Objectives: We aimed to assess the predicting value of C-reactive protein (CRP)/albumin ratio (CAR) in the development of Oxygenation impairment (OI) in the patients with Stanford type-B acute aortic dissection (AAD). Methods: This study included 133 patients (age = 58.8 ± 12.0 years, median age = 61 years, Male/Female = 117/16) diagnosed as Stanford type-B AAD accompanied by hypertension from July 2012 to May 2020. Clinical data were retrospectively extracted from the database. The patients in this study were divided into OI group (oxygenation index ≤ 200) and non-OI group (oxygenation index > 200). Clinical characteristics in both groups were compared, and predicting value of CAR in the development of OI was assessed. Results: Patients in OI group had higher peak body temperature (37.94 ± 0.62 vs. 37.67 ± 0.51 ℃, P =.010), higher levels of serum CRP (41.74 ± 27.71 vs 15.21 ± 19.66 mg/L, P =.000) and plasma B-type natriuretic peptide (292.14 ± 251.11 vs 179.80 ± 241.27 ng/L, P =.016), lower levels of albumin (34.00 ± 5.14 vs 37.72 ± 5.24 g/L, P =.000), and higher CAR (1.27 ± 0.89 vs 0.41 ± 0.53, P =.000). In multivariate regression analysis, CAR (odds ratio: 5.215, 95 % CI: 2.682; 10.137, P =.000) and the peak body temperature (odds ratio: 2.905, 95 % CI: 1.255; 6.724, P =.013) could significantly predict the OI development. The AUC for CAR was 0.831 (95 % CI: 0.756–0.907). An optimal cutoff value for CAR for predicting OI was ≥ 0.70, with a sensitivity of 67.5 % and a specificity of 88.2 %. Conclusions: Compared with CRP or albumin alone, the CAR might be a more accurate marker in predicting OI development in Stanford type-B AAD.

BackgroundIt is well known that motor features of Parkinson’s disease (PD) commonly begin on one side of the body and extend to the other side with disease progression. The onset side generally remains more severely affected over the course of the disease. However, the pathophysiology underlying the asymmetry of motor manifestations remains unclear. The purpose of the present study is to examine whether alterations in neuronal activity in the subthalamic nucleus (STN) associate with PD severity.MethodsMicroelectrode recording was performed in the STN during targeting for 30 patients in the treatment of deep brain stimulation. The mean spontaneous firing rate (MSFR), power density spectral analysis, and correlations were calculated. Characteristics of subthalamic oscillatory activity were compared between two hemispheres. UPDRS III scores during “Off” and “On” states were obtained for the body side of initial symptoms (BSIS) and the body side of extended symptoms (BSES).ResultsThere were significant differences of MSFR (41.3 ± 11.0 Hz vs 35.2 ± 10.0 Hz) and percentage of ß frequency oscillatory neurons (51.3% vs 34.9%) between BSIS and BSES. The percentage of ß frequency oscillatory neurons correlated with the bradykinesia/rigidity scores for both sides (p < 0.05). In contrast, the percentage of tremor frequency oscillatory neurons was significantly higher in the BSES than that in the BSIS. In particular, these neurons only correlated with the tremor scores of the BSES (p < 0.05).ConclusionThe results suggest that increased neuronal firing rate and ß frequency oscillatory neurons in the STN are associated with contralateral side motor severity and its progression. Tremor frequency oscillatory neurons are less observed in the STN of the BSIS suggesting that ß oscillatory activity dominates and tremor frequency oscillatory activity reciprocally declines.

To obtain high energy storage dielectric materials with high thermal conductivity for electronic devices, we designed and constructed three-dimensional (3D) BN-BT/PVDF skeleton structure composites based on the ice template method and freeze-drying technology. Phonon thermal conductivity and intrinsic thermal conductivity can be improved by the hexagonal boron nitride (h-BN) thermal conductive skeleton orientation along the ice crystal on nanoscale. The 3D BN-BT/PVDF skeleton structure composites with high thermal conductivity and high energy storage can be obtained by impregnating the barium titanate (BT) modified polyvinylidene fluoride (PVDF) precursor into a 3D thermal conductive skeleton. When the h-BN content is 25 wt%, the comprehensive performance of BN-BT/PVDF composite are the most excellent with the energy storage density of 2.6355 J/cm3, which is 5.64 times of pure PVDF, and the thermal conductivity can reach 0.302 W/(m·K), an improvement of 152%, showing a promising industrial application prospects.

