Abstract

Abstract Japanese Hayabusa2 spacecraft has successfully carried out an impact experiment using a small carry-on impactor (SCI) on an asteroid (162173) Ryugu. We examine the size distribution of particles inside and outside an artificial impact crater (the SCI crater) based on the images taken by the optical navigation camera onboard the Hayabusa2 spacecraft. The circumferential variation in particle size distribution inside the SCI crater is recognized and we interpret that major circumferential variation is caused by the large boulders inside the SCI crater that existed prior to the impact. The size distribution inside the SCI crater also shows that the subsurface layer beneath the SCI impact site had a large number of particles with a characteristic size of – 9 cm, which is consistent with the previous evaluations. On the other hand, the size distribution outside the SCI crater exhibits the radial variation, implying that the deposition of ejecta from the SCI crater is involved. The slope of the size distribution outside the crater at small sizes differs from the slope of the size distribution on the surface of Ryugu by approximately 1 or slightly less. This is consistent with the claim that some particles are buried in fine particles of the subsurface origin included in ejecta from the SCI crater. Thus, the particle size distributions inside and outside the SCI crater reveal that the subsurface layer beneath the SCI impact site is rich in fine particles with – 9 cm in size while the particles on the surface have a size distribution of a power-law form with shallower slopes at small sizes due to the deposition of fine ejecta from the subsurface layer. Finally, we discuss a process responsible for this difference in particle size distribution between the surface and the subsurface layers. The occurrence of segregation in the gravitational flow of particles on the surface of Ryugu is plausible. Graphical Abstract

Abstract

Preceding a core-collapse supernova (CCSN), various processes produce an increasing amount of neutrinos of all flavors characterized by mounting energies from the interior of massive stars. Among them, the electron antineutrinos are potentially detectable by terrestrial neutrino experiments such as KamLAND and Super-Kamiokande (SK) via inverse beta decay interactions. Once these pre-supernova (pre-SN) neutrinos are observed, an early warning of the upcoming CCSN can be provided. In light of this, KamLAND and SK, both located in the Kamioka mine in Japan, have been monitoring pre-SN neutrinos since 2015 and 2021, respectively. Recently, we performed a joint study between KamLAND and SK on pre-SN neutrino detection. A pre-SN alert system combining the KamLAND detector and the SK detector was developed and put into operation, which can provide a supernova alert to the astrophysics community. Fully leveraging the complementary properties of these two detectors, the combined alert is expected to resolve a pre-SN neutrino signal from a 15 M _⊙ star within 510 pc of the Earth at a significance level corresponding to a false alarm rate of no more than 1 per century. For a Betelgeuse-like model with optimistic parameters, it can provide early warnings up to 12 hr in advance.

Abstract

Among multimessenger observations of the next Galactic core-collapse supernova, Super-Kamiokande (SK) plays a critical role in detecting the emitted supernova neutrinos, determining the direction to the supernova (SN), and notifying the astronomical community of these observations in advance of the optical signal. In 2022, SK has increased the gadolinium dissolved in its water target (SK-Gd) and has achieved a Gd concentration of 0.033%, resulting in enhanced neutron detection capability, which in turn enables more accurate determination of the supernova direction. Accordingly, SK-Gd’s real-time supernova monitoring system has been upgraded. SK_SN Notice, a warning system that works together with this monitoring system, was released on 2021 December 13, and is available through GCN Notices. When the monitoring system detects an SN-like burst of events, SK_SN Notice will automatically distribute an alarm with the reconstructed direction to the supernova candidate within a few minutes. In this paper, we present a systematic study of SK-Gd’s response to a simulated Galactic SN. Assuming a supernova situated at 10 kpc, neutrino fluxes from six supernova models are used to characterize SK-Gd’s pointing accuracy using the same tools as the online monitoring system. The pointing accuracy is found to vary from 3° to 7° depending on the models. However, if the supernova is closer than 10 kpc, SK_SN Notice can issue an alarm with three-degree accuracy, which will benefit follow-up observations by optical telescopes with large fields of view.

