The effect of pre-stretching and under-aging treatment on microstructure and fatigue crack resistance of Al-4.1Cu-1.27Mg-0.47Mn-0.02Ti alloy has been investigated in present work. The results showed that the fatigue strength corresponding to 107 stress cycle numbers of the sample pre-stretched 4% and under-aged at for 2 hours reached up to 280MPa, 80MPa higher than that pre-stretched 0% and peak-aged at for 8 hours. Microstructural observations showed that fine and uniformly dispersed S’ plates were present in the sample treated by pre-stretching and under-aging, whereas, a coarse S’ plate was formed in the sample when treated by 0% pre-stretching and peak-aging. It is indicated that the pre-stretching and under-aging treatment promotes the dispersoid formation of strengthening particles, enhancing the fatigue crack resistance and resultant fatigue strength. The effect of perioperative acupuncture on accelerating gastrointestinal function recovery has been reported in colorectal surgery and distal gastrectomy (Billroth-II). However, the evidence in pancreatectomy and other gastrectomy is still limited. A prospective, randomized controlled trial was conducted between May 2018 and August 2019. Consecutive patients undergoing pancreatectomy or gastrectomy in our hospital were randomly assigned to the electroacupuncture (EA) group and the control group. The patients in the EA group received transcutaneous EA on Bai-hui (GV20), Nei-guan (PC6), Tian-shu (ST25), and Zu-san-li (ST36) once a day in the afternoon, and the control group received sham EA. Primary outcomes were the time to first flatus and time to first defecation. In total, 461 patients were randomly assigned to the groups, and 385 were analyzed finally (EA group, n = 201; control group, n = 184). Time to first flatus (3.0 ± 0.7 vs 4.2 ± 1.0, P < 0.001 ) and first defecation (4.2 ± 0.9 vs 5.4 ± 1.2, P < 0.001 ) in the EA group were significantly shorter than those in the control group. Of patients undergoing pancreatectomy, those undergoing pancreaticoduodenectomy and intraoperative radiation therapy (IORT) surgery benefitted from EA in time to first flatus ( P < 0.001 ) and first defecation ( P < 0.001 ), while those undergoing distal pancreatectomy did not ( P flatus = 0.157 , P defecation = 0.007 ) completely. Of patients undergoing gastrectomy, those undergoing total gastrectomy and distal gastrectomy (Billroth-II) benefitted from EA ( P < 0.001 ), as did those undergoing proximal gastrectomy ( P = 0.015 ). Patients undergoing distal gastrectomy (Billroth-I) benefitted from EA in time to first defecation ( P = 0.012 ) but not flatus ( P = 0.051 ). The time of parenteral nutrition, hospital stay, and time to first independent walk in the EA group were shorter than those in the control group. No severe EA complications were reported. EA was safe and effective in accelerating postoperative gastrointestinal function recovery. Patients undergoing pancreaticoduodenectomy, IORT surgery, total gastrectomy, proximal gastrectomy, or distal gastrectomy (Billroth-II) could benefit from EA. Objective This study aims to evaluate the safety and efficacy of lobaplatin in prophylactic hyperthermic intraperitoneal chemotherapy (HIPEC) for advanced gastric cancer. Methods Advanced gastric cancer patients who underwent radical gastric resection and/or prophylactic HIPEC were systematically reviewed in our department from January 2016 to June 2017. All enrolled patients were grouped in either HIPEC or non-HIPEC groups. Clinical data were collected and analyzed. Results A total of 129 patients were enrolled with 61 cases in the HIPEC group and 68 in the non-HIPEC group. The two groups were well balanced in terms of clinical characteristics. In patients of the HIPEC group, three suffered leakage from the duodenal stump or anastomosis, one suffered abnormal bleeding and two were found to have abnormal routine blood tests; no significant difference in adverse events between groups, however, was noted (p > 0.05) and most patients recovered uneventfully. During follow-up, peritoneal recurrence was significantly less among HIPEC patients (p = 0.029), with only three suffering peritoneal recurrence, as compared to 12 non-HIPEC patients. In addition, the estimated illness-specific 3-year disease-free survival rate was significantly higher in the HIPEC group as compared to the non-HIPEC group (89.4% vs.73.9%; p = 0.031). Conclusion Lobaplatin in prophylactic HIPEC is safe for advanced gastric cancer patients after treatment by radical resection and can effectively improve illness-specific 3-year disease-free survival.

Background. The prognostic relevance of gastric tumor location has been reported and debated. Our study was conducted to examine the differences in clinicopathological features, prognostic factors, and overall survival (OS) between patients with proximal gastric cancer (PGC) and distal gastric cancer (DGC). Patients and Methods. Patients with PGC or DGC were identified from the China National Cancer Center Gastric Cancer Database (NCCGCDB) during 1997–2017. Survival analysis was performed via Kaplan-Meier estimates and Cox proportional hazards models. Results. We reviewed 16,119 cases of gastric cancer patients, including 6,479 of PGC and 9,640 of DGC. PGC patients presented as older patients (61.5 versus 56.4 years, P<0.001) and more males (82.9% versus 68.2%, P P P Conclusion. In China, PGC significantly differed from DGC in clinicopathological characteristics and prognostic factors. However, there was no significant relationship between survival outcome and gastric tumor location.

