Abstract The group fuzzy extractor allows group users to extract and reproduce group cryptographic keys from their individual non-uniform random sources. It can be easily used in group-oriented cryptographic applications. However, current group fuzzy extractors are not dynamic, i.e. they spend a large cost when dealing with user revocation. In this work, we propose the formal definition and construction of dynamic group fuzzy extractor (DGFE) to address this issue. For the revocation, DGFE allows unrevoked group users to reproduce updated group keys from the existing group help data. Meanwhile, it prevents any revoked group user from generating new group keys using the previously authorized individual help data. We propose a DGFE construction based on the revocable group signature. Furthermore, we give formal proofs of reusability, anonymity and traceability of our construction.

Objective Although the advantages of postoperative braces have been verified in many fields, it is not clear whether postoperative braces can help reduce patients' adverse psychological emotions such as kinesiophobia, anxiety, and depression. This study aims to analyze whether the use of a postoperative brace helps reduce adverse psychological emotions in adolescent idiopathic scoliosis (AIS) patients undergoing spinal deformity surgeries. Methods All consecutive patients who underwent spinal corrective surgeries at our institution between April 2023 and July 2023 formed the prospective cohort. Outcome measures were collected in the preoperative period, 3 months after surgery, and 6 months after surgery. All patients were assessed using the Tampa scale for kinesiophobia (TSK), the hospital anxiety and depression scale (HADS), and the numerical rating scale (NRS). A statistical model of propensity score matching was used to eliminate potential selection bias and maintain comparability. Multivariate linear regression models were used to determine the relationship between postoperative brace and adverse psychological emotions. Results After propensity score matching, this study ultimately enrolled 150 patients. There were no significant differences between the two groups in terms of demographic and perioperative variables. The fully adjusted model showed that the TSK scores of the non‐brace group at the 3‐month (ꞵ = 2.50, 95% CI 0.80–4.20, p = 0.005) and 6‐month follow‐up (ꞵ = 2.75, 95% CI 0.75–4.74, p = 0.007) were significantly higher than those of the brace group. The HADS score of the non‐brace group at the 3‐month follow‐up was significantly higher than that of the brace group (ꞵ = 1.75, 95% CI 0.28–3.22, p = 0.019). The NRS score of the non‐brace group at the 3‐month follow‐up was significantly higher than that of the brace group (ꞵ = 0.69, 95% CI 0.05–1.33, p = 0.034). At the 6‐month follow‐up, there were no significant difference for HADS score or NRS score between the two groups. Conclusion In the early postoperative period, the postoperative brace could provide AIS patients with psychological supports and help them reduce the frequency of adverse psychological emotions. The postoperative brace could continuously improve the fear of movement within 6 months after surgery, and help reduce anxiety, depression, and pain within 3 months after surgery.

Abstract Background Triglyceride glucose (TyG) index and its related parameters have been introduced as cost-effective surrogate indicators of insulin resistance, while prospective evidence of their effects on atherosclerotic cardiovascular disease (ASCVD) remained scattered and inconsistent. We aimed to evaluate the association of TyG and its related parameters with new-onset ASCVD, and the predictive capacity were further compared. Method A total of 95,342 ASCVD-free participants were enrolled from the Kailuan study. TyG and its related parameters were defined by fasting blood glucose, triglyceride, body mass index (BMI), waist circumstance (WC) and waist-to-height ratio (WHtR). The primary outcome was incident ASCVD, comprising myocardial infarction (MI) and ischemic stroke (IS). Cox proportional hazard models and restricted cubic spline (RCS) analyses were adopted to investigate the association between each index and ASCVD. The C-index, integrated discrimination improvement (IDI), and net reclassification improvement (NRI) were used for comparison of their predictive value for ASCVD. Results During a median follow-up of 15.0 years, 8,031 new cases of ASCVD were identified. The incidence rate of ASCVD increased along with elevated levels of each index, and the relationships were found to be nonlinear in the RCS analyses. The hazard ratio (HR) and 95% confidence interval (95% CI) for ASCVD was 1.39 (1.35, 1.43), 1.46 (1.41, 1.50), 1.50 (1.46, 1.55), and 1.52 (1.48, 1.57) per 1 IQR increase of baseline TyG, TyG-BMI, TyG-WC, and TyG-WHtR, respectively, and the association were more pronounced for females and younger individuals aged

Abstract Digital dental technology covers oral cone-beam computed tomography (CBCT) image processing and low-dose CBCT dental applications. A low-dose CBCT image enhancement method based on image fusion is proposed to address the need for subzygomatic small screw insertion. Specifically, firstly, a sharpening correction module is proposed, where the CBCT image is sharpened to compensate for the loss of details in the underexposed/over-exposed region. Secondly, a visibility restoration module based on type II fuzzy sets is designed, and a contrast enhancement module using curve transformation is designed. In addition to this, we propose a perceptual fusion module that fuses visibility and contrast of oral CBCT images. As a result, the problems of overexposure/underexposure, low visibility, and low contrast that occur in oral CBCT images can be effectively addressed with consistent interpretability. The proposed algorithm was analyzed in comparison experiments with a variety of algorithms, as well as ablation experiments. After analysis, compared with advanced enhancement algorithms, this algorithm achieved excellent results in low-dose CBCT enhancement and effective observation of subzygomatic small screw implantation. China is one of the largest volatile organic compounds (VOCs) emitters worldwide. The emission levels of and harm caused by VOCs have attracted much attention. China has implemented multiple policies for VOCs prevention and control but lacks economic control measures for VOCs In this study, the input-output (IO) price model was used to simulate and analyze the emission reduction and economic effects resulting from the imposition of a VOCs environmental protection tax (EPT) in 31 provinces in China. The results show that, first, the collection of a VOCs-EPT can achieve not only VOCs emission reductions but also the synergistic emission reductions of other major pollutants. Second, the collection of a VOCs-EPT could have a negative impact on the macroeconomy, i.e., the greater the tax scope and the higher the tax rate are, the greater the negative economic impact. Third, differences in the level of economic development, the structure of pollution emissions and the stringency of tax policies among regions would cause the emission reduction effect and related negative economic impact to vary across regions. Finally, the collection of a VOCs-EPT could have heterogeneous impacts on various industries, as high-emission industries would suffer greater negative impacts. Therefore, each region should set tax rates that match its provincial economic and environmental development levels. Furthermore, a VOCs-EPT can be levied on key industries, and reasonable preferential tax policies can be formulated to reduce negative macroeconomic benefits.

