12. Relationships between the Physical Activity Intensity and the Medical Expenditure of Middle-Aged and Elderly People: Parsing from the CHARLS Database

Abstract

There are many studies on the impact of physical activity on health but few studies on the relationship between physical activity and medical expenditure among the elderly. Based on the China Health and Retirement Longitudinal Survey (CHARLS) database and selected 4456 valid samples, this paper used a two-part model to analyze the effects of high, moderate, and low physical activity intensity on medical expenditure. It is found that the intensity of physical activity was negatively correlated with medical expenditure, and the medical expenditure of the high physical activity intensity group was significantly lower than that of the low physical activity intensity group. For example, compared to people with no physical activity, the total medical expenditure decreased by 22.4%, 40.4%, and 62.5% per week in those with low, moderate, and high physical activity intensity. Thus, the government should provide more places for the elderly to exercise, planning special exercise areas for the elderly in community playgrounds, such as a dancing square, which will also help the elderly to increase their amount of exercise per week and develop a daily exercise habit.

Abstract

Objective: The present study was aimed to comprehensively characterize the epidemiological, clinicopathological characteristics, treatments, and prognosis of intraoral spindle cell carcinoma (SpCC). Materials and Methods: Patients diagnosed with intraoral SpCC at our institution in the past 15 years (2005–2019) were screened from inpatient disease registry. All relevant data concerning patients with intraoral SpCC were retrieved. Previous reports about intraoral SpCC with adequate clinicopathological data in both English literature and Chinese literature were collected. Eligible cases were further reviewed and pooled for statistical analyses. Results: Six patients (5 females and 1 male; average age: 59 years) with intraoral SpCC were histopathologically diagnosed and surgically treated at our institution. The literature review identified another 63 published cases from 34 articles. Most cases were presented in the fifth to seventh decade of life with a male preponderance. Gingiva (23/69, 33.3%) was the most common site followed by the tongue (19/69, 27.5%) and buccal mucosa (8/69, 11.6%). Complete surgical ablation remains the primary treatment option. Tumor size, pathological grades, cervical node metastasis, and distant metastasis were significantly associated with reduced survival. Conclusions: Intraoral SpCC is an uncommon and aggressive malignancy with dismal prognosis. Much attention and effort are needed to characterize this rare entity and improve its clinical outcomes. Abstract

The longitudinal-torsion compound ultrasonic vibration dry milling characteristics were experimentally studied for AlMgSc alloys formed by arc micro-casting and forging additive manufacturing. The influence rules of ultrasonic milling parameters, such as cutting force, surface morphology, work hardening and tool wear, were discussed about the dry milling forming characteristics of AlMgSc alloys. Results show that the arc micro-casting and forging additive manufactured AlMgSc alloys which have more dense microstructure, higher strength and hardness comparing with conventional forging and common arc added material forming aluminum alloys, may produce larger milling frces in ultrasonic dry milling processes and be more likely to occur work hardening phenomenon. And the longitudinal-torsion compound ultrasonic vibration dry milling may get lower surface roughness and is helpful to reduce the tool wear comparing with conventional dry milling.