This study investigated the correlation between the expression of 3-phosphoinositide-dependent protein kinase-1 (PDK1) and the prognosis of gastric carcinoma patients. A total of 156 paired tumor and matched normal samples were collected from patients of gastric carcinoma who underwent surgical resection. The expression of PDK1 was analyzed by real-time quantitative PCR and immunohistochemistry method. Potential correlation between PDK1 protein expression and the clinicopathological characteristics was determined by chi-square test and Spearman correlation analysis. The influence of PDK1 expression on 5-year survival rate and survival length was determined by Kaplan-Meier analysis. The expression of PDK1 mRNA and protein were significantly higher in tumor samples comparing to those in adjacent normal samples (paired t test, P = 0.007). Immunohistochemical staining results indicated that PDK1 protein level was positively correlated with infiltration (P = 0.006). However, no associations with age, sex, clinical stage, lymph node metastasis, and distant metastasis were observed (P > 0.05). The study determined the biogas production potential of different concentrations of biogas slurry co-digested with cornstalk at the TS of 8%. It turned out that pure biogas slurry mixed with cornstalk had the best performance. Besides, using biogas slurry without adding any water could keep the pH of the system at 6.9-7.2, while acidification happened when using other concentrations of biogas slurry and the pH went below 6.4.

Seepage properties of fractured rock mass are of prime importance for hydraulic engineering and accurate description of rock fracture geometry parameters is an important and basic task in rock hydraulics. In this paper, an improved K-means clustering algorithm for structural plane of fractured rock mass was first brought forward and the corresponding Matlab program for discontinuity orientations partitioning was compiled and then used in the fitting analysis of dominant orientations of certain dam foundation rock mass. On this basis, combining calculation formulas of multi-group fractures, the permeability tensor and principle value was calculated for the actual dam foundation. The results provide a theoretical and computational reference for other similar projects.

The main objectives of this research were to determine the effect of leachate refluence on biogas production for dry mesophilic co-fermentation of chicken manure and corn straw. The biogas production, the ratio of biogas production, methane content and pH were analyzed. The results showed that the leachate refluence has a significant impact on biogas production of dry co-fermentation. The cumulative biogas yield of the once in 48h has an increase by 10% and 5% for no reflux and once in 24h, respectively. The leachate refluence has little influence on the methane content, but it has good effect to keep pH in the optimum rang.

A well-known observation with respect to cancer biology is that transformed cells display a disturbed cytoskeleton, The underlying mechanisms, however, remain only partly understood. In an effort to identify possible mechanisms, we compared the proteome of pancreatic cancer with matched normal pancreas and observed diminished protein levels of gelsolin-an actin filament severing and capping protein of crucial importance for maintaining cytoskeletal integrity-in pancreatic cancer. Additionally, pancreatic ductal adenocarcinomas displayed substantially decreased levels of gelsolin as judged by Western blot and immunohistochemical analyses of tissue micoarrays, when compared with cancerous and untransformed tissue from the same patients (P 0.05), suggesting that post-transcriptional mechanisms mediate low gelsolin protein levels. In apparent agreement, high activity ubiquitin-proteasome pathway in both patient samples and the BxPC-3 pancreatic cancer cell line was detected, and inhibition of the 26s proteasome system quickly restored gelsolin protein levels in the latter cell line. The status of ubiquitinated gelsolin is related to lymph node metastasis of pancreatic cancer. In conclusion, gelsolin levels are actively downregulated in pancreatic cancer and enhanced targeting of gelsolin to the ubiquitin-proteasome pathway is an important contributing factor for this effect.

The GaAs photocathode has widely been used in optoelectronic devices such as image intensifiers, photomultiplier tubes, but these devices is inevitable to withstand a variety of mechanical vibration. In order to study the mechanical vibration impact on the photoemission performance of GaAs photocathode, GaAs photocathode image intensifier is researched in this paper. The spectral response of the GaAs photocathode before and after 5~55Hz scan frequency, 14Hz, 33Hz, 52Hz stay frequency, 5～60Hz scan frequency mechanical vibration respectively was tested, then the parameter of photocathode was calculated by MATLAB software according to quantum efficiency formula, the quantum efficiency curve were fitted. The results show that surface escape probability is increased after photocathode is subjected to mechanical vibration, so that its photoemission performance will be improved. We think this phenomenon is due to the GaAs photocathode surface Cs-O reconstruction. This finding provided a new method to enhance the photoemission performance of photocathode.

A new type of ultra-wideband (UWB) printed monopole antennas is presented, which is composed of a circular or annular patch and a trapeziform ground plane with a tapered CPW feeder in the middle. Both simulated and experimental results are presented, showing good agreement and therefore verifying validity of the design. The proposed antenna with a circular monopole patch achieves an 11.6:1 measured ratio bandwidth of VSWR ⩽2 (from 0.79 GHz to 9.16 GHz), while that with an annular patch obtains a measured ratio impedance bandwidth of 10.6:1 (from 0.87 GHz to 9.47 GHz). In addition, these designs exhibit nearly omnidirectional radiation patterns with simple compact structures, which are attractive in communications and others UWB applications.

A leaf-shaped monopole antenna with an extremely wide bandwidth is introduced. The antenna is composed of a leaf-shaped metal sheet with three circular holes vertically mounted on the ground plate covered by a dielectric layer. Its measured −10-dB return-loss frequency range is from 1.3 to 29.7 GHz with ratio bandwidth of about 22:1. This antenna provides bandwidth covering the L, C, X, and Ku bands and exhibits a nearly omnidirectional radiation pattern with a simple structure, which is suitable for various military and commercial ultra-wideband applications. © 2006 Wiley Periodicals, Inc. Using the projected augmented wave potential by the density functional theory based upon gradual gradient approach method and the slab model, from the calculated surface, we identify the relaxed atoms sites of GaAs(110) surface, the electronic structure of elements K and O adsorpted on binding sites of ideal GaAs(110) surface have also been calculated, especially the total energy of the adsorption system. The comparison results of calculated total energy showed: for K and O elements at highest coverage of Θ=1ML on GaAs(110) surface, they were not formed to local domain of competitive chemical adsorption, while they were formed to a compound uniformity phase of cooperative chemical adsorption. Our calculated results providing theoretical basis and reference for the application of alkali oxidation adsorpted on GaAs surface to form a negative electron affinity photocathode.

