2. Efficacy of a 'provisional incision' for longitudinal flap division after free-flap surgery

Summary: Fix and flap surgery for severe open limb fractures is already a standard treatment. In cases where the fracture is complicated or accompanied by bone defects, secondary surgery is required for fracture sites covered with a myocutaneous flap after the soft tissue condition has stabilized. We applied the delayed procedure concept used for distant flaps and attempted to prevent postoperative myocutaneous flap necrosis by performing a provisional incision prior to the longitudinal incision of the flap. We report the course of five cases of the longitudinal division of the myocutaneous flap using “provisional incision” after free-flap surgery for severe open fracture and verify its usefulness. In this case series, five patients with severe open limb fractures treated from 2020 to 2021 who underwent longitudinal incision of the myocutaneous flap using provisional incision after free-flap surgery were included. The types of flaps used for soft tissue reconstruction in the acute phase, the reasons for the need for secondary surgery, the period from soft tissue reconstruction to additional surgery, and the healing status of soft tissue after secondary surgery were all investigated retrospectively. The types of flaps used for soft tissue reconstruction were latissimus dorsi myocutaneous flap in four cases and anterolateral thigh flap in one case. The breakdown of secondary surgery was osteosynthesis in one case, plate removal in one case, and bone cement removal and autologous bone grafting in three cases. The period from soft tissue reconstruction to secondary surgery ranged from 6 weeks to 4 months. In all cases, the wound healed without necrosis of the myocutaneous flap. For the treatment of severe open limb fractures, longitudinal division of the myocutaneous flap using “provisional incision” is a safer approach to the necessary secondary surgery and reduces the possibility of necrosis of the flap.

The single image super-resolution (SISR) technique refers to improving the resolution over the original image. In recent years, we use deep learning-based convolutional neural networks to improve the spatial resolution of images more reasonably. To train such deep learning models, we use training samples consisting of the original HR images and LR images obtained by bicubic downsampling. However, this method of training data using downsampling has a negative impact when applying the trained model to real images. That is because the downsampling function that occurs in the real image is unknown, and the hypothetically created LR image does not represent the resolution degradation that can realistically occur. Therefore, SISR methods that use realistic degradation called real-world super-resolution (RWSR) have been proposed. In this paper, we investigate how such RWSR methods using realistic degradation affect the SISR performance of satellite images. The results of applying the trained model to optical satellite images show that the RWSR method is not the most effective way to handle optical satellite images when compared to the deep learning method without modeling the degradation. In particular, we showed that the effect of RWSR with not only upsampling but also noise and blur removal is significant in the visibility of optical satellite images. Moreover, pre-trained RWSR models can be an aid in visually deciphering ground objects.

4. A new method to quantify particulate sodium and potassium salts (nitrate, chloride, and sulfate) by thermal desorption aerosol mass spectrometry

The reaction of sea salt (or biomass burning) particles with sulfuric acid and nitric acid leads to the displacement of chloride relative to sodium (or potassium). We have developed a new particle mass spectrometer to quantify non-refractory and refractory sulfate aerosols (referred to as refractory aerosol thermal desorption mass spectrometer: rTDMS). The combination of a graphite particle collector and a carbon dioxide laser enables high desorption temperature (blackbody equivalent radiation temperature of up to 930 ∘C). Ion signals originating from evolved gas molecules are detected by a quadrupole mass spectrometer. Here we propose a new method to quantify the mass concentrations of sodium nitrate (NaNO3: SN), sodium chloride (NaCl: SC), sodium sulfate (Na2SO4: SS), potassium nitrate (KNO3: PN), potassium chloride (KCl: PC), and potassium sulfate (K2SO4: PS) particles by using the rTDMS. Laboratory experiments were performed to test the sensitivities of the rTDMS to various types of particles. We measured ion signals originating from single-component particles for each compound and found a good linearity (r2>0.8) between the major ion signals and mass loadings. We also measured ion signals originating from internally mixed SN + SC + SS (or PN + PC + PS) particles and found that the temporal profiles of ion signals at m/z 23 (Na+) (or 39; K+) were characterized by three sequential peaks associated with the evolution of the desorption temperature. We tested potential interferences in the quantification of sea salt particles under real-world conditions by artificially generating “modified” sea salt particles from a mixture of diluted seawater and SN (or SS) solution. The SS/SC ratios estimated from the ion signals at m/z 23, 36 (H35Cl+), and 48 (SO+) agreed well with those predicted from the solution concentrations to within ∼10 %. The SN/SC ratios estimated from the ion signals at m/z 30 (NO+) and 36 also agreed with those predicted from the solution concentrations to within ∼15 %, whereas the SN/SC ratios estimated from m/z 23 were significantly lower than the predicted values. Based on these experimental results, the applicability of the rTDMS to ambient measurements of sea salt particles is discussed.

