6. Utilising volunteer infection studies in the characterisation of anaemia in early malaria

Abstract

The rapid emergence and spread of antimalarial resistance, especially artemisinin resistance in Plasmodium falciparum, has led to the use of volunteer infection studies (VIS) in the drug development pipeline. This thesis aims to characterise malaria-associated anaemia using the data from an induced blood stage model (IBSM trial), pooled analysis of existing haematology data and the investigation of the iron-infection axis. Investigation of the iron-infection axis has not been conducted previously in VIS studies. First, the haematology and parasite dataset were expanded with the execution of two IBSM VIS, one using a new bioreactor manufactured malaria cell bank (MCB) and the other a historical MCB. The clinical studies identified that both MCB were safe to use in malaria-naïve, healthy participants, and that the parasite growth and clearance profiles of the new biomanufactured MCB were comparable to the existing P. falciparum MCB used. Secondly, the haematology and parasite data from 315 participants inoculated with either P. falciparum (n=269) or P. vivax (n=46) were interrogated in a pooled analysis. The effects of age, sex, drug and parasitaemia and recrudescence were evaluated against the fractional fall in haemoglobin, haemoglobin nadir and the reticulocyte response. The fractional fall in haemoglobin, attributable to malaria was ~4% in P. falciparum and ~5% in P. vivax after correction for phlebotomy; the contribution of parasitised erythrocytes to the malaria attributable losses was less than 1% in both species. Lastly, malaria has recently been described as a cause of iron deficiency, which is a significant public health threat in malaria endemic regions. An exploratory study to characterise the markers of iron metabolism in early malaria was conducted in retrospective samples from previous IBSM VIS. The iron-axis was characterised in P. vivax and artemisinin-resistant K13 strain of P. falciparum, as well as fully drug-sensitive P. falciparum. Those inoculated with P. falciparum had depletion of their body iron stores, with a ~23% reduction in their log ferritin/soluble transferrin receptor index. Reduced baseline iron stores were associated with a reduced reticulocyte response (r=0.39, p=0.015) in those inoculated with P. falciparum. Finally, in those inoculated with P. falciparum, increased iron stores were associated with increased post-treatment liver transaminases (ALT-r=0.54, p<0.001; AST-r=0.31, p=0.047). These studies have characterised malaria-attributable malaria in experimental and low-level malaria and have extended the existing information on iron-axis in early P. falciparum infection, as well a possible mechanism for elevations in liver transaminases in malaria VIS.

19. Patients, performance and parlours : the perception and socially constructed practises of dental sedation clinics

Abstract

Conscious Sedation is a pharmacological intervention which enables anxious patients to have dental treatment. Although there is a strong research tradition into the efficacy of sedation modalities, there is a weak evidence base for the experience of sedation by those who use it. The aim of this thesis was to explore patients’, referrers’ and providers’understandings and experiences of conscious sedation and the Secondary Care Sedation Clinics which use it. A qualitative study was undertaken of stakeholders’ experiences of conscious sedation provided by five Secondary Care Sedation Clinics within the United Kingdom. Data were collected through thirty one semi-structured interviews, which were transcribed verbatim and subsequently analysed using a constant comparative method. The data show that sedation and secondary care sedation clinics are imbued with a variety of interpretations by differing participants. Patients primarily perceive sedation clinics as access points for addressing dental needs, whilst clinicians also anticipate an influential role in rehabilitating patients to primary care and recognise the importance of such settings for training future dentists. Successful sedation provision requires a variety of work, and sedation clinics play a hosting role to visiting patients as hinterlands to the dental world. The outcome of patients’engagement with sedation clinics varies from breaking avoidant patterns to creating a cycle of sedation dependency, whilst the process of sedation performance has a potential negative impact for providers. 2 The purposes and processes reported by participants mirror those of Victorian domestic parlours. This thesis suggests a novel theoretical construct of clinical ‘Parlours’. Such frontier social structures provide safe interaction for patients in a temporarily hosting environment. They require front-stage performance augmented by back-stage work, and seek to influence patients in the long-term whilst providing short-term clinical services. Further research is required to explore the transferability of such a concept to other clinical settings.

Abstract

Background Plasmodium vivax has been proposed to infect and replicate in the human spleen and bone marrow. Compared to Plasmodium falciparum, which is known to undergo microvascular tissue sequestration, little is known about the behavior of P. vivax outside of the circulating compartment. This may be due in part to difficulties in studying parasite location and activity in life. Methods and findings To identify organ-specific changes during the early stages of P. vivax infection, we performed 18-F fluorodeoxyglucose (FDG) positron emission tomography/magnetic resonance imaging (PET/MRI) at baseline and just prior to onset of clinical illness in P. vivax experimentally induced blood-stage malaria (IBSM) and compared findings to P. falciparum IBSM. Seven healthy, malaria-naive participants were enrolled from 3 IBSM trials: NCT02867059, ACTRN12616000174482, and ACTRN12619001085167. Imaging took place between 2016 and 2019 at the Herston Imaging Research Facility, Australia. Postinoculation imaging was performed after a median of 9 days in both species (n = 3 P. vivax; n = 4 P. falciparum). All participants were aged between 19 and 23 years, and 6/7 were male. Splenic volume (P. vivax: +28.8% [confidence interval (CI) +10.3% to +57.3%], P. falciparum: +22.9 [CI -15.3% to +61.1%]) and radiotracer uptake (P. vivax: +15.5% [CI -0.7% to +31.7%], P. falciparum: +5.5% [CI +1.4% to +9.6%]) increased following infection with each species, but more so in P. vivax infection (volume: p = 0.72, radiotracer uptake: p = 0.036). There was no change in FDG uptake in the bone marrow (P. vivax: +4.6% [CI -15.9% to +25.0%], P. falciparum: +3.2% [CI -3.2% to +9.6%]) or liver (P. vivax: +6.2% [CI -8.7% to +21.1%], P. falciparum: -1.4% [CI -4.6% to +1.8%]) following infection with either species. In participants with P. vivax, hemoglobin, hematocrit, and platelet count decreased from baseline at the time of postinoculation imaging. Decrements in hemoglobin and hematocrit were significantly greater in participants with P. vivax infection compared to P. falciparum. The main limitations of this study are the small sample size and the inability of this tracer to differentiate between host and parasite metabolic activity. Conclusions PET/MRI indicated greater splenic tropism and metabolic activity in early P. vivax infection compared to P. falciparum, supporting the hypothesis of splenic accumulation of P. vivax very early in infection. The absence of uptake in the bone marrow and liver suggests that, at least in early infection, these tissues do not harbor a large parasite biomass or do not provoke a prominent metabolic response.