Abstract Acute lung injury (ALI) and oxygenation impairment (OI) frequently occur in the patients with acute aortic dissection (AAD), which may necessitate mechanical ventilation and result in adverse outcomes. This paper aims to increase clinicians’ awareness of the severe respiratory complications in the patients with AAD, and provide the overview of the epidemiology, adverse outcomes, pathogenesis, predictive markers and therapeutic modalities of the concurrent conditions. Currently, it is considered that inflammatory response plays a great role in the pathogenesis of ALI and OI in the patients with AAD, but the definite pathogenesis remains unclear. Given the great importance of the prediction of the occurrence of the severe respiratory complication at a very early stage, some inflammatory biomarkers have been investigated to predict the occurrence of ALI and OI in several studies. C-reactive protein was found to have a significant predictive effect for the development of ALI and OI. Early use of beta-blockers and the use of bindarit could prevent the occurrence of OI and ALI. Ulinastatin could also improve oxygenation in the patients with type-A AAD. Prevention and management of ALI and OI in AAD remain a great challenge. The definite pathogenesis should be clearly clarified and further studies should be performed to look for potential effective way to predict and manage the severe respiratory conditions.

Background: This study aims to investigate the altered spontaneous neural activity in patients with Parkinson's disease (PD) revealed by amplitudes of low-frequency fluctuations (ALFF) of resting-state fMRI, and the feasibility of using ALFF as neuroimaging predictors for motor improvement after bilateral subthalamic nucleus (STN) deep brain stimulation (DBS). Methods: Fourty-four patients and 44 healthy controls were included in this study. First, the ALFF of patients with PD was compared with that of controls; then significant clusters were correlated with motor improvement after DBS (unified Parkinson's disease rating scale (UPDRS-III)) and other clinical variables. Second, regression and classification of the machine learning models were conducted to predict motor improvement after DBS. Receiver operating characteristic (ROC) analysis was used to evaluate the performance of the classification model. Results: Compared with healthy controls, patients with PD showed increased ALFF in the bilateral motor area and decreased ALFF in the bilateral temporal cortex and cerebellum. Abstract Background Currently no optimal localization technique has been established for localization of ground glass opacity (GGO). We aimed to introduce a localization technique using geometric localization for peripheral GGO. Methods We delineated the location of pulmonary GGO using geometric method which was similar with localization of a point in a spatial coordinate system. The localization technique was based on the anatomical landmarkers (ribs or intercostal spaces, capitulum costae and sternocostal joints). The geometric parameters were measured on preoperative CT images and the targeted GGO could be identified intraoperatively according to the parameters. We retrospectively collected the data of the patients with peripheral GGOs which were localized using this method and were wedge resected between June 2019 and July 2020. The efficacy and feasibility of the localization technique were assessed. Results There were 93 patients (male 34, median = 55 years) with 108 peripheral GGOs in the study. All the targeted GGOs were successfully wedge resected in the operative field with negative surgical margin at the first attempt. For each GGO, the localization parameters could be measured in 2–4 min (median = 3 min) on CT images before operation, and surgical resection could be completed in 5–10 min (median = 7 min). A total of 106 (98.15%) GGOs achieved sufficient resection margin. No complications and deaths occurred related to the localization and surgical procedure. Conclusions The localization technique can achieve satisfactory localization success rate and good safety profile. It can provide an easy-to-use alternative to localize peripheral GGO.