Abstract

We report the first search result for the flux of astrophysical electron antineutrinos for energies ${ \mathcal O }(10)\,\mathrm{MeV}$ in the gadolinium-loaded Super-Kamiokande (SK) detector. In 2020 June, gadolinium was introduced to the ultrapure water of the SK detector in order to detect neutrons more efficiently. In this new experimental phase, SK-Gd, we can search for electron antineutrinos via inverse beta decay with efficient background rejection thanks to the high efficiency of the neutron tagging technique. In this paper, we report the result for the initial stage of SK-Gd, during 2020 August 26, and 2022 June 1 with a 22.5 × 552 kton · day exposure at 0.01% Gd mass concentration. No significant excess over the expected background in the observed events is found for the neutrino energies below 31.3 MeV. Thus, the flux upper limits are placed at the 90% confidence level. The limits and sensitivities are already comparable with the previous SK result with pure water (22.5 × 2970 kton · day) owing to the enhanced neutron tagging. Operation with Gd increased to 0.03% started in 2022 June.

Background: The Global Anticoagulant Registry in the FIELD–Atrial Fibrillation (GARFIELD-AF) is an ongoing prospective noninterventional registry, which is providing important information on the baseline characteristics, treatment patterns, and 1-year outcomes in patients with newly diagnosed non-valvular atrial fibrillation (NVAF). This report describes data from Indian patients recruited in this registry. Methods and results: A total of 52,014 patients with newly diagnosed AF were enrolled globally; of these, 1388 patients were recruited from 26 sites within India (2012–2016). Abstract

We compute the contribution from the twist-3 fragmentation function for light hadron production in collisions between transversely and longitudinally polarized protons, i.e., p↑p→→hX, which can cause a double-spin asymmetry (DSA) ALT. This is a naïve T-even twist-3 observable that we analyze in collinear factorization using both Feynman gauge and lightcone gauge as well as give a general proof of color gauge invariance. So far only twist-3 effects in the transversely polarized proton have been studied for ALT in p↑p→→hX. However, there are indications that the naïve T-odd transverse single-spin asymmetry (SSA) AN in p↑p→hX is dominated not by such distribution effects but rather by a fragmentation mechanism. Therefore, one may expect similarly that the fragmentation contribution is important for ALT. Given possible plans at RHIC to measure this observable, it is timely to provide a calculation of this term.

Abstract

Neurofibromatosis type 1 (von Recklinghausen's disease) is an inherited nervous system disease affecting around 1 in 3000 and is characterized by café-au-lait spots, pigmented hamartomas of the iris (Lisch nodules), and multiple neurofibromas. Plexiform neurofibromas can show intratumoral bleeding, and the fragile surrounding blood vessels and soft tissues carry a risk of escalation to lethal bleeding. We encountered a 59-year-old man with neurofibromatosis type 1 who presented with a rapidly growing massive hematoma in the left buttock after hitting it against a chair 12 hours earlier. He was tachycardic and hypotensive (hemoglobin, 5.7 g/dl), with severe swelling and tenderness in the left gluteal region. Plain computed tomography revealed a large, hyperdense, subcutaneous soft-tissue mass on the left buttock measuring 23 × 12 × 24 cm. Selective angiography demonstrated significant arterial supply to the lesion from a highly hypertrophied left L4 lumbar artery as well as the left superior gluteal artery. Embolization of these vessels was performed to reduce tumor blood supply. After returning to the intensive care unit, a central area of skin necrosis was noted. The wound was debrided; healing with scarring after 6 weeks with the tumor returning to its original size within 6 months. Hemorrhage in neurofibromatosis type 1 is attributed to a friable vasculature secondary to arterial dysplasia or vascular invasion by the neurofibroma. As neurofibroma is highly vascular, vessel rupture can occur spontaneously or with trivial trauma. Arterial embolization represents the method of choice for treating intratumoral bleeding in neurofibromatosis type 1, as a minimally invasive means of controlling arterial bleeding.

