Abstract

Summary Background The mechanism underlying statin-induced event reduction in patients with acute coronary syndrome remains unclear. Aims To assess the efficacy of rosuvastatin 20 mg versus atorvastatin 80 mg in reducing the apolipoprotein B/apolipoprotein A-1 (apoB/apoA-1) ratio at 3 months. Non-inferiority of rosuvastatin 20 mg versus atorvastatin 80 mg in reducing low-density lipoprotein cholesterol at 1 and 3 months was also assessed. Methods Patients with non-ST-elevation acute coronary syndrome were enrolled into this randomized, double blind, parallel-group trial. Results In total, 753 patients (369, rosuvastatin 20 mg; 384, atorvastatin 80 mg) were included in the intention-to-treat analysis; 478 patients (226, rosuvastatin 20 mg; 252, atorvastatin 80 mg) were included in the per-protocol analysis. Rosuvastatin 20 mg was more effective than atorvastatin 80 mg in decreasing apoB/apoA-1 ratio at 1 month (−44.4% vs −42.9%, p = 0.02) but not at 3 months (both −44.4%, p = 0.87). Low-density lipoprotein cholesterol decreased by ∼50% after 1 and 3 months in both groups. Non-inferiority of rosuvastatin 20 mg versus atorvastatin 80 mg was demonstrated at 1 month (difference, −0.3% [95% confidence interval, −2.7; +2.1]), but not at 3 months (+1.0% [−1.6; 3.5]) (intention-to-treat analysis). In the per-protocol analysis, non-inferiority of rosuvastatin 20 mg was demonstrated at both 1 (−0.7% [−3.5; 2.0]) and 3 (−0.5% [−3.5; 2.5]) months. Conclusion In patients with non-ST-elevation acute coronary syndrome, rosuvastatin 20 mg decreased apoB/apoA-1 ratio at 1 month more than atorvastatin 80 mg. No difference could be shown at 3 months; thus, the primary endpoint was not met.

Abstract

Nine patients, 4 with chronic obstructive pulmonary disease (COPD) and 5 with obstructive sleep apnoea syndrome (OSAS) were monitored during sleep, rest and exercise. Left ventricular ejection fraction (LVEF) was investigated using gated equilibrium 99mtechnetium ventricular scintigraphy during rapid eye movement (REM) sleep, during exercise, and during wakeful rest. Control wakeful rest periods used for comparison with a study state (either REM sleep or exercise) were always selected during the same circadian segment as that state. Myocardial stress thallium-201 scintigraphy was performed during, and 4 h after, exercise, and results were compared to a daytime rest period. Several patients had myocardial hypoperfusion despite a normal electrocardiographic (ECG) treadmill test. During REM sleep, all patients exhibited a significant change in LVEF (greater than 5%) compared to wakefulness. During exercise, 5 subjects increased their LVEF normally (greater than 5%) and 4 (1 COPD, 3 OSAS) decreased it. All patients had a similar change (increase or decrease) during REM and at maximal exercise. Our results suggest that REM sleep in COPD and in OSAS can produce a myocardial stress as great as that produced by exercise. We conclude that REM sleep, like exercise, is a state in which morbidity may become higher and that it may account for mortality in COPD and OSAS patients with compromised myocardial circulation.

Abstract

OBJECTIVE: To report the results of the fourth national study into the practice of euthanasia, physician-assisted suicide, and other medical decisions around the end of life in The Netherlands in 2005, after the Dutch law regulating the ending of life by a physician at the request of a patient and physician-assisted suicide had come into effect in 2002. DESIGN: Descriptive study and longitudinal comparison. METHOD: A questionnaire was mailed to the physicians that had filled out a death certificate for a case of death included in the sample of 6860 deaths. The response rate was 77.8%. RESULTS: In 2005, of all deaths in The Netherlands, 1.7% was the result of euthanasia and 0.1% was the result of physician-assisted suicide. These percentages were significantly lower than those in 2001, when 2.6% of all deaths resulted from euthanasia and 0.2% from assisted suicide. Of all deaths, 0.490 was the result of the termination of life without an explicit request by the patient. Continuous deep sedation, possibly resulting in hastening of death, was used in 7.1% of all deaths in 2005, which is significantly higher than the 5.6% in 2001. In 73.9% ofall cases of euthanasia or assisted suicide in 2005, life was terminated by means of neuromuscular relaxants and/or barbiturates; opioids were used in 16.2% of cases. In 2005, 80% of all cases of euthanasia and assisted suicide were reported to a regional assessment committee. Physicians were more likely to report their end-of-life practices if they considered them to be euthanasia or assisted suicide, which was rarely the case when morphine had been used. CONCLUSION: The coming into effect of the Dutch euthanasia act was followed by a modest decrease in the rates of euthanasia and physician-assisted suicide. This decrease may have resulted from the increased application of other methods for reducing severe suffering in the last phase of life, such as palliative sedation