BackgroundNumerous randomized controlled trials (RCTs) conducted in China have shown that jinlida granules are a promising traditional Chinese medicine (TCM) for the treatment of persons with type 2 diabetes mellitus (T2DM). Controversial results have been reported in different RCTs. The aim of our study was to evaluate the adjuvant hypoglycemic effect of jinlida granules on persons with T2DM and to explore the source of heterogeneity between these RCTs.Materials and methodsMedical article databases were individually searched by two authors for RCTs that provided data regarding the effect of jinlida granules in the treatment of T2DM before 1 June 2022. The methodological quality of the included RCTs was comprehensively assessed by two authors. Data from RCTs with low risk of bias were pooled using Stata SE 12.0 (random-effects model). Evidence derived from the meta-analysis will be assessed according to the GRADE system.ResultsTwenty-two RCTs were eventually included in the systematic review and three RCTs with low risk of bias were analyzed in the meta-analysis. Compared with the control groups, significant changes were found in lowering glycosylated hemoglobin a1c (mean difference -0.283 with 95% CI -0.561, -0.004; P=0.046), and were not found in lowering 2-hour postprandial glucose (mean difference -0.314 with 95% CI -1.599, 0.972; P=0.632) and fasting blood glucose (mean difference -0.152 with 95% CI -0.778, -0.474; P=0.634) in the jinlida groups. The GRADE-assessed evidence quality for the outcomes was moderate.ConclusionThe adjuvant hypoglycemic effect of jinlida granules on adult Chinese persons with T2DM was statistically found in lowering HbA1c and was not statistically found in lowering FPG and 2h-PG. Evidence grading should be considered moderate, and the results should be interpreted cautiously. Whether the efficacy of HbA1c-lowering related to clinical significance remains to be investigated in future RCTs. Differences in HbA1c, FPG and 2h-PG at baseline and high risk of bias were important source of heterogeneity between these RCTs. There are a large number of tunnels built in permafrost regions every year in China. When investigating the road tunnels that have been in operation in cold regions, it was found that most of these tunnels have various freezing damages. Therefore, in the design, construction, operation, and maintenance of tunnels in cold regions, there are still many technical problems that need to be solved urgently. Among them, the problems of heat preservation, anti-freezing, and anti-drainage are the key technical problems to determine whether the tunnels in cold regions are affected by freezing damage. The project was based on the completed highway tunnels in Inner Mongolia. Given a series of special problems faced by the construction and management of tunnels in cold regions, a large number of reference documents were consulted. This paper designed thermal insulation, antifreeze, and anti-drainage technology for tunnels in the cold area of Jinpen Bay. The research in this paper can effectively reduce the probability of the occurrence of tunnel freezing damage, ensure the safety of the long-term operation of the tunnel lining structure, and greatly reduce maintenance costs, which have good economic and social benefits.

Abstract Background Increasing evidence demonstrate that the gut microbiota is involved in the pathogenesis of liver diseases, and faecal microbiota transplantation is considered to be a promising new treatment option. However, there are no reports on the intestinal flora of asymptomatic HBV carriers using next-generation sequencing. This study intends to investigate the potential role of the intestinal microflora in predicting the progression of Hepatitis B patients in different non-cancerous stages. Results A total of 266 patients with different stages of Hepatitis B and 31 healthy controls were included in this study. Some of the subjects (217 cases) underwent 16S rRNA gene sequencing. Compared with the control group (CK), the α diversity of patients in Group A (HBV carrier) slightly increased, while that of patients in the other three groups decreased. Each group of patients, especially those in Group C (cirrhosis) and Group D (acute-on-chronic liver failure), could be separated from the CK using weighted UniFrac PCoA and ANOSIM. LEfSe revealed that 40 taxa belonging to three phyla had an LDA larger than 4. In addition to the comparison between Group B (chronic Hepatitis B) and Group C, the specific flora and potential taxonomic function were also identified. Different microbial communities were found to be highly correlated with clinical indicators and the Child-Pugh scores. Changes in the microbial community were highly related to the alternations of host metabolism, which in turn, was related to the development of Hepatitis B. Our analysis identified a total of 47 strains with potential biomarker functions at all levels except for the phylum level. Conclusions Faecal microbiota transplantation of some potential beneficial bacteria can change with the occurrence of disease, and HBV carriers might be the most suitable donors.