Abstract

Studies of meteorite analysis and theoretical modeling have indicated the possibility that some carbonaceous near-Earth asteroids are thermally altered due to radiative heating during close approaches to the Sun in addition to parent body processes (Nakamura, 2005; Marchi et al., 2009; Chaumard et al., 2012). In April 2019, the Hayabusa2 mission successfully completed an artificial impact experiment on the carbonaceous near-Earth asteroid 162173 Ryugu (Arakawa et al., 2020), which provided an opportunity to investigate the effects of radiative heating through the exposed subsurface material. Here we report observations of the Ryugu’s subsurface material by the Near-Infrared Spectrometer (NIRS3) on the Hayabusa2 spacecraft. Spectra of the subsurface material exhibit a slightly stronger and peak-shifted hydroxyl absorption feature compared to that observed for the surface, indicating that space weathering and/or radiative heating caused a subtle change in the spectrum of Ryugu surface. However, the shape of the absorption feature still suggests that the subsurface material experienced heating above 300 ˚C similar to the surface. In contrast, our thermal modeling shows that radiative heating does not increase the subsurface temperature at 1 m depth above 200 ˚C even if the semimajor axis is reduced down to 0.344 au. This supports that the Ryugu material would have been preferentially altered due to radiogenic and/or impact heating on the parent body rather than radiative heating.

Abstract

In this study, delayed detached-eddy simulations (DDESs) based on the Spalart–Allmaras turbulence model are investigated for separation flows. Three simple modifications are considered: the Reynolds-averaged Navier–Stokes (RANS) turbulence model coefficient, Crot, is calibrated to achieve a better leading-edge separation prediction in DDES; the large-eddy simulation (LES) coefficient, CDES, is assessed to obtain better dissipation control in DDES; and dirty-cell treatments in three-dimensional unstructured grids are conducted for a smooth LES/RANS transition. Numerical results confirm the effects of the three aforementioned steps, such as the reproducibility of the measured pressure distribution over the main wing in unsteady turbulence simulations of low-speed buffet around the NASA Common Research Model. Thus, these modifications will potentially serve as good alternatives, without major programming efforts, to the conventional approaches for practitioners.

Abstract

Choroidal anastomosis, a hemorrhage-prone periventricular collateral manifestation in Moyamoya disease, outflows to the cortex posterior to the central sulcus. The objective of the present study was to test whether the angiographic extent of revascularization posterior to the central sulcus contributes to the postoperative reduction of choroidal anastomosis.This retrospective cohort study included choroidal anastomosis-positive hemispheres before direct bypass surgery. The postoperative reduction of choroidal anastomosis was determined by a consensus of 2 raters according to the previous research. An imaging software automatically traced the angiographic revascularization area, which was subsequently divided into anterior and posterior parts by an anatomic line corresponding to the central sulcus. Each area was quantitatively measured as a percentage relative to the whole supratentorial area.Postoperative reduction of choroidal anastomosis was achieved in 68 (85.0%) of the 80 included hemispheres. The revascularization area posterior to the central sulcus was significantly larger in the hemispheres with reduction than in those with no reduction (mean, 15.2% [SD, 7.1%] versus 4.2% [SD, 3.4%],The results suggest that a larger revascularization posterior to the central sulcus is associated with postoperative reduction of choroidal anastomosis regardless of the extent of anterior revascularization. It might facilitate optimal selection of the revascularization site for preventing hemorrhage.

Abstract

The influence of acoustic characteristics of in-flight noise on airplane operations and sound impression has been investigated through objective and subjective measure including the flight simulation. The authors verified the influence of the noise conditions including seven conditions, in which the sound pressure levels and frequency characteristics of them were parametrically changed, on the airplane operations using shortterm flight simulation. Resultantly, the change of the sound pressure levels of the in-flight noise did not greatly affect the operation accuracy although clearly affected the subjective impressions. On the other hand, it has been suggested that the change of the frequency characteristics of in-flight noise affected not only the subjective impressions but also the operation accuracy.

