It is commonly assumed that efforts to simultaneously develop energy efficient building technologies and to improve indoor environmental quality (IEQ) are unfeasible. The primary reason for this is that IEQ improvements often require additional ventilation that is costly from an energy standpoint. It is currently thought that health and productivity in work and learning environments requires adequate, if not superior, IEQ. Despite common assumptions, opportunities do exist to design building systems that provide improvements in both energy efficiency and IEQ. This report outlines the selection of a heating, ventilation, and air conditioning (HVAC) system to be used in demonstrating such an opportunity in a field study using relocatable school classrooms. Standard classrooms use a common wall mounted heat pump HVAC system. After reviewing alternative systems, a wall-mounting indirect/direct evaporative cooling system with an integral hydronic gas heating is selected. The anticipated advantages of this system include continuous ventilation of 100 percent outside air at or above minimum standards, projected cooling energy reductions of about 70 percent, inexpensive gas heating, improved airborne particle filtration, and reduced peak load electricity use. Potential disadvantages include restricted climate regions and possible increases in indoor relative humidity levels under some conditions.

AIMS Our aim was to report on a survey initiated by the European Association of Percutaneous Cardiovascular Interventions (EAPCI) collecting the opinion of the cardiology community on the invasive management of acute coronary syndrome (ACS), before and after the MATRIX trial presentation at the American College of Cardiology (ACC) 2015 Scientific Sessions. METHODS AND RESULTS A web-based survey was distributed to all individuals registered on the EuroIntervention mailing list (n=15,200). A total of 572 and 763 physicians responded to the pre- and post-ACC survey, respectively. The radial approach emerged as the preferable access site for ACS patients undergoing invasive management with roughly every other responder interpreting the evidence for mortality benefit as definitive and calling for a guidelines upgrade to class I. The most frequently preferred anticoagulant in ACS patients remains unfractionated heparin (UFH), due to higher costs and greater perceived thrombotic risks associated with bivalirudin. However, more than a quarter of participants declared the use of bivalirudin would increase after MATRIX. CONCLUSIONS The MATRIX trial reinforced the evidence for a causal association between bleeding and mortality and triggered consensus on the superiority of the radial versus femoral approach. The belief that bivalirudin mitigates bleeding risk is common, but UFH still remains the preferred anticoagulant based on lower costs and thrombotic risks.

AIMS Our aim was to report on a survey initiated by the European Association of Percutaneous Cardiovascular Interventions (EAPCI) concerning opinion on the evidence relating to dual antiplatelet therapy (DAPT) duration after coronary stenting. METHODS AND RESULTS Results from three randomised clinical trials were scheduled to be presented at the American Heart Association Scientific Sessions 2014 (AHA 2014). A web-based survey was distributed to all individuals registered in the EuroIntervention mailing list (n=15,200) both before and after AHA 2014. A total of 1,134 physicians responded to the first (i.e., before AHA 2014) and 542 to the second (i.e., after AHA 2014) survey. The majority of respondents interpreted trial results consistent with a substantial equipoise regarding the benefits and risks of an extended versus a standard DAPT strategy. Two respondents out of ten believed extended DAPT should be implemented in selected patients. After AHA 2014, 46.1% of participants expressed uncertainty about the available evidence on DAPT duration, and 40.0% the need for clinical guidance. CONCLUSIONS This EAPCI survey highlights considerable uncertainty within the medical community with regard to the optimal duration of DAPT after coronary stenting in the light of recent reported trial results. Updated recommendations for practising physicians to guide treatment decisions in routine clinical practice should be provided by international societies.