Abstract Background Acute aortic dissection (AAD) is an acute life-threatening cardiovascular disease, which is frequently complicated with oxygenation impairment (OI). We aim to investigate predictors of the development of OI in the patients with AAD. Methods We retrospectively collected clinical data of AAD in hypertensive patients from July 2012 to March 2020. The patients included in this study were divided into OI (+) group (oxygenation index≤200) and OI (−) group (oxygenation index> 200). Both groups were compared according to demographic and clinical characteristics, and laboratory findings. Characteristics of hypertension in the patients with AAD were described. Predictors for the development of OI were assessed. And cutoff values were determined by receiver operating characteristics (ROC) curve. Results A total of 208 patients were included in this study and the incidence of OI was 32.2%. In OI (+) group, patients had significantly higher peak body temperature (37.85 ± 0.60 vs 37.64 ± 0.44 °C, P = .005), higher levels of CRP (42.70 ± 28.27 vs 13.90 ± 18.70 mg/L, P = .000) and procalcitonin (1.07 ± 3.92 vs 0.31 ± 0.77μg/L, P = .027), and lower levels of albumin (34.21 ± 5.65 vs 37.73 ± 4.70 g/L, P = .000). Spearman’s rank correlation test showed that the minimum oxygenation index was positively correlated with albumin, and was negatively correlated with the peak body temperature, serum CRP, procalcitonin, BNP and troponin. The stepwise multiple linear regression analysis showed that the peak body temperature, serum CRP and albumin were independently associated with development of OI. An optimal cutoff value for CRP for predicting OI was ≥9.20 mg/L, with a sensitivity of 91.0% and a specificity of 61.0%. Conclusions The peak body temperature, serum CRP and albumin were independent predictors of OI development in the patients with AAD. The serum CRP on admission≥9.20 mg/L might be a valuable and reliable indicator in predicting the development of OI.

In dialogue systems, understanding the user utterances is crucial for providing appropriate responses. A traditional dialogue act classification (DA) task is to classify each user reply into “ACCEPT, REJECT, PROPOSE, and others”. In contrast, in this paper, we define the DA task on multiple round conversations between humans. The re-defined task is to classify a full dialogue according to the intention of one participant. We term this task as intention classification (IC). We, then, propose a hybrid neural network-based ensemble model for solving this problem. Two novel ensemble schemes are introduced for combining the classification results or features from various classifiers. One is ensembling features from each individual classifier using stacking, and we term this scheme as SFE. The other is adding wrong examples' weight to loss functions of each individual classifier using the AdaBoost scheme, and we term this scheme as MN-Ada. We have empirically examined the performance of the proposed ensemble schemes by using three popular deep neural networks, as well as one newly modified networks for IC. Extensive experiments have been conducted on a Chinese dialogue corpus. Our model can achieve state-of-the-art accuracy on the experimental dialogue corpus.

Nowadays, most end-to-end task-oriented dialog systems are based on sequence-to-sequence (Seq2seq), which is an encoder-decoder framework. These systems sometimes produce grammatically correct, but logically incorrect responses. This phenomenon is usually due to information mismatching. To solve this problem, we introduce hierarchical attention and knowledge matching networks with information enhancement (HAMI) for task-oriented dialog systems. It contains a hierarchical attention dialog encoder (HADE) that models dialogs at the word and sentence level separately. HADE can focus on important words in a dialog history and generate context-aware representations. HAMI also contains a unique knowledge matching module to detect entities and interact with a knowledge base (KB). Then, dialog history and KB result representations serve as guidance for system response generation. Finally, HAMI's loss function is designed with an information regularization term to emphasize the importance of entities. The experimental results show that HAMI improves 9.8% in entity F1 compared with vanilla Seq2seq. HAMI also outperforms state-of-the-art models in both the entity F1 and dialog accuracy metrics.