Abstract

Many small boulders with reflectance values higher than 1.5 times the average reflectance have been found on the near-Earth asteroid 162173 Ryugu. Based on their visible wavelength spectral differences, Tatsumi et al. (2021) defined two bright boulder classes: C-type and S-type. These two classifications of bright boulders have different size distributions and spectral trends. In this study, we measured the spectra of 79 bright boulders and investigated their detailed spectral properties. Analyses obtained a number of important results. First, S-type bright boulders on Ryugu have spectra that are similar to those found for two different ordinary chondrites with different initial spectra that have been experimentally space weathered the same way. This suggests that there may be two populations of S-type bright boulders on Ryugu, perhaps originating from two different impactors that hit its parent body. Second, the model space-weathering ages of meter-size S-type bright boulders, based on spectral change rates derived in previous experimentally irradiated ordinary chondrites, are 0.1-1 Myr, which is consistent with the crater retention age (, Comment: 21 pages, 20 Figures, 1 Table, Published in Icarus, November 15 2021, 369 114591

Abstract

Importance: Sodium glucose cotransporter 2 inhibitors reduce morbidity and mortality in patients with heart failure and reduced ejection fraction (HFrEF). Clinicians may find estimates of the projected long-term benefits of sodium glucose cotransporter 2 inhibitors a helpful addition to clinical trial results when communicating the benefits of this class of drug to patients. Objective: To estimate the projected long-term treatment effects of dapagliflozin in patients with HFrEF over the duration of a patient’s lifetime. Design, Setting, and Participants: Exploratory analysis was performed of Dapagliflozin and Prevention of Adverse Outcomes in Heart Failure (DAPA-HF), a phase 3 randomized, placebo-controlled clinical trial conducted at 410 sites in 20 countries. Patients with an ejection fraction less than or equal to 40% in New York Heart Association functional classification II to IV and elevated plasma levels of N-terminal pro B-type natriuretic peptide were enrolled between February 15, 2017, and August 17, 2018, with final follow-up on June 6, 2019. Mean (SD) duration of follow-up was 17.6 (5.2) months. Interventions: Dapagliflozin, 10 mg, once daily vs placebo in addition to standard therapy. Main Outcomes and Measures: The primary composite outcome was time to first hospitalization for heart failure, urgent heart failure visit requiring intravenous therapy, or cardiovascular death. The trial results were extrapolated to estimate the projected long-term treatment effects of dapagliflozin over the duration of a patient’s lifetime for the primary outcome and the secondary outcome of death from any cause. Results: A total of 4744 patients (1109 women [23.4%]; 3635 men [76.6%]) were randomized in DAPA-HF, with a mean (SD) age of 66.3 (10.9) years. The extrapolated mean event-free survival for an individual aged 65 years from a primary composite end point event was 6.2 years for placebo and 8.3 years for dapagliflozin, representing an event-free survival time gain of 2.1 years (95% CI, 0.8-3.3 years; P = .002). When considering death from any cause, mean extrapolated life expectancy for an individual aged 65 years was 9.1 years for placebo and 10.8 years for dapagliflozin, with a gain in survival of 1.7 years (95% CI, 0.1-3.3; P = .03) with dapagliflozin. Similar results were seen when extrapolated across the age range studied. In analyses of subgroups of patients in DAPA-HF, consistent benefits were seen with dapagliflozin on both event-free and overall survival. Conclusions and Relevance: These findings indicate that dapagliflozin provides clinically meaningful gains in extrapolated event-free and overall survival. These findings may be helpful in communicating the benefits of this treatment to patients with HFrEF. Trial registration: ClinicalTrials.gov Identifier: NCT03036124.

