2. Sustainable Development Through a Mobile Application for a Community Clinic

Abstract

Implementing Information and Communication Technology (ICT) solutions can alleviate pressing problems in society and are a central component of sustainable development. Often, healthcare addresses the symptoms without approaching the socioeconomic limiters that can lead to reduced individual economic freedoms from receiving healthcare. This paper investigates the question: How can technology and training interventions enable clinicians to offer care that addresses the socioeconomic limitations of their patients? This paper observes the implementation of a mobile app designed to offer people who cannot access health resources in Omaha, Nebraska, a city in the Midwestern United States. This study follows the design science and action research approach, with clinicians participating in developing the mobile app. As a result of COVID, patients no longer have access to the free clinic because it was shut down. The app is available to the broader community needing basic resources to stay healthy. Through sets of application revisions and observations of usage, this paper arrives at insights into how such applications can support multi-ethnic and underserved communities. 21. Generalized Eigensystem techniques for the inverse problem of electrocardiography applied to a realistic heart-torso geometry

Abstract

We have previously proposed two novel solutions to the inverse problem of electrocardiography, the generalized eigensystem technique (GES) and the modified generalized eigensystem technique (tGES), and have compared these techniques with other numerical techniques using both homogeneous and inhomogeneous eccentric spheres model problems. In those studies we found our generalized eigensystem approaches generally gave superior performance over both truncated singular value decomposition (SVD) and zero-order Tikhonov regularization (TIK). In this paper we extend the comparison to the case of a realistic heart-torso geometry. With this model, the GES and tGES approaches again provide smaller relative errors between the true potentials and the numerically derived potentials than the other methods studied. In addition, the isopotential maps recovered using GES and tGES appear to be more accurate than the maps recovered using either SVD and TIK. Abstract

A case is reported of a 69-year-old man who had two heart attacks and subsequently developed heart arrhythmia (uncoordinated, ineffective heartbeat). Antiarrhythmic agents were not only ineffective, but arrhythmia continued after coronary bypass surgery. Treatment with amiodarone, an antiarrhythmic drug, was ineffective, and appeared to lead to pneumonia, a common complication of the drug. Steroid treatment was then tried to overcome the suspected pulmonary toxicity, and the patient improved. Antibiotics were then used to treat a fever and white blood cell elevation. Heart symptoms like those in myocardial infarction (heart attack) developed, and full block of electrical conduction in the heart occurred, and he soon died. Upon autopsy, infection with Aspergillus, a mold, was found in the lungs and throughout the heart muscle. This type of infection is rare in the heart, and usually occurs in subjects whose immune systems are compromised, usually from malignancy or chemotherapy. The study suggests that the primary infection was aspergillus pneumonia, which is difficult to culture and diagnose. The infection then spread to the heart, and the steroid treatment and two courses of antibiotics may have increased the risk of this spread. The infection in the heart was not typical, in that it affected the nerves involved in electrical conduction, but not coronary arteries or the bypass grafts. The study suggests that such fungal infections may increase in prevalence with the increased use of amiodarone and treatment of its pulmonary complications with steroids. Abstract

ICDs can affect a patient's perceived quality- of-life (QOL). This article describes the QOL in patients who participated in The CABG Patch Trial. This trial evaluated the potential benefit of empiric ICD implantation in patients with an increased risk of arrhythmic cardiac death as determined by reduced ejection fraction (< 0.36) and an abnormal signal- averaged ECG. Patients were randomized to control (no ICD) or treatment (ICD) limbs. QOL was measured using the SF-36 and other measures among 490 (68%) of 719 patients available at 6-month follow-up. Analysis was performed on 228 control patients (those without ICDs) and 262 patients with ICDs. Results: Six months after having CABG surgery, patients in the ICD group had lower levels of psychological well-being than those in the control group. In addition, compared to controls, patients whose ICDs had delivered therapy reported feeling less healthy, had reduced physical and emotional role functioning, and had lower levels of psychological well-being. Conclusion: Strategies aimed at easing patients' adjustment to ICDs should be developed and tested for efficacy in the setting of ICD prophylaxis. [ABSTRACT FROM AUTHOR]

Abstract

Cognitive load is one important contributor to system usability, and minimizing unnecessary cognitive load can help users better process system information. Cognitive load theory, an educational theory, defines three types of cognitive load: intrinsic, extraneous, and germane (Sweller et al. 1998). Intrinsic cognitive load is a necessary part of completing a task. Extraneous cognitive load is introduced by the format and presentation of material, and can be minimized with design help. Germane cognitive load is the effect of the effort learners put into understanding. Important to our study here are intrinsic and extraneous cognitive load. Systems can be designed to minimize the negative influences of extraneous cognitive load. Using eye tracking, we can understand how system design influences cognitive load, and correlating with reported cognitive load can clarify differences between extraneous and intrinsic cognitive load in system design. We have designed a novel personal health record pre-visit form. Data from the pre-visit form helps the healthcare team understand the patient's current status and needs, and ensures needs are met. The system consists of 10 sequential screens where patients input their information such as past surgical history, medications, allergies, and family history. As a part of the development of the system, we have conducted usability tests with 32 adult cardiovascular patients from the University of Nebraska Medical Center's Heart and Vascular Center. Patients used the pre-visit form to enter their personal health information. After each screen, patients filled out measures of cognitive load using the NASA Task Load Index (TLX). Throughout the interaction, their gaze was tracked using an EyeTech eye tracker. Using the combination of self-reported cognitive load (e.g. NASA TLX) during system interaction and eye tracking, we will gain an understanding of how gaze behavior is influenced by cognitive load. Our initial analysis of the TLX data has shown that cognitive load varies depending on the type of information requested from patients, and that tasks requiring significant information recall create cognitive load in patients (Pachunka et al. 2019).