- Abstract
Aims: Our aim was to report on a survey initiated by the European Association of Percutaneous Cardiovascular Interventions (EAPCI) concerning opinion on the evidence relating to dual antiplatelet therapy (DAPT) duration after coronary stenting. Methods and results: Results from three randomised clinical trials were scheduled to be presented at the American Heart Association Scientific Sessions 2014 (AHA 2014). A web-based survey was distributed to all individuals registered in the EuroIntervention mailing list (n=15,200) both before and after AHA 2014. A total of 1,134 physicians responded to the first (i.e., before AHA 2014) and 542 to the second (i.e., after AHA 2014) survey. The majority of respondents interpreted trial results consistent with a substantial equipoise regarding the benefits and risks of an extended versus a standard DAPT strategy. Two respondents out of ten believed extended DAPT should be implemented in selected patients. After AHA 2014, 46.1% of participants expressed uncertainty about the available evidence on DAPT duration, and 40.0% the need for clinical guidance. Conclusions: This EAPCI survey highlights considerable uncertainty within the medical community with regard to the optimal duration of DAPT after coronary stenting in the light of recent reported trial results. Updated recommendations for practising physicians to guide treatment decisions in routine clinical practice should be provided by international societies.

- Abstract
Potassium azide (KN3) and 2‐chloro‐6‐(trichloromethyl) pyridine (N‐Serve) were evaluated as nitrification inhibitors for anhydrous NH3field applied on irrigated and nonirrigated Ritzville silt loam and on nonirrigated Naff silt loam in eastern Washington. Formulations of KN3, N‐Serve in liquid NH3, or NH3alone were applied to fallow soil in midsummer at a rate of 90 kg N/ha. Irrigations were 15 cm of water sprinkler applied 1 day or 2 weeks after fertilizer application, and 10 to 15 cm of water each time at 4, 8, and 13 weeks after NH3application. The NH3retention zone was sampled for NH+4and NO−3periodically through December for the Naff soil and through February for the Ritzville soil. Both KN3and N‐Serve effectively inhibited nitrification of the applied NH3on nonirrigated Ritzville soil when temperature and soil moisture were favorable for rapid nitrification. However, KN3was completely ineffective following irrigation or, for the Naff soil, after rainwater penetrated below the retention zone 2 weeks after N application. Where irrigated 1 day or 2 weeks after fertilization application, all of the applied N had disappeared from the initial NH3retention zone in the Ritzville soil in 8 to 13 weeks for both NH3alone and NH3+ KN3. Results with the Naff soil for these applications were similar to results with the irrigated Ritzville soil. By contrast, N‐Serve effectively suppressed nitrification under leaching and nonleaching conditions. For the Ritzville soil, total N uptake by the wheat (Triticum aestivumL.) crop for different rates of fertilizer application followed the order of NH3+ KN3> NH3+ N‐Serve > NH3alone, but grain yields with NH3+ inhibitor were not different from yields with NH3alone. For the Naff soil there was no N‐uptake or grain‐yield response to N rates, and thus no response to the inhibitors.

- Abstract
The design of enzymes with complex active sites that mediate multistep reactions remains an outstanding challenge. With serine hydrolases as a model system, we combined the generative capabilities of RFdiffusion with an ensemble generation method for assessing active site preorganization to design enzymes starting from minimal active site descriptions. Experimental characterization revealed catalytic efficiencies ( k
cat / Km ) up to 2.2x105 M-1 s-1 and crystal structures that closely match the design models (Cα RMSDs < 1 Å). Selection for structural compatibility across the reaction coordinate enabled identification of new catalysts in low-throughput screens with five different folds distinct from those of natural serine hydrolases. Our de novo approach provides insight into the geometric basis of catalysis and a roadmap for designing enzymes that catalyze multistep transformations.- Published

There are repeated calls for remote sensing observations to produce accessible data products that improve our understanding and conservation of biodiversity. The Biodiversity Survey of the Cape (BioSCape) addresses this need by integrating field, airborne, satellite, and modeling datasets to advance the limits of global remote sensing of biodiversity. Over six weeks, an international team of ~150 scientists collected data across terrestrial, marine, and freshwater ecosystems in South Africa. In situ biodiversity observations of plant and animal communities, estuaries, kelp, and plankton were made using traditional field methods as well as novel approaches like environmental DNA and acoustic surveys. Biodiversity observations were accompanied by an unprecedented combination of airborne imaging spectroscopy and lidar measurements acquired across 45,000 km

