Abstract

Using declassified colonial and British records in Hong Kong and London, as well as memoirs of former leftists and newspapers, this article explores the strategies the Hong Kong colonial government employed in a propaganda campaign to garner political support of the rural population in the New Territories, a porous land frontier during the Cold War. It also analyses the varying political orientations of migrant farmers, who often had received economic benefits from both the colonial government and the leftist organizations. This article reveals that the colonial government established the Vegetable Marketing Organization (VMO), a state-owned enterprise, to first nationalize the vegetable wholesale market in the immediate post-war period, and subsequently used it to combat increasing political influence and anti-government activities of the communist-controlled Society of Plantations. Despite the improvement of the livelihood of immigrant farmers, the VMO Scheme failed to out-compete the Society economically, which was ultimately eliminated by draconian measures. Through studying the agrarian politics and economic contestations in Hong Kong's rural area, this article provides a lens on how the Cold War was played out at a village level in East Asia. 8. Diffuse esophageal spasm: a rare motility disorder not characterized by high-amplitude contractions

Abstract

During normal swallowing, a progressive wave of muscular relaxation and contraction (peristalsis) propels food through the esophagus to the stomach. A painful swallowing disorder known as diffuse esophageal spasm (DES) occurs in a small number of patients. Definitive criteria for DES are not agreed upon (although high-amplitude esophageal contractions are often used as a diagnostic marker), and the prevalence of this condition is not fully known. To further characterize DES, the records of 1,480 patients who were referred for evaluation of chest pain and difficulty swallowing, and who underwent esophageal manometry (recording of intraesophageal pressure changes during swallowing) were reviewed. A diagnosis of DES was made when more than 10 percent but less than 100 percent of the esophageal contractions during swallows were simultaneous (i.e. not peristaltic). According to this criterion, 56 of the patients (4 percent) were diagnosed with DES. Of these 56 patients, only two (4 percent) showed evidence of high-amplitude esophageal contractions; in both cases these were peristaltic contractions. No patient showed evidence of high-amplitude simultaneous contractions; simultaneous contractions were uniformly of lower amplitude than peristaltic contractions. Excessively high pressure of the lower esophageal sphincter (LES), the muscular ring separating the esophagus from the stomach, was found in 5 of the 56 DES patients (9 percent); poor relaxation of the LES during swallowing was found in 7 of the 56 patients (13 percent). Hence, DES is a rare finding based on manometric diagnosis and among those patient with confirmed DES, the occurrence of high-amplitude contractions is also rare. 12. The prevalence of abnormal esophageal test results in patients with cardiovascular disease and unexplained chest pain

Abstract

Chest pain resulting from ischemic heart disease, or the lack of adequate oxygen supply to heart muscle, may be indistinguishable from chest pain caused by disorders of the esophagus. However, the two disorders have very different prognoses. Little attention has been paid to the prevalence of esophageal disorders among patients with cardiovascular disease, despite the fact that the chest pain in many such patients might respond to treatment. To investigate the prevalence of esophageal disorders among heart patients, 220 patients with confirmed heart disease and persistent chest pain were examined. A control group of 159 patients with chest pain but without evidence of cardiac disease were also examined. Positive indications for esophageal disorders included abnormal motility and esophageal spasms. Abnormal motility was common, and ranged from 24 to 37 percent among the patients. Negative results occurred in the groups with the following frequencies: noncardiac chest pain, 55 percent; coronary artery disease, 62 percent; mitral valve prolapse, 42 percent; and bypass/angiography patients, 59 percent. There were no distinct differences between the noncardiac chest pain group and the other patient groups. It has been suggested that in some patients esophageal pain may precipitate myocardial ischemia, and there is some suggestion that esophageal disorders may lower the threshold for cardiac chest pain. The relatively high prevalence of esophageal disturbances among the cardiac patients suggests that at least some pain may be effectively treated. 50. Reversible Esophageal Motility Disorder in a Patient with Sarcoidosis.

Abstract

We report the case of a patient who presented with new onset of dysphagia for solids and liquids. Her condition was subsequently diagnosed as sarcoidosis on the basis of granulomatous inflammation in tissue obtained from transbronchial fine needle aspiration, noncaseating granulomas in a full-thickness lip biopsy, mild uveitis on slit lamp examination, and serum angiotensin-converting enzyme activity of 120 U/L. Esophageal manometry revealed a nonspecific esophageal motility disorder with 60% nontransmitted contractions and a lower esophageal sphincter that failed to relax with wet swallows. The patient was treated with 40 mg prednisone per day, and within 2 wk her dysphagia had markedly improved. Prednisone was continued with a slow taper, and after 1 month her dysphagia had resolved and her esophageal manometry was normal. To our knowledge, this is the first case of sarcoidosis presenting with dysphagia and documented esophageal dysmotility in which symptoms and manometric abnormalities normalized after therapy with systemic corticosteroids.