Plant based natural products have been widely used as antibacterial and insect repellent agents globally. Because of growing resistance in bacterial plant pathogens and urban pests to current methods of control, combined with the long- and short-term negative impact of certain chemical controls in humans, non-target organisms, and the environment, finding alternative methods is necessary to prevent and/or mitigate losses caused by these pathogens and pests. The antibacterial and insect repellent activities of essential oils of novel cultivars of catnip (Nepeta cataria L. cv. CR9) and oregano (Origanum vulgare L. cv. Pierre) rich in the terpenes nepetalactone and carvacrol, respectively, were evaluated using the agar well diffusion assay and petri dish repellency assay. The essential oils exhibit moderate to high antibacterial activity against three plant pathogens, Pseudomonas cichorii, Pseudomonas syringae and Xanthomonas perforans of economic interest and the individual essential oils, their mixtures and carvacrol possess strong insect repellent activity against the common bed bug (Cimex lectularius L.), an urban pest of major significance to public health. In this study, the essential oils of catnip and oregano were determined to be promising candidates for further evaluation and development as antibacterial agents and plant-based insect repellents with applications in agriculture and urban pest management.

One of the major factors controlling the distribution and abundance of marine submerged aquatic vegetation (SAV) is light availability. Reduced water clarity due to sediment loading from rivers greatly affects the health and coverage of seagrasses and seaweeds. Monitoring SAV using unmanned aerial vehicles (UAV) has been getting attention because of its cost-effectiveness and ease of use. In this research, a low-cost UAV was utilized to assess the impacts of river discharges on SAV in Busuanga Island, Philippines. Linear regression was performed to determine the effectivity and accuracy of UAV-based percent cover estimation compared to established field survey methods of monitoring SAV. Water quality was estimated in the study area by performing spatial interpolation methods of in situ measurement of turbidity, chlorophyll, temperature, salinity, and dissolved oxygen using a multi-parameter water quality sensor. Current velocity and tidal fluctuations were monitored using bottom-mounted sensors deployed near the river mouth and in seagrass and seaweed areas with relatively good water clarities. Four stations were surveyed using automated UAV missions which were flown simultaneously with field observations. Each station surveyed has varying distances from the river mouth. Results from the classification of the UAV data and field survey show that SAV is more abundant as the distance from the river mouth increases and the turbidity decreases. Classification overall accuracies of UAV orthophotos ranging from 87.91–93.41% were achieved using Maximum Likelihood (ML) Classification. Comparison of field-based and UAV-based survey of percent cover of seagrasses show an overestimation of 1.75 times from the UAV compared to field observations.

The Cherenkov Telescope Array (CTA) is a next-generation ground-based observatory for gamma-ray astronomy at very high energies. The Large-Sized Telescope prototype (LST-1) is located at the CTA-North site, on the Canary Island of La Palma. LSTs are designed to provide optimal performance in the lowest part of the energy range covered by CTA, down to ≃20 GeV. LST-1 started performing astronomical observations in 2019 November, during its commissioning phase, and it has been taking data ever since. We present the first LST-1 observations of the Crab Nebula, the standard candle of very-high-energy gamma-ray astronomy, and use them, together with simulations, to assess the performance of the telescope. LST-1 has reached the expected performance during its commissioning period—only a minor adjustment of the preexisting simulations was needed to match the telescope’s behavior. The energy threshold at trigger level is around 20 GeV, rising to ≃30 GeV after data analysis. Performance parameters depend strongly on energy, and on the strength of the gamma-ray selection cuts in the analysis: angular resolution ranges from 0.°12–0.°40, and energy resolution from 15%–50%. Flux sensitivity is around 1.1% of the Crab Nebula flux above 250 GeV for a 50 hr observation (12% for 30 minutes). The spectral energy distribution (in the 0.03–30 TeV range) and the light curve obtained for the Crab Nebula agree with previous measurements, considering statistical and systematic uncertainties. A clear periodic signal is also detected from the pulsar at the center of the Nebula.