- Abstract
Background: There are several surgical options each with their potential for complications, differences in length of procedure, and in meaningful outcomes. This study aims to explore those outcomes after Roux-en-Y Gastric Bypass (RYGB) and Loop Duodenal Switch (LDS)., Objective: The purpose of this project is to offer a comparative analysis of RYGB and LDS at the NorthShore University Health System for up to 4 years postoperatively., Methods: A retrospective review of a prospectively maintained database was queried for all patients who underwent RYGB and LDS from 2019 to 2023. Demographic, preoperative, post-operative data, and Quality of Life (QOL) data were included. χ
2 and Kruskal-Wallis tests were used for comparison. Statistical significance was set at p < 0.05., Results: Patient database included 238 patients who underwent RYGB, and 54 who underwent LDS. Initial BMI was significantly higher in LDS compared to RYGB (56.9 ± 8.0; 46.5 ± 7.3; p < 0.01). There were no statistically significant differences between reported comorbidities. There were no differences in intraoperative complications between the two groups, however postoperative complications were significantly higher in the LDS population (16.7%, 7.1%; p < 0.01). Percent total body weight loss (%TBWL) was significantly different at 2 years post operatively with LDS having more %TBWL than RYGB (LDS: N = 10, %TBWL = 44.7 ± 14.1%; RYGB: N = 47) There were no statistically significant differences at any other postoperative time point. Subgroup analysis was completed in patients with initial BMI 50. There were no significant differences at any postoperative time point. QOL data showed no significant difference between both procedures at all postoperative timepoints., Conclusion: Patients who undergo LDS are more likely to experience postoperative complications compared to RYGB with no added benefit in weight loss or comorbidity resolution up to 3 years post operatively., Competing Interests: Declarations. 42. Arteriovenous shunts of the cervical spine: patient demographics, presentation, patterns of high-risk venous drainage, and updated classification.

- Abstract
Background: Intracranial dural arteriovenous (AV) fistula classifications focus on presence/absence of retrograde flow in the cortical veins of the brain as this angiographic finding portends a worse prognosis. However, prior categorization systems of AV shunts in the spine do not incorporate these features. We propose an updated classification for spinal shunting lesions that terms any shunting lesion with retrograde flow in any cortical vein of the brain or spinal cord medullary vein as "high risk". To present this classification, we analyzed our center's most recent experience with cervical spine shunting lesions., Methods: The electronic medical record at our institution was reviewed to identify shunting lesions of the cervical spine and patient demographics/presentation. Comprehensive craniospinal digital subtraction angiograms were evaluated to classify shunt location, type (arteriovenous malformation (AVM) vs arteriovenous fistula (AVF)), and presence of high-risk venous drainage., Results: Some 52 lesions were identified and categorized as pial/dural/epidural/paravertebral AVFs and intramedullary/extraspinal AVMs. Lesions were classified as high risk or not depending on the presence of retrograde flow into at least one vein that directly drains the spinal cord or brain. All patients who presented with either hemorrhage or infarct had underlying high-risk lesions. Additionally, 50% (17/34) of symptomatic patients with high-risk lesions presented with neurological extremity symptoms (OR=10.0, p=0.037) most of which fit a myelopathic pattern., Conclusion: We present an updated classification system for shunting lesions of the spine that focuses on high-risk retrograde flow to the brain or spine in addition to anatomical location in order to better inform patient management., Competing Interests: Competing interests: None declared., (© Author(s) (or their employer(s)) 2024. Purpose: Prior investigations regarding the effect of obesity on inguinal hernia repair have been mixed. The aim of our study was to retrospectively compare perioperative outcomes, recurrence rate, and quality of life between obese and non-obese patients undergoing inguinal hernia repair., Methods: Patients who underwent inguinal hernia repair by any approach at a single institution were identified from a prospectively maintained quality database. Patients with a body mass index (BMI) greater than or equal to 30 kg/m
2 were considered obese. Quality of life was measured with the Surgical Outcomes Measurement System (SOMS) and Carolinas Comfort Scale (CSS) surveys. Differences between obese and non-obese patients were assessed using independent samples t-tests, Wilcoxon rank-sum, and chi-square tests., Results: Between 2010 and 2021, a total of 5575 patients underwent inguinal hernia repair. Fifteen percent of patients were identified as obese (835 patients, mean BMI 33.2 ± 3.3 kg/m2 ). A significantly higher percentage of obese patients were diabetic, and operative time and estimated blood loss were higher in the obese group (all p < 0.001). Rates of hernia recurrence in obese patients was significantly more likely than in non-obese patients (4.2% vs 2.0%, p < 0.001). Up to 2 years postoperatively, a greater percentage of obese patients reported worse quality of life on the SOMS and more bothersome symptoms on the CCS., Conclusions: Inguinal hernia repair in obese patients is a more technically challenging operation. Long-term follow-up revealed a greater risk of hernia recurrence and worse quality of life up to 2 years postoperatively in this patient population., (© 2024. 44. Measured air quality impacts after teaching parents about cooking ventilation with a video: a pilot study.

