1. Arginase-1 and Treg Profile Appear to Modulate Inflammatory Process in Patients with Chronic Gastritis: IL-33 May Be the Alarm Cytokine in H. pylori-Positive Patients

Abstract

Helicobacter pylori (H. pylori) is a highly prevalent bacterium in our environment, directly involved in various upper digestive tract diseases, such as gastritis, peptic ulcer, and gastric cancer. Several molecules activating the immune system have been reported to be involved in containing H. pylori infection. This study is aimed at analyzing the mRNA expression of the cytokines IFN-γ, IL-17, IL-10, TGF-β, IL-6, IL-22, IL-23, and IL-33; transcription factors T-bet, RORC, and FOXP3; enzymes ARG1, ARG2, and NOS2; and neuropeptides VIP and TAC and their respective receptors VIPR1 and TACR1 in the stomach lining of patients with severe digestive disorders. One hundred and twenty six patients have been evaluated, presenting with symptoms in the upper digestive tract, with the clinical indication for an Upper Digestive Endoscopy exam. Two fragments of the mucosa of the gastric body and antrum have been collected for anatomopathological examination and to analyze the expression of enzymes, cytokines, and transcription factors using qPCR. Expression of the ARG1 gene was seen as significantly higher in the group of patients with chronic inactive gastritis than in the control group. Expression of the TGF-β gene and its FOXP3 transcription factor was significantly higher in the group of chronic inactive gastritis patients than in the control. Expression of IFN-γ, IL-17, IL-10, and TGF-β and the transcription factors, T-bet and RORC, in the presence or absence of H. pylori showed no significant difference. However, the expression of FOXP3 was significantly lower in H. pylori-positive patients than that in H. pylori-negative patients. ARG1 and Treg profile appeared to be modulating the inflammatory process, protecting patients from the tissue lesions with chronic inactive gastritis. Furthermore, we suggest that IL-33 may be a crucial mediator of the immune response against an infection, after gastric mucosal damage.

Abstract

The balancing of energy and water supply-demand involves a challenging issue. Policymakers at both global and national levels are striving to ensure the security of energy and water supply while supporting economic growth. This study investigates the nexus between water and power and explores potential future directions at global and national perspectives. It considers the case of Iran, as a resource-rich developing economy, to assess real-world influences and potential outcomes. The research proposes an enhanced layered uncertainties scenario synthesizing model to develop and evaluate plausible futures within a 10-year planning horizon (2033). The model employs a novel data-gathering procedure based on experts’ inputs and intuitionistic fuzzy averaging to facilitate reliable judgments in a highly uncertain environment. Findings reveal that, from the global perspective, an energy transition is likely to gain momentum during the next decade, but the water intensity (quantity of water supplied per unit of GDP) is expected to gradually decrease. Providing reliable and secured electricity and water supply, especially for the less developed economies is likely to become an increasingly challenging matter, hindering their potential financial and technological development. The period to 2033 in Iran is expected to be shaped by political issues and international sanctions, making them the most significant future drivers. These factors are likely to have a substantial impact on Iran's decisions regarding energy and water development, as they may affect foreign investment, the cost of money, and water competition diplomacy.

Abstract

ABSTRACT ChatGPT, a language-learning model chatbot, has garnered considerable attention for its ability to respond to users’ questions. Using data from 14 countries and 186 institutions, we compare ChatGPT and student performance for 28,085 questions from accounting assessments and textbook test banks. As of January 2023, ChatGPT provides correct answers for 56.5 percent of questions and partially correct answers for an additional 9.4 percent of questions. When considering point values for questions, students significantly outperform ChatGPT with a 76.7 percent average on assessments compared to 47.5 percent for ChatGPT if no partial credit is awarded and 56.5 percent if partial credit is awarded. Still, ChatGPT performs better than the student average for 15.8 percent of assessments when we include partial credit. We provide evidence of how ChatGPT performs on different question types, accounting topics, class levels, open/closed assessments, and test bank questions. We also discuss implications for accounting education and research.

