The aim of this study is to evaluate the efficacy and safety of Ma’s bamboo-based medicinal moxibustion for low back pain in LDH.Methods: A total of 312 LDH patients with low back pain were randomized to receive Ma’s bamboo-based medicinal moxibustion (MBMM) or acupuncture (AT). The primary efficacy measure was the change of Visual Analogue Scale (VAS) on the 14th day compared with that at baseline. The secondary efficacy measures included VAS score, Oswestry disability index (ODI), modified Japanese Orthopaedic Association (M-JOA) score, and the content of β-endorphin (β-EP) and substance-P (SP). The safety measures included the occurrence of adverse events and the changes in laboratory indicators.Results: In total, 304 patients were incorporated for the analysis of efficacy, including 96 males and 208 females, aged 21– 65 years. There was no statistically significant difference in the change of VAS score between the two groups on the 14th day [mean difference (95% CI) = − 2.31 (− 2.48, − 2.13) and − 2.28 (− 2.45, − 2.11), respectively; p = 0.819]. The VAS, ODI, and M-JOA scores changed after the intervention in both groups (p < 0.001), with increased β-EP content (p = 0.014, p = 0.032) and decreased SP content (p < 0.001, p = 0.048). The ODI score (p = 0.039) and M-JOA score (p = 0.032) of the MBMM group on the 28th day were lower than those of the AT group.Conclusion: The efficacy of Ma’s bamboo-based medicinal moxibustion therapy in relieving low back pain of LDH patients is comparable to that of acupuncture, and it has post-effect advantages in improving lumbar dysfunction and daily living ability, which can be used as a safe and effective alternative method for LDH treatment.Keywords: LDH, low back pain, Ma’s bamboo-based medicinal moxibustion, acupuncture, randomized controlled trial

Extracellular vesicles (EVs), recognized as multifunctional drug delivery systems in biomedicine, have gained accumulative interest. Specifically, engineered EVs have emerged as a promising therapeutic approach for targeted drug delivery, potentially addressing the complexities of CKD management. In this review, we systematically dissect EVs, elucidating their classification, biogenesis, composition, and cargo molecules. Furthermore, we explore techniques for EV engineering and strategies for their precise renal delivery, focusing on cargo loading and transportation, providing a comprehensive perspective. Moreover, this review also discusses and summarizes the diverse therapeutic applications of engineered EVs in CKD, emphasizing their anti-inflammatory, immunomodulatory, renoprotective, and tissue-regenerating effects. It has been documented that human activities are causing the rapid loss of taxonomic, phylogenetic, genetic and functional diversity in soils. However, it remains unclear how modern intensive rice cultivation impacts the soil microbiome and its functionality. Here we examined the microbial composition and function differences between a buried Neolithic paddy soil and an adjacent, currently-cultivated paddy soil using high throughput metagenomics technologies. Our results showed that the currently cultivated soil contained about 10-fold more microbial biomass than the buried one. Analyses based on both 16S rRNA genes and functional gene array showed that the currently cultivated soil had significantly higher phylogenetic diversity, but less functional diversity than the buried Neolithic one. The community structures were significantly different between modern and ancient soils, with functional structure shifting towards accelerated organic carbon (C) degradation and nitrogen (N) transformation in the modern soils. This study implies that, modern intensive rice cultivation has substantially altered soil microbial functional structure, leading to functional homogenization and the promotion of soil ecological functions related to the acceleration of nutrient cycling which is necessary for high crop yields.

Microbial decomposition of soil carbon in high-latitude tundra underlain with permafrost is one of the most important, but poorly understood, potential positive feedbacks of greenhouse gas emissions from terrestrial ecosystems into the atmosphere in a warmer world. Using integrated metagenomic technologies, we showed that the microbial functional community structure in the active layer of tundra soil was significantly altered after only 1.5 years of warming, a rapid response demonstrating the high sensitivity of this ecosystem to warming. The abundances of microbial functional genes involved in both aerobic and anaerobic carbon decomposition were also markedly increased by this short-term warming. Consistent with this, ecosystem respiration (R eco) increased up to 38%. In addition, warming enhanced genes involved in nutrient cycling, which very likely contributed to an observed increase (30%) in gross primary productivity (GPP). However, the GPP increase did not offset the extra R eco, resulting in significantly more net carbon loss in warmed plots compared with control plots. Altogether, our results demonstrate the vulnerability of active-layer soil carbon in this permafrost-based tundra ecosystem to climate warming and the importance of microbial communities in mediating such vulnerability.