Freshwater phytoplankton communities can be classified into a variety of functional groups that are based on physiological, morphological, and ecological characteristics. This classification method was used to study the temporal and spatial changes in the phytoplankton communities of Gaozhou Reservoir, which is a large municipal water source in South China. Between January 2015 and December 2017, a total of 155 taxa of phytoplankton that belong to seven phyla were identified. The phytoplankton communities were classified into 28 functional groups, nine of which were considered to be representative functional groups (relative biomass > 10%). Phytoplankton species richness was greater in the summer and autumn than in the winter and spring; cyanobacterial blooms occurred in the spring. The seasonal succession of phytoplankton functional groups was characterized by the occurrence of functional groups P (Staurastrum sp. and Closterium acerosum) and Y (Cryptomonas ovata and Cryptomonas erosa) in the winter and spring, and functional groups NA (Cosmarium sp. and Staurodesmus sp.) and P (Staurastrum sp. and Closterium acerosum) in the summer and autumn. The temperature, nitrogen, and phosphorus levels were the main factors driving seasonal changes in the phytoplankton communities of Gaozhou Reservoir. The functional group M (Microcystis aeruginosa) dominated the community during the cyanobacterial blooms in spring 2016, with the maximum algal cell density of 3.12 × 108 cells L−1. Relatively low temperature (20.8 °C), high concentrations of phosphorus (0.080–0.110 mg L−1), suitable hydrological and hydrodynamic conditions (e.g., relatively long retention time), and relatively closed geographic location in the reservoir were the key factors that stimulated the cyanobacterial blooms during the early stages.

Diabetes, especially type 2, has been rapidly increasing all over the world. Although many drugs have been developed and used to treat diabetes, side effects and long-term efficacy are of great challenge. Therefore, natural health product and dietary supplements have been of increasing interest alternatively. In this regard, Chinese herbs and herbal products have been considered a rich resource of product development. Although increasing evidence has been produced from various scientific studies, the mechanisms of action are lacking. Here, we have proposed that many herbal monomers and formulae improve glucose homeostasis and diabetes through the BMID axis; B represents gut microbiota, M means mucosal immunity, I represents inflammation, and D represents diabetes. Chinese herbs have been traditionally used to treat diabetes, with minimal side and toxic effects. Here, we reviewed monomers such as berberine, ginsenoside, M. charantia extract, and curcumin and herbal formulae such as Gegen Qinlian Decoction, Danggui Liuhuang Decoction, and Huanglian Wendan Decoction. This review was intended to provide new perspectives and strategies for future diabetes research and product.

Thermal energy storage (TES) is a highly effective approach for mitigating the intermittency and fluctuation of renewable energy sources and reducing industrial waste heat. We report here recent research on the use of composite phase change materials (PCM) for applications over 700 °C. For such a category of material, chemical incompatibility and low thermal conductivity are often among the main challenges. Our aims are to address these challenges through the formulation of form-stable composite PCMs and to understand their thermophysical properties. The eutectic K2CO3-Na2CO3 salt was used as a PCM with MgO as a form stabilizer. We found that such a formulation could maintain shape stability with up to 60 wt.% PCM. With a melting point of ~710.1 °C and an energy density as high as 431.2 J/g over a temperature range between 550 °C and 750 °C, the composite PCM was shown to be thermally stable up to 885 °C. An addition of 10 wt.% SiC enhanced the overall thermal conductivity from 1.94 W·m−1 K−1 to 2.28 W·m−1 K−1, giving an enhancement of 17.53%. Analyses of thermal cycling data also showed a high extent of chemical compatibility among the ingredients of the composite PCM.