Abstract

Chronic constipation in children and adolescents is relatively common and a reason for consultation with pediatricians and proctologists. Most cases respond to medical treatment. Advanced cases of megacolon and megarectum can be treated surgically by Duhamel technique.This case report describes a 15-year-old patient with chronic intestinal constipation refractory to clinical treatment associated with megacolon and megarectum, which was surgically treated. Resumo: A constipação intestinal crônica em crianças e adolescentes é relativamente comum e motivo de consultas a pediatras e coloproctologistas. A maioria dos casos responde ao tratamento clínico. Casos avançados de megacolon e megarreto podem ser tratados cirurgicamente através da cirurgia de Duhamel.Este relato de caso descreve um paciente de 15 anos de idade com quadro de constipação intestinal crônica refratária ao tratamento clínico associada a megacolon e megarreto, o qual foi tratado cirurgicamente. Abstract

A hidradenite supurativa perianal é uma doença inflamatória crônica de etiologia multifatorial que possui uma prevalência de 4%, acometendo principalmente indivíduos entre a segunda e terceira décadas de vida, além de ser mais frequente em mulheres. O acometimento de ductos das glândulas apócrinas causa lesões do tipo abscesso, nódulo, túneis ou cicatrizes. Foi discutida ainda a etiopatogenia da doença em questão e as estratégias de diagnóstico e tratamento específico. Este estudo de caso buscou evidenciar as manifestações clínicas da hidradenite supurativa e a construção do diagnóstico diferencial de doença inflamatória intestinal. Apresentamos o caso de um paciente com lesões pustulosas em região perianal há dois anos. Após exame clínico e biópsia, foi diagnosticado com quadro de hidradenite supurativa perianal, descartando a suspeita de doença inflamatória intestinal fistulizante. O paciente foi submetido a ressecção cirúrgica associada a sessões de câmara hiperbárica com cicatrização completa das lesões e bom prognóstico e melhora do paciente.

16. Internal audit stigma impairs internal audit outcomes

Abstract

Prior research finds that the internal audit function (IAF) plays a critical role in organizations, yet there is still a stigma toward the profession. We examine how this stigma affects internal audit outcomes, using three different data sources: survey results from parts of Europe (113 observations) and the United States (124 observations) for the year 2017 and an experiment (65 observations) in 2018. We find that when internal auditors in parts of Europe and the U.S. believe there is a negative stigma about internal auditing, they report negative work outcomes, including less ability to add value, less influence in the organization, more resistance to implementing their recommendations, and more pressure to change audit findings. Our experimental results confirm the survey findings and provide further evidence that negative stigma causes participants to perceive less value in internal audit reports and that internal audit recommendations are less influential in decision-making. Taken together, the results suggest that negative perceptions of internal audit have a significant impact on the profession

Abstract

Background: Surgical aortic valve replacement and transcatheter aortic valve replacement (TAVR) are now both used to treat aortic stenosis in patients in whom life expectancy may exceed valve durability. The choice of initial bioprosthesis should therefore consider the relative safety and efficacy of potential subsequent interventions. Objectives: The aim of this study was to compare TAVR in failed transcatheter aortic valves (TAVs) versus surgical aortic valves (SAVs). Methods: Data were collected on 434 TAV-in-TAV and 624 TAV-in-SAV consecutive procedures performed at centers participating in the Redo-TAVR international registry. Propensity score matching was applied, and 330 matched (165:165) patients were analyzed. Principal endpoints were procedural success, procedural safety, and mortality at 30 days and 1 year. Results: For TAV-in-TAV versus TAV-in-SAV, procedural success was observed in 120 (72.7%) versus 103 (62.4%) patients (p = 0.045), driven by a numerically lower frequency of residual high valve gradient (p = 0.095), ectopic valve deployment (p = 0.081), coronary obstruction (p = 0.091), and conversion to open heart surgery (p = 0.082). Procedural safety was achieved in 116 (70.3%) versus 119 (72.1%) patients (p = 0.715). Mortality at 30 days was 5 (3%) after TAV-in-TAV and 7 (4.4%) after TAV-in-SAV (p = 0.570). At 1 year, mortality was 12 (11.9%) and 10 (10.2%), respectively (p = 0.633). Aortic valve area was larger (1.55 ± 0.5 cm2 vs. 1.37 ± 0.5 cm2; p = 0.040), and the mean residual gradient was lower (12.6 ± 5.2 mm Hg vs. 14.9 ± 5.2 mm Hg; p = 0.011) after TAV-in-TAV. The rate of moderate or greater residual aortic regurgitation was similar, but mild aortic regurgitation was more frequent after TAV-in-TAV (p = 0.003). Conclusions: In propensity score–matched cohorts of TAV-in-TAV versus TAV-in-SAV patients, TAV-in-TAV was associated with higher procedural success and similar procedural safety or mortality.

