Objective To observe the difference of hypotensive effect of sarcubitril/valsartan between hypertensive patients with increased left atrial diameter (LAD) and with normal LAD. Methods A total of 80 patients who were diagnosed as essential hypertension in Nanjing First Hospital from June 2021 to January 2022 were selected, and they all did not take antihypertensive drugs in the past two weeks. According to the size of LAD, they were divided into first group (LAD≥40 mm, n=50) and second group (LAD＜40 mm, n=30) . After enrollment, all patients were given a daily dose of 100 mg of sarcubitril/valsartan. If the blood pressure reached the standard after 1 week［systolic blood pressure (SBP) ＜140 mmHg and diastolic blood pressure (DBP) ＜90 mmHg］, continue to use 100 mg of sarcubitril/valsartan. If not, increase the dose to 200 mg of sarcubitril/valsartan per day. If the blood pressure reaches the standard after 2 weeks, continue to use 200 mg dose. If the blood pressure does not reach the standard, add it to 400 mg dose per day or add other kinds of antihypertensive drugs according to the patient's individualized situation. The changes of blood pressure level and the rate of reaching the standard of blood pressure in the two groups for 4 weeks (28th day) were observed and compared. Results Compared with second group, the decrease of SBP ［7th day: (11.06±1.36) mmHg vs (7.53±1.31) mmHg, P＜0.01; 14th day: (19.70±2.62) mmHg vs (13.80±1.92) mmHg, P＜0.01］ and the decrease of DBP ［7th day: (5.76±2.63) mmHg vs (4.47±1.31) mmHg, P=0.014; 14th day: (10.02±3.17) mmHg vs (8.23±2.84) mmHg, P=0.013］ were significant higher in the first group. After four weeks, there was no significant difference in SBP, DBP levels and the distribution of proportion of combined use of antihypertensive drugs between the two groups (P＞0.05) . Conclusion Compared with hypertensive patients with normal LAD, sarcubitril/valsartan has a better effect on blood pressure reduction in patients with increased LAD is more superior, and the blood pressure reduction range is very obvious in the first two weeks after use.

BACKGROUND: Low back pain is highly prevalent and the main cause of years lived with disability, but data on the burden and trends of low back pain (LBP) in adolescents and young adults (AYAs) are sparse. OBJECTIVE: To assess trends in the burden of LBP among AYAs aged 15–39 years at the global, regional and national levels from 1990 to 2019. METHODS: Data from the Global Burden of Disease (GBD) 2019 were used to analyze incidence, prevalence and Disability-adjusted life year (DALY) due to LBP at global, regional, and national levels. Joinpoint regression analysis calculated the average annual percentage changes (AAPC). Then analyse the association between incidence, prevalence and DALYs and socioeconomic development using the GBD Socio-demographic Index (SDI). Finally, projections were made until 2030 and calculated in Nordpred. RESULTS: The incidence, prevalence and DALYs rates (95%UI) were 2252.78 (1809.47–2784.79), 5473.43 (4488.62–6528.15) and 627.66 (419.71–866.97) in 2019, respectively. From 1990 to 2019, the incidence, prevalence, and DALYs rates AAPC (95%CI) were - 0.49 (- 0.56 to - 0.42), - 0.58 (- 0.65 to - 0.51) and - 0.57 (- 0.64 to - 0.5), respectively. Incidence, prevalence, and DALYs rates in South Asia, East Asia, High-income North America, Western Europe, and Australasia decreased with SDI. Incidence, prevalence, and DALYs rates in Central Asia, Central Europe, and Eastern Europe decreased and then increased with SDI. At the national level, the incidence, prevalence, and DALYs rates are high in the United States and low in India and China. From the 2020 to 2030, most regions is predicted to decline. CONCLUSION: LBP in AYAs is a major global public problem with a high burden. There are large differences in incidence, prevalence and DALYs across SDIs, regions and countries. there is still a need to focus on LBP in AYAs and tailor interventions to reduce the future burden of this condition. The study on the evolution of natural ferroan brucite (MgFe(OH)2) was beneficial to discovering the function of brucite in the geologic cycle and directing the reasonable utilization of brucite resources. In this study, natural MgFe(OH)2 was characterized in detail and Rietveld refinement was employed to determine the precise content and existing environment of Fe2+. The reaction products of MgFe(OH)2 under various atmospheric conditions, including CO2, O2 and a mixed atmosphere of CO2 and O2 were innovatively investigated to gain insights into the evolution process and the crucial role played by Fe2+. Accordingly, the evolution mechanism of MgFe(OH)2 under different atmospheres was afforded based on the characterization and molecular simulations like electron transfer and binding energy. Fe2+ in MgFe(OH)2 layers could be oxidized by O2 easily and give positive layers, CO32− produced by CO2 dissolving in water simultaneously was attracted to finally produce CO32− intercalated MgFe- layered double hydroxides (MgFe-CO32−-LDHs) which could be used as adsorbents, catalysts and so on. This process was supported by thermodynamics and also the dominant evolution route due to the dynamic reason. The existence of Fe2+ in MgFe(OH)2 resulted in the diversity of the evolution products. This work highlighted the composition and structure of evolution products of natural MgFe(OH)2 under different environment as well as its possible application field. [ABSTRACT FROM AUTHOR]