Abstract Background Patients with myocardial infarction (MI) frequently experience a heightened incidence of depression, thereby increasing the risk of adverse cardiovascular events. Consequently, early detection and intervention in depressive symptoms among patients with MI are imperative. Shexiang Baoxin Pills (SBP), a Chinese patent medicine employed for the treatment of MI, exhibits diverse mechanisms targeting this condition. Nevertheless, its therapeutic efficacy on postmyocardial infarction depressive symptoms remains unclear. The aim of this study is to investigate the effectiveness and mechanism of SBP in managing depression during acute myocardial infarction (AMI). Methods A rat model combining MI and depression was established, and the rats were randomly divided into four groups: the model (MOD) group, SBP group, Fluoxetine (FLX) group, and Sham group. After 28 days of drug intervention, cardiac function was assessed using echocardiography while behavior was evaluated through sucrose preference test (SPT), forced swimming test (FST), and open‐field test (OFT). Additionally, levels of inflammatory factors in serum and hippocampus were measured along with NLRP3 inflammasome‐related protein expression via Western blotting and immunofluorescence. Results SBP can enhance cardiac function in rats with AMI and depression, while significantly ameliorating depressive‐like behavior. Compared to the Sham group, levels of IL‐1β, IL‐18, TNF‐α, and other inflammatory factors were markedly elevated in the MOD group. However, expressions of these inflammatory factors were reduced to varying degrees following treatment with SBP or FLX. Analysis of NLRP3 inflammasome‐related proteins in the hippocampus revealed a significant upregulation of IL‐1β, IL‐18, NLRP3, ASC, caspase‐1, and GSDMD in the MOD group; conversely, these measures were significantly attenuated after SBP intervention. Conclusion We have observed a significant amelioration in depression‐like behavior upon SBP administration during the treatment of AMI, suggesting that this effect may be attributed to the inhibition of NLRP3‐mediated pyroptosis. Fungal keratitis is a severely vision-threatening corneal infection, where the prognosis depends on both fungal virulence and host immune defense. Inappropriate host responses can induce substantial inflammatory damage to the cornea. Therefore, in the treatment of fungal keratitis, it is important to concurrently regulate the immune response while efforts are made to eliminate the pathogen. Ebselen is a widely studied organo-selenium compound and has been demonstrated to have antifungal, antibacterial, anti-inflammatory, and oxidative stress-regulatory properties. The effectiveness of ebselen for the treatment of fungal keratitis remains unknown. In this study, ebselen was demonstrated to produce a marked inhibitory effect on Aspergillus fumigatus (A. fumigatus), including spore germination inhibition, mycelial growth reduction, and fungal biofilm disruption. The antifungal activity of ebselen was related to the cell membrane damage caused by thioredoxin (Trx) system inhibition-mediated oxidative stress. On the contrary, ebselen enhanced the antioxidation of Trx system in mammalian cells. Further, ebselen was proven to suppress the expressions of inflammatory mediators (IL-1β, IL-6, TNF-α, COX-2, iNOS, and CCL2) and reduce the production of oxidative stress-associated indicators (ROS, NO, and MDA) in fungi-stimulated RAW264.7 cells. In addition, ebselen regulated PI3K/Akt/Nrf2 and p38 MAPK signaling pathways, which contributed to the improvement of inflammation and oxidative stress. Finally, we verified the therapeutic effect of ebselen on mouse fungal keratitis. Ebselen improved the prognosis and reduced the fungal burden in mouse corneas. Expressions of inflammatory mediators, as well as the infiltration of macrophages and neutrophils in the cornea were also obviously decreased by ebselen. In summary, ebselen exerted therapeutic effects by reducing fungal load and protecting host tissues in fungal keratitis, making it a promising treatment for fungal infections.

Abstract Background Mesial tipping of posterior teeth occurs frequently during space closure with clear aligners (CAs). In this study, we proposed a new modification of CA by localized thickening of the aligner to form the enhanced structure and investigate its biomechanical effect during anterior retraction. Methods Two methods were employed in this study. First, a finite element (FE) model was constructed, which included alveolar bone, the first premolars extracted maxillary dentition, periodontal ligaments (PDL), attachments and aligners. The second method involved an experimental model—a measuring device using multi-axis transducers and vacuum thermoforming aligners. Two groups were formed: (1) The control group used common CAs and (2) the enhanced structure group used partially thickened CAs. Results FE model revealed that the enhanced structure improved the biomechanics during anterior retraction. Specifically, the second premolar, which had a smaller PDL area, experienced a smaller protraction force and moment, making it less likely to tip mesially. In the same vein, the molars could resist movement due to their larger PDL area even though they were applied larger forces. The resultant force of the posterior tooth was closer to the center of resistance, reducing the tipping moment. The canine was applied a larger retraction force and moment, resulting in sufficient retraction of anterior teeth. The experimental model demonstrated a similar trend in force variation as the FE model. Conclusions Enhanced structure allowed force distribution more in accordance with optimal principles of biomechanics during the extraction space closure while permitting less mesial tipping and anchorage loss of posterior teeth and better retraction of anterior teeth. Thus, enhanced structure alleviated the roller coaster effect associated with extraction cases and offered a new possibility for anchorage reinforcement in clear aligner therapy.

RSPO1, also referred to R-spondin1, plays a crucial role in female sex determination and differentiation in both mammals and teleosts. Nevertheless, limited documentation exists regarding its contribution to the subsequent ovarian development in teleosts. In this research, Qi River crucian carp (Carassius auratus) was employed as a model organism to investigate the function of Rspo1 during the initial stages of ovarian growth. To clarify the expression patterns, a polyclonal antibody targeting Rspo1 was generated. According to Western blot analysis, Rspo1 was able to identify a band that matched the size of Rspo1 in adult Qi River crucian carp. Furthermore, during the development of the ovary, the expression of Rspo1 peaked at 90 days after hatching (dah), a critical period for the ovary development in Qi River crucian carp, then decreased. Immunohistochemical (IHC) analysis revealed the widespread presence of Rspo1 in oogonia, stage I and II oocytes, as well as certain stage III oocytes, stromal cells, and granulosa cells throughout the ovary. To further confirm the essential roles of Rspo1 in early ovary development, we used dsRNA to suppress its expression for a duration of 30 days starting at 90 dah. Following this, there was a significant reduction in the number of oocytes. Additionally, the expression of genes associated with female development, such as β-catenin, foxl2, cyp19a1a, and figlα, exhibited a noticeable decline. Conversely, the expression of apoptosis-promoting factors caspase3 and p53 increased significantly. The gene ki-67, serving as a marker for cell proliferation, experienced a decrease. These findings suggest that Rspo1 may exert influence on ovarian development through the regulation of germ cell proliferation. The inhibition of Rspo1 expression during the crucial phase of ovarian growth in Qi River crucian carp is anticipated to induce somatic cell apoptosis and perturb the structural integrity of the ovary. Our investigation offers significant elucidation on the molecular mechanisms governing the involvement of Rspo1 in the initial stages of ovarian development in teleosts.

Abstract Background This study aimed to investigate sex differences in risk factors for suicide attempts in first‐episode and drug naive (FEDN) major depressive disorder (MDD) with comorbid subclinical hypothyroidism (SCH). Methods A total of 1034 FEDN MDD patients with comorbid SCH were enrolled. The Hamilton Depression Scale (HAMD), Hamilton Anxiety Scale (HAMA), and Positive and Negative Syndrome Scale (PANSS) positive subscale were used to assess patients’ symptoms. Thyroid hormone levels and metabolic parameters were measured. Results MDD patients with SCH had a significantly higher risk of suicide attempts than those without SCH (25.4% vs. 12.2%). Logistic regression showed that HAMA score, thyroid stimulating hormone (TSH) levels, and thyroid peroxidase antibody (TPOAb) levels were significantly associated with an increased risk for suicide attempts in both male and female MDD patients comorbid SCH, while low‐density lipoprotein cholesterol (LDL‐C) was significantly associated with an increased risk for suicide attempts only in male patients, HAMD score and systolic blood pressure were significantly associated with an increased risk for suicide attempts only in female patients. Conclusion SCH comorbidities may increase suicide attempts in MDD patients. Our results showed significant sex differences in clinical and metabolic factors associated with suicide attempts among FEDN MDD patients with comorbid SCH, highlighting appropriate sex‐based preventive interventions are needed.

Abstract Background High cardiovascular health (CVH) was associated with lower risk of cardiovascular disease (CVD) and longer life expectancy. However, whether life years lived without CVD could increase faster than or at least at the same pace as total lifespan remains unknown. We aimed to explore the associations of CVH status with total life expectancy and life years lived with and without CVD among middle-aged and elderly men and women. Methods We included 65,587 participants aged ≥ 45 years from Kailuan study, who were recruited during June 2006 to October 2007. CVH was scored and classified (low [0–49 points], moderate [50–79 points] and high [80–100 points]) with Life’s Essential 8, incorporating evaluations of health behaviors and factors. All-cause mortality and incident non-fatal CVD were recorded from baseline to December 31, 2020. The multi-state life table was adopted to explore the associations of CVH status with total life expectancy and life years lived with and without CVD. Results Six thousand fifty eight cases of incident non-fatal CVD and 10,580 cases of deaths were identified. Men aged 45 years with low, moderate, and high CVH had a life expectancy of 33.0, 36.5 and 38.5 years, of which 7.8 (23.6%), 6.0 (16.3%) and 3.7 years (9.6%) were spent with CVD. For women, the corresponding life expectancy was 36.6, 43.6 and 48.6 years, and the remaining life years lived with CVD were 7.8 (21.3%), 6.0 (13.7%) and 4.5 years (9.3%), respectively. The benefits of high CVH were persistent across lifespan from age 45 to 85 years and consistent when CVH was evaluated with health behaviors and factors alone. Conclusions High CVH compared with low CVH was associated with longer total life expectancy and fewer years spent with CVD, indicating that promoting CVH is of great importance for CVD prevention and healthy ageing in China.