To study the feasibility and clinical value of 13th lymph nodes in predicting general lymph nodes metastases for periampullary carcinoma.A total of 77 patients with pathologically confirmed periampullary carcinoma were recruited. And 26 (18 males and 8 females, age 38-79 years) of them underwent Whipple procedures during which 1% methylene blue or nanogate carbon was injected into tumor bed. The other 51 patients as controls (33 males, 18 females, age 38-78 years) . The dyed 13th lymph nodes were biopsied. Then routine lymphadenectomy was performed and their pathological results were analyzed.In the experimental group, the lymph node drainage area of cancer was identified. However, in contrast with 51 patients in the control group, the labeling of 13th lymph nodes did not significantly increase the number of cleaned lymph nodes (15.8 (3-54) vs 17.6 (6-40) , P = 0.460).13th lymph nodes are non-suitable as sentinels for predicting general lymph nodes metastases of periampullary carcinoma.

Postoperative hospital stay after pancreaticoduodenectomy (PD) is relatively longer than after other gastrointestinal operations. The aim of the current study was to investigate the risk factors associated with prolonged hospital stay after PD.Patients who had PD at the Cancer Hospital of Chinese Academy of Medical Sciences between December 2008 and November 2012 were selected for this retrospective study. Clinical and pathological data were collected and analyzed. The primary outcome was postoperative length of stay. Normal discharge or recovery was defined as a postoperative hospital stay of no more than 10 days; otherwise it was defined as delayed discharge or recovery (including hospital death).A total of 152 patients were enrolled in the present study. Postoperative hospital stay was (19.7 ± 7.7) days (range 7-57). Of the 152 patients, 67 were discharged within the normal time and 85 had delayed discharge. Postoperative complications occurred in 62.5% (95/152), and the mortality rate was 3.29% (5/152). Multiple regression analysis showed that delayed discharge was significantly associated with postoperative complications (adjusted odds ratio (OR) 10.40, 95% confidence interval (CI) 3.58-30.22), age (adjusted OR 4.09, 95% CI 1.16-14.39), body mass index (BMI) (adjusted OR 4.40, 95% CI 1.19-16.23), surgical procedure (adjusted OR 26.14, 95% CI 4.94-153.19), blood transfusion (adjusted OR 7.68, 95% CI 2.09-28.27), and fluid input (adjusted OR 3.47, 95% CI 1.24-11.57).Postoperative complications increase the time to postoperative hospital discharge. The length of hospital stay after PD is also associated with age, BMI, blood transfusion, surgical procedure, and fluid input. Further studies with more patients are needed in future.

With the rapid development of computer technology and mobile devices, high requirements for computer vision are more and more critical. In particular, current research on moving target detection based on still scene or hardware platform with turntable and gyroscope, can not meet the requirements of portable mobile equipment. Moving target detection and tracking on mobile platforms is attracting more and more attention. What makes the task even more challenging is when the camera is non-stationary ,due to the random motion of camera caused by bumps and swings of vehicle and handheld, and parallax problem caused by 3D scene. The essential problem in this case lies in distinguishing between global motion induced by camera and independent motion caused by moving targets. To solve above problems, the three-dimensional reconstruction method based on camera calibration technology is always introduced, such as the fundamental matrix and trifocal tensor, but that are appropriate for large classes of problems and situations without considering the complexity and speed of processing. This paper proposes a new robust algorithm GMOS (Global Motion Of Scene) based on the global motion of scene for moving object detection on a freely moving camera. By modeling for GMOS with the adjacent optical flow field of the image sequence, the proposed method is able to detect and separate the moving targets simply and fast from the global motion model without three-dimensional reconstruction. According to the GMOS model, we can describe the movement of the camera through a GMOS vector which is independent of two viewpoints between the adjacent images, and compensate for the overall movement caused by camera movement. The results of theory analysis and experimentations on numerous real world videos demonstrate that the proposed method GMOS could separate the independent objects fast and robustly under the premise of the high accuracy and robustness.

It introduces a new method to achieve the passenger flow statistics in stereo vision according to the original depth image output by the monocular Xtion sensor, aiming at the problem of algorithm with large amounts of data and realization of single field with dual camera on the basis of stereo vision. Double Xtion sensors are used to expand the range of view angle because of the monocular Xtion sensor’s limitations, whose view range is 45°*58° with small transverse view range and can’t meet the passenger flow statistics. Due to the characteristics of constant physical space dimensions, use the improved SIFT (Scale Invariant Features Transform) feature algorithm to realize the auto - stereoscopic splice of binocular original depth images. Firstly, the feature points of the reference image (the image to be matched) and the subsequent image (the image to be matched with the reference image) are obtained by SIFT algorithm, getting the location, scale and direction of the feature points and the feature points are described by means of the 128-dimensional vector .Secondly, complete the match of the feature points of the two images to calculate overlapping area, using the nearest neighbor method. Finally, image stitching is completed based on multi-resolution wavelet transform, which contains three-dimensional spatial information of the human body, thus use a method to analysis comprehensively the depth image for field detection and tracking based on the features such as the head shape, the head area the spatial position relation of the human head and shoulder and so on. The experimental results show that this method not only improve the detection accuracy and efficiency, reduce the amount of operation data, so that the system is simple in structure, but also solve many problems of passenger flow statistics based on video stream in the system, accuracy up to 93%, having high and practical application value.

