Aim: Malignant chest wall tumors are rare in children. We aimed to study the management of the Ewing family of tumors (EFT) in the chest walls of children. Materials and Methods: The files of patients diagnosed with primitive neuro-ectodermal tumor and Ewing sarcoma of the chest wall in the prior 2 decades were retrospectively reviewed. Results: A total of 15 children with a median age of 10 (8-14) years were included. Their symptoms were palpable mass (n=6), chest pain (n=4), B symptoms (n=2), cough (n=2) and abdominal pain. Their history revealed delayed diagnoses in 1/3 of the patients due to a misinterpretation of the symptoms. All children except for one had tru-cut biopsy and upfront chemotherapy. The median greatest diameter of mass at diagnosis was 100 (67-148) mm and 51 (39-100) mm at preoperative imaging after chemotherapy. Pleural effusion (n=8), costal destruction (n=9), extension to neural foramina (n=3), pulmonary or diaphragmatic nodule (n=5) and distant bone metastasis (n=4) were present at diagnosis in some cases. More than one surgery was performed in 9 (60%) of the children in order to remove the primary tumor and metastases. Costal excision (n=11), diaphragmatic resection (n=6) and chest wall reconstruction with graft material were performed on 8 patients. All cases had radiotherapy postoperatively. Mortality occurred in 9 (60%) patients due to local recurrences or metastases. Only 3 children reached 5-year survival and were considered to be cured. The remaining 3 patients completed their first line oncologic treatments and were under surveillance without treatment. Conclusion: The EFT in the chest wall are aggressive tumors with poor prognosis despite multimodality treatment. Background. For children with non-rhabdomyosarcoma soft tissue sarcomas, a risk-adapted treatment approach is generally used in order to minimize treatment-related morbidity and mortality in low-risk patients and maximize the benefit in high-risk patients. Our aim in this review is to discuss the prognostic factors, riskadapted treatment options and the details of radiotherapy. Methods. The publications reached by searching the keywords 'pediatric soft tissue sarcoma', 'nonrhabdomyosarcoma soft tissue sarcoma (NRSTS)', and 'radiotherapy' in Pubmed database were evaluated in detail. Results. Today, based on prospective COG-ARST0332 and EpSSG studies, a risk-adapted multimodal treatment approach has become the standard in pediatric NRSTS. According to them, adjuvant chemotherapy/ radiotherapy can be safely omitted in low-risk patients, while adjuvant chemotherapy/radiotherapy or both are recommended in intermediate and high-risk groups. Recent prospective studies for pediatric patients have reported excellent treatment outcomes with smaller radiotherapy fields and lower doses than adult series. The primary goal of surgery is maximal tumor resection with negative margins. In cases that are initially unresectable, neoadjuvant chemotherapy and radiotherapy should be considered. Conclusions. A risk-adapted multimodal treatment approach is the standard in pediatric NRSTS . Surgery alone is sufficient in low-risk patients, and adjuvant therapies may safely be omitted. On the contrary, in intermediateand high-risk patients, adjuvant treatments should be applied to reduce recurrence rates. In unresectable patients, the chance of surgery increases with the neoadjuvant treatment approach and thus treatment results may improve. In the future, outcome might be improved with further clarification of molecular features and targeted therapies in such patients. [ABSTRACT FROM AUTHOR]