Abstract Background The risk of cardiovascular disease (CVD) depended on the magnitude and exposure duration of insulin resistance (IR). This study aimed to investigate the associations of cumulative metabolic score for IR (cumMETS-IR) with incident CVD, and to further explore the modulated effects of time course of METS-IR accumulation. Methods We enrolled 47,270 participants without CVD and underwent three examinations during 2006–2010 from the Kailuan study. CumMETS-IR from 2006 to 2010 were calculated as the mean values of METS-IR between consecutive examinations multiplying by time intervals between visits. Time course of METS-IR accumulation was calculated as the slope of METS-IR versus time. Hazard ratios (HRs) and 95% confidence intervals (CIs) for CVD risk were calculated with multivariable-adjusted Cox regressions. Results During a median follow-up of 10.99 years, we identified 3184 cases of incident CVD. The risk of incident CVD increased with increasing cumMETS-IR (HR, 1.77; 95% CI 1.58–1.98 for the Q4 versus Q1 group), exposure duration (HR, 1.60; 95% CI 1.45–1.77 for 6 years versus 0 years), and cumulative burden (HR, 1.49; 95% CI 1.37–1.61 for burden ≥ 0 versus

Abstract Background Glioma cells have increased intake and metabolism of methionine, which can be monitored with 11 C-L-methionine. However, a short half-life of 11 C (~ 20 min) limits its application in clinical practice. It is necessary to develop a methionine metabolism genes-based prediction model for a more convenient prediction of glioma survival. Methods We evaluated the patterns of 29 methionine metabolism genes in glioma from the Cancer Genome Atlas (TCGA). A risk model was established using Lasso regression analysis and Cox regression. The reliability of the prognostic model was validated in derivation and validation cohorts (Chinese Glioma Genome Atlas; CGGA). GO, KEGG, GSEA and ESTIMATE analyses were performed for biological functions and immune characterization. Results Our results showed that a majority of the methionine metabolism genes (25 genes) were involved in the overall survival of glioma (logrank p and Cox p < 0.05). A 7-methionine metabolism prognostic signature was significantly related to a poor clinical prognosis and overall survival of glioma patients (C-index = 0.83). Functional analysis revealed that the risk model was correlated with immune responses and with epithelial-mesenchymal transition. Furthermore, the nomogram integrating the signature of methionine metabolism genes manifested a strong prognostic ability in the training and validation groups. Conclusions The current model had the potential to improve the understanding of methionine metabolism in gliomas and contributed to the development of precise treatment for glioma patients, showing a promising application in clinical practice.

Dissimilatory iron reduction (DIR) coupled with carbon cycling is increasingly being recognized as an influential process in freshwater wetland soils and sediments. The role of DIR in organic matter (OM) mineralization, however, is still largely unknown in lake sediment environments. In this study, we clarified rates and pathways of OM mineralization in two shallow lakes with seasonal hydrological connectivity and different eutrophic situations. We found that in comparison with the domination of DIR (55%) for OM mineralization in Lake Xiaoxingkai, the contribution of methanogenesis was much higher (68%) in its connected lake (Lake Xingkai). The differences in rates and pathways of sediment OM mineralization between the two lakes were attributed to higher concentrations of carbonate associated iron oxides (Fecarb) in Lake Xiaoxingkai compared to Lake Xingkai (P = 0.002), due to better deposition mixing, more contributions of terrigenous detrital materials, and higher OM content in Lake Xiaoxingkai. Results of structural equation modeling showed that Fecarb and total iron content (TFe) regulated 25% of DIR in Lake Xiaoxingkai and 76% in Lake Xingkai, accompanied by a negative effect of TFe on methanogenesis in Lake Xingkai. The relative abundance and diversity of Fe-reducing bacteria were significantly different between the two lakes, and showed a weak effect on sediment OM mineralization. Our findings emphasize the role of iron minerals and geochemical characterizations in regulating rates and pathways of OM mineralization, and deepen the understanding of carbon cycling in lake sediments.

To investigate the effects of replacing fish meal with fermented Antarctic krill meal on growth performance, body color, and serum biochemical indexes of koi carp, 450 koi carp with an average body weight of (4.92±0.22) g were randomly divided into five groups with three replicates per group and 30 fish per replicate. In the control group, we added unfermented defatted Antarctic krill meal, and in the experimental groups, we added defatted Antarctic krill meal fermented by Enterococcus faecalis, Lactobacillus plantarum, Clostridium butyricum, and compound bacteria (1:1:1), which were termed as ES (control group), EF, LP, CB, and MIX, respectively. The addition amount of Antarctic krill meal in each group was 200 g/kg, and the experiment lasted for 70 days. The results showed the following: Compared with those in the control group, the replacement of fish meal with fermented Antarctic krill meal significantly improved final body weight (FBW), weight gain rate (WGR), and specific growth rate (SGR) (P0.05). Based on the above results, using complex bacteria to ferment krill meal in the koi carp feed is recommended with a 20% addition level of defatted krill meal, which can effectively improve the absorption and conversion utilization rate of krill meal.

BackgroundLung cancer (LC) is the second-highest incidence and the first-highest mortality cancer worldwide. Early screening and precise treatment of LC have been the research hotspots in this field. Artificial intelligence (AI) technology has advantages in many aspects of LC and widely used such as LC early diagnosis, LC differential classification, treatment and prognosis prediction.ObjectiveThis study aims to analyze and visualize the research history, current status, current hotspots, and development trends of artificial intelligence in the field of lung cancer using bibliometric methods, and predict future research directions and cutting-edge hotspots.ResultsA total of 2931 articles published between 2003 and 2023 were included, contributed by 15,848 authors from 92 countries/regions. Among them, China (40%) with 1173 papers,USA (24.80%) with 727 papers and the India(10.2%) with 299 papers have made outstanding contributions in this field, accounting for 75% of the total publications. The primary research institutions were Shanghai Jiaotong University(n=66),Chinese Academy of Sciences (n=63) and Harvard Medical School (n=52).Professor Qian Wei(n=20) from Northeastern University in China were ranked first in the top 10 authors while Armato SG(n=458 citations) was the most co-cited authors. Simultaneously, there is a need to enhance collaboration not only among various countries and institutions but also between high-quality medical and industrial entities.

Chemotherapeutic drugs have low aggregation rates and short retention times in tumors, severely limiting their clinical application. Additionally, the development and spread of solid tumors is highly dependent on neovascularization. An RGD-modified acid-responsive peptide was designed to encapsulate the chemotherapeutic drug paclitaxel (PTX) and the antiangiogenic drug sorafenib (SF), enabling them to self-assemble into nanoparticles (NPs) in a physiological environment. In an acidic environment, the release of PTX and SF was achieved when the drug-loaded experimental peptide PS/Pep1 transformed from spherical NPs into aggregates containing nanofibers, which effectively prolonged the retention of the encapsulated drug and increased drug accumulation. Furthermore, in vivo experiments provided compelling evidence of the strong inhibitory effects of PS/Pep1 on tumor growth and metastasis, as well as its effective suppression of angiogenesis. This targeted NP system that undergoes a morphological shift in the tumor microenvironment holds promise for enhancing the efficacy of the combined administration of chemotherapeutic and antiangiogenic drugs to inhibit tumor growth and metastasis.