In order to improve the measurement accuracy of MCP current gain and provide highly reliable feedback to the MCP production, based on MCP current gain test principle, a device with adjustable incident illumination for measuring the MCP current gain was established. Under MCP voltage of 800V, 900V, and 1000V, incident illumination of photocathode varying from 0.1mlx to 8mlx, MCP current gain by this device was measured. According to the curves of current gain, we knew more about the relationship among MCP current gain, MCP voltage and photocathode incident illumination. It was indicated that MCP current gain saturation was not obvious with photocathode incident illumination less than 6mlx and MCP voltage 800V, while with more than 4mlx and 900V, the MCP current gain began to saturate, and with more than 1mlx and 1000V, the MCP current gain began to saturate. Owing to the characterizations of MCP, the reason why the MCP current gain decreased was analyzed. With the MCP voltage of the 3rd generation low-light-level image intensifier tube ranging between 800V and 1000V, the photocathode incident illumination for MCP current gain measurement should be less than 1mlx. In this study, the determined range of photocathode incident illumination during MCP current gain measurement and the improved test accuracy of the MCP current gain provides effective reference for the production procedure of the 3rd generation low-light-level image intensifier tube.

To study the chemical effect of direct current arc plasma igniter, the emission spectrum of plasma jet was measured, and the active particles produced by the interaction of plasma jet with atmospheric air were analyzed. The NO and CO volume fractions were measured quantificationally by smoke analyzer at the 8cm downstream the plasma igniter exit, and the changing law between arc current and NO, CO volume fractions was obtained. The results show that the plasma jet interacting with atmospheric air produced active particles (H, O, N), charged particles (O2 +, N2+), and excited particles (N2 (A3), N2 (B3), N2 (C3), N2 (a1), O2 (a1), O2 (b1)). The NO and CO volume fractions increased with rising of are current and feedstock argon flow rate.

To evaluate the value of sentinel lymph node biopsy in diagnosis of cervical metastasis in patients with early oral squamous cell carcinoma.Cochrane library, evidence-based medicine (EBM), PubMed, China national knowledge internet (CNKI) were searched for 2001 - 2011 literatures on sentinel lymph node biopsy in patients with oral squamous cell carcinoma as early diagnosis of cervical metastasis. The results were analysed by using Meta-analysis software Metadisc1.4.There were 12 studies in which, a total of 793 patients with oral squamous cell carcinoma were involved in Meta-analysis. The combined sensitivity and specificity were 0.86 (95%CI: 0.81 - 0.90) and 0.99 (95%CI: 0.98 - 1.00) respectively.For early oral cancer patients, the sensitivity and specificity of sentinel lymph node biopsy are relatively high. The sentinel lymph node biopsy could be used to select which patients need neck dissection.

To summarize the experiences in the treatment of complicated orbital fractures.A total of 89 patients with complicated orbital fractures treated in Department of Oral and Maxillofacial Surgery, China Medical University from January 2005 to January 2010 were retrospectively reviewed. The classification of these cases included naso-orbital-ethmoid fracture, frontal orbital fracture and orbitozygomatic fracture. All patients were followed up for 6 - 36 months.The orbital frame was repaired or reconstructed in these patients. The function of lacrimal pathways was improved. All the patients and the physicians were satisfied with the surgical effects. However, recurrence of deformity after endophthalmas correction was found in several cases.The experiences, comprehensive management of complicated orbital fractures by team approaches, concluded from this study could be expanded. There are still challenges in the treatment of complicated orbital fractures, such as severe endophthalmas deformity, recurrence of endophthalmas deformity and malunion of complicated orbital fracture.

To assess the value of intraoperative radiotherapy (IORT) in the combined treatment of locally advanced pancreatic cancer.All patients with locally advanced pancreatic cancer at our hospital from January 2007 to December 2009, judged as unresectable and confirmed by histology or cytology, were recruited into this prospective study. They were randomly assigned into the IORT group (n=31) and control group (n=34). The IORT group received IORT plus internal drainage or laparotomy. The control group had internal drainage or laparotomy only. The evaluation of adverse results of two groups included: intraoperative and postoperative adverse events, recent post-operative side effects, analgesic effect, the level of tumor marker such as CA19-9 and the long-term survival.There was no difference in operation duration, intraoperative hemorrhage and postoperative recovery. Significant differences were found in hematotoxicology, analgesic effect, tumor marker decreasing and long-term survival.IORT is a safe, reliable and easy-to-master technique without any obvious side effect. Its analgesic effect is better than the control group. Also IORT can retard the tumor growth and improve the patient survival.

To investigate the classification of alar base depression, so as to provide the reference for the surgical management of secondary nasal deformity of unilateral cleft lip.From Jul. 2008 to Feb. 2009, 26 cases with secondary deformity of unilateral cleft lip were treated. All the patients underwent 3-dimensional CT for maxillary measurement. The nasal soft tissue measurement was performed pre- and post-operatively. The relationship between the maxillary and soft tissue at alar base was analyzed. The nasal deformity was classified.The location of alar base was not related to the form of piriform aperture, but the bony defect at the alar base was correlated to the patient satisfactory. The nasal deformity was graded as I when the depression at alar base was less than 4.5 mm in depth, as II when it was 4.5-5.0 mm in depth, and as III when it was more than 5 mm in depth. The deformity could be corrected with only soft tissue plasty for grade I, with soft tissue plasty or artificial implants for grade II, with combined bone autograft or alveolar cleft repair for grade III.The depression at maxillary does not necessarily result in alar base depression. The alar base can be adjust to proper position through operation. The operation should be designed based on the preoperative nasal measurement.

