21. HIV risk behaviours and solutions among men who purchase sex in Indonesia

Abstract

There has been a limited focus on HIV among men who purchase sex (MWPS) in many countries. Systematic information is lacking on the HIV prevalence in this population, and on interventions conducted to reduce HIV risk. Looking specifically to Indonesia, MWPS are understood to be at higher risk of HIV infection compared with men in the general population but there has been a relative lack of attention put towards identifying characteristics such as the rate of condom use and HIV testing in this population and developing appropriate interventions. Accordingly, the studies included in the thesis contribute to filling the knowledge gap for this population in regard to estimation of rate of condom use and HIV testing and evaluation of control strategies among MWPS in Indonesia. A range of research designs and analytical approaches was applied. The first study is a meta-analysis of the burden of HIV among MWPS in low- and middle-income countries (LMICs). We found that HIV prevalence was elevated compared to the population as a whole, and that there was a strong association (RR 1.95) between purchasing sex and HIV prevalence. The second study is a systematic review of HIV interventions conducted among MWPS, or groups often used as proxies for MWPS, in LMICs to reduce their risk of HIV infection. We observed that only a few studies had evaluated the impact of interventions on HIV testing rates. The third study is a survey in Bali, Indonesia, which identifies rates and predictors of condom use and HIV testing among MWPS in this setting. The study found a high condom use (88.5%), but low HIV testing rates (8.1%) among this group. The fourth study explores in more detail why such a low HIV testing phenomenon among MWPS occurs in this setting and the preferences for testing strategies. Men‘s preference for HIV self-testing over HIV testing at the clinic was expressed. The fifth study is a study piloting the HIV self-testing strategies to identify whether this method could improve HIV testing rates among this group. A pilot single-arm non-randomised trial study was conducted among men presenting at the brothels area and found high acceptability of this testing strategy to improve HIV testing rates among MWPS; HIV testing acceptance was 6-7 times higher than the HIV testing history of less than 8.1% found in the third study. Together, the five studies provide insight into estimation of the HIV burden and evaluation of control strategies among MWPS in Indonesia.

Abstract

Introduction: In Indonesia, a country with around 280 million people and the second-highest tuberculosis (TB) incidence rate in the world, the impact of the COVID-19 pandemic on TB care needs careful assessment so that future response strategies can be strengthened. We conducted a study comparing TB testing and treatment rates before and during the first 2 years of the COVID-19 pandemic in Indonesia, and the reasons for any disruptions to care., Methods: We conducted retrospective secondary data analysis and qualitative interviews in Yogyakarta and Bandung, Indonesia. Routine data on TB testing and treatment were sourced from the national TB information system operated by the Indonesian Ministry of Health. TB testing and treatment outcomes were compared between two time periods: pre-COVID (2018-19); and during COVID-19 (2020-21). In-depth interviews were conducted with patients and health workers to explore their experiences in accessing and providing TB services during the pandemic., Results: There was a 45% (21 937/39 962) reduction in the number of patients tested for TB during the pandemic compared with pre-COVID-19, while the proportion of TB tests returning a positive result increased from 12% (4733/39 962) to 50% (10 945/21 937). The proportion of TB patients completing treatment increased by 2.6% during the pandemic, yet the proportion cured and the number of patients successfully treated both decreased (by 7% and 4.4%, respectively). Our qualitative interviews highlighted several factors influencing TB service access and delivery, including fear of being diagnosed with COVID-19 during TB-related clinic visits, fear of COVID-19 exposure among patients and health workers, healthcare facilities prioritising COVID-19 over other services, and mandatory mobility restrictions affecting both patients and health workers., Conclusion: The COVID-19 pandemic impacted TB testing and treatment outcomes in Bandung and Yogyakarta. 46. Lived Experience, Social Support, and Challenges to Health Service Use during the COVID-19 Pandemic among HIV Key Populations in Indonesia.

Abstract

The COVID-19 has greatly affected the lives, health and social well-being of people globally including presenting special challenges in low to middle income countries for people living with HIV. This study investigates the pandemic experiences of the four key HIV-positive populations in Indonesia: men who have sex with men, transgender women, female sex workers, and people who use drugs. In-depth interviews were conducted with a convenience sample of 22 key population members recruited through 9 nongovernment HIV agencies in Jakarta and Bali, Indonesia. Indonesia's Large-scale Social Restrictions Policy mandating physical distancing and stay-at-home orders had been in effect for 7-10 months. The interviews were audio-recorded, transcribed, and coded using NVivo ™ (R1.7) software A grounded theory approach identified key concepts along with similarities, differences, and reoccurring patterns of COVID-related lived experience among participants. Participants recounted the impact of both the pandemic and the Restriction Policy on their interpersonal, financial, medical, and psychosocial well-being. When in need, they turned to formal and informal sources of financial and social support plus their own resourcefulness. Along with other factors, HIV medication shortages, HIV and COVID-related stigma, and fear of acquiring COVID-19 negatively impacted antiretroviral adherence and the use of health services. The results point to the latent consequences of government attempts to curb a pandemic through public health lockdowns and enforced policies of physical separation. Its findings reveal the importance of ensuring that public safety nets for HIV key populations are available to supplement more informal personal sources of needed support., Competing Interests: Competing interests.