We study the broadband emission of Mrk 501 using multiwavelength observations from 2017 to 2020 performed with a multitude of instruments, involving, among others, MAGIC, Fermi's Large Area Telescope (LAT), NuSTAR, Swift, GASP-WEBT, and the Owens Valley Radio Observatory. Mrk 501 showed an extremely low broadband activity, which may help to unravel its baseline emission. Nonetheless, significant flux variations are detected at all wave bands, with the highest occurring at X-rays and very-high-energy (VHE) γ -rays. A significant correlation (>3 σ ) between X-rays and VHE γ -rays is measured, supporting leptonic scenarios to explain the variable parts of the emission, also during low activity. This is further supported when we extend our data from 2008 to 2020, and identify, for the first time, significant correlations between the Swift X-Ray Telescope and Fermi-LAT. We additionally find correlations between high-energy γ -rays and radio, with the radio lagging by more than 100 days, placing the γ -ray emission zone upstream of the radio-bright regions in the jet. Furthermore, Mrk 501 showed a historically low activity in X-rays and VHE γ -rays from mid-2017 to mid-2019 with a stable VHE flux (>0.2 TeV) of 5% the emission of the Crab Nebula. The broadband spectral energy distribution (SED) of this 2 yr long low state, the potential baseline emission of Mrk 501, can be characterized with one-zone leptonic models, and with (lepto)-hadronic models fulfilling neutrino flux constraints from IceCube. We explore the time evolution of the SED toward the low state, revealing that the stable baseline emission may be ascribed to a standing shock, and the variable emission to an additional expanding or traveling shock.

LiteBIRD is a JAXA-led Strategic Large-Class mission designed to search for the existence of the primordial gravitational waves produced during the inflationary phase of the Universe, through the measurements of their imprint onto the polarization of the cosmic microwave background (CMB). These measurements, requiring unprecedented sensitivity, will be performed over the full sky, at large angular scales, and over 15 frequency bands from 34 GHz to 448 GHz. The LiteBIRD instruments consist of three telescopes, namely the Low-, Medium- and High-Frequency Telescope (respectively LFT, MFT and HFT). We present in this paper an overview of the design of the Medium-Frequency Telescope (89–224 GHz) and the High-Frequency Telescope (166–448 GHz), the so-called MHFT, under European responsibility, which are two cryogenic refractive telescopes cooled down to 5 K. They include a continuous rotating half-wave plate as the first optical element, two high-density polyethylene (HDPE) lenses and more than three thousand transition-edge sensor (TES) detectors cooled to 100 mK. We provide an overview of the concept design and the remaining specific challenges that we have to face in order to achieve the scientific goals of LiteBIRD.

We report a characterization of the multiband flux variability and correlations of the nearby (z=0.031) blazar Markarian 421(Mrk 421) using data from Mets ̈ahovi, Swift, Fermi-LAT, MAGIC, FACT, and other collaborations and instruments from 2014November till 2016 June. Mrk 421 did not show any prominent flaring activity, but exhibited periods of historically low activity above 1 TeV (F>1TeV<1.7×10−12ph cm−2s−1) and in the 2–10 keV (X-ray) band (F2−10 keV<3.6×10−11erg cm−2s−1),during which the Swift-BAT data suggest an additional spectral component beyond the regular synchrotron emission. The highest flux variability occurs in X-rays and very high-energy (E>0.1 TeV)γ-rays, which, despite the low activity, show a significant positive correlation with no time lag. The HRkeV and HRTeV show the harder-when-brighter trend observed in many blazars, but the trend flattens at the highest fluxes, which suggests a change in the processes dominating the blazar variability. Enlarging our dataset with data from years 2007 to 2014, we measured a positive correlation between the optical and the GeV emission over a range of about 60 d centred at time lag zero, and a positive correlation between the optical/GeV and the radio emission over a range of about 60 d centred at a time lag of 43+9−6d. This observation is consistent with the radio-bright zone being located about 0.2 parsec downstream from the optical/GeV emission regions of the jet. The flux distributions are better described with a lognormal function in most of the energy bands probed, indicating that the variability in Mrk 421 is likely produced by a multiplicative process.

Background Right ventricular fractional area change (RVFAC) as right ventricular function is recently referred as an independent predictor of sudden cardiac death (SCD). Light ventricular function is related with SCD and adopted as a criteria of ICD implantation for primary prophylaxis. The purpose of this study was to evaluate the association of RVFAC and appropriate ICD therapy. Methods Consecutive patients who underwent initial ICD implantation for any diseases except for hypertrophic cardiomyopathy and Brugada syndrome and long QT syndrome were retrospectively enrolled from 2012 to 2018. Transthoracic echocardiographic parameters before ICD implantation were evaluated to investigate the association with appropriate ICD therapy. We analyzed RV function of patients undergoing echocardiography one year after implantation. RVFAC was measured by one physician and one echocardiologist. Results In total, 172 patients (60.3±13.6 years, 131 males) including 63 ischemic cardiomyopathy were enrolled. Ninety patients received an ICD as a secondary prophylaxis. Mean LVEF and RVFAC were 38.3±14.3% and 35.8±8.82%, respectively. There was no correlation between RVFAC and LVEF (correlation coefficient =0.274). Regarding appropriate ICD therapy events, the best cut-off value of RVFAC was 34.8%. The odds ratio of low RVFAC was 2.731 (95%CI: 1.456-5.121, P Conclusion RVFAC and RV function improvement may be important to stratify the prognosis of ICD patients.