Summary: Background: The atherogenicity of remnant cholesterol (RC), a contributor to residual risk of cardiovascular events, has been underlined by recent guidelines. We aimed to evaluate the relationship between RC levels and the efficacy and safety of genotype-guided dual antiplatelet therapy in the CHANCE-2 trial. Methods: This post-hoc study used data from the CHANCE-2 trial, which was a randomised, double-blind, placebo-controlled trial of 6412 patients (aged >40 years) enrolled from 202 hospitals in China, between Sept 23, 2019, and March 22, 2021, who carried CYP2C19 loss-of-function alleles and had either an acute minor stroke or high-risk transient ischaemic attack (TIA), and could start treatment within 24 h of symptom onset. Participants received either (1:1) ticagrelor plus aspirin (control) or clopidogrel plus aspirin (intervention) and the effect of reducing the 3-month risk of any new stroke was assessed (ischemic or haemorrhagic, primary outcome). From the CHANCE-2 study population, we enrolled 5890 patients with complete data on RC. The cutoff point of RC for distinguishing patients with greater benefit from ticagrelor-aspirin versus clopidogrel-aspirin was determined with subpopulation treatment effect pattern plot. The primary efficacy and safety outcome was recurrent stroke and severe or moderate bleeding within 90 days, respectively. CHANCE-2 is registered with ClinicalTrials.gov, NCT04078737. Findings: The cutoff to define elevated RC was 0.91 mmol/L. Ticagrelor-aspirin versus clopidogrel-aspirin was associated with a reduced risk of recurrent stroke in patients with non-elevated RC levels (122 [5.3%] versus 179 [7.8%]; hazard ratio [HR], 0.68; 95% confidence interval [CI], 0.54–0.85), but this benefit was absent in those with elevated RC levels (58 [8.4%] versus 48 [7.3%]; HR, 1.10; 95% CI, 0.73–1.65; P-interaction = 0.03). When analyzed as a continuous variable, the benefit of ticagrelor-aspirin on recurrent stroke decreased as RC levels increased. The rates of severe or moderate bleeding between treatment groups were similar across RC categories (0.3% versus 0.3%, P-interaction = 0.95). Interpretation: Our post-hoc findings suggest that RC could be a potential biomarker to discriminate patients who received more benefits from ticagrelor-aspirin versus clopidogrel-aspirin therapy in CYP2C19 loss-of-function carriers with minor stroke or TIA. These findings need to be validated in an independent study. Abstract Background There are many studies on the relationship between vitamin D and coronavirus disease 2019 (COVID-19), while the results are matters of debate and the mechanisms remain unknown. The present study was performed to assess the impact of serum 25-hydroxyvitamin D [25(OH)D] levels on the severity of disease in hospitalized COVID-19 patients and identify potential mechanisms of 25(OH)D alterations. Methods A total of 399 hospitalized COVID-19 patients were recruited from three centers between December 19, 2022, and February 1, 2023. Medical history, laboratory examination, and radiologic data were retrospectively collected. The patients were divided into four groups based on disease severity. Serum 25(OH)D levels in the patients were determined by the electrochemiluminescence method and cytokines were detected by flow cytometry. The relationship between serum 25(OH)D status and the severity of COVID-19, and the correlation between 25(OH)D levels and cytokines in COVID-19 patients were assessed. Results Levels of 25(OH)D were significantly lower in the deceased group than in the other three groups (P 0.05). The levels of 25(OH)D (odds ratio = 0.986, 95% confidence interval: 0.973–0.998, P = 0.024) and IL-5 (odds ratio = 1.239, 95% confidence interval: 1.104–1.391, P = 0.04) were independent risk factors for the severity of COVID-19 disease upon admission. Serum 25(OH)D levels were able to predict the mortality of patients with COVID-19, and the predictive value was even higher when combined with IL-5 levels and eosinophil (Eos) count. Abstract Background Atherogenic index of plasma (AIP) has been confirmed as a novel marker for myocardial infarction (MI), but few evidence on the long-term AIP and MI risk in general populations. We thus aimed to evaluate the relationships of cumulative exposure to AIP and its accumulation time course with the risk of MI. Methods A total of 54,440 participants were enrolled in the Kailuan study. Time-weighted cumulative AIP was calculated as the weighted sum of the mean AIP value for each time interval, then normalized by total exposure duration, the exposure duration was from 2006 to 2010. Eco-engineering is an important tool for wetland restoration, but there are still large theoretical and application gaps in the knowledge of the effects of eco-engineering implementation on the interactions between environmental conditions and organisms during wetland restoration processes. In this study, we investigated water quality parameters and plankton communities in a national wetland park to clarify the mechanism of changes in plankton community structure and their ecological networks before and after the eco-engineering project. Undoubtedly, we found water quality was significantly improved with increased metazooplankton diversity after the implementation of eco-engineering. Ecological engineering reduced the effect of farmland drainage on the restored wetland and changed the phytoplankton community structure, which significantly reduced the relative abundance of Cyanobacteria and increased the relative abundance of Bacillariophyta. The structural equation modeling revealed that the total effect of metazooplankton on phytoplankton was significantly enhanced and associated with weakened relationships between phytoplankton and environmental variables after eco-engineering. In addition, the ecological network analysis also showed that the network connection between phytoplankton and metazooplankton was stronger after the eco-engineering implementation, leading to an enhanced biotic interactions in different trophic levels. These results indicate that the main approach to regulating primary producers in wetland ecosystems changed from “bottom-up” control to a combination of “bottom-up” and “top-down” control under the intervention of artificial recovery measures. Our findings shed new light on the effects of eco-engineering on the interactions between water quality and organisms and provide a scientific basis for the sustainable management of wetland ecosystems.

Abstract In this study, we aimed to determine whether liraglutide could effectively reduce insulin resistance (IR) by regulating Sestrin2 (SESN2) expression in L6 rat skeletal muscle cells by examining its interactions with SESN2, autophagy, and IR. L6 cells were incubated with liraglutide (10–1000 nM) in the presence of palmitate (PA; 0.6 mM), and cell viability was detected using the cell counting kit-8 (CCK-8) assay. IR-related and autophagy-related proteins were detected using western blotting, and IR and autophagy-related genes were analyzed using quantitative real-time polymerase chain reaction. Silencing SESN2 was used to inhibit the activities of SESN2. A reduction in insulin-stimulated glucose uptake was observed in PA-treated L6 cells, confirming IR. Meanwhile, PA decreased the levels of GLUT4 and phosphorylation of Akt and affected SESN2 expression. Further investigation revealed that autophagic activity decreased following PA treatment, but that liraglutide reversed this PA-induced reduction in autophagic activity. Additionally, silencing SESN2 inhibited the ability of liraglutide to up-regulate the expression of IR-related proteins and activate autophagy signals. In summary, the data showed that liraglutide improved PA-induced IR in L6 myotubes by increasing autophagy mediated by SESN2.

ABSTRACTObjective The quantification of cardiovascular health (CVH) was updated by the American Heart Association recently by using the “Life’s Essential 8” (LE8) score. We aimed to investigate the associations of baseline and longitudinal CVH status measured by the new LE8 score (except for blood pressure) with the risk of hypertension.Methods A total of 52 990 participants with complete data on LE8 metrics and without hypertension were enrolled from the Kailuan study, Tangshan, China. The associations of incident hypertension with the overall baseline, time-updated, and time-varying CVH score (ranging 0 [lowest] to 100 [highest]), and each component of LE8, were assessed by Cox regressions.Results During a median follow-up of 10.73 years 28 380 cases of incident hypertension were identified. The risk of hypertension attenuated with increased CVH score (Ptrend < 0.0001), the hazard ratios (HRs) in high CVH versus low CVH group was 0.54 (95% confidence interval [CI], 0.51–0.57) for baseline CVH, 0.47 (95% CI, 0.45–0.50) for time-updated CVH, and 0.59 (95% CI, 0.55–0.63) for time-varying CVH. The predictive value of CVH in predicting hypertension improved by using LE8 than using Life’s Simple 7 metrics. Among LE8 components, body mass index score was the strongest risk factor for hypertension. Subgroup analyses showed that the benefit of a higher CVH score on hypertension was more prominent in young adults and in women (Pinteraction < 0.05).Conclusions A higher CVH score assessed by new LE8 is associated with a lower risk of subsequent hypertension, especially young adults and women.

