering sexuality education to teachers and people with disability, then trained and supported teachers working in special disability schools to provide sexuality education to students. An evaluation explored the acceptability and effectiveness of the project, utilising baseline and endline surveys and interviews. Findings: From 2016 to 2018, the CSE disability project reached teachers and students in nine special disability schools in Fiji. At project completion, teachers reported increased confidence overall in teaching about sexuality and relationships, and students demonstrated increased knowledge regarding puberty and body changes. Interview findings suggested the project contributed to overcoming silence and stigma for people with disability and promoting safety for young people. Conclusion: Age- and developmentally appropriate sexuality education for people with disability is essential for equipping them with necessary knowledge and skills. The CSE disability project utilised a collaborative, capacity building approach to co-design and implement sexuality education in special disability schools in Fiji. With commitment from stakeholders, this could be sustained and expanded to support students with disability in achieving optimal sexual and reproductive health.

Abstract

Background Current estimates of the economic burden of respiratory syncytial virus (RSV) are needed for policymakers to evaluate adult RSV vaccination strategies. Methods A cost-of-illness model was developed to estimate the annual societal burden of RSV in US adults aged ≥60 years. Additional analyses were conducted to estimate the burden of hospitalized RSV in all adults aged 50–59 years and in adults aged 18–49 years with potential RSV risk factors. Results Among US adults aged ≥60 years, the model estimated 4.0 million annual RSV cases (95% uncertainty interval [UI], 2.7–5.6 million) and an annual economic burden of $6.6 billion (95% UI, $3.1–$12.9 billion; direct medical costs, $2.9 billion; indirect costs, $3.7 billion). The 4% of RSV cases that were hospitalized contributed to 94% of direct medical costs. Additional analyses estimated $422 million in annual hospitalization costs among all adults aged 50–59 years. Among adults aged 18–49 years with RSV risk factors, annual per capita burden was highest among people with congestive heart failure at $51 100 per 1000 people. Discussion The economic burden of RSV is substantial among adults aged ≥50 years and among adults aged 18–49 years with RSV risk factors, underscoring the need for preventive interventions for these populations. Abstract

Much of what has been written about local, state, and national approaches related to the desegregation of public schools focuses on administrative concerns, policy debates, and other issues faced by schools, yet there are still many stories of initiatives designed to facilitate school integration in the twentieth century to be told. The lives, the struggles, and the triumphs of people who did so much to transform the nature of public education are often lost in the mist of time; resurrecting their stories is often the challenge of the historian. This paper evolved from research the author began over ten years ago while writing the centennial history, "The Curry School of Education at the University of Virginia, 1905-2005: Preparing Men and Women for Leadership in Scientific Educational Work."

Abstract

This article explores the challenges and adaptations we undertook to engage people with different communication needs, specifically those living with motor neurone disease (MND), in qualitative research interviews. While interviewing those in advanced stages of illness has raised ethical concerns, participants continue to report value in contributing to research. We reflect on our experiences of conducting qualitative interviews with people living with MND and provide guidance for researchers facing similar challenges. MND significantly impairs communication abilities, impacting speech, gestures, and facial expressions. Communication aids such as augmented and alternative communication (AAC) devices are often key tools for communication. We draw on two research studies using expanded interview techniques to accommodate these devices and provide space for participants to express their views. Interview methods were modified to cater for participant fatigue, setting and individual communication needs. While acknowledging the limitations, we explore how digital platforms, such as MS Teams and Zoom, enabled online interviews, offering advantages for both participants and researchers. Joint interviews involving participants and family members were found to be crucial, considering the interdependence of experiences in illness. Family members often aided communication and enriched data by providing context. Email interviews were another valuable method, allowing participants time to respond and consider their answers, which was particularly useful for those with limited speech. By incorporating a variety of qualitative methods and adapting approaches, we aimed to create inclusive and meaningful research experiences for individuals with different communication needs, ensuring their voices and experiences were captured effectively. Abstract

Home has become established as the preferred place of death within health policy and practice in the UK and internationally. However, growing awareness of the structured inequalities underpinning end-of-life care and the challenges for family members undertaking care at home raise questions about the nature of patient and public preferences and priorities regarding place of death and the feasibility of home management of the complex care needs at the end-of-life. This paper presents findings from a qualitative study of 12 patients' and 34 bereaved family caregivers' perspectives and priorities regarding place of death. Participants expressed complex and nuanced accounts in which place of death was not afforded an overarching priority. The study findings point to public pragmatism and flexibility in relation to place of death, and the misalignment of current policy with public priorities that are predominantly for comfort and companionship at the end-of-life, regardless of place. 45. Key Issues in an International Clinical Experience for Graduate Students in Education: Implications for Policy and Practice