Abstract

Analyses of meteorites and theoretical models indicate that some carbonaceous near-Earth asteroids may have been thermally altered due to radiative heating during close approaches to the Sun1–3. However, the lack of direct measurements on the subsurface doesn’t allow us to distinguish thermal alteration due to radiative heating from parent-body processes. In April 2019, the Hayabusa2 mission successfully completed an artificial impact experiment on the carbonaceous near-Earth asteroid (162173) Ryugu4,5, which provided an opportunity to investigate exposed subsurface material and test potential effects of radiative heating. Here we report observations of Ryugu’s subsurface material by the Near-Infrared Spectrometer (NIRS3) on the Hayabusa2 spacecraft. Reflectance spectra of excavated material exhibit a hydroxyl (OH) absorption feature that is slightly stronger and peak-shifted compared with that observed for the surface, indicating that space weathering and/or radiative heating have caused subtle spectral changes in the uppermost surface. The strength and shape of the OH feature suggests that the subsurface material experienced heating above 300 °C, similar to the surface. In contrast, thermophysical modelling indicates that radiative heating cannot increase the temperature above 200 °C at the estimated excavation depth of 1 m, even at the smallest heliocentric distance possible for Ryugu. This supports the hypothesis that primary thermal alteration occurred on Ryugu’s parent body. Hayabusa2 created an artificial crater on Ryugu to analyse the subsurficial material of the asteroid. Results show that the subsurface is more hydrated than the surface. It experienced alteration processes that can be traced back to Ryugu’s parent body.

Abstract

Background Blood pressure (BP) variability has reportedly been a risk factor for various clinical events. To clarify the influence of BP visit‐to‐visit variability on adverse events in patients with nonvalvular atrial fibrillation, a post hoc analysis of the J‐RHYTHM Registry was performed. Methods and Results Of 7406 outpatients with nonvalvular atrial fibrillation from 158 institutions, 7226 (age, 69.7±9.9 years; men, 70.7%), in whom BP was measured 4 times or more (14.6±5.0 times) during the 2‐year follow‐up period or until occurrence of an event, constituted the study group. SD and coefficient of variation of BP values were calculated as BP variability. Thromboembolism, major hemorrhage, and all‐cause death occurred in 110 (1.5%), 121 (1.7%), and 168 (2.3%) patients, respectively. When patients were divided into quartiles of systolic BP‐SD (P =0.015 for thromboembolism; HR, 2.60, 95% CI, 1.36–4.97, P =0.004 for major hemorrhage; and HR, 1.85, 95% CI, 1.11–3.07, P =0.018 for all‐cause death) after adjusting for components of the CHA 2 DS 2 ‐VASc score, warfarin and antiplatelet use, atrial fibrillation type, BP measurement times, and others. These findings were consistent when BP‐coefficient of variation was used instead of BP‐SD. Conclusions Systolic BP visit‐to‐visit variability was significantly associated with all adverse events in patients with nonvalvular atrial fibrillation. Further studies are needed to clarify the causality between BP variability and adverse outcomes in patients with nonvalvular atrial fibrillation. Abstract

We present the first observation by the Telescope Array Surface Detector (TASD) of the effect of thunderstorms on the development of cosmic ray single count rate intensity over a 700 km$^{2}$ area. Observations of variations in the secondary low-energy cosmic ray counting rate, using the TASD, allow us to study the electric field inside thunderstorms, on a large scale, as it progresses on top of the 700 km$^{2}$ detector, without dealing with the limitation of narrow exposure in time and space using balloons and aircraft detectors. In this work, variations in the cosmic ray intensity (single count rate) using the TASD, were studied and found to be on average at the $\sim(0.5-1)\%$ and up to 2\% level. These observations were found to be both in excess and in deficit. They were also found to be correlated with lightning in addition to thunderstorms. These variations lasted for tens of minutes; their footprint on the ground ranged from 6 to 24 km in diameter and moved in the same direction as the thunderstorm. With the use of simple electric field models inside the cloud and between cloud to ground, the observed variations in the cosmic ray single count rate were recreated using CORSIKA simulations. Depending on the electric field model used and the direction of the electric field in that model, the electric field magnitude that reproduces the observed low-energy cosmic ray single count rate variations was found to be approximately between 0.2-0.4 GV. This in turn allows us to get a reasonable insight on the electric field and its effect on cosmic ray air showers inside thunderstorms.