ABSTRACTAlthough previous studies have reported the dysregulation of respiratory tract microbiota in infectious diseases, insufficient data exist regarding respiratory microbiota imbalances in the lower respiratory tracts (LRTs) of children with Mycoplasma pneumoniae pneumonia (MPP). Here, we analysed the microbial community using 16S rRNA gene sequencing. Finally, bronchoalveolar lavage fluid (BALF) samples from 158 children with MPP and 29 with bacterial or viral pneumonia (control group) were collected. The diversity of the microbial community was significantly different between the two groups. A significantly increased abundance of Tenericutes and Mycoplasma was detected in the MPP group, exceeding 67% and 65% of the total bacterial population, respectively. Using Mycoplasma abundance as the diagnostic method, the sensitivity and specificity of the model was 97.5% and 96.6%, respectively. Abstract Purpose Asprosin is a newly discovered adipose factor secreted by white fat, which is involved in glucose metabolism and inflammation. Neuregulin-4 (Nrg-4) is a new adipose factor released from brown adipose tissue and is considered to play an important role in metabolism. This study aims to explore the association between serum Asprosin, Nrg-4 level and coronary heart disease(CHD) in patients with type 2 diabetes mellitus(T2DM) and the diagnostic value. Patients and methods 157 patients with T2DM were enrolled from Affiliated Hospital of Chengde Medical University between December 2020 to July 2021. These patients were divided into T2DM without CHD group (T2DM-0, n = 80) and T2DM with CHD (T2DM-CHD, n = 77). Serum Asprosin and Nrg-4 expression was detected by enzyme-linked immunosorbent assay, and the correlations between Asprosin or Nrg-4 and clinical and biochemical indicators were analyzed. A receiver operating characteristics curve analysis and area under the curve (AUC) were used to evaluate diagnostic accuracy. In order to study the changes of the structure and chemical durability with Fe/P mole ratio(r(Fe/P)), a series of P2O5-Fe2O3-Al2O3-Na2O-ZrO2 glass-ceramic samples with different Fe/P mole ratios were prepared by the melting-quenching method. X-ray diffraction was used for phase analysis, and Raman spectroscopy, X-ray photoelectron spectroscopy and M-ssbauer spectroscopy were used for structural analysis. n(Fe3+)/(n(Fe2+)+n(Fe3+)) mole ratios increase with the increase of Fe/P mole ratio when the Fe/P mole ratio is less than 0.18. When the Fe/P mole ratio is between 0.18 and 0.32, n(Fe3+)/(n(Fe2+)+n(Fe3+)) fluctuates around 60%. The samples with Fe/P mole ratio between 0.2 and 0.3 have the best chemical durability and relatively low crystal fraction.

In the context of wetland restoration, the reconstruction of an ecosystem’s structure typically manifests within a relatively short timeframe, while the restoration of its function often necessitates an extended period of time following the implementation of restoration measures. Consequently, it becomes imperative to engage in the comprehensive, long-term dynamic monitoring of restored wetlands to capture timely information regarding the ecological health status of wetland restoration. In this paper, we aimed to precisely assess the ecosystem health of a typical wetland that had been converted from farmland to wetland in Fujin National Wetland Park in 2022. We selected 18 ecological, social, and economic indicators to establish a wetland ecological health evaluation model, and then used the method of an analytic hierarchy process (AHP) to calculate the weights for each indicator and acquire the ecological health index (EHI) score. The results of our study revealed that the ecosystem health index was 3.68, indicating that the FNWP wetland ecosystem was in “good” condition; this result was mainly affected by wetland water quality (0.382). The ecological health assessment of restored wetlands can monitor wetland ecological resources and provide a scientific basis for the management and protection of restored wetlands.

Abstract Background The mechanisms linking obesity to cardiovascular disease (CVD) are still not clearly defined. Individuals who are overweight or obese often develop insulin resistance, mediation of the association between obesity and CVD through the insulin resistance seems plausible and has not been investigated. This study aimed to evaluate whether and to what extend the effect of general and central obesity on cardiovascular disease (CVD) is mediated by insulin resistance. Methods A total of 94,136 participants without CVD at baseline were recruited from the Kailuan study. Insulin resistance was evaluated by the triglyceride-glucose (TyG) index, calculating as ln [fasting triglyceride (mg/dL) × fasting glucose (mg/dL)/2]. Mediation analysis using a new 2-stage regression method for survival data proposed by Valeri and VanderWeele was to explore the mediating effects of the TyG index on the association between obesity and CVD. Results During a median follow-up of 13.01 years, we identified 7327 cases of CVD. Mediation analyses showed that 47.81% of the total association (hazard ratio [HR], 1.18; 95% confidence interval [CI], 1.12–1.24) between overweight and CVD was mediated through the TyG index (HR [indirect association], 1.07; 95% CI, 1.07–1.09), and the proportion mediated was 37.94% for general obesity. For central obesity, analysis by waist circumference, waist/hip, and waist/height categories yielded an attenuated proportion mediated of 32.01, 35.02, and 31.06% for obesity, taken normal weight as reference. Conclusions The association between obesity and CVD was mediated by TyG index, suggesting proper control of insulin resistance can be effective to reduce the effects of obesity on CVD.

Abstract Adsorption desorption is an important behavior affecting the migration of phenanthrene in soil. In this study, three typical soils of loess, silts and silty sand in Guanzhong Basin, Shaanxi Province, China were used as adsorbents. Batch equilibrium experiments were carried out to study the adsorption desorption kinetics and isotherm of phenanthrene in different soils. Response surface method (RSM) was used to study the effects of temperature, pH, phenanthrene concentration and organic matter content on soil adsorption of phenanthrene. The results showed that after adsorption, the outline of soil particles became more blurred and the degree of cementation increased. The kinetic adsorption of phenanthrene by soil conforms to the quasi second-order kinetic model, and the adsorption desorption isotherm is nonlinear and conforms to the Freundlich model. Due to the difference of soil properties, the adsorption amount of phenanthrene by soil is loess > silty sand > silts. The thermodynamic results show that the adsorption of phenanthrene by soil is spontaneous and endothermic, and the desorption is spontaneous and exothermic. Through RSM, the interaction between phenanthrene concentration and soil organic matter in Loess and silts is significant, and the interaction between temperature and soil organic matter in silty sand is significant. Among the four factors affecting the adsorption rate of loess, silts and silty sand, soil organic matter is the most significant. The theoretical optimum adsorption rates of loess, silts and silty sand are 98.89%, 96.59% and 93.37% respectively.

Objective: The modification of bean dregs by steam explosion (SE) combined with high solids enzymatic digestion to extract soluble dietary fiber (SDF) was investigated to evaluate the modification effect of soybean dregs and the enhancement of the high solid enzymatic digestion process. Methods: The bean dregs were modified by SE technology. The microstructure, chemical composition, functional groups and thermal stability of bean dregs before and after SE modification were characterized by scanning electron microscopy, Fourier transform infrared spectroscopy and differential scanning calorimetry. SDF was prepared by SE combined with high-solid enzymatic hydrolysis, and the enzymatic hydrolysis effect of bean dregs before and after SE was compared. Results: The SE treatment significantly improved the structure and properties of soybean dregs, which contributed to the conversion of insoluble dietary fiber (IDF) into SDF. The SDF content of soybean dregs was increased to 3.85 times that of raw materials under the optimal SE condition. After SE, the microstructure of bean dregs showed a loose honeycomb porous structure, the water solubility was increased by 5.13 times, and the water holding capacity, oil holding capacity and water swelling were reduced. The maximum pyrolysis peak temperature and enthalpy change of soybean dregs were increased and the thermal stability was improved. The composition and structure of enzymatic substrates and the flow state of enzymatic system were prompted by SE, so as to reduce the solid load restriction of enzymatic hydrolysis process of soybean dregs. It benefited to increase the concentration of the enzymatic substrate, lower the amount of enzymes and the enzymatic hydrolysis time. Under the enzymatic hydrolysis conditions at the maximum solid loading of 20% and the minimum enzyme consumption of 5 FPU/g, the yield of SDF in SE bean dregs reached 43.75%, which was 3.76 times higher than that of raw materials. Through SE, the reaction equilibrium time of enzymatic hydrolysis was decreased by about 50%. Conclusion: SE modification combined with high-solid enzymatic hydrolysis treatment technology would provide technical support for the efficient utilization and application for the large amount of bean dregs.