- Abstract
Background: Cooking-related emissions contribute to air pollutants in the home and may influence children's health outcomes., Objective: In this pilot study, we investigate the effects of a cooking ventilation intervention in homes with gas stoves, including a video-based educational intervention and range hood replacement (when needed) in children's homes., Methods: This was a pilot (n = 14), before-after trial (clinicaltrials.gov #NCT04464720) in homes in the San Francisco Bay Area that had a school-aged child, a gas stove, and either a venting range hood or over-the-range microwave/hood. Cooking events, ventilation use, and indoor air pollution were measured in homes for 2-4 weeks, and children completed respiratory assessments. Midway, families received this intervention: (1) education about the hazards of cooking-related pollutants and benefits of both switching to back burners and using the range hood whenever cooking and (2) ensuring the range hood met airflow and sound performance standards. The educational intervention was delivered via a video developed in conjunction with local youth., Results: We found substantially increased use of back burners and slight increases in range hood use during cooking after intervening. Even though there was no change in cooking frequency or duration, these behavior changes resulted in decreases in nitrogen dioxide (NO2), including significant decreases in the total integrated concentration of NO2 over all cooking events from 1230 ppb*min (IQR 336, 7861) to 756 (IQR 84.0, 4210; p < 0.05) and NO2 collected on samplers over the entire pre- and post-intervention intervals from 10.4 ppb (IQR 3.5, 47.5) to 9.4 (IQR 3.0, 36.1; p < 0.005). There were smaller changes in PM2.5, and no changes were seen in respiratory outcomes., Impact: This pilot before-after trial evaluated the use of a four-minute educational video to improve cooking ventilation in homes with gas stoves and one or more school-aged children. Participant behavior changed after watching the video, and there were decreases in indoor air pollutant concentrations in the home, some of which were significant. This brief video is now publicly available in English and Spanish (wspehsu.ucsf.edu/projects/indoor-air-quality), and this provides suggestive evidence of the utility of this simple intervention, which could be particularly beneficial for households that have children with asthma., Competing Interests: Competing interests The authors declare no competing interests. 45. Multi-Sensor Soil Probe and Machine Learning Modeling for Predicting Soil Properties.

- Abstract
We present a data-driven, in situ proximal multi-sensor digital soil mapping approach to develop digital twins for multiple agricultural fields. A novel Digital Soil Core
TM (DSC) Probe was engineered that contains seven sensors, each of a distinct modality, including sleeve friction, tip force, dielectric permittivity, electrical resistivity, soil imagery, acoustics, and visible and near-infrared spectroscopy. The DSC System integrates the DSC Probe, DSC software (v2023.10), and deployment equipment components to sense soil characteristics at a high vertical spatial resolution (mm scale) along in situ soil profiles up to a depth of 120 cm in about 60 s. The DSC Probe in situ proximal data are harmonized into a data cube providing vertical high-density knowledge associated with physical-chemical-biological soil conditions. In contrast, conventional ex situ soil samples derived from soil cores, soil pits, or surface samples analyzed using laboratory and other methods are bound by a substantially coarser spatial resolution and multiple compounding errors. Our objective was to investigate the effects of the mismatched scale between high-resolution in situ proximal sensor data and coarser-resolution ex situ soil laboratory measurements to develop soil prediction models. Our study was conducted in central California soil in almond orchards. We collected DSC sensor data and spatially co-located soil cores that were sliced into narrow layers for laboratory-based soil measurements. Partial Least Squares Regression (PLSR) cross-validation was used to compare the results of testing four data integration methods. Method A reduced the high-resolution sensor data to discrete values paired with layer-based soil laboratory measurements. Method B used stochastic distributions of sensor data paired with layer-based soil laboratory measurements. Method C allocated the same soil analytical data to each one of the high-resolution multi-sensor data within a soil layer. Method D linked the high-density multi-sensor soil data directly to crop responses (crop performance and behavior metrics), bypassing costly laboratory soil analysis. Overall, the soil models derived from Method C outperformed Methods A and B. Soil predictions derived using Method D were the most cost-effective for directly assessing soil-crop relationships, making this method well suited for industrial-scale precision agriculture applications.- Published