This paper reports the effect of a hydrothermal process for alumina sol on the crystal structure of alumina gel derived from hydrothermally treated alumina sol to help push forward the development of low temperature synthesis of α-Al2O3. White precipitate of aluminum hydroxide was prepared with a homogeneous precipitation method using aluminum nitrate and urea in aqueous solution. The obtained aluminum hydroxide precipitate was peptized by using acetic acid at room temperature, which resulted in the production of a transparent alumina sol. The alumina sol was treated with a hydrothermal process and transformed into an alumina gel film by drying at room temperature. Crystallization of the alumina gel to α-Al2O3 with 900 °C annealing was dominant for a hydrothermal temperature of 100 °C and a hydrothermal time of 60 min, as production of diaspore-like species was promoted with the hydrothermal temperature and time. Excess treatments with hydrothermal processes at higher hydrothermal temperature for longer hydrothermal time prevented the alumina gel from being crystallized to α-Al2O3 because the excess hydrothermal treatments promoted production of boehmite.

This paper describes a method to fabricate gadolinium hydroxide (Gd(OH)3) nanoparticles. An opaque solution was prepared by adding basic anion exchange resin (BAER) to a Gd(NO3)3 aqueous solution at room temperature and aging the solution for 12–24 h; the solution became basic because of the exchange of H2O with OH−. The particles in the opaque solution have a needle structure, and their crystal structure was hexagonal Gd(OH)3. Their longitudinal and lateral average particle sizes tend to increase in the ranges of 175.0–222.1 and 33.9–52.3 nm when the aging time increases from 12 to 24 h, respectively. The relaxivity value for T1-weighted imaging was 0.79 mM−1 s−1 for the solution that was prepared at the aging time of 18 h, which was ca. 20% of that for a commercial Gd complex contrast agent.