Abstract Background Future risk of cardiovascular disease (CVD) and mortality is associated with cumulative amount TyG index (cumTyG) exposure, while whether time course of TyG accumulation modulates the risk remains unclear. This study sought to examine the associations of cumTyG index accumulation time course with the risk of CVD and all-cause mortality. Methods We enrolled 51,734 participants free of CVD and underwent three examinations at year 2006, 2008, and 2010. CumTyG from baseline to the third examination was calculated. Time course of cumTyG accumulation was calculated as the slope of TyG versus time from 2006 to 2010, or as splinting the overall TyG index accumulation into early (cumTyG06 − 08) and late accumulation (cumTyG08 − 10). Extracellular vesicles (EVs) with a variety of bioactive molecules can be continuously secreted during the storage of blood components, which can promote cell-cell interaction and mediate cell-to-cell signal transmission. EVs are widely involved in inflammation, metabolism and immunization. In recent years, studies have reported that EVs in the blood components can not only cause transfusion-related lung injury through activating neutrophils, but also act on monocytes to stimulate inflammatory response. EVs can play different regulatory effects on the immune system of patients. In this review, the role and mechanism in transfusion-related immunomodulation (TRIM) of EVs in different blood components have been summarized.

Background The American Heart Association recently proposed an updated definition of cardiovascular health (CVH) named as Life's Essential 8. We aimed to explore the association between this latest published CVH measurement and years lived without cardiovascular disease (CVD) among the Chinese population. Methods and Results We included 89 755 adults free of CVD at baseline from the Kailuan study. The CVH of each participant was scored (from 0 point to 100 points) and classified (low [0–49 points], moderate [50–79 points], and high [80–100 points]) according to Life's Essential 8, which incorporated 8 components covering health behaviors and health factors. Incident CVD was documented through follow‐ups from baseline (June 2006 to October 2007) until December 31, 2020. CVD‐free life years from age 30 to 80 years associated with different CVH scores were estimated using flexible parametric survival models. A total of 9977 incident CVDs were recorded. We observed a gradient relationship between CVH score and years lived without CVD. The age‐ and sex‐adjusted CVD‐free life years (95% CI) were 40.7 (40.3–41.0) years for low CVH, 43.3 (43.0–43.5) years for moderate CVH, and 45.5 (45.1–45.9) years for high CVH. Similar trends were noted when individual subtypes of CVD were investigated, and high CVH evaluated by health behaviors and health factors was also related to longer CVD‐free life years. Conclusions A higher CVH evaluated by the updated Life's Essential 8 metrics was significantly associated with a greater number of life years without CVD, indicating the importance of promoting CVH for healthy aging in China.

Background Healthy individuals with normal level of serum uric acid (SUA) may not be truly at the lowest risk of cardiovascular disease (CVD). This study aimed to assess the association of SUA levels with CVD and its subtypes in people without CVD risk factors and determine a suitable target of SUA to prevent CVD. Methods and Results We enrolled 25 284 participants who were free of CVD, absent of CVD risk factors, and with an SUA level between 180 and 359 μmol/L (3–6 mg/dL) at baseline from the Kailuan study. Cox proportional hazards models were applied to calculated adjusted hazard ratio (HR) and 95% CI for the risk of CVD and its subtypes. During a median follow‐up of 12.97 years (interquartile range, 12.68–13.16 years), we identified 1007 cases of CVD. There was an increase in the risk of incident CVD with increasing SUA levels (Ptrend=0.0011). Compared with participants with SUA levels of 180 to 239 μmol/L (3–4 mg/dL), the HR of CVD was 1.12 (95% CI, 0.96–1.31) and 1.28 (95% CI, 1.08–1.52) for SUA levels of 240 to 299 μmol/L (4–5 mg/dL) and 300 to 359 μmol/L (5–6 mg/dL), respectively. A multivariable‐adjusted spline regression model showed a J‐shaped association between SUA and the risk of CVD. Similar results were observed for stroke and myocardial infarction. Conclusions The risk of incident CVD increased with elevating SUA levels among individuals without hyperuricemia or other traditional CVD risk factors. These findings highlighted the importance of primordial prevention for SUA level increase along with other traditional CVD risk factors.

Background The multitrajectory model can identify joint longitudinal patterns of different lipids simultaneously, which might help better understand the heterogeneous risk of premature cardiovascular disease (CVD) and facilitate targeted prevention programs. This study aimed to investigate the associations between multitrajectories of lipids with premature CVD. Methods and Results The study enrolled 78 526 participants from the Kailuan study, a prospective cohort study in Tangshan, China. Five distinct multitrajectories of triglyceride, low‐density lipoprotein cholesterol (LDL‐C), and high‐density lipoprotein cholesterol over 6‐year exposure were identified on the basis of Nagin's criteria, using group‐based multitrajectory modeling. During a median follow‐up of 6.75 years (507 645.94 person‐years), 665 (0.85%) premature CVDs occurred. After adjustment for confounders, the highest risk of premature CVD was observed in group 4 (the highest and increasing triglyceride, optimal and decreasing LDL‐C, low and decreasing high‐density lipoprotein cholesterol) (hazard ratio [HR], 2.13 [95% CI, 1.36–3.32]), followed by group 5 (high and decreasing triglyceride, optimal and increasing LDL‐C, low and decreasing high‐density lipoprotein cholesterol) (HR, 2.07 [95% CI, 1.45–2.98]), and group 3 (optimal and increasing triglyceride, borderline high and increasing LDL‐C, optimal and decreasing high‐density lipoprotein cholesterol) (HR, 1.90 [95% CI, 1.32–2.73]). Conclusions Our results showed that the residual risk of premature CVD caused by increasing triglyceride levels remained high despite the fact that LDL‐C levels were optimal or declining over time. These findings emphasized the importance of assessing the joint longitudinal patterns of lipids and undertaking potential interventions on triglyceride lowering to reduce the residual risk of premature CVD, even among individuals with optimal LDL‐C levels.

The 2008 8.0 Ms Wenchuan earthquake caused serious ecological degradation in the northwest of China’s Sichuan Province. Elucidating landscape fragmentation and spatial pattern of post-earthquake landscape is of great interest in order to improve ecological restoration and predict the spatial distribution of damaged ecosystems in earthquake-affected areas. We used four real-time remote sensing images to analyze the landscape pattern characteristics and spatial autocorrelation of the Longxi River Basin before and after the Wenchuan earthquake during the period from 2005 to 2015. In the study period, the degree of landscape fragmentation increased 1 year after the earthquake in 2009 but decreased in 2011 and 2015. The spatial distribution of forest, farmland, and shrub-grassland had significantly positive spatial correlation and the characteristics of spatial aggregation from 2005 to 2015. Construction land had no significant spatial correlation before the earthquake in 2005, but significantly positive spatial correlation after the earthquake, while traffic land had no significant spatial correlation across the study period. Unlike the other landscape types, geological disasters changed from a significantly negative spatial correlation before the earthquake to a significantly positive spatial correlation after the earthquake. However, the positive autocorrelation of all landscape types decreased with the increase of space distance, but in different distance-decay rates. The High-high spatial aggregation areas of geological disasters and construction land clustered gradually upstream of the basin and distributed in Longchi town, respectively; those of farmland distributed in the southeast of the basin increased in 2009 but then decreased, while those of forests had the opposite pattern as the dominant landscape type after the earthquake. Our results elucidated the spatial structure and distribution features of the Longxi River Basin to give a theoretical foundation for assessing the effects of ecological restoration and reconstruction management in earthquake-affected areas.