Abstract

A hidradenite supurativa perianal é uma doença inflamatória crônica de etiologia multifatorial que possui uma prevalência de 4%, acometendo principalmente indivíduos entre a segunda e terceira décadas de vida, além de ser mais frequente em mulheres. O acometimento de ductos das glândulas apócrinas causa lesões do tipo abscesso, nódulo, túneis ou cicatrizes. Foi discutida ainda a etiopatogenia da doença em questão e as estratégias de diagnóstico e tratamento específico. Este estudo de caso buscou evidenciar as manifestações clínicas da hidradenite supurativa e a construção do diagnóstico diferencial de doença inflamatória intestinal. Apresentamos o caso de um paciente com lesões pustulosas em região perianal há dois anos. Após exame clínico e biópsia, foi diagnosticado com quadro de hidradenite supurativa perianal, descartando a suspeita de doença inflamatória intestinal fistulizante. O paciente foi submetido a ressecção cirúrgica associada a sessões de câmara hiperbárica com cicatrização completa das lesões e bom prognóstico e melhora do paciente.

Abstract

AIMS: The burden of cardiovascular disease is increasing worldwide, which has to be reflected by cardiovascular (CV) research in Europe. CardioScape, a FP7 funded project initiated by the European Society of Cardiology (ESC), identified where CV research is performed, how it is funded and by whom. It could be transformed into an on-line and up-to-date resource of great relevance for researchers, funding bodies and policymakers and could be a role model for mapping CV research funding in Europe and beyond. METHODS AND RESULTS: Relevant funding bodies in 28 European Union (EU) countries were identified by a multistep process involving experts in each country. Projects above a funding threshold of 100 k€ during the period 2010-2012 were included using a standard questionnaire. Results were classified by experts and an adaptive text analysis software to a CV-research taxonomy, integrating existing schemes from ESC journals and congresses. An on-line query portal was set up to allow different users to interrogate the database according to their specific viewpoints. CONCLUSION: CV-research funding varies strongly between different nations with the EU providing 37% of total available project funding and clear geographical gradients exist. Data allow in depth comparison of funding for different research areas and led to a number of recommendations by the consortium. Abstract