Abstract

The near-Earth asteroid (162173) Ryugu displays a Cb-type average spectrum and a very low average normal albedo of 0.04. Although the majority of boulders on Ryugu have reflectance spectra and albedo similar to the Ryugu average, a small fraction of boulders exhibit anomalously high albedo and distinctively different spectra. A previous study (Tatsumi et al., 2021) based on the 2.7-km observations and a series of low-altitude (down to 68 m) descent observations conducted prior to the first touchdown have shown that the spectra of these anomalous boulders can be classified into two distinct groups corresponding to S and C type asteroids. The former originate most likely from an impactor that collided with Ryugu's parent body, whereas the latter may be from portions of Ryugu's parent body that experienced a different temperature history than experienced by the majority of boulder materials. In this study, we analyzed images captured after the first touchdown to determine the quantitative properties of these bright boulders on Ryugu. We measured the sizes of more than a thousand bright boulders and characterized the morphologic properties of the largest ones. Analyses revealed many properties of bright boulders important for the evolution of Ryugu and its parent body. The size-frequency distributions of S- and C-type bright boulders follow power laws. We obtained the ratios of the total volume and surface area of S-type bright boulders to those of average dark boulders on the Ryugu surface. Also, many of the bright boulders are embedded in a larger substrate boulder, suggesting that they have experienced mixing and conglomeration with darker fragments on the parent body, rather than gently landing on Ryugu during or after its formation by reaccumulation. Abstract

In this paper we report the first close, high-resolution observations of downward-directed terrestrial gamma-ray flashes (TGFs) detected by the large-area Telescope Array cosmic ray observatory, obtained in conjunction with broadband VHF interferometer and fast electric field change measurements of the parent discharge. The results show that the TGFs occur during strong initial breakdown pulses (IBPs) in the first few milliseconds of negative cloud-to-ground and low-altitude intracloud flashes, and that the IBPs are produced by a newly-identified streamer-based discharge process called fast negative breakdown. The observations indicate the relativistic runaway electron avalanches (RREAs) responsible for producing the TGFs are initiated by embedded spark-like transient conducting events (TCEs) within the fast streamer system, and potentially also by individual fast streamers themselves. The TCEs are inferred to be the cause of impulsive sub-pulses that are characteristic features of classic IBP sferics. Additional development of the avalanches would be facilitated by the enhanced electric field ahead of the advancing front of the fast negative breakdown. In addition to showing the nature of IBPs and their enigmatic sub-pulses, the observations also provide a possible explanation for the unsolved question of how the streamer to leader transition occurs during the initial negative breakdown, namely as a result of strong currents flowing in the final stage of successive IBPs, extending backward through both the IBP itself and the negative streamer breakdown preceding the IBP., Comment: Typo fixed and reference added. Abstract

We report wound management using a vacuum-assisted closure (VAC) system for the cannula sites of long-term extracorporeal biventricular assist devices (BiVADs). A 23-year old Chinese female patient with a diagnosis of giant cell myocarditis needed extracorporeal BiVADs for more than 9 months. When the cannula sites appeared necrotic 3 months after BiVADs placement, she underwent negative pressure wound therapy prophylactically for four cannula sites, using a VAC system for 3 months, followed by no infections. Such prophylactic VAC therapy may be useful to avoid cannula site infections, which is still a fatal adverse event causing sepsis, especially in patients with extracorporeal BiVADs.

Abstract

Long-term survival of patients with neonatal-onset carbamoyl-phosphate synthetase 1 deficiency (CPS1D), an autosomal recessive disorder characterized by repeated, life-threatening hyperammonemia, is rare. We describe the diagnosis and clinical management of a teenager with neonatal-onset CPS1D who did not undergo therapeutic liver transplantation.Following emergent neonatal therapy, the patient was diagnosed with CPS1D based on clinical, radiological, biochemical and genetic analyses. Her clinical course, neurobehavioral development and therapeutic interventions are presented and discussed.Born from nonconsanguineous parents, the proband underwent phototherapy for neonatal jaundice, associated with acute encephalopathy, apnea and cerebral edema. Based on blood and urinary biochemical abnormalities, neonatal-onset CPS1D was diagnosed. Her hyperammonemia was corrected by hemodialysis, followed by sodium benzoate, L-arginine, levocarnitine and protein-free diet therapy. Because of a relapse and persistent neurobehavioral regression by age 1, a planned liver transplantation was cancelled. At age 10, sodium phenylbutyrate was substituted as ammonia scavenger. Genetic testing revealed compound heterozygote c.2359CT (R787X) and c.236+6TC variants of CPS1, confirming her diagnosis. Despite severe neurological sequelae, the patient is 16 and in stable condition.Our case suggests that early hemodialysis and pharmacologic interventions for acute neonatal hyperammonemia can improve the prognosis of patients with neonatal-onset CPS1D.