Objective: This study aimed to optimize the extraction process of essential oil from Cinnamomum cassia Presl. and reveal its effect on flavor compounds. Methods: The aqueous two-phase extraction of essential oil was optimized by orthogonal design, and the yield of the essential oil from Cinnamomum cassia Presl. was employed as the index to evaluate the outcome. The flavor compounds of cinnamon volatile oil were analyzed by Gas Chromatography-Mass Spectrometry (GC-MS) combined with principal component analysis. Results: The optimal extraction conditions were as follows: The mass fraction of ethanol 25%, the mass fraction of sodium dihydrogen phosphate 34%, the solid-liquid ratio 1:15 g/mL, the extraction time 60 min, and the temperature of extraction 35 ℃. Under these conditions, the yields of cinnamon volatile oil and cinnamaldehyde were 2.82% and 0.037%, which were 1.07 times and 1.31 times higher than those of the conventional steam distillation method. Furthermore, a total of 14 and 15 chemical compounds were identified in the essential oil of Cinnamomum cassia Presl. by two-phase extraction and steam distillation method, respectively. The results demonstrated that most of these compounds were terpenes and aldehydes. Compared with the conventional steam distillation method, the main components of the volatile oil prepared by aqueous two-phase extraction were distinct, and the latter contained more terpene components such as α-pinene (59.93%), cadina-1(10),4-diene (10.20%), α-muurolene (7.35%), cycloalfalfa (2.44%) and yilanene (1.50%). Conclusion: The aqueous two-phase extraction has several advantages of mild conditions, high yield and easy recovery of solvent for the extraction of cinnamon volatile oil. It can be used as an effective method for the extraction and separation of volatile oil from spices.

Abstract Background Coronary heart disease (CHD) is one of the most common causes of morbidity and mortality in type 2 diabetes mellitus (T2DM). Oxidative stress is one of the important contributors to the pathogenesis of CHD. Sestrin2 is a stress-induced antioxidant protein that plays a important role in T2DM and CHD. However, the relationship between serum Sestrin2 levels and T2DM with CHD remains unclear. Aim This study aimed to investigate the relationship between serum Sestrin2 levels and CHD in patients with type 2 diabetes. Methods A total of 70 T2DM patients with CHD and 69 T2DM patients were enrolled in this study. Clinical features and metabolic indices were identified. Serum Sestrin2 was measured by ELISA. Results Serum Sestrin2 levels in T2DM-CHD groups were significantly lower compared with the T2DM group (11.17 (9.79, 13.14) ng/mL vs 9.46 (8.34, 10.91) ng/mL). Abstract Background Long-term patterns of serum uric acid (SUA) and their association with the risk of myocardial infarction (MI) and mortality are poorly characterized as prior studies measured SUA at a single time point. This study aimed to identify SUA trajectories and determine their associations with incident MI and all-cause mortality. Methods We included 85,503 participants who were free of MI in or prior 2012 from the Kailuan study. SUA trajectories during 2006–2012 were identified by group-based trajectory modeling. Cox proportional hazard models were used to assess the association of SUA trajectories with MI and all-cause mortality. Results We identified three SUA trajectories during 2006–2012: low-stable (n=44,124, mean SUA: 236–249 μmol/L), moderate-stable (n=34,431, mean SUA: 324–354 μmol/L) and high-stable (n=6,984, mean SUA: 425–463 μmol/L). During a median follow-up of 6.8 years, we documented 817 (0.96%) incident MI and 6498 (7.60%) mortality. Compared with the low-stable group, high-stable group experienced a higher risk of MI (hazard ratio [HR], 1.35; 95% confidence [CI], 1.07–1.71) and all-cause mortality (HR, 1.22; 95% CI, 1.12–1.33). Multiple sensitivity analyses yielded similar results. Abstract Background Recent studies have suggested that triglyceride-glucose (TyG) index is an independent predictor of cardiovascular disease (CVD). However, the impact of long-term visit-to-visit variability in TyG index on the risk of CVD is not known. We aimed to investigate the longitudinal association between baseline and mean TyG index as well as TyG index variability and incident CVD in a Chinese population. Methods We included 49,579 participants without previous history of CVD in the Kailuan study who underwent three health examinations (2006, 2008, and 2010) and were followed up for clinical events until 2019. TyG index was calculated as Ln [fasting triglyceride (mg/dL) × fasting glucose (mg/dL)/2]. We measured TyG index variability as the SD of the residuals obtained from a linear regression on the three TyG index measurements for each individual. Multivariate-adjusted Cox models were used to estimate the adjusted hazard ratio (aHR) and 95% confidence interval (CI) with incident CVD. Results During a median follow-up time of 9.0 years, 2404 developed CVD. The highest tertile (T3) of baseline and mean TyG index were each associated with higher CVD incidence as compared with the lowest tertile (T1): aHR, 1.25; 95% CI 1.11–1.42; and aHR 1.40; 95% CI 1.24–1.58, respectively. Tertile 3 of TyG index variability was associated with increased CVD incidence compared to T1 group (aHR, 1.12; 95% CI 1.01–1.24). Similar findings were observed in a series of sensitivity analyses. Conclusion Higher TyG index level and greater TyGindex variability were each independently associated with a higher incidence of CVD.

Background and aims: Urine ketone bodies have been considered as predictors of stroke in diabetic patients, however, the role of urine ketone bodies in the prognosis of stroke has not been investigated well. This study aimed to investigate the association between urine ketone bodies and adverse outcomes in patients with acute ischemic stroke (AIS) or transient ischemic attack (TIA). Methods: This study enrolled 14 015 patients with AIS or TIA who screened for urine ketone bodies from the Third China National Stroke Registry. Status of urine ketone bodies were classified into negative, suspicious positive and positive. The outcomes were all-cause death and poor functional outcomes (modified Rankin Scale [mRS] 2–6/3-6) at 3 months and 1 year. Multivariable Cox proportional hazards regressions and logistic regressions were adopted to explore the associations. Results: Participants with negative, suspicious positive and positive urine ketone bodies were 12979 (92.61%), 480 (3.42%) and 556 (3.97%). After multivariate adjustment, patients with positive urine ketone bodies had a higher risk of all-cause death (hazard ratio, 1.74; confidence interval [CI], 1.07–2.83), a higher proportion of mRS score 2–6 (Odds ratio [OR], 1.85; 95% CI, 1.51–2.27), mRS score 3–6 (OR, 2.00; 95% CI, 1.61–2.48) at 3 months, compared to those with negative urine ketone bodies. Significant associations persisted at 1 year. Furthermore, there was no significant interaction of diabetes status and alcohol use with urine ketone bodies. Conclusions: Positive urine ketone bodies can independently predict all-cause death and poor functional outcomes in patients with AIS or TIA.

Chromium (Cr) is the seventh most abundant chemical element in the Earth’s crust, and Cr(III) and Cr(VI) are common stable valence states of Cr. Several Cr-containing substances, such as FeOCr2O3 and stainless-steel products, exist in nature and in life. However, Cr(VI) is toxic to soil, microorganisms, and plants and poses a serious threat to human health through direct and indirect exposure. By collecting published journal literature, we found that Cr(VI) can cause acute and chronic toxicity in organisms and has carcinogenic effects, and the mechanisms causing these toxicity include endoplasmic reticulum stress, autophagy and apoptosis. However, the relationship between these mechanisms remains unclear. Many methods have been researched to purify chromium, but each of these methods has its own advantages and disadvantages. Therefore, this review summarizes the hazards of chromium and the mechanisms of chromium toxicity after entering cells and provides a number of methods for chromium contamination management, providing a direction for the next step in chromium toxicology and contamination decontamination research.

In this paper, an effective synthesis method for sparse uniformly excited concentric ring array (CRA) element positions featuring a minimum sidelobe level (SLL) is presented. This method is based on the chaos sparrow search algorithm (CSSA), which can search for the optimal solution under multiple constraints. By improving the constraint on the number of array elements on each ring, the solution range of the optimal solution is further reduced, and the global search ability of the algorithm is effectively improved by introducing tent mapping into the algorithm to initialize the population. Numerical examples are presented to assess the effectiveness and reliability of the proposed method, showing that it can achieve better results than existing methods in the design of sparse concentric ring array arrangements.