Background For decades, ICD is a well-established therapy for improving prognosis of structural heart disease with severe cardiac dysfunction, and ICD for primary prophylaxis against sudden cardiac death were routinely provided. However, long-term prognosis and clinical course are different in each individual patient with an ICD, and it is moreover unclear what kind of factors might have influences on their clinical outcomes. Purpose The aim of this study is to clarify long-term prognosis and predictors of future major adverse cardiac events (MACEs) in HFrEF patients with an ICD as primary prophylaxis in Japanese population. Methods We retrospectively analyzed our ICD database. Patients underwent primary prophylactic ICD implantation from 2006 to 2020 at our institute and met the criteria of ICD recommendation of the latest Japanese guideline. Its requirements are receiving optimal medication therapy, symptomatic heart failure (New York Heart Association classification II or greater), and severe cardiac dysfunction (left ventricular ejection fraction (LVEF) is 35% or less). Additionally, prior NSVT is considered Class I ICD recommendation. In the case of ischemic cardiomyopathy (ICM), ICD implantation was done at least 40 days after myocardial infarction and at least 90 days after revascularization. MACEs were defined as composite outcome of cardiovascular death, heart failure hospitalization, and appropriate ICD therapies. Results A total of 148 consecutive patients were enrolled (male, 120 (81%); age, 62.1±11.8 years; LVEF, 23.0±5.86%; left ventricular end-diastolic diameter (LVDd), 67.6±9.26mm; paroxysmal or persistent atrial fibrillation (AF), 38 (26%); NSVT, 113 (76%); use of class III antiarrhythmic drugs, 48 (32%); ICM, 49 (33%); cardiac resynchronization therapy (CRT), 63 (43%)). One hundred twenty patients (81%) were programmed with a shock-only zone over 200 beats per minute. The median follow-up duration was 58.5 months. Among those 148 patients, MACEs were occurred to 60 patients (41%). As a result of dividing all patients into two groups by the occurrence of MACE, LVEF and LVDd were worse in MACE(+) group, whereas, MACE(−) had greater number of co morbidities. The results of cox-regression analysis showed LVDd (HR: 1.07, 95% CI: 1.03–1.12, p Conclusions The incidence of MACEs in patients with an ICD and severe HFrEF was substantially high in this Japanese population. Background Patients with hypertrophic cardiomyopathy (HCM) are at high risk of lethal arrhythmias, and implantable cardioverter defibrillators (ICD) are widely used for prevention of sudden cardiac death (SCD). Apical HCM is a phenotype variant of HCM, with hypertrophy predominantly affecting apex, that was initially described 30 years ago. Apical HCM patients may have different clinical prognosis compared with other subsets of HCM. In previous studies, apical HCM patients seem to have a more benign prognosis than other types of HCM. However, little is known about the long-term outcomes of apical HCM patients and there are many unclear points. Moreover, there are few reports about the clinical prognosis in apical HCM patients with an ICD. Objective The aim of this study is to identify the difference between the prognosis of apical and the other types of HCM patients with an ICD. Methods We retrospectively analyzed the database of our ICD clinic. All subjects underwent ICD implantation between October 2006 and September 2018. We classified HCM patients into LV outflow tract obstruction (LVOTO) and midventricular obstruction (MVO), apical HCM and other non-obstructive types. We divided all the patients into apical and other types of HCM, and examined their background, incidence of appropriate ICD therapies, hospitalization for heart failure, electrical storm and death. Results A total of consecutive 64 Japanese HCM patients with an ICD (follow-up period, 86±24 months; age, 65±14 years; male sex, 83%; left ventricular ejection fraction, 56±14%; LV max wall-thickness, 19±7mm; LV apical aneurysm, 9.4%; 5-year risk of SCD, 4.4±2.1) were enrolled in this study. We classified them into 14 apical HCM and 50 other types of HCM patients. The clinical characteristics and major clinical events of these patients are shown in the Table 1. During the follow-up periods, there were no significant differences in the incidence of electrical storm, hospitalization for heart failure and death between the 2 groups (p=0.11; p=0.60; p=0.39). Appropriate ICD therapies occurred in 6 of 14 (43%) patients with apical HCM and 5 of 50 (10%) patients with other types of HCM (p=0.010). The risk factors of patients with apical HCM patients are shown in Table 2. Conclusions Appropriate ICD therapy was more prevalent in patients with apical HCM, compared to patients with other types of HCM. Aggressive intervention such as catheter ablation for ventricular tachycardia and ventricular fibrillation may be considered in patients with apical HCM and higher score of 5-year risk of SCD. Background Triglyceride (TG) deposit cardiomyovasculopathy (TGCV) is a novel cardiovascular disorder and was recently encoded as an orphan disease in Europe (ORPHA code: 565612). Defective intracellular lipolysis results in TG accumulation in the myocardium and coronary arteries in TGCV. The myocardial washout rate (WR) of iodine-123-β-methyl-p-iodophenylpentadecanoic acid (BMIPP) is an essential indicator to evaluate myocardial lipolysis in vivo, and decreased WR ( Purpose To clarify clinical findings of TGCV including comorbid conditions and laboratory findings. Methods We enrolled 234 patients who underwent BMIPP scintigraphy between September 2015 and July 2019. The distributions of TGCV in each comorbidity, cardiac functions and laboratory findings were investigated. Results In total, 104 patients were diagnosed with definitive TGCV. The BMIPP WR of TGCV patients was −1.37±10.6%. TGCV patients had various comorbid conditions, including coronary artery disease (75%), diabetes mellitus (56%), and heart failure (21%). Left ventricular ejection fraction (LVEF) of TGCV patients was significantly lower than that of non-TGCV patients (38.1±18.0% vs. 43.6±18.9%, p-value=0.026). Moreover, among those who did not take lipid-lowering drugs, there was no difference in the serum TG level between TGCV and non-TGCV patients (TGCV: n=44, 127±84.6 mg/dL, non-TGCV: n=66, 133±70.7 mg/dL, p-value=0.73). Conclusions TGCV patients showed multiple coexistence of coronary artery disease, diabetes mellitus, or heart failure with lower LVEF. Serum TG level was not significantly different between TGCV and non-TGCV patients. Serum TG did not affect the intracellular TG accumulation in TGCV patients directly, and this result was consistent with the pathophysiological hypothesis that the TG accumulation in the myocardial cytoplasm is due to intracellular lipase dysfunction. Background Patient-reported outcome (PRO2) score composed solely of the stool frequency and bleeding components from partial Mayo (pMayo) score are used for the evaluation of clinical symptom in the patients with Ulcerative Colitis (UC). Although pMayo and PRO2 score also perform as well as the full Mayo score to identify the disease activity and clinical response, it is unclear whether serial daily PRO2 score are relevant in clinical practice. Moreover, its clinical significance for long-term outcome has not been established. The aim of this study istherefore to clarify the relationship between daily PRO2 records using a smart device application and prognosis. Methods This study was approved by the ethics committee in Tokyo Medical and Dental University (TMDU). The application was originally developed by the cooperation with TMDU and Hitachi ltd. Patients with UC input their symptoms by themselves in accordance with PRO2 once a day using the application on their smart device. The alert for their symptom exacerbation is set according to our logic. Patient's condition including alert status is confirmed by the doctor once a week using doctor’s PC. Medical information such as therapeutics and blood examination, was extracted from the electronic medical chart in the hospital. The application data was combined with medical information on a daily basis. We defined a start or change of the treatment with prednisone or biologics as treatment change for the outcome of this study. The difference of PRO2 score before the date of treatment change is calculated for each patient and the odds ratio (OR) and AUC of the ROC curve are estimated by univariate logistic regression analysis. Results From July 2020 to March 2022, 183 UC patients participated in this study. Mean age was 46.6 years. Male to female ratio was 81:102. Mean observation period was 346 days. The number of the patients with alert of symptom exacerbation averaged 5.79/week. There were 27 patients with treatment change during the period. PRO2 score in the patients with treatment change was significantly increased compared with the patients with no change in treatment. The OR [95%CI] of the PRO2 change from 90 days to 5 days before treatment change was 7.708 [2.713-21.902], the AUC [95%CI] was 0.872 [0.772-0.973], the OR [95%CI] of the PRO2 change from 10 days to 5 days was 3.309 [1.601-6. 841] and AUC [95%CI] was 0.679 [0.569-0.789]. Conclusion PRO2 variation over 90 days was highly reliable for predicting treatment change. Further analysis will clarify the clinical significance of serial daily PRO2 score recording.