- Abstract
Background: Despite excellent surgical outcomes, a minority of qualified patients undergo weight loss surgery. Endoscopic Sleeve Gastroplasty (ESG), an incisionless procedure, has proven to be effective in achieving weight loss and comorbidity improvement. We aim to compare outcomes of ESG to those of Laparoscopic Sleeve Gastrectomy (LSG)., Method: A retrospective review of a prospective database of patients who underwent ESG and LSG at NorthShore University HealthSystem from 2016 to 2023 was completed. Demographic and outcome data were analyzed. Pre- and post-surgical data were compared using chi-square and two-sample t tests. Improvement or resolution of obesity-related comorbidities were also assessed., Results: A total of 212 LSG and 68 ESG patients were reviewed. ESG patients were older (47 ± 10 vs. 43 ± 12, p = 0.006) and less obese (BMI 37.0 ± 5.5 vs. 45.8 ± 0.4, p < 0.001) than LSG patients. Median length of stay after ESG was 0 days and after LSG 1 day (p < 0.001). Severe adverse events were seen less frequent after ESG (1.47%, vs 3.77%). LSG achieved more significant %TBWL at 6 months (25.2 ± 8.9 vs 14.9 ± 7.4), 1 year (27.5 ± 10.8 vs 14.1 ± 9.8), and 2 years (25.7 ± 10.8 vs 10.5 ± 8.8, all p < 0.001) after surgery when compared to ESG. LSG achieved significantly greater %EWL compared to ESG at 6 months (57.0 ± 20.7 vs 50.4 ± 29.2, p = 0.137), 1 year (61.4 ± 24.6 vs 46.5 ± 34.0, p = 0.026), and 2 years postoperatively (59.7 ± 25.5 vs 32.6 ± 28.2, p = 0.001). There were no statistically significant differences in rates of improvement or resolution of diabetes, obstructive sleep apnea, hyperlipidemia, or hypertension., Conclusion: ESG is an effective procedure for weight loss and comorbidity resolution. Obesity-related comorbidities are comparably improved and resolved following ESG vs LSG. Although the weight loss in LSG is significantly higher, patients can expect a shorter hospital length of stay and a lower rate of complications after ESG. 47. Non-Markovian Dynamic Models Identify Non-Canonical KRAS-VHL Encounter Complex Conformations for Novel PROTAC Design.