To compare the difference of clinicopathological characteristics between colorectal signet-ring cell carcinoma and mucinous adenocarcinoma.The clinicopathological and survival data of 65 patients with colorectal signet-ring carcinoma and 166 with mucinous adenocarcinoma were retrospectively analyzed.Such clinical characteristics as gender, gross anatomical classification, preoperative carcinoembryonic antigen level and hepatic metastasis or not had no significant difference between two groups (P0.05) while the difference of such characteristics as age, location of tumor, vascular tumor embolus, N stage, T stage, AJCC stage, preoperative obstruction and the ratio of radical resection between them was significant (P0.05). The overall 3, 5-year survival of the whole group was 56.7% and 31.6% respectively. The 3, 5-year survival and median survival time (MST) in the signet-ring cell carcinoma and the mucinous adenocarcinoma groups were 33.1%, 14.8%, 24.0 months and 64.1%, 36.6%, 41.5 months respectively. The pathological type of signet-ring cell carcinoma was an independent risk factor of survival in the whole group.Compared to colorectal mucinous adenocarcinoma, signet-ring cell carcinoma has a higher degree of malignancy and the patients have a worse survival.

To study the role of slow-release 5-fluorouracil implantation in treatment of unresectable pancreatic cancer.85 cases of untreated patients with locally advanced pancreatic cancer (LAPC) were randomized into two groups: Trial group: slow-release 5-fluorouracil implantation (50 patients) and control group (35 patients). Observing the objective tumor response, clinical benefit response, toxicity, complications and survival of patients of the two groups.In the trial group the overall response rate (PR + NC) was 76.0%, and the clinical benefit response rate was 52.0%. No toxicity was observed. Pancreatic fistula occurred in 2 patients. The median survival time of the two groups was 9.0 months and 4.0 months, respectively. The survival rates of 6- and 12-month were 56.8% vs. 31.4% and 22.9% vs. 2.9% in the two groups, respectively (P = 0.012).Slow-release 5-fluorouracil implantation is a simple, safe and effective method in treatment of LAPC.

To study the role of (125)I seed implantation in the treatment of unresectable pancreatic cancer.From April 2004 to march 2006, 66 untreated patients with locally advanced pancreatic cancer (LAPC) were randomized into two groups: Group A: (125)I seeds implantation (n = 31) and Group B: control (n = 34). The objective tumor response, clinical benefit response, toxicity, complications and survival of two groups were observed.In Group A, the overall response rate (PR + NC) was 80.6%. Clinical benefit response rate was 54.8%. No toxicity was observed. Gastrointestinal hemorrhage and pancreatic fistula occurred in 1 patient respectively in Group A. The survival rates of 6 and 12 months were 56.0% vs 31.4% and 16.8% vs 2.9% respectively in two groups (P0.05). The median survival time of two groups was 8.0 months vs 4.0 months (P0.05).(125)I seed implantation is a simple, safe and effective method in the treatment of locally advanced pancreatic cancer.

To assess the risk factors of stress-related ulcer and gastrointestinal hemorrhage after pancreaticoduodenectomy.From May 1999 to July 2007, 285 periampullary cancer patients underwent pancreaticoduodenectomy in our hospital. The clinical data, pathological results, type of operation, and postoperative treatment were retrospectively analyzed. Patients with stress-related ulcer and gastrointestinal hemorrhage were selected for risk factor analysis, and other patients were taken as control group.35 patients (12.3%) developed stress-related ulcer and gastrointestinal hemorrhage following pancreaticoduodenectomy. Pathological examination showed pancreatic cancer in 5 cases, duodenal cancer in 8, common bile duct cancer in 10, ampullary carcinoma in 11, and solid-pseudopapillary tumors in 1. Single variate analysis demonstrated that alcohol, preoperative bilirubin level, operation time, lymph node metastasis, prealbumin decrease after operation and other complication were significantly associated with the stress-related ulcer and gastrointestinal hemorrhage. Logistic regression in multivariate analysis revealed that preoperative bilirubin level, operation time, other complication, prealbumin decrease after surgery were independent risk factors.Stress-related ulcer and gastrointestinal hemorrhage are one of the most common complications after pancreaticoduodenectomy. Preoperative bilirubin level, operation time, other complications, and prealbumin decrease after operation are four independently risk factors.

To investigate the method and value of tru-cut biopsy (TCB) combined with fine needle aspiration biopsy (FNAB) in the pathological diagnosis of pancreatic carcinoma during operation.From April 2007 to October 2008, 22 cases who were suspected to suffer from pancreatic carcinoma were enrolled into this prospective study. All of them underwent a tru-cut biopsy combined with fine needle aspiration biopsy for the pathological diagnosis during operation.Of the 22 patients, 20 were finally diagnosed as having pancreatic carcinoma, while 2 having pancreatitis. The diagnosis of pancreatic carcinoma was confirmed in 19 by tru-cut biopsy combined with fine needle aspiration biopsy, while other 3 cases were not confirmed as pancreatic carcinoma. Among those 3 cases, one was diagnosed as having pancreatic carcinoma with hepatic metastasis by liver nodular biopsy, one as suffering from autoimmune pancreatitis, and another case as having chronic pancreatitis confirmed by follow-up for 9 months without any changes after the operation. The accuracy of FNA, TCB and FNA combined with TCB in the diagnosis for suspected pancreatic cancer were 86.4%, 90.9%, and 95.5%, respectively. No pancreatic fistula and bleeding developed after operation.Tru-cut biopsy is more accurate in diagnosis for the suspected pancreatic cancer than fine needle aspiration biopsy during operation. Tru-cut biopsy combined with fine needle aspiration biopsy can improve the accuracy of diagnosis, and is a safe and effective diagnostic method.