INTRODUÇÃO: Helicobacter pylori (H. pylori) é uma bactéria altamente prevalente em nosso meio e está diretamente envolvida em várias doenças do trato digestivo superior como gastrite, úlcera péptica e câncer gástrico. Neste estudo foram pesquisados dados preditivos do ponto de vista clínico, endoscópico, anatomopatológico bem como os mecanismos da resposta imune frente à presença desta bactéria Várias moléculas ativadoras do sistema imune têm sido descritas e estudadas na tentativa de conter a infecção pelo H pylori. OBJETIVOS: Estudar a presença de infecção por H. pylori na mucosa gástrica com a história clínica, antecedentes clínicos, doenças concomitantes, dados no exame de endoscopia digestiva alta (EDA) do exame anatomopatológico (EAP). Na resposta imune, analisar nos fragmentos do corpo gástrico por qPCR, a expressão mRNA das citocinas IFN- , IL-17, IL-10, TGF- , IL-6, IL-22, IL-23, IL-33; dos fatores de transcrição T-bet, ROR t e FOXP3; das enzimas ARG1, ARG2 e NOS2; dos neuropeptídeos VIP e TAC, e seus respectivos receptores VIP-R1 e TAC-R1. Comparar a expressão desses genes com a presença ou ausência da bactéria H. pylori. MATERIAL E MÉTODOS: foram avaliados 126 pacientes que apresentavam sintomas do trato digestivo superior e com indicação clínica de exame de Endoscopia Digestiva Alta (EDA). Antes da realização do exame de endoscopia digestiva alta foi feito um questionário clínico detalhado e durante o exame endoscópico realizaram-se biópsias múltiplas do esôfago, corpo e antro gástrico e teste rápido da urease. Fragmentos da mucosa do corpo e do antro gástrico foram coletados para o exame anatomopatológico e para análise da expressão das enzimas, citocinas e fatores de transcrição por meio de PCR em tempo real, qPCR. Introdução: Helicobacter pylori (H. pylori) é um importante agente etiológico de várias doenças do trato digestivo superior como gastrite, úlcera péptica e câncer gástrico. A expressão de fatores de virulência pela bactéria, principalmente o gene cagA contribui para uma maior patogenicidade da bactéria na mucosa gástrica. Objetivos: estudar a mucosa do antro gástrico de pacientes infectados pelo H. pylori e comparar com a história clínica, antecedentes clínicos e cirúrgicos, doenças concomitantes, dados do exame de endoscopia digestiva alta, do exame anatomopatológico e analisar a expressão dos fatores de virulência cagA, sítios de fosforilação CagA EPIYA, vacA, iceA1, iceA2 e babA2, e sua influência na expressão do TLR-4, IL-8, CCL-2 e CCR-2 nos indivíduos com gastrite ativa e H. pylori positivo. Material e Métodos: foram avaliados 126 pacientes submetidos ao exame de endoscopia digestiva alta (EDA) com queixas clínicas do trato digestivo superior. Para esta avaliação, antes do exame de EDA foi feito um questionário clínico e durante o exame endoscópico, realizaram-se biópsias múltiplas do esôfago, corpo e antro gástrico e teste rápido da urease. O material obtido foi encaminhado ao laboratório de patologia para exame anatomopatológico (EAP) e parte do material procedente do antro gástrico foi identificada a positividade do H. pylori através da expressão do gene 16S rRNA. Destes, 38 (30.1%) pacientes apresentaram H. pylori positivo, distribuídos da seguinte forma: 32 (84,2%) com gastrite ativa e 6 (15,8%) sem gastrite ativa. Foi pesquisado nessas amostras com H. pylori positivo a expressão de cagA, sítios de fosforilação CagA EPIYA, vacAm1, vacAm2, vacAs1, vacAs2, iceA1, iceA2, babA2, TLR-4, IL-8, CCL-2 e CCR-2, por meio da PCR. Abstract

Galectins are mediators that play an important role in the inflammatory response and in this study we analyzed the expression of Galectins (Gal) -1, -3 and -9 in biopsies of the gastric antrum of patients with upper gastrointestinal symptoms.44 patients with upper digestive tract symptoms were evaluated, and underwent Upper Digestive Endoscopy examination. Sections of the gastric antrum were fixed in buffered formaldehyde at 4% in order to perform the anatomopathological examination and immunohistochemical analysis for Galectins-1, -3 and -9 expression. Fresh sections of gastric antrum were used for DNA extraction and evaluation of Helicobacter pylori (H. pylori). P values0.05 were considered statistically significant.Gal-1 was significantly more expressed on stroma than epithelium (p0.0001), whereas Gal-3 and Gal-9 were more expressed on epithelium (p0.0001). Gal-3 was found to be significantly higher in the stroma of patients with H. pylori infection, mainly on Cag-A positive H. pylori (p0.0001). Gal-9 was down modulated in stroma of patients with chronic gastritis.Up modulation of Gal-3 expression was associated with H. pylori infection and down modulation of Gal-9 with the inflammatory process of chronic gastritis.