For forest management or monitoring, it is required to constantly measure several parameters of each tree, such as height, diameter at breast height, and trunk volume. Terrestrial laser scanner has been used for this purpose instead of human workers to reduce time and cost for the measurement. In order to use point cloud captured by terrestrial laser scanner in the above applications, it is an important step to extract all trees or their trunks separately. For this purpose, we propose an interactive system in which a user can intuitively and efficiently extract each trunk by a simple editing on the distance image created from the point cloud. We demonstrate the effectiveness of our proposed system from various experiments.

The Kuril harbour seal Phoca vitulina stejnegeri is an endangered species which inhabits southeastern Hokkaido, Japan. Its population declined precipitously from between 1500 and 4800 individuals in the 1940s to a few hundred individuals in the early 1970s. The causes of this decline are thought to be commercial harvesting, bycatch in autumn set-net salmon fishing, and other human activities, including coastal fisheries. To quantify Kuril seal population trends, counts were performed each year at haul-out sites during the pupping season from 1974 to 2010 and during the moulting season from 1983 to 2010. The average population growth rate was ~4% per annum over the past 37 yr. Two haul-out sites (Kenbokki Island and Hattaushi) from which the species had disappeared in the early 1980s showed no evidence of being recolonized. Commercial harvesting ended in the late 1980s and probably had an effect on population trends until the 1990s. The bycatch of seals during autumn set-net salmon fishing in the 2000s remained similar to, or slightly greater than, that during the 1980s. Recently, seals have been observed at 9 haul-out sites during the pupping/moulting season along the coast of southeastern Hokkaido. Approximately 70% of the seals found were at Cape Erimo (~500 seals) and at Daikoku Island and Akkeshi (~250 seals).

Based on two summer spatio-temporal data sets obtained from the northern South China Sea shelf and basin, this study reveals contrasting relationships among bacterial production (BP), dissolved organic (DOC) and primary production (PP) in the transition zone from the neritic to the oceanic regions. Inside the mid-shelf (bottom depth 100 days in the outer shelf, indicating that riverine (Pearl River) DOC might be more labile. The actual mechanism for this is unknown, but might relate to higher inorganic nutrient supply from river/terrestrial sources. The positive correlation of the BP / PP ratios vs. phosphate (and nitrate) concentrations in the inner shelf implies that if anthropogenic mineral loading keeps increasing in the foreseeable future, the near-shore zone may become more heterotrophic, rendering the system a stronger source of CO2.

Study question What was the impact on treatment age in Japan after a subsidy policy change that set age limits for assisted reproductive technology (ART) treatment? Summary answer The national policy introducing age limits in the subsidy may have promoted ART treatment among younger women. What is known already Japan has provided partial subsidies for ART treatment since 2004. To promote treatment at a younger age, the government introduced a subsidy policy in 2016 that set age limits: up to six treatment cycles for women younger than 40 years of age; and up to three cycles for women between 40 and 42 years old. However, two out of 47 prefectures continued to provide subsidies to women aged 43 and older. Study design, size, duration We conducted a time series analysis of the utilisation of ART before and after the introduction of age limits, using data from the Japanese national ART registry from 2012 to 2016. Participants/materials, setting, methods We described the number of fresh and frozen treatment cycles, comparing the number between 45 prefectures that followed the national policy change (hereafter, prefectures with age limits) and two prefectures that did not (hereafter, prefectures without age limits). Ordinary least squares regression models were used to assess the impact of the policy change by prefecture on the number of ART cycles by women of different ages. Main results and the role of chance The overall number of fresh and frozen ART cycles continuously increased in all age groups from 2012 to 2016. Meanwhile, the number of fresh ART cycles among women aged ≤ 36 and 37-39 years in 2016 increased from the previous year by + 4.0% and +1.8% in prefectures with age limits, whereas it decreased in prefectures without age limits: -3.1% and -2.3%, respectively. The number of fresh ART cycles among women aged 40-42 and 43-45 years in prefectures with age limits in 2016 changed by + 1.5% and -0.1%, respectively, whereas it increased considerably in prefectures without age limits by + 9.6% and +65.4%, respectively. Similar changes were shown for the frozen cycles. After controlling for underlying time trends and prefectural characteristics, the policy change significantly increased the number of fresh and frozen ART treatment cycles among women aged ≤ 36 years and decreased the treatment cycles of women aged 40-42 years. Limitations, reasons for caution We evaluated the change observed in the year of the policy change and could not assess longer-term trends. Additionally, unobserved factors might have contributed to the change in treatment numbers. Wider implications of the findings The introduction of a policy to set an age limit for the partial ART subsidy resulted in a significant increase in treatment even among age groups younger than the boundary groups. The policy change might have conveyed educational messages regarding the benefits of early treatment. Trial registration number not applicable

