An 82-year-old, right-handed male patient was admitted to the emergency room because of continuous rhythmic clonic movements of the right forearm and hand. He was diagnosed with epilepsia partialis continua (EPC) and treated successfully with levetiracetam. In conclusion, we recommend levetiracetam as the first line treatment for EPC, which is mostly refractory to classical antiepileptic drugs. [ABSTRACT FROM AUTHOR]

Background And Purpose: Elevated early troponin T levels are a biomarker of acute coronary syndrome. But recent studies suggest possible association of troponin with stroke. We aimed to investigate the relationship between carotid stenosis, carotid plaque morphology and elevated troponin levels. Methods: We retrospectively reviewed patient files with acute ischaemic stroke who are admitted to Ankara Atatürk Education and Research Hospital Neurology clinic between June and September 2018. We included patients whose troponin levels were measured during the first admission at the emergency room. We also noted patient demographics, clinical characteristics, cardiac variables and if there is insular area infarct. We excluded patients under 18-years-old, patients with chronic renal failure and pregnant women. Results: We reviewed 63 patient's files. The mean age was 74.70 [51-98]. Thirty-four of patients were women (54%). There was a statistically significant difference between with high levels of troponin T and carotid plaque morphology (p=0.029) and diabetes (p=0.049). Also there was statistically significant and moderate correlation between tropoin T and CRP levels (p=0.006, r=0.343) Conclusions: A possible cause of troponin T elevation in acute stroke may be vascular smooth muscle damage due to inflammation in especially complicated carotid atherosclerotic plaques. Objective: There is little research about the effect of Restles Leg Syndrome (RLS) on academic success and day time sleepiness. In this study, we aimed to determine the effect of RLS on day time sleepiness and the effect of RLS on academic gadepoint average (GPA). Methods: Our study included 171 preclinic students in 2. and 3. years at AYBU Medical Faculty. RLS diagnosis was made according to International Classification of Sleep Disorders-3. International Restless Legs Syndrome Study Group (IRLSSG) survey was used to define the intensity of RLS. Epworth Sleepiness Scale (ESS) was used tofor the evaluation of the day time sleepiness of the students. Chi-square test was used to for the statistical analysis between GPA and RLS severity, RLS frequency and day time sleepiness. Results: One-hundred-and-twenty of the students were women (70.2%), 51 of the students were men (29.8%). Seventeen of the students diagnosed as RLS (9.9%). Thirteen of them were women (76.5%) and only 4 of them were men (23.5%). GPA of the students with RLS was statically lower than the other students' GPA(p=0.02). There was no statistically significant difference between GPA and RLS intensity (p=0.516). There was statistically significant difference between the frequency of RLS symptoms and GPA (p=0.045). Among students with RLS, students who have excessive day time sleepiness have statistically significantly lower GPA (p=0.038). Discussion: RLS is a very common sleep related movement disorder which causes academic failure at school and concentration difficulty as well as in somnia and excessive day time sleepiness. Objectives: The aim of this study was to investigate the reliability of the Revised Illness Perception Questionnaire (IPQ) and to determine the effects of earthquake experience on the perception in migraine patients., Methods: The sample was composed of 62 outpatients, consisting of with migraine diagnosis who were in Erciş during earthquake (n=33) and who had never had any earthquake experience (n=29).The interview form, IPQ-R and Beck Depression Scale (BDS) were applied. The study was carried out on migraine patients whose mean age was 31 and who had been diagnosed since 7.8 years. Comparison of groups with earthquake experience (group1) and without experience (group 2) there were no difference in point of demographic findings and disease severity (p>0.05)., Results: In the part concerning the manifestations of the disease, the most frequently manifestations were found pain, headache and fatige. The test was determined to be reliable. Illness coherence and timeline (cyclic) subscale scores (p<0.05) and BDS score (z: -2.006, p<0.05)were significantly higher in group 1. Although an earthquake caused an increase in depression scores did not cause much change in the IPQ-R scores. Group1 understand disease better and realize of the cyclical nature of the disease. Other perception parameters of the disease were same in both groups., Conclusion: A severe life event such as an earthquake did not much change IPQ-R scores in migraine patients. The results of this study demonstrated that IPQ could be used reliably in the Turkish migraine patients.