Ovarian cancer (OC) is the seventh most prevalent type of cancer in women and the second most common cause of cancer-related deaths in women worldwide. Because of the high rates of relapse, there is an immediate and pressing need for the discovery of innovative sensitive biomarkers for OC patients. Using TCGA and GSE26712 datasets, we were able to identify 17 survival-related DEGs in OC that had differential expression. CLDN4 was the gene that caught our attention the most out of the 17 important genes since its expression was much higher in OC samples than in nontumor samples. The findings of the ROC assays then confirmed the diagnostic utility of the test in screening OC specimens to differentiate them from nontumor specimens. Patients with high CLDN4 expression predicted a shorter overall survival (OS) and disease-specific survival (DSS) than those with low CLDN4 expression, according to clinical research. Patients with low CLDN4 expression predicted longer OS and DSS. Analysis using both univariate and multivariate techniques revealed that CLDN4 expression was an independent factor associated with a poor prognosis for OS and DSS. Based on multivariate analysis, the C-indexes and calibration plots of the nomogram suggested an effective predictive performance for OC patients. After that, we investigated whether or not there was a link between the infiltration of immune cells and the expression of the CLDN4 gene. We found that the expression of CLDN4 was positively associated with Th17 cells, NK CD56bright cells, while negatively associated with Th2 cells, pDC, and T helper cells. In the end, we carried out RT-PCR on our cohort and confirmed that the level of CLDN4 expression was noticeably elevated in OC specimens in comparison to nontumor tissues. The diagnostic usefulness of CLDN4 expression for OC was also validated by the findings of ROC tests. Thus, our findings revealed that CLDN4 may serve as a predictive biomarker in OC to assess both the clinical outcome of OC patients and the level of immune infiltration.

Metabolic profiling using nonsputum samples has demonstrated excellent performance in diagnosing infectious diseases. But little is known about the lipid metabolism alternation in children with tuberculosis (TB). Therefore, the study was performed to explore lipid metabolic changes caused by Mycobacterium tuberculosis infection and identify specific lipids as diagnostic biomarkers in children with TB using UHPLC-MS/MS. Plasma samples obtained from 70 active TB children, 21 non-TB infectious disease children, and 21 healthy controls were analyzed by a partial least-squares discriminant analysis model in the training set, and 12 metabolites were identified that can separate children with TB from non-TB controls. In the independent testing cohort with 49 subjects, three of the markers, PC (15:0/17:1), PC (17:1/18:2), and PE (18:1/20:3), presented with high diagnostic values. The areas under the curve of the three metabolites were 0.904, 0.833, and 0.895, respectively. The levels of the altered lipid metabolites were found to be associated with the severity of the TB disease. Taken together, plasma lipid metabolites are potentially useful for diagnosis of active TB in children and would provide insights into the pathogenesis of the disease.

IntroductionAsthma is primarily divided into two categories: type 2 (T2-high) and non-type 2 (T2-low). A relationship between asthma severity and vitamin D deficiency has been identified, but its impact on each asthma endotype remains unknown.MethodsWe clinically examined the influence of vitamin D on patients with T2-high (n = 60) or T2-low asthma (n = 36) compared with controls (n = 40). Serum 25(OH)D levels, inflammatory cytokines and spirometry were measured. Mouse models were then used to further analyze the effects of vitamin D on both asthmatic endotypes. BALB/c mice were fed with vitamin D-deficient (LVD), -sufficient (NVD), or -supplemented diets (HVD) throughout lactation and offspring followed the same diet after weaning. Offspring were sensitized/challenged with ovalbumin (OVA) to establish “T2-high” asthma or OVA combined with ozone exposure (OVA + ozone) to induce “T2-low” asthma. Spirometry and serum, bronchoalveolar lavage fluid (BALF), and lung tissues were analyzed.ResultsSerum 25(OH)D levels were decreased in asthmatic patients compared with controls. Patients with vitamin D deficiency (Lo) had varying degrees of elevation of the pro-inflammatory cytokines IL-5, IL-6, and IL-17A, decreased expression of the anti-inflammatory cytokine IL-10, and altered forced expiratory volume in the first second as a percentage of predicted value (FEV1%pred) in both asthmatic endotypes. Vitamin D status had a stronger correlation with FEV1%pred in T2-low asthma than T2-high asthma, and 25(OH)D level was only positively linked to maximal mid-expiratory flow as a percentage of predicted value (MMEF%pred) in the T2-low group. Inflammation, hyperresponsiveness, and airway resistance (RL) was increased in both asthma models compared with controls while vitamin D deficiency further increased airway inflammation and airway obstruction. These findings were particularly prominent in T2-low asthma.DiscussionThe potential function and mechanisms of vitamin D and both asthma endotypes should be studied individually, and further analysis of the potential signaling pathways involved with vitamin D on T2-low asthma is warranted.

Objectives: Our study analyzed the performance of Xpert MTB/RIF Ultra (Ultra) on stool and gastric aspirate (GA) samples for the diagnosis of pediatric pulmonary tuberculosis (TB) caused by Mycobacterium tuberculosis in a high-burden area of China. Methods: Children with presumptive TB were enrolled in two hospitals in Sichuan Province (July 2019–Oct 2020). Because of the unavailability of sputum for etiological testing, gastric samples were aspirated and tested by bacterial culture, acid-fast bacillus microscopy, and Ultra. Stool samples were tested simultaneously using Ultra and Xpert. Results: In total, 141 children with active TB and 34 with non-TB respiratory tract infections were enrolled. Ultra-stool (60.3%, 85/141) and Ultra-GA (52.5%, 74/141) tests were similarly sensitive (p = 0.187). Among the subset of 48 children with confirmed TB, Ultra testing was equally sensitive on stool and GA samples (85.4%, 41/48). The agreement between Ultra-stool and Ultra-GA was moderate in children with active TB (kappa value = 0.527). After integrating Ultra-GA and Ultra-stool outcomes, 70.9% (100/141) of the children were considered to have confirmed TB. The specificities of Ultra-stool and Ultra-GA were 97.1% (33/34) and 100% (34/34), respectively (p = 0.314). Conclusions: In children, stools can be used as alternative samples to GAs for Ultra tests. Stool- and GA-based Ultra tests are appropriate for bacteriological TB confirmation.

Abstract Background The impact of long-term serum uric acid (SUA) exposure and time course of SUA accumulation on diabetes mellitus (DM) is unknown. This study aimed to evaluate the association of cumulative SUA (cumSUA) exposure and its accumulation time course with risk of DM. Methods This prospective study included 46,434 participants without DM and underwent three examinations at 2006, 2008, and 2010. CumSUA from 2006 to 2010 was calculated, multiplying mean values between consecutive examinations by time intervals between visits. Time course of SUA accumulation was categorized as the slope of SUA versus time from 2006 to 2010, or by splitting the overall accumulation into an early (cumSUA06-08) and late accumulation (cumSUA08-10). Results During 6.99 years of follow-up, we identified 2971 incident DM cases. In the fully adjusted model, a higher risk of DM was observed in participants with the highest quartile of cumSUA (hazard ratio [HR], 1.31; 95% confidence interval [CI], 1.17–1.46), cumulative burden >0 (HR, 1.23; 95% CI, 1.08–1.40), and with 6 year of hyperuricemia exposure duration (HR, 1.25; 95% CI, 1.01–1.55). When considering the time course of SUA accumulation, participants with a negative slope (HR, 1.05; 95% CI, 1.01–1.12), or combined with cumSUA ≥ median and a negative slope had elevated risk of DM (HR, 1.58; 95% CI, 1.18–2.11). Conclusions Incident DM risk depends on cumulative exposure of SUA and time course of SUA accumulation. Early SUA accumulation resulted in a greater risk increase compared with later accumulation, emphasizing the importance of optimal SUA control early in life.