To investigate the factors influencing recurrence and metastasis following curative resection of pancreatic ductal adenocarcinoma and analyze the prognosis.The clinicopathological and follow-up data of 56 patients who underwent curative resection for pancreatic ductal adenocarcinoma between Jan. 1997 and Dec. 2006 in this hospital were analyzed retrospectively.The recurrence rate after curative resection was 73.2% (41/56). The recurrence rate after operation at the time of 3 months, half year, 1 year and 2 years was 26.8% (15/56), 51.8% (29/56), 64.3% (36/56) and 69.6% (39/56), respectively. Hepatic metastasis and local recurrence accounted for 36.6% and 31.7% of the cases, respectively. The 3-year accumulated survival of this group was 22.7%. The symptom presenting time, back pain, preoperative level of CA19-9, tumor size, AJCC stage and T stage were correlated with metastasis/recurrence. Univariate analysis revealed that the preoperative level of CA19-9, T stage and the tumor size were prognostic factors. Cox regression analysis revealed that only tumor size was an independent prognostic factor.The metastasis or recurrence mostly occurs within 2 years after curative resection, and the liver is the most common site of metastasis. High recurrence rate is the major reason causing the failure of curative resection and short survival time after operation. The symptom presenting time, back pain, preoperative level of CA19-9, tumor size, AJCC stage and T stage are correlated with metastasis/recurrence. The tumor size is an independent prognostic factor.

To analyze the clinicopathological features and their relation to treatment and prognosis in different gastric carcinoid subtypes.The data of surgically treated 39 patients with gastric carcinoids (9 of type I and 30 of type III) were retrospectively analyzed. Univariate and multivariate analysis were performed using Chi square test (chi(2)) and Cox model, respectively. The survival rates were analyzed by Kaplan-Meier method, and the factors affecting survival by Log rank test.Of the 9 patients with type I carcinoids, 5 underwent endoscopic or surgical resection, and extra antrectomy was performed in 2 patients simultaneously. 3 cases had a proximal gastrectomy, and 1 underwent total gastrectomy. Among the 30 patients with type III gastric carcinoids, 21 underwent radical resection, 6 had a palliative resection, and the remaining 3 underwent exploration and biopsy only due to invasion into adjacent organs and distant metastasis. Infiltration beyond the submucosa was found in all 30 type III gastric carcinoid patients, but in only 1 of 9 patients with type I gastric carcinoids. Regional lymph node metastases were found in 27 of 30 type III carcinoid cases, but in none of type I. Distant metastases occurred in 5 patients of type III carcinoid (4 in the liver and 1 in the ovary). There were statistically significant differences between type I and type III carcinoids in the sex, tumor number, location, size and infiltration depth of the tumors, the regional lymph node metastasis, distant metastasis and lymphatic emboli (P0.05 in all). The overall 5-year survival rate was 49.7% for the whole group, and 100.0% and 37.2% for type I and type III carcinoids, respectively. Univariate analysis revealed that the number of tumor, tumor size (2 cm), serosal invasion, regional lymph node metastasis and distant metastasis were all significant factors affecting the survival (P0.05 in all). However, by multivariate analysis, only distant metastasis was found to be a significant prognostic predictor.The prognosis of type III carcinoids is much poorer than that of type I. Subtyping of gastric carcinoids is helpful in guiding clinical management, and also in prediction of malignant potential and prognosis.

To analyze the prognostic factors of colorectal cancer patients with synchronous liver metastasis treated by simultaneous colorectal and liver resection.The clinical and follow-up data of 44 colorectal cancer patients with synchronous liver metastases who underwent simultaneous colorectal and liver resection from Jan. 1993 to Jan. 2003 were analyzed retrospectively. Survival rate was estimated by Kaplan-Meier method, and was compared using log-rank test. Prognostic factors were analyzed by multivariate Cox proportional hazards model.The overall 1-, 3- and 5-year survival rates were 86.3%, 40.9% and 25.0%, respectively. The lymph node metastasis and vascular invasion by cancer cells from the primary tumour were found to affect prognosis significantly, while gender, age, tumor location, histopathological types, the number and distribution of liver metastases were not. Multivariate analysis revealed that the lymph node metastasis was the only independent prognostic factor.Simultaneous liver and colorectal resection can be performed and may achieve good outcome in colorectal cancer patients with synchronous liver metastases, especially in those without lymph node metastasis.

To investigate the expression of annexin in human pancreatic cancer and to elucidate its role in oncogenesis of pancreatic cancer.A pancreatic carcinoma cell line Suit-II with high-expression of annexin I gene was adopted. Three subtypes of annexin I -siRNA sequences and a non-related fragment were combined, and the eukaryotic expression vectors bearing siRNA fragments were constructed. Then they were transfected into pancreatic carcinoma cells to knock down the expression of annexin I by RNAi. After knocking down the expression of annexin I , the growth speed, cell cycling, morphological features and apoptosis of pancreatic carcinoma cells were examined by RT-PCR and MTT test.When the expression of annexin I was blocked, the growth speed of pancreatic carcinoma cells was significantly decreased, the morphological features were changed and pronounced apoptosis occurred.Annexin I can modulate pancreatic carcinoma cell cycle, promote the cell proliferation, increasingly stimulate the cell growth, and suppress the process of apoptosis in pancreatic carcinoma cells.