However, full cycles of standard R-CHOP present the risk of severe bleeding or perforation, even leading to emergency surgery, especially for those with deep lesions in their first 1– 2 cycles of treatment. This study aims to explore the safety and efficacy of fractioned R-CHOP (rituximab d0, 50% dose of CHOP d1 and d5) followed by standard R-CHOP cycles in PG-DLBCL patients guided by endoscopic ultrasonography (EUS).Patients and Methods: Thirty-one PG-DLBCL patients were analyzed in this retrospective study. All patients had lesions infiltrated to at least the 3rd layer of the stomach under EUS at baseline. Patients switched to standard R-CHOP if they showed the reduced infiltrated layers and restricted lesions after fractioned R-CHOP cycles.Results: The overall response rate, 5-year progression-free survival (PFS) and overall survival (OS) of patients in our study were 93.5%, 75% and 84%, respectively. No treatment delay or dosage reduction from gastric adverse event was observed. None of the patients in our study suffered from severe bleeding or perforation during the treatment. Kaplan–Meier analyses showed that PG-DLBCL patients characterized by multiple localization, lesions ≥ 3cm, having B symptoms, lower serum albumin level, and elevated LDH level were associated with worse PFS and OS.Conclusion: Our data indicate that it might be an effective approach in treating deeply infiltrated PG-DLBCL patients by switching fractioned R-CHOP to standard R-CHOP cycles guided by EUS.Keywords: primary gastric diffuse large B-cell lymphoma, endoscopic ultrasonography, R-CHOP, prognosis

In this paper, we investigate the Bloch bands and develop a linear response theory for nonlinear systems, where the interplay between topological parameters and nonlinearity leads to new band structures. The nonlinear system under consideration is described by the Qi–Wu–Zhang model with Kerr-type nonlinearity, which can be treated as a nonlinear version of Chern insulator. We explore the eigenenergies of the Hamiltonian and discuss its Bloch band structures as well as the condition of gap closing. A cone structure in the ground Bloch band and tubed structure in the excited Bloch band is found. We also numerically calculate the linear response of the nonlinear Chern insulator to external fields, finding that these new band structures break the condition of adiabatic evolution and make the linear response not quantized. This feature of response can be understood by examining the dynamics of the nonlinear system. [ABSTRACT FROM AUTHOR]

As a new quantum state, topological insulators have become the focus of condensed matter and material science. The open system research of topological insulators has aroused the interest of many researchers. In this paper, we study the response of topological insulators to a single-mode quantized field with momentum. We solve the ground state of the system after the addition of a single-mode light field with adjustable photon momentum. To be specific, the analytical solution of Hall conductance has an additional correction compared with the closed system, and the Hall conductance can no longer be expressed by the weighted sum of the Chern numbers. Furthermore, the topological properties are analyzed and discussed through the results of an instance with their illustration. The system still has a topological phase transition and the critical point of the topological phase is robust to the single-mode quantized field. [ABSTRACT FROM AUTHOR]

Geogrid is a horizontal reinforcement material that is used in geosynthetic-reinforced pile-supported (GRPS) embankments in freeways with soft soil. Nevertheless, the long-term deformation mechanics of geogrid in the GRPS embankment under cyclic loading is not clear enough. This paper presents a laboratory model study of a GRPS embankment to study the long-term geogrid strain under cyclic loading. The effects of parameters such as number of load cycles, load frequency, geogrid layer numbers, and length of supporting piles on the long-term strain of geogrid were investigated. The results showed that geogrid strain increased with the number of load cycles, mainly developed in the first 10 000 load cycles and increased slowly in the later period. When the frequency was increased from 1 to 5 Hz, geogrid strain increased dramatically. When compared with the condition of a single geogrid layer, the upper layer of two geogrid layers caused a less geogrid strain. Compared with long piles and short piles GRPS embankments, a sufficient membrane effect of geogrid was found for long-short piles GRPS embankments. Finally, the 95% prediction interval proposed in this paper could be expressed using a logarithmic function, providing a theoretical foundation for future engineering applications. Unidirectional reflectionlessness is investigated in a waveguide quantum electrodynamics system that consists of a cavity and a Λ -type three-level quantum dot coupled to a one-dimensional plasmonic waveguide. Analytical expressions of transmission and reflection coefficients are derived and discussed for both resonant and off-resonant couplings. By appropriately modulating the coupling strength, phase shift and Rabi frequency, unidirectional reflectionlessness is observed at the exceptional points (EPs). And unidirectional coherent perfect absorption is exhibited at the vicinity of the EP. These results might find applications in designing quantum devices of photons, such as optical switches and single-photon transistors. [ABSTRACT FROM AUTHOR]