- Abstract
Targeted protein degradation (TPD) is emerging as a promising therapeutic approach for cancer and other diseases, with an increasing number of programs demonstrating its efficacy in human clinical trials. One notable method for TPD is Proteolysis Targeting Chimeras (PROTACs) that selectively degrade a protein of interest (POI) through E3-ligase induced ubiquitination followed by proteasomal degradation. PROTACs utilize a warhead-linker-ligand architecture to bring the POI (bound to the warhead) and the E3 ligase (bound to the ligand) into proximity. The resulting non-native protein-protein interactions (PPIs) formed between the POI and E3 ligase lead to the formation of a stable ternary complex, enhancing cooperativity for TPD. A significant challenge in PROTAC design is the screening of the linkers to induce favorable non-native PPIs between POI and E3 ligase. Here, we present a physics-based computational protocol to predict noncanonical and metastable PPI interfaces between an E3 ligase and a given POI, aiding in the design of linkers to stabilize the ternary complex and enhance degradation. Specifically, we build the non-Markovian dynamic model using the Integrative Generalized Master equation (IGME) method from ∼1.5 ms all-atom molecular dynamics simulations of linker-less encounter complex, to systematically explore the inherent PPIs between the oncogene homologue protein and the von Hippel-Lindau E3 ligase. Our protocol revealed six metastable states each containing a different PPI interface. We selected three of these metastable states containing promising PPIs for linker design. Our selection criterion included thermodynamic and kinetic stabilities of PPIs and the accessibility between the solvent-exposed sites on the warheads and E3 ligand. One selected PPIs closely matches a recent cocrystal PPI interface structure induced by an experimentally designed PROTAC with potent degradation efficacy. Enzymes that proceed through multistep reaction mechanisms often utilize complex, polar active sites positioned with sub-angstrom precision to mediate distinct chemical steps, which makes their de novo construction extremely challenging. We sought to overcome this challenge using the classic catalytic triad and oxyanion hole of serine hydrolases as a model system. We used RFdiffusion
1 to generate proteins housing catalytic sites of increasing complexity and varying geometry, and a newly developed ensemble generation method called ChemNet to assess active site geometry and preorganization at each step of the reaction. Experimental characterization revealed novel serine hydrolases that catalyze ester hydrolysis with catalytic efficiencies ( kcat / Km ) up to 3.8 x 103 M-1 s-1 , closely match the design models (Cα RMSDs < 1 Å), and have folds distinct from natural serine hydrolases. In silico selection of designs based on active site preorganization across the reaction coordinate considerably increased success rates, enabling identification of new catalysts in screens of as few as 20 designs. Our de novo buildup approach provides insight into the geometric determinants of catalysis that complements what can be obtained from structural and mutational studies of native enzymes (in which catalytic group geometry and active site makeup cannot be so systematically varied), and provides a roadmap for the design of industrially relevant serine hydrolases and, more generally, for designing complex enzymes that catalyze multi-step transformations.- Published

In recent years, reinforcement learning (RL) has emerged as a valuable tool in drug design, offering the potential to propose and optimize molecules with desired properties. However, striking a balance between capabilities, flexibility, reliability, and efficiency remains challenging due to the complexity of advanced RL algorithms and the significant reliance on specialized code. In this work, we introduce ACEGEN, a comprehensive and streamlined toolkit tailored for generative drug design, built using TorchRL, a modern RL library that offers thoroughly tested reusable components. We validate ACEGEN by benchmarking against other published generative modeling algorithms and show comparable or improved performance. We also show examples of ACEGEN applied in multiple drug discovery case studies. ACEGEN is accessible at https://github.com/acellera/acegen-open and available for use under the MIT license.

- Abstract
Background and Purpose: Hereditary hemorrhagic telangiectasia (HHT) is an autosomal dominant disease that causes vascular malformations in a variety of organs and tissues, including brain AVMs. Because brain AVMs have the potential to cause disabling or fatal intracranial hemorrhage, detection of these lesions before rupture is the goal of screening MR imaging/MRA examinations in patients with HHT. Prior studies have demonstrated superior sensitivity for HHT-related brain AVMs by using postcontrast MR imaging sequences as compared with MRA alone. We now present data regarding the incremental benefit of including arterial spin-labeling (ASL) perfusion sequences as part of MR imaging/MRA screening in patients with this condition., Materials and Methods: We retrospectively analyzed 831 patients at the UCSF Hereditary Hemorrhagic Telangiectasia Center of Excellence. Of these, 42 patients had complete MR imaging/MRA, ASL perfusion scans, and criterion-standard DSA data. Two neuroradiologists reviewed imaging studies and a third provided adjudication when needed., Results: Eight patients had no brain AVMs detected on DSA. The remaining 34 patients had 57 brain AVMs on DSA. Of the 57 identified AVMs, 51 (89.5%) were detected on ASL and 43 (75.4%) were detected on conventional MR imaging/MRA sequences ( P = .049), with 8 lesions detected on ASL perfusion but not on conventional MR imaging., Conclusions: ASL provides increased sensitivity for brain AVMs in patients with HHT.