Purpose: The American Brachytherapy Association is attempting to develop standards for delivering brachytherapy, although differences in practice have been reported in the literature. This study evaluated vaginal cuff brachytherapy (VBT) practice and quality of life-related recommendations among Turkish radiation oncologists. Material and methods: A nationwide web-based 17-item survey was distributed to the members of the Turkish Society for Radiation Oncology. These members received e-mail notifications, and a link was posted on the Turkish Society for Radiation Oncology internet site to solicit voluntary responses The survey addressed the simulation processes, target volume, prescribed dose, delivery schedules, and recommendations related to vaginal side effects. Results: Fifty-seven radiation oncologists responded to the survey. The most used dose fraction schemes for adjuvant VBT were 7 Gy × 3 fractions (30%), 5.5 Gy × 5 fractions (26%), and 6 Gy × 5 fractions (28%). The preferred VBT scheme was 5 Gy × 3 fractions (50%) when the external beam radiotherapy (EBRT) dose was 45 Gy external radiotherapy, while the preferred schemes were 6 Gy × 3 fractions (30%) or 5 Gy × 3 fractions (32%) when the external radiotherapy dose was increased to 50.4 Gy. One-half of the respondents delivered VBT twice a week, and the dose was prescribed to 0.5 cm from vaginal mucosa by 86% of the respondents. There was no common definition for the dose prescription length, which was defined as 3 cm from the vaginal cuff in 33% of responses and as 4 cm in 35% of responses. For serous and clear cell histological types, 38% of the respondents targeted "full cylinder length". To prevent vaginal side effects, 78% of the respondents recommended using a vaginal dilator and/or sexual intercourse after VBT. Conclusions: This survey revealed variations in the clinical practice of VBT among Turkish radiation oncologists, which suggests that standardization is necessary. [ABSTRACT FROM AUTHOR]

Amaç: Hodgkin Lenfoma (HL) tedavisinde radyoterapinin (RT) rolü geçmişten günümüze çok değişikliğe uğramıştır. Günümüzde risk adaptif kemoterapi (KT) ve KT'ye yanıta göre RT yaklaşımları benimsenmektedir. Bugün KT sonrası pekiştirme amaçlı RT endikasyonu konulan hastalarda standart yaklaşım tutulu saha RT (ISRT) olarak kabul edilmektedir. ISRT'nin sınırları 2020 International Lymphoma Radiation Oncology Group (ILROG) kılavuzu ile belirlenmiş olsa da tedavi hacimleri konusunda gri alanlar ve klinisyenlerin kararına kalan bazı durumlar mevcuttur. Biz bu çalışma ile ülkemizde erişkin HL tedavisinde radyasyon onkologlarının (RO) tedavi eğilimlerini belirlemeyi ve bu eğilimleri klinik tecrübeye göre analiz etmeyi amaçladık. Gereç ve Yöntem: Bu çalışmada erişkin HL tedavisinde RT eğilimlerini saptamak amacı ile klinik senaryolar içeren anket oluşturulmuş (Şekil 1.) ve anketin değişik klinik tecrübeye sahip RO tarafından yanıtlanması istenmiştir. İstatistiksel analizler SPSS versiyon 23 ile yapılmıştır. Bulgular: Çalışmaya 26 RO katılmış ve dahil olan hekimler klinik tecrübesine göre (<10 yıl vs. =10 yıl) iki gruba ayrılmıştır. Sorulara verilen yanıtların klinik tecrübeye göre değerlendirildiği sonuçlar Tablo 1'de verilmiştir. Hekimlerin 13'ü (%50) 10 yıl ve üzeri tecrübeye, 13'ü (%50) 10 yıl altı tecrübeye sahiptir. Klinik tecrübesi fazla hekimlerin 4'ünün tedavi alanı olarak tutulu alan RT (IFRT) tercih ettikleri ve genel olarak klinik tecrübesi fazla grupta daha geniş tedavi alanlarının tercih edildiği görülmektedir. Çalışmaya katılan RO'larının 21'i (%80,8) ILROG kılavuzunun tüm olgularda çözüm sağlamadığını, ancak bazı olgularda günlük pratikte yol gösterici oluğunu belirtmişlerdir. Sonuç: Erişkin HL tedavisinde ISRT konusunda önerilerinde bulunan son kılavuz olan ILROG 2020 her klinik senaryo için ihtiyacı karşılamamakta, özellikle; tanıda büyük hacimli (bulky) tutulum, kemoterapi (KT) sonrası rezidü hastalık, ara PET-BT'de Deauville skor 3 hastalık, dalak tutulumu gibi senaryolarda klinik yaklaşımda heterojeniteye sebep olmaktadır. TROD hematolojik onkoloji-pediatrik onkoloji ve TBI çalışma grubu olarak bu konuda çalıştay planlamakta ve kendi kılavuzumuzu oluşturmak istemekteyiz. Amaç: Lokal ileri servikal kanserinin standart