Abstract

Blowing a crater in asteroid Ryugu The Hayabusa2 spacecraft was designed to collect samples from the nearby asteroid (162173) Ryugu and return them to Earth for laboratory analysis. Arakawa et al. describe how the spacecraft's Small Carry-on Impactor was fired into the asteroid's surface, producing an artificial impact crater. Analysis of the resulting plume of ejecta, recorded by a remote camera, sets an upper limit on the strength of the rubble-pile surface. The crater has a semicircular shape, probably due to a large boulder buried close to the impact location. The crater exposed material from Ryugu's subsurface, which has not been subjected to space weathering, that is suitable for collection by Hayabusa2. Science , this issue p. 67

Abstract

Motivated by the detection of a significant dipole structure in the arrival directions of ultrahigh-energy cosmic rays above 8 EeV reported by the Pierre Auger Observatory (Auger), we search for a large-scale anisotropy using data collected with the surface detector array of the Telescope Array Experiment (TA). With 11 years of TA data, a dipole structure in a projection of the right ascension is fitted with an amplitude of 3.3+- 1.9% and a phase of 131 +- 33 degrees. The corresponding 99% confidence-level upper limit on the amplitude is 7.3%. Funding Acknowledgements Young Investigator Grant of Japanese Society of Cardiovascular Anesthesia Background / Introduction Evidences have shown that cardiac function decline and systolic phase change are caused by massive afterload of the stenosed aortic valve in patients with severe aortic valve stenosis. As a result of the reduction of the left ventricular afterload by valve replacement, the cardiac function recovers. However, it has not been clarified yet how the changes in cardiac function affect the relationship between the right and left heart systems, as well as the systole phase. TAPSE and MAPSE are known as indices of simple cardiac function evaluation by measuring the movement distance of the atrioventricular annulus. We obtained these indices (i.e. TAPSE, MAPSE) within the same heartbeat using speckle tracking analysis of the atrioventricular annulus and evaluated the changes in cardiac function and phase between the right and left heart systems. Purpose To reveal the relationship of cardiac function and time phase between the right and left heart systems by evaluating the maximum movement distance and time of the atrioventricular annulus within the same heartbeat and the same view in patients with severe aortic valve stenosis before and after TAVI. Methods A prospective cohort study was conducted. We recruited and analyzed 44 patients with severe aortic valve stenosis who received TAVI treatment, able to record baseline before treatment and follow-up 6 months and 12 months after treatment at our hospital from March 2017 to May 2019. Patients were excluded if more than 2 degree of atrioventricular valve regurgitation or incomplete data. The apical four-chamber view was used for speckle tracking analysis with the origin of the apical extension and region of interest (ROI) of the mitral annulus and tricuspid annulus. For each patient before treatment, at 6 months and 12 months after treatment, the maximum contraction distance (DM), maximum contraction time (TM) of the mitral valve annulus, maximum contraction distance (DT) and maximum contraction time (TT) of the tricuspid annulus were measured. Maximum contraction distance ratio (DM/ DT) and maximum contraction time ratio (TM/ TT) were calculated. For statistical analysis, t-test and ANOVA were used, and a significance threshold of p Results TM/ TT decreased at 12 months after TAVI, and DM/ DT increased significantly at 6 months and 12 months after TAVI when compared to baseline before treatment. Conclusions In patients with severe aortic valve stenosis, the correction of cardiac function difference between the right and left heart systems occurs from 6 months after TAVI. Moreover, the correction of contraction phase difference between the right and left heart systems at 12 months after TAVI.