To evaluate fine needle localized biopsy under mammography-guiding and skin incision selection by hookwire under ultrasound-guiding for patients with breast microcalcification.Breast microcalcification of 178 patients treated from May 2000 to November 2006 were resected after localized with fine needle under X-ray mammography-guiding. Among them, 62 patients received the selection of hookwire under ultrasound-guiding.Breast cancer was detected in 58 patients (32.6%). Among them, 32 (55.1%) cases were carcinoma in situ, 11 (19.0%) intraductal carcinoma with early infiltration, 15 (25.9%) infiltrative ductal carcinoma, and 4 infiltrative ductal carcinoma with lymph node metastasis. The overall 5-year survival rate was 100%. Compared with that in needle guided group, the acceptable rate of cosmetic results was higher in hookwire group (P = 0.022).For breast microcalcification, mammography guided needle biopsy with ultrasound selected skin incision is an effective and accurate diagnostic method.

To investigate the clinical pathological characteristics and treatment of primary splenic tumor.The clinical data of 43 patients with primary splenic rumors, 23 males and 20 females, aged 44.7 (19 - 66), treated in the Cancer Hospital, Chinese Academy of Medical Science from Feb 1972 through Mar 2006 were analyzed.Of the 43 cases, 21 cases (48.8%) were found in physical examination. 42 cases underwent splenectomy and 1 case underwent rumor biopsy. Sixteen cases were with benign splenic tumors, including 15 cases of hemangioma and 1 case of lymphangioma. Of the 24 malignant cases, 16 suffered from splenic lymphoma and 8 from angiosarcoma. Most lymphoma cases survived and the mean three year surviving rate was 88.7%. The mean surviving time of angiosarcoma was only 10.4 months.Primary splenic tumors lack specificity; B-ultrasonography and CT are primary examination methods. Surgery is an effective method in treatment of primary splenic tumors.

To investigate the factors affecting the result and selection of local excision for low rectal cancer.The clinical data of 101 patients with low rectal cancer treated by local excision were retrospectively analyzed. Survival was estimated using the Kaplan-Meier. The factors influencing on the survival were analyzed using univariate (Log rank) and multivariate (Cox model) analysis methods.Of 101 patients in this series, 91 patients underwent transanal excision, 9 had transsacral excision, 1 recieved transvaginal excision. Postopertative complication developed in 6 patients (5.9%). No death occurred within 30 postoperative days. Five T4 patients underwent preoperative radiotherapy, and 34 received postoperative radiotherapy. The overall 5-year survival rate was 91.0% for the whole group, and it was 100%, 92.6%, 77.1%, 83.3% for patients with Tis, T1, T2, and T3/T4 lesion, respectively. The incidence of local recurrence was 15. 8%. Univariate analysis revealed that pathological T stage, tumor size (3 cm), lymphovascular invasion, ulcerative lesion, adjuvant radiotherapy and local recurrence were significant factors affecting the survival (P0.05). However, by multivariate analysis, only tumor size (3 cm) and local recurrence were found to be the significant prognostic predictors.The important selection criteria for local excision in the treatment of low rectal cancer may include T1 stage, well or moderate differentiation,tumor sizeor = 3 cm, no lymphovascular invasion.

To explore diagnosis and surgery therapy and factors influencing the prognosis of tumor of body and tail of the pancreas.The clinical data of 117 cases of tumor of tail and body of pancreas who underwent operation were analysed.Imaging and tumor marker detection can give exact diagnosis to cases with tumors of body and tail of pancreas. The medial survival time of radical resection, palliative resection and exploratory laparotomy were 18, 8 and 3.5 months.Atypical signs should be noticed, combined imaging and serum detection can diagnose the cases exactly, radical resection is the unique method for long survival, the diameter of the tumor and degree of infiltration to major vessels and organs is important.

To assess the clinical value of sigmoid coloplasty of super-lower anastomosis.Thirty-five patients underwent sigmoid coloplasty (treatment group), while 33 patients received straight sigmoidorectostomy (control group). Complication rate and anal function were observed and compared between the two groups.The complication rate was 17.1% and 21.2% in the study group and control group respectively (P0.05). Average stool frequency were 2 times (1-5.5 times) a day in the study group and 3.5 times (1-9 times) a day in the control group 12 months after operation (P0.01). The fecal continence and self-satisfaction were improved significantly in the study group than that of the control group(P0.01, respectively). The postoperative anal function score was 2.57 in the study group and 7.21 in the control group, there was significant difference between the two groups (P0.01).Compared with straight sigmoidorectostomy, sigmoid coloplasty applied in super-lower anastomosis can significantly improve the recovery of anal function without increasing complication rates.

The purpose of this study was to evaluate the feasibility of VEGF165 gene transfection into bone marrow stromal cells and make the groundwork of VEGF gene therapy for the revascularization of bone tissue engineering.Bone marrow stromal cells (BMSC) were cultured in vitro, and were tested for the dexamethasone-induced potential to form mineralized-like tissue in culture in the presence of ascorbic acid and beta-glycerophosphate. The vector pcDNA3.1-VEGF165 was transfected into bone marrow stroma cells with the method of liposome mediated or Sofast transfectam reagent. The expression level of human VEGF165 in the transformed cells was detected by immunocytochemistry.The percent of the positive cells after VEGF165 gene transfection mediated by liposome and cationic polymer was 11.34 and 11.42 respectively. There was no significant difference in gene transfer efficacy between liposome and cationic polymer mediated gene delivery. While the percent of the number of survival cells after gene transfection was 74.60 and 85.88 respectively, the cytoxity of liposome was a little higher than that of cationic polymer.The experiment demonstrated that rhVEGF165 was successfully expressed in BMSC. This result could serve as a basis for the next step of revascularization of bone tissue engineering.

