14. Prenatal and neonatal brain structure and white matter maturation in children at high risk for schizophrenia

Abstract

Objective: Schizophrenia is a neurodevelopmental disorder associated with abnormalities of brain structure and white matter, although little is known about when these abnormalities arise. This study was conducted to identify structural brain abnormalities in the prenatal and neonatal periods associated with genetic risk for schizophrenia. Method: Prenatal ultrasound scans and neonatal structural magnetic resonance imaging (MRI) and diffusion tensor imaging were prospectively obtained in the offspring of mothers with schizophrenia or schizoaffective disorder (N=26) and matched comparison mothers without psychiatric illness (N=26). Comparisons were made for prenatal lateral ventricle width and head circumference, for neonatal intracranial, CSF, gray matter, white matter, and lateral ventricle volumes, and for neonatal diffusion properties of the genu and splenium of the corpus callosum and corticospinal tracts. Results: Relative to the matched comparison subjects, the offspring of mothers with schizophrenia did not differ in prenatal lateral ventricle width or head circumference. Overall, the high-risk neonates had nonsignificantly larger intracranial, CSF, and lateral ventricle volumes. Subgroup analysis revealed that male high-risk infants had significantly larger intracranial, CSF, total gray matter, and lateral ventricle volumes; the female high-risk neonates were similar to the female comparison subjects. There were no group differences in white matter diffusion tensor properties. Conclusions: Male neonates at genetic risk for schizophrenia had several larger than normal brain volumes, while females did not. To the authors' knowledge, this study provides the first evidence, in the context of its limitations, that early neonatal brain development may be abnormal in males at genetic risk for schizophrenia.

A newborn baby who has a seizure shortly after birth may have developed brain damage two or three days before birth, not during delivery. This was demonstrated in a study of 30 such babies, who had elevated blood levels of nucleated red blood cells. This takes two or three days to happen after blood oxygen levels drop.

Abdominal measurements made from midterm fetal ultrasound scans do not seem to be useful in predicting premature birth risk or birth weight complications. Statistical analyses were performed on abdominal measurements from 1,589 ultrasound scans taken at a mean of 19.2 weeks of pregnancy. There was a trend between large and small abdominal measurements and large and small weights at birth, respectively. However, calculated percentile rankings could not be consistently used to predict birth weight or premature birth.

Abstract

The effect of maternal size on the outcome of pregnancy has been studied in the maternal weight range of 100 to 200 pounds. The results provide information for assessing the risk of health problems at the initial prenatal visit. The maternal body mass index is calculated as body weight in kilograms divided by height in meters. This measurement has been shown to be more appropriate than weight alone in predicting disease and death rates in nonpregnant women. The validity of using the body mass index to assess the risk of developing adverse weight-related problems in pregnancy was determined in 6,270 pregnant women. The body mass index increased with age, number of births, and increased gestational age (length of pregnancy). It was greater in black women, compared with nonblack women. Extremely large values for maternal weight and body mass index were effective in assessing the risk of adverse obesity-related outcomes, including pregnancy-related diabetes and hypertensive disorders, abnormally large fetal body, and difficult labor. Body mass index was more effective than maternal weight in predicting the risk of pregnancy-related diabetes. Extremely low values for maternal weight and body mass index were predictive of infants who were small for their gestational ages, born prematurely, had low birth weights, and died at childbirth. These findings suggest that calculating body mass index is not more advantageous than simply weighing women when trying to predict the risk of adverse maternal weight-related pregnancy outcomes. Abstract

The use of telemedicine for reading fetal ultrasounds appears to be feasible and reliable. Telemedicine involves reading images remotely over computer and telephone lines either in real time or at a later time. Fetal ultrasounds of 35 women in their 14th to 38th week of pregnancy were analyzed simultaneously by the attending ultrasound technicians and by a remote physician. The technicians and the physician completely agreed on 66% of the findings. Visualization problems obscured 13 of the structures. This set-up required three digital telephone lines.

Certain abnormal chromosome patterns may be more prevalent than others found by prenatal genetic testing. Abnormal chromosome patterns may be associated with abnormal fetal development such as Down syndrome. Researchers identified 664 fetuses with abnormal chromosome patterns out of 12,454 fetuses that underwent amniocentesis or chorionic villus sampling. The most common abnormal chromosome patterns were autosomal, sex, or marker aneuploidies (16%), and structural chromosomal rearrangements (26%). A standard test known as fluorescent in situ hybridization would not have detected 31% of the abnormalities. Ultrasound examination detected fetal abnormalities in 45% of the de novo chromosome rearrangements. Conclusive testing may need to be weighed against the time required.