Study question Does sperm DNA integrity affect clinical outcomes of ICSI? Summary answer Use of high DNA integrity sperm selected by microfluidics sperm sorting results in lower miscarriage rates in the patients of 39-years old and more. What is known already High sperm DNA damage is associated with decreased normal fertilization, embryo development and pregnancy rates, and an increased miscarriage rate. On the other hand, oocytes from older women have decreased pregnancy rate, and increased miscarriage rate because of possibility of low ability to repair sperm with DNA fragmentation, and dramatical increases of aneuploidy as women age. A microfluidic sperm selection chamber (MSS, ZyMōt™; DxNow) is a device designed to collect sperm with higher chromatin integrity than density gradient centrifugation (DGC). Study design, size, duration Sperm analysis was performed by sperm chromatin dispersion (SCD) test and comet assay in the same sample of 15 cases between October 2020 and February 2021. ICSI outcomes by DGC and MSS were compared with blastocyst development, and pregnancy rates in vitrified-thawed single blastocyst transfers cycle for 518 cases between August 2018 and May 2021. Participants/materials, setting, methods SCD test was optimized as a rapid procedure, with sperm showing a halo deemed normal, and those without a halo abnormal. Comet assay results were analyzed using CometScore 2.0, with comparison of %Tail DNA. ICSI outcomes were analyzed using multiple logistics regressions of male and female ages. Main results and the role of chance We found a positive correlation between male age and sperm DNA fragmentation rates in raw semen using SCD test (r = 0.70) and Comet assay (r = 0.42). Higher DNA integrity sperm could select using MSS than DGC. In this study with ICSI outcomes, 170 of 318 (53.5%) blastocyst transfers resulted in pregnancy, and 49 (28.8%) subsequently miscarried. The data were classified according to less than or more than 39 years old of male age detected by multiple logistics regressions. In patients with ≥39 years of male age, the female age was significantly higher and blastocyst and pregnancy rates were significantly lower, and the miscarriage rate was significantly higher than Limitations, reasons for caution The sample size for each study was small. Analysis of sperm DNA fragmentation and samples in ICSI outcomes were not the same. The retrospective nature of ICSI outcomes in this study does not allow controlling of unknown confounders. Wider implications of the findings Sperm DNA fragmentation depended on male age affected fertility outcomes. However, when male age is higher, masking the effect of male age by female age. Background Right ventricular fractional area change (RVFAC) as right ventricular function is recently referred as an independent predictor of sudden cardiac death (SCD). The purpose of this study was to evaluate the association of RVFAC and appropriate ICD therapy in order to determine the cut-off value of RVFAC. Methods Consecutive patients who underwent initial ICD implantation for any diseases except for non-dilated phase hypertrophic cardiomyopathy and channelopathy were retrospectively enrolled from 2012 to 2018. Primary endpoint was an initial appropriate ICD therapy. Transthoracic echocardiographic parameters before ICD implantation were evaluated by one physician and one echocardiologist to be validated. Right ventricular dimensions and function were also measured to be analyzed. Results In total, 172 patients (60.3±13.6 years, 131 males) including 63 ischemic cardiomyopathy were enrolled. Ninety patients received an ICD as a secondary prophylaxis. Mean LVEF and RVFAC were 38.3±14.3% and 35.8±8.82%, respectively. There was little correlation between RVFAC and LVEF (correlation coefficient =0.274). Regarding appropriate ICD therapy events, the best cut-off value of RVFAC was 34.8%. The odds ratio of low RVFAC was 2.731 (95%CI: 1.456-5.121, P=0.00174). Secondary prophylactic cohort with low RVFAC showed highest incidence of appropriate ICD therapy as shown in the figure. In multivariate analysis, only low RVFAC is an independent predictor of appropriate ICD therapy (HR: 3.53, 95%CI:1.78- 6.99, P=0.0003). Conclusion Low RVFAC seemed independently associated with increased appropriate ICD therapy.