Serum tumour markers carcinoembryonic antigen (CEA), carbohydrate antigen 19-9 (CA19-9) and CA242 were investigated to evaluate the values of single and combined test in the diagnosis and prognosis of pancreatic cancer.Pre-operative serum CEA, CA19-9 and CA242 were measured in 105 pancreatic cancers, 70 non-pancreatic malignancies and 30 benign pancreatic diseases.The sensitivity of CA19-9 alone was the highest in pancreatic cancer patients (80%), but the specificity was significantly lower than that of CEA and CA242 (P0.01). The combination of CEA and CA242 could increase the specificity to 92%. In serum CA242 positive patients, the survival time was remarkably shorter than that of patients with negative result (P0.01). The survival time in patients with more than two markers positive expression of CEA, CA19-9 and CA242 was obviously shorter than that of only one or no marker positive expression (P0.05).The diagnostic rate of CA19-9 in pancreatic cancer is better than that of CEA and CA242. Combined detection of CEA and CA242 can improve the diagnostic specificity obviously. High levels of serum markers are associated with advanced stage of the disease. Patients with two or three markers positive expression of CEA, CA19-9, and CA242 simultaneously had a shorter survival time.

AIM: To explore the feasibility and oncologic outcomes of segmental jejunal resection on the left side of the mesenteric vessels in patients with tumors of the angle of Treitz using data from a single center. METHODS: Thirteen patients with tumors of the angle of Treitz who underwent surgery at our institution were prospectively followed. A segmental jejunal resection on the left side of the mesenteric vessels was performed in all patients. Formalin-fixed and paraffin-embedded tumor samples were examined. The primary end point of this analysis was disease-free survival. RESULTS: In this study, there were 8 males and 5 females (mean age, 50.1 years; range, 36-74 years). The mean tumor size was 8.1 cm (range, 3.2-15 cm). Histologic examination showed 11 gastrointestinal stromal tumors (GISTs) and 2 adenocarcinomas. Five of the GIST patients presented with potential low risk, and 6 presented with intermediate and high risk, according to the National Institutes of Health criteria. One potentially high-risk patient showed tumor progression at 46 mo and died 52 mo after surgery. One patient with locally advanced adenocarcinoma received neoadjuvant chemotherapy and adjuvant radiotherapy, but the disease progressed, and the patient died 9 mo after surgery. One GIST patient without progression died 16 mo after surgery because of a postoperative intestinal obstruction. The median overall survival rate was 84.6 mo, and the median disease-free survival rate was 94.5 mo. CONCLUSION: The overall survival of patients with tumors of the angle of Treitz was encouraging even when the tumor size was relatively large. A segmental resection on the left side of the mesenteric vessels is considered to be a reliable and curative option for tumors of the angle of Treitz.

Hepatocellular carcinoma (HCC) is a common tumor worldwide with an extremely poor prognosis. Alpha-fetoprotein (AFP) is the only available HCC serological biomarker and has limited effect at early stages. Previous work has proven that AFP-producing and -nonproducing HCC cells have different gene/protein expression profiles. To identify additional candidates that might aid in HCC diagnosis, in addition to giving a better understanding of the mechanisms underlying hepatocarcinogenesis, subcellular fraction proteins from multiple liver cancer (HepG2, Hep3B and SK-HEP-1) and normal cells (HL-7702) were separated and identified using a 2-DE/MALDI-TOF-TOF approach. In total, 50 differentially-expressed proteins between liver cancer and normal liver cells were identified from different fractions including the cytosol, membrane/organelles and nuclear proteins. Among them, the tumor-related protein Annexin A2, was verified to be significantly upregulated in HCC tissues (63.5%, 80/126) especially in AFP-normal cases based on large-scale clinical specimens. Serum levels of Annexin A2 measured by an established ELISA assay were found to be significantly elevated in HCC patients compared with healthy individuals. The serum concentration was also correlated with histological grade. Combined use of AFP and Annexin A2 could increase the diagnostic performance of HCC. These data suggest that Annexin A2 may serve as a novel serological/histological candidate involved in HCC, especially among individuals with normal levels of serum AFP. AIM: To investigate the expression of annexin I in pancreatic cancer and its relationship with the clinicopathologic factors, and to evaluate its potential clinical significance. METHODS: Annexin I expression was analyzed by Western blot and immunohistochemical staining in pancreatic adenocarcinoma and multi-tissue microarrays (MTAs). RESULTS: Western blot analysis showed that annexin I was overexpressed in 84.6% (11/13) pancreatic ductal adenocarcinomas. Immunohistochemistry analysis of pancreatic cancer in MTAs showed that annexin I protein was 71.4% (30/42) positive which was markedly increased compared with that in the tumor matched normal pancreas tissues 18.4% (7/38) (P < 0.01). In the meantime, the high expression of annexin 1 was correlated with the poor differentiation of pancreatic adenocarcinoma. CONCLUSION: Annexin 1 overexpression is a frequent biological marker and correlates with the differentiation of pancreatic cancer during tumorigenesis.