29. Early vs Standard Discharge After Transcatheter Aortic Valve Implantation: A Systematic Review and Meta-analysis

Abstract

Objectives: We sought to assess the clinical outcomes of patients undergoing transcatheter aortic valve implantation (TAVI) with early discharge (ED) versus standard discharge (SD) pathways.\ud \ud Background: Minimalist approaches for TAVI have been developed targeting different aspects of the procedure such as local anesthesia or sedation, intra-procedural imaging, vascular access, post-operative monitoring and care, and discharge planning. Their incorporation into routine clinical practice aim to reduce length of hospital stay (LoS) and healthcare cost utilisation without adversely affecting outcomes when compared to standard approaches. \ud \ud Methods: We conducted a literature search of MEDLINE and EMBASE to identify studies that investigated ED (≤3 days) versus SD in TAVI patients. Random-effects meta-analyses were used to estimate the effect of ED compared with SD with regards to 30-day mortality after discharge, 30-day re-admission rate and need for permanent pacemaker implantation (PPI) following discharge.\ud \ud Results: Eight studies including 1,775 participants (ED n=642) fulfilled the inclusion criteria. The mean age was 82.4 years and Society of Thoracic Surgeons score was 6.7. Meta-analyses evaluating discharge to 30-day mortality (OR: 0.65, 95%CI 0.23-1.82, I2=0%) and discharge to 30-day new PPI (OR: 1.61, 95%CI 0.19-13.71, I2=40%) showed no significant difference of an ED compared to a SD strategy. Notably, ED patients were less likely to be re-admitted after ED when compared to SD patients (OR: 0.63, 95%CI 0.41-0.98, P=0.04, I2=0%).\ud \ud Conclusion: ED following uncomplicated TAVI is safe in terms of discharge to 30-day mortality or need for PPI following discharge. Moreover, ED patients experienced a lower rate of re-admissions. These data support the safety of programs aiming an ED pathway in selected TAVI patients. Institutional protocols with the input from different members of the multi-disciplinary heart team should be devised to optimize discharge processes to improve healthcare resource utilisation.

Abstract

Laser-driven high-harmonic generation provides the only demonstrated route to generating stable, tabletop attosecond x-ray pulses but has low flux compared to other x-ray technologies. We show that high-harmonic generation can produce higher photon energies and flux by using higher laser intensities than are typical, strongly ionizing the medium and creating plasma that reshapes the driving laser field. We obtain high harmonics capable of supporting attosecond pulses up to photon energies of 600 eV and a photon flux inside the water window (284 to 540 eV) 10 times higher than previous attosecond sources. We demonstrate that operating in this regime is key for attosecond pulse generation in the x-ray range and will become increasingly important as harmonic generation moves to even longerwavelength driving fields.

Abstract

The influence of crushed sample particle size on low-pressure gas adsorption and desorption behavior of shales and their measurement is an issue of significant current interest in this new era focused on shale gas and oil resources. Here we study two samples of distinct Indian shales, with different organic contents, ages, levels of thermal maturity, and pore-size distributions crushed to four different particle size ranges [S1 (1 mm to 500 mu m), S2 (500-212 mu m), S3 (212-75 mu m), and S4 (75-53 mu m)]. Low-pressure gas adsorption analysis with nitrogen and carbon dioxide gases reveals significant and complex impacts of particle-crush sizes on the measured pore structure characteristics for the two shales. The CO2 results suggest that at the smallest (S4) particle -crush size evaluated, low-pressure gas adsorption measurements record more, finer nanopores (i.e., less than about 8 angstrom), fewer larger nanopores (i.e., greater than about 8 angstrom), and a lower overall nanopore surface volume. The N-2 results show an overall increase in macro-pore volume at the smallest particle-crush size. The results imply that while more, smaller pores are exposed to gas adsorption at the smaller crush sizes, a significant number of nanopores are in some way altered and are not recorded as part of the measured nanopore-size distribution >8 angstrom by low-pressure CO2 adsorption analysis. Fractal dimensions of one shale varied across a range of particle-crush sizes, whereas the fractal dimensions of the other shale studied did not. The analyses suggest that low-pressure gas adsorption results conducted with samples of very small particle -crush sizes should be viewed with caution.

Abstract

Chronic constipation in children and adolescents is relatively common and a reason for consultation with pediatricians and proctologists. Most cases respond to medical treatment. Advanced cases of megacolon and megarectum can be treated surgically by Duhamel technique. This case report describes a 15-year-old patient with chronic intestinal constipation refractory to clinical treatment associated with megacolon and megarectum, which was surgically treated. Resumo A constipação intestinal crônica em crianças e adolescentes é relativamente comum e motivo de consultas a pediatras e coloproctologistas. A maioria dos casos responde ao tratamento clínico. Casos avançados de megacolon e megarreto podem ser tratados cirurgicamente através da cirurgia de Duhamel. Este relato de caso descreve um paciente de 15 anos de idade com quadro de constipação intestinal crônica refratária ao tratamento clínico associada a megacolon e megarreto, o qual foi tratado cirurgicamente.