43. Negative prognostic implications of non-sustained ventricular tachycardias in patients after prophylactic defibrillator implantation

Background Non-sustained ventricular tachycardia (NSVT) is frequent phenomenon in severe heart failure with reduced ejection fraction (HFrEF) patients, and causes any negative impacts on such patients. In the Japanese Circulation Society (JCS) and Japanese Heart Rhythm Society (JHRS) guidelines, NSVT is regarded as a major component of indication for implantable cardioverter defibrillator (ICD) implantation. However, the long-term prognostic significance of NSVT in severe HFrEF is incompletely resolved. Purpose The aim of this study is to investigate the relation between prior NSVT episodes and major adverse cardiac events (MACEs) in HFrEF patients with an ICD as primary prophylaxis. Methods We retrospectively analyzed our ICD database. Patients underwent primary prophylactic ICD implantation from 2007 to 2018 following ICD recommendation of JCS and JHRS guidelines. Patients met the criteria of receiving optimal medication therapy, symptomatic heart failure (New York Heart Association classification II or greater), and severe cardiac dysfunction (left ventricular ejection fraction (LVEF) is 35% or less). In the case of ischemic cardiomyopathy (ICM), implantation of ICD was done at least 40 days after myocardial infarction and at least 90 days after revascularization. Incidence of NSVT episodes were identified through daily electrocardiogram (ECG), Holter ECG or monitor ECG in the hospital. MACEs were defined as composite outcome of cardiovascular death, heart failure hospitalization, and appropriate ICD therapies. Results A total of 148 consecutive patients were enrolled (male, 120 (81%); age, 62.1±11.8 years; LVEF, 23.0±5.86%; left ventricular end-diastolic diameter (LVDd), 67.6±9.26mm; paroxysmal or persistent atrial fibrillation (AF), 38 (26%); NSVT, 113 (76%); use of class III antiarrhythmic drugs, 48 (32%); ICM, 49 (33%); cardiac resynchronization therapy (CRT), 63 (43%)). The median follow-up duration was 58.5 months. As a result of comparison of the Kaplan-Meier curve between NSVT group (n=113) and non-NSVT group (n=35), cardiovascular death, heart failure hospitalization, and appropriate ICD therapy were not statistically different (Figures). Of those, MACEs were occurred to 60 patients (41%). The results of cox-regression analysis showed LVDd (HR: 1.07, 95% CI: 1.03-1.12, p Conclusions In this Japanese population, the long-term prognosis of severe HFrEF patients is considered to be comparable regardless of prior NSVT episodes. However, the incidence of MACEs in patients with severe HFrEF after ICD implantation was substantially high. ICM, left ventricle size, and atrial fibrillation were the potential risk factors for MACEs as the previous reports showed.

- Abstract
We present the results of a model-based search for continuous gravitational waves from the low-mass X-ray binary Scorpius X-1 using LIGO detector data from the third observing run of Advanced LIGO and Advanced Virgo. This is a semicoherent search that uses details of the signal model to coherently combine data separated by less than a specified coherence time, which can be adjusted to balance sensitivity with computing cost. The search covered a range of gravitational-wave frequencies from 25 to 1600 Hz, as well as ranges in orbital speed, frequency, and phase determined from observational constraints. No significant detection candidates were found, and upper limits were set as a function of frequency. The most stringent limits, between 100 and 200 Hz, correspond to an amplitude h(0) of about 10(-25) when marginalized isotropically over the unknown inclination angle of the neutron star's rotation axis, or less than 4 x 10(-26) assuming the optimal orientation. The sensitivity of this search is now probing amplitudes predicted by models of torque balance equilibrium. For the usual conservative model assuming accretion at the surface of the neutron star, our isotropically marginalized upper limits are close to the predicted amplitude from about 70 to 100 Hz; the limits assuming that the neutron star spin is aligned with the most likely orbital angular momentum are below the conservative torque balance predictions from 40 to 200 Hz. Assuming a broader range of accretion models, our direct limits on gravitational-wave amplitude delve into the relevant parameter space over a wide range of frequencies, to 500 Hz or more.

Ectopic ovary is a rare gynaecological condition that results in problems with menstruation and pregnancy and may develop into a malignant tumour. However, as the condition is often asymptomatic, diagnosis is difficult and frequently delayed. We report a case of a 42-year-old female who presented with a 10-day history of abdominal pain. The patient underwent surgery that confirmed the diagnosis of an ectopic ovary with an internal abscess. The findings of our study indicate that ectopic ovaries can present with an abscess. Ectopic ovaries should be included in the differential diagnosis of masses with internal abscesses.