tedavisi eşzamanlı kemoterapi ile birlikte eksternal radyoterapi ve brakiterapidir (BRT). Çalışmamızın amacı lokal ileri serviks kanseri veya inoperable endometrium kanseri nedeniyle intrauterin BRT uygulanması planlanan 18 yaş üstü hastalarda BRT uygulaması öncesinde ve fraksiyon arasında BAÖ'nin tekrarlanarak anksiyete düzeyinin başlangıca göre farklılığının saptanması, tedavi uygulamaları ve hasta özelliklerinin hasta anksiyetesi üzerine etkisinin değerlendirilmesidir. Gereç ve Yöntem: Anksiyete düzeyini değerlendirmek için 0-21 arasında puanlanan Beck Anksiyete Ölçeği (BAÖ), ağrı düzeyini değerlendirmek içinse 1-10 arasında puanlanan Yüz İfadelerini Derecelendirme Ölçeği kullanılmıştır. Dokuz merkezde intrauterin BRT uygulanan toplam 121 hastaya tanı anında, birinci ve son fraksiyonda BAÖ, birinci ve son fraksiyonda ağrı ölçeği (AÖ) uygulandı. BAÖ ve AÖ skoru arasındaki ilişki için lineer regresyon analizi, BAÖ ve AÖ skorlaması ile çeşitli hasta özellikleri arasındaki ilişki için Kruskal Wallis ve Bağımsız Örneklem T testi kullanıldı. Bulgular: Hastaların tanı zamanı Beck skoru medyan değeri tanı anı 7, birinci fraksiyon sonrası 10 ve son fraksiyonda ise 7 bulunmuştur. Tanı zamanı yapılan BAÖ ile birinci fraksiyon sonrası yapılan BAÖ kıyaslandığında skor istatistiksel anlamlı olarak artış göstermiştir (p=0,047). Birinci ve son fraksiyon sonrası yapılan yapılan BAÖ kıyaslandığında ise son fraksiyonda skorda istatistiksel anlamlı bir azalma görülmüştür (p=0,003). Tanı zamanı yapılan BAÖ ile son fraksiyon sonrası yapılan BAÖ kıyaslandığında ise skorda istatistiksel anlamlılık bulunmamıştır (p=0,970). Hastaların ilk fraksiyon sonrası yapılan AÖ skoru medyan değeri 4, son fraksiyon sonrası yapılan AÖ skoru medyan değeri 3 olarak bulunmuştur. Hastaların ilk ve son fraksiyon sonrası AÖ istatistiksel olarak kıyaslandığında anlamlı fark bulunmamıştır. Birinci fraksiyon sonrası yapılan BAÖ ile AÖ da arasında yapılan lineer regresyon analizinde istatistiksel anlamlı olarak BAÖ arttıkça AÖ skorunun arttığı görülmüştür (p=0,002). Son fraksiyon sonrası yapılan BAÖ ile AÖ skoru arasında yapılan lineer regresyon analizinde istatistiksel anlamlı olarak BAÖ arttıkça AÖ skorunun arttığı görülmüştür. Doğum sayısı ile tanı zamanı yapılan BAÖ skoru ilişkisi için yapılan lineer regresyon analizinde iki parametre arası doğrusal bir ilişki bulunmuştur (p<0,01). Doğum sayısı ile birinci fraksiyonda yapılan AÖ için yapılan lineer regresyon analizinde iki parametre arası bir ilişki bulunamamıştır (p=0.3). Tanı zamanı Beck skoru; menapoz durumu, eğitim durumu,önceki jinekolojik operasyon öyküsü, ailede kanser geçmişi, BRT uygulama tipi, sonda kullanımı, sigara ve alkol durumuna göre istatistiksel farklılık göstermedi. Tanı zamanı Beck skoru görsel ve sözel olarak brakiterapi alan hastalarda en yüksek iken, sadece sözel eğitim alanlarda en düşüktü. Sonuç: Çalışmamızın sonucunda intrakaviter brakiterapi tedavisi alan hastalarda tanı anı Beck skoru ilk fraksiyonda artış gösterirken son fraksiyonda ise azalmıştır. AÖ skorlaması ise ilk fraksiyondan son fraksiyona gelindiğinde azalmıştır. Hastaların ağrı düzeyinin azalmamasına rağmen yapılan işlemin bilinmesi hastaların anksiyete düzeylerinde istatistiksel olarak belirgin azalma sağlamıştır. Ayrıca anksiyete seviyesi ile ağrı düzeyinin doğru orantılı olması hastaların anksiyetesini kontrol altına alabilirsek ağrı kontrolünün de daha kolay yapılabileceğini düşündürmektedir. Ewing sarcoma (ES) is a rare tumor in infants and prognosis is controversial. There are no standard recommendations for treatment in such very young patients. Generally, radiotherapy (RT) is not a part of treatment in infants due to the risk of severe late side effects. In this case report, we report a 7-month-old boy with diagnosis of left mastoid bone ES with lung metastases at diagnosis, showing a rapidly fatal outcome despite aggressive systemic chemotherapy and RT without surgery.