The generalized wall function by Shih et al. [Report No. M-1999-209398 (1999)], which accounts for non-equilibrium effects by the presence of favorable and adverse pressure gradients in turbulent flows, is addressed with the aim of performing high Reynolds number large-eddy simulations of the wall-bounded flow. The model uses a corrected law of the wall with a pressure gradient contribution to approximate the wall stress and applies to the entire viscous layer, buffer layer, and inertial region. A fully developed channel flow is first tested to validate the solver and model implementation, and then the wall function is assessed for the flow over a periodic hill. Wall-resolved simulations are in good agreement with reference results. A priori investigation with own experimental results corroborates the mathematical form of the model and suggests using different coefficients. The wall-modeled simulations show that the implemented wall model is able to improve the wall shear stress predictions compared to a standard equilibrium wall model. It corrects the underestimation of wall shear stresses by equilibrium models in the favorable pressure gradient region and the overestimation of wall shear stresses in the adverse pressure gradient region. The positions of the separation and reattachment points are also in good agreement with reference results. Furthermore, the prediction of the wall shear stress maximum in the favorable pressure gradient zone at the windward side of the hill is quite robust against coarsening the wall-normal grid spacing. Recent studies have suggested an important role of gut microbiota (GM) in modulating the host metabolism and immune system, leading to alterations in bone mass. Probiotic bacteria administered in adequate amounts can alter the composition of GM and confer health benefits to the host. Given the inflammatory osteolysis that occurs in wear debris-induced prosthesis loosening, we examined whether the probiotic Lactobacillus casei could reduce osteolysis in a mouse calvarial resorption model. In this study, L. casei markedly protected mice from CoCrMo particles (CoPs)-induced osteolysis. Osteoclast gene markers and the number of osteoclasts were significantly decreased in L. casei-treated mice. Probiotic treatment decreased the M1-like macrophage phenotype indicated by downregulation of tumor necrosis factor α (TNF-α), interleukin (IL)-6 and inducible nitric oxide synthase (iNOS) and increased the M2-like macrophage phenotype indicated by upregulation of IL-4, IL-10 and arginase. Collectively, these results indicated that the L. casei treatment modulated the immune status and suppressed wear particle-induced osteolysis in vivo. Thus, probiotic treatment may represent a potential preventive and therapeutic approach to reduced wear debris-induced osteolysis. Oysters hold a key ecological position in intertidal ecosystems worldwide, and their distribution across various sea areas is largely influenced by the range of salinity. Sodium-potassium ATPase (NKA), a central ion pump involved in maintaining cellular osmotic balance, plays a pivotal role in oyster adaptation to salinity fluctuations. This study identified one NKA-a subunit and two distinct NKA-ß subunits by utilizing genome assemblies and transcriptomics data from multiple oyster species. Furthermore, we discovered a conserved NKA-interacting protein (NKAIN) in oysters. Through transcriptome assembly, we identified five different splice variants of NKAIN in oysters. Sequence alignment and phylogenetic analysis confirmed the high conservation of NKA-a and -ß subunits in oysters, albeit with significant distinction between the two -ß subunits. Multi-population comparative transcriptomic analysis illuminated the regulatory roles of NKA and NKAIN in the adaptive responses to salinity stress. The findings shed light on the compositional details and osmotic regulation mechanisms of the oyster's NKA-NKAIN system, thereby enhancing our understanding of the comprehensive osmotic adaptation mechanisms in oysters.
