3. Developing the green operating room: exploring barriers and opportunities to reducing operating room waste.

Abstract

Summary: The Australian health care system contributes 7% of the national greenhouse gas emission footprint and generates massive waste streams annually. Operating rooms are a particular hotspot, generating at least 20% of the total hospital waste.A systematic search of several global academic databases was conducted in mid‐2022 (articles from 1992 to 2022) for peer‐reviewed research relevant to waste management in the operating rooms. We then used thematic analysis to enumerate and characterise the strategies and barriers to sustainable waste management in the operating room.The waste reduction strategies focused on avoidance of high carbon products; correct waste segregation and reduced overage; reusing, reprocessing, and repurposing devices; and improved recycling. The first barrier identified was a constrained interpretation of the concept of "first do not harm", ingrained in surgeons' practices, in prioritising single‐use surgical products. The second barrier was ineffective or insufficient waste education. The third barrier was the immediate cost of implementing waste management compared with the long term realisation of environmental and economic benefits. The last barrier to implementing institutional practice change was the lack of policies and regulations at the local hospital, federal and international levels.We also evaluated the knowledge gaps in current surgical waste research, including lack of benchmarking data and standardised regulations concerning reusable or reprocessed devices, as well as the methods used to promote pro‐sustainability behavioural change. Abstract

Australia's neighbor Papua New Guinea suffers from some of the highest snakebite rates in the world, with some of the most venomous snakes on Earth. A worldwide shortage of antivenoms is a global problem, and some 100,000 people each year die from snake bites. Perhaps some progress can be made with new charitable contributions, but the problems may be multiplied by the privatization of antivenom manufacturers, making them subject to market forces.

Abstract

Carukia barnesi (Cb), Malo kingi (Mk) and Chironex fleckeri (Cf) are dangerous Australian box jellyfish species that provoke distinct and not well understood envenomation syndromes. Specifically, Cb and Mk are small, rare and able to induce a systemic syndrome of generalised muscle pain and catecholamine excess termed “Irukandji syndrome”; Cf has been widely regarded as one of the most venomous organisms in the animal kingdom causing severe sting site pain combined with potentially lethal cardiotoxicity. Building on past studies of major chirodropid and carybdeid species venoms, this study compared the utility of various cubozoan specific antibody reagents to better define the relationships between venom proteins from both exemplar Irukandji species (Cb and Mk) and the archetype C. fleckeri box jellyfish. With the aid of commercial ovine derived Cf-specific antivenom, mouse antibodies reactive to Cb and Mk and rabbit antibodies specific to two Cf toxins (CfTX-1 and 2), as well as human sera, the cross-reactivity of jellyfish species-specific polyclonal antibodies against these three cubozoan venoms was investigated. Immunoblot assays revealed distinc levels of immune recognition across the three species, indicating that Mk specific reagents may bind both Irukandji and Cf venoms. Irukandji venom appears to be antigenic with the exception of a few proteins in the range of 43/46 kDa maybe homologous to CfTX-1 and 2. The implications of such antibody binding for future antivenom development require further investigation.

Abstract

Funnel-web spider, Hadronyche infensa, venomous bites in coastal southeast Queensland as seen in five patients, spring and summer, 1992-98, at a hospital are discussed. Two required antivenom with low doses effective. Where the bites are not reported frequently, clinicians and the community should be educated about risks and immediate management. Using gloves while gardening might reduce danger. The spiders do go into houses. Linens should not be put on the ground outside and should be checked if it hangs to the floor. This is the first time bites of this spider have been reported in this region. Pressure-immobilization first aid was used in most cases. Symptoms are given.