Serious phytoplankton blooms occurred repeatedly every spring in many bays of the Three-Gorges Reservoir. However, little was known about the impact of phytoplanktivorous fish on spring phytoplankton community structure in the Three-Gorges Reservoir. In this study, an enclosure experiment was conducted to assess the impact of silver carp (Hypophthalmichthys molitrix) on the plankton community structure and water quality of the Three-Gorges Reservoir during 2 April to 2 May 2008. The field experiment was performed in six enclosures. Stocking silver carp into enclosures caused a dramatic change in the aquatic ecosystem. For example, pH, transparency, DO and phosphate were reduced, while chlorophyll-a concentration and turbidity increased dramatically. Furthermore, during the enclosure experiment, some zooplankton such as rotifer and copepoda were significantly reduced, and some phytoplankton and protozoa were significantly increased by silver carp. The predation by silver carp might have increased phytoplankton biomass through reducing the densities of some zooplankton which directly fed on phytoplankton. The subzones of the intercritical heat-affected zone (IC HAZ) of low-carbon bainitic steel were simulated by using a Gleeble-3500 simulator to study the impact toughness. The results showed that the IC HAZ is not entirely brittle and can be further divided into three subzones according to the impact toughness or peak welding temperature; the invariant subzone heated between the critical transformation start temperature (Ac1) and 770 °C exhibited unchanged high impact toughness. Furthermore, an extremely low impact toughness was found in the embrittlement subzone, heated between 770 and 830 °C, and the reduction subzone heated between 830 °C and the critical transformation finish temperature (Ac3) exhibited toughness below that of the original metal. The size of the blocky martensite-austenite (M-A) constituents was found to have a remarkable level of influence on the impact toughness when heated below 830 °C. Additionally, it was found that, once the constituent size exceeds a critical value of 3.0 µm at a peak temperature of 770 °C, the IC HAZ becomes brittle regardless of lath or twinned martensite constitution in the M-A constituent. Essentially, embrittlement was observed to occur when the resolved length of initial cracks (in the direction of the overall fracture) formed as a result of the debonding of M-A constituents exceeding the critical Griffith size. Furthermore, when the heating temperature exceeded 830 °C, the M-A constituents formed a slender shape, and the impact toughness increased as the area fraction of the slender M-A constituents decreased.

Certain epidemiological studies have suggested exposure to polybrominated diphenyl ethers (PBDEs) affect the production and secretion of thyroid hormones (TH); however, conflicting results have been reported in different studies. There is not a convincing conclusion about this debate to date.To perform a meta-analysis determining if there are correlations between PBDEs exposure and the serum levels of TH. Medical and scientific literature databases were searched for articles that met the eligibility criteria. The included articles were assessed for methodological quality. The correlation coefficient values or regression coefficient values between PBDEs and thyroid stimulating hormone (TSH) or total thyroxine (TT4) from each article were used for analysis.Sixteen articles were included in this meta-analysis. Pearson correlation coefficients (r) were directly collected or calculated from data given in the articles. Then, Fisher's z transformation was performed to convert each correlation coefficient to an approximately normal distribution. For z values between PBDEs exposure and TSH levels, the pooled z value for 18 studies was 0.08 (95% CI: -0.06, 0.22), and indicated significant heterogeneity (I2 values = 90.7%). Several epidemiological studies have indicated that an increased risk of depression was associated with long-term exposure to PM2.5. The objective of our two-sample Mendelian randomization study was to determine the causal relationship between long-term exposure to particulate matter 2.5 and the risk of depression. A two-sample Mendelian randomization study was performed based on GWAS summary data. Forty-six PM2.5-related single nucleotide polymorphisms were suitable for the analysis as instrumental variables. The random-effect model of inverse-variance weighted and the other four methods (weighted median, MR-Egger, Simple mode and weighted mode) were all performed for the analysis. Additionally, multivariate Mendelian randomization analysis was also completed. Our two-sample Mendelian randomization study indicated that exposure to particulate matter 2.5 has a significantly positive impact on the risk of depression (P = 0.026, random-effect model of inverse-variance weighted). After adjusting for smoking and body mass index in our multivariate Mendelian randomization analysis separately, the relationship between exposure to PM2.5 and the risk of depression remained significant. Based on current GWAS data, our study supplies potential evidence that long-term exposure to PM2.5 is a risk factor for depression.