Abstract

This study examines international clinical experiences in England with graduate education students from The University of Connecticut (UConn) and The University of Virginia (UVA) in the United States of America. Limited research available about international clinical experiences in the field of Education focuses primarily to only describe programs and provide authors' anecdotal evidence about the benefits and challenges of these experiences. This study is significant because it provides data about both the anticipated and actual benefits and challenges of an international clinical experience in education. Focus group interviews were chosen as the method of data collection to best explore and understand the full range of perceptions of the students. Data yielded findings not reported in previous information about international placements: significant differences existed between the benefits and challenges anticipated by the graduate students and the actual benefits and challenges they experienced; and developing skills to self-reflect and reflect with peers and supervisors about their professional practice was a significant actual benefit experienced by all students. Students further reported that they did not anticipate developing reflective skills because it had not been a significant benefit of their undergraduate clinical experiences in the United States. In contrast, literature about teacher and teacher candidate development cites that the ability to reflect on professional practice is one of the most successful means of professional development. Based on information in the literature and analysis of the focus group discussions, the conclusion outlines implications for policy, practice, and future research to improve clinical experiences in education both locally and internationally. The significance of the benefits of developing reflective skills during clinical experiences is also discussed.

46. Characterising the molecular function of the Rho GTPase RhoJ in endothelial cells

Abstract

RhoJ is an endothelial expressed Rho GTPase that localises to focal adhesions and regulates cell migration and tube formation. Previous work in our laboratory determined that RhoJ activation negatively regulates focal adhesion (FA) numbers, and interacts with the FA proteins GIT1 and β-PIX. The studies presented in this thesis aimed to characterise the role of RhoJ in modulating FA dynamics, further investigate its interactions with the GIT/PIX complex, and generate a RhoJ knockout mouse to determine its function in vivo. Silencing RhoJ led to prolonged FA disassembly in migrating human umbilical vein endothelial cells (HUVEC), while adhesions in cells expressing a dominant active (da) mutant of RhoJ disassembled more rapidly. Interactions between daRhoJ and GIT1, GIT2 and β-PIX were identified, and the localisation of any member of the RhoJ/GIT/PIX complex to FAs was found to be dependent on each of the other components. daRhoJ expression increased levels of GIT2 protein, while silencing RhoJ reduced GIT2 Tyr 392 phosphorylation. In vivo, the growth of subcutaneous tumours was reduced in RhoJ knockout mice compared to wild type controls. In conclusion, the data presented in this thesis show that RhoJ regulates FA disassembly, most likely in concert with the GIT/PIX proteins. This in turn influences endothelial cell migration and ultimately angiogenesis.

Abstract

BackgroundIdentifying immunologic mechanisms that contribute to premature cardiovascular disease (CVD) among HIV-positive patients will inform prevention strategies.MethodsCoronary artery calcium (CAC) progression was studied in an HIV cohort. Immunophenotypes were measured on baseline cryopreserved peripheral blood mononuclear cells using multicolor flow cytometry. Logistic regression identified predictors of CAC progression after adjusting for traditional and HIV-related risk factors.ResultsBaseline characteristics for the analysis cohort (n=436) were median age 42 years, median CD4 cell count 481 cells/μl, and 78% receiving antiretroviral therapy. Higher frequencies of CD16 monocytes were associated with greater likelihood of CAC progression, after adjusting for traditional and HIV risk factors [odds ratio per doubling was 1.66 for CD14/CD16 (P=0.02), 1.36 for CD14/CD16 (P=0.06), and 1.69 for CD14/CD16 (P=0.01)]. Associations for CD16 monocytes persisted when restricted to participants with viral suppression. We found no significant associations for CAC progression with other cellular phenotypes, including T-cell activation and senescence markers.ConclusionCirculating CD16 monocytes, potentially reflecting a more pro-atherogenic subpopulation, independently predicted greater CAC progression among HIV-infected persons at low risk for AIDS. In contrast to T-cell abnormalities classically associated with AIDS-related disease progression, these data highlight the potential role of monocyte activation in HIV-related CVD risk.

Abstract

Talking about death and dying is promoted in UK health policy and practice, from a perception that to do so encourages people to plan for their end of life and so increase their likelihood of experiencing a good death. This encouragement occurs alongside a belief that members of the public are reluctant to talk about death, although surveys suggest this is not the case. This paper describes findings from a research study in which people participated in deliberative discussion groups during which they talked about a range of topics related to death, including talking about death, the good death, choice and planning and compassionate communities. Here we report what they had to say in relation to talking about death and dying. We identified three themes: 1. The difference between talking about death as an abstract concept and confronting the certainty of death, 2. how death and dying presents issues for planning and responsibility, and 3. approaches to normalising death within society. For our participants, planning was considered most appropriate in relation to wills and funerals, while dying was considered too unpredictable to be easy to plan for; they had complex ideas about the value of talking about death and dying.
