Abstract

Background: Although the effects of the trauma center(TC) were researched in several studies, there have been few studies on changes in the regional mortality due to the implementation of a TC.An emergency medical center (EMC) and TC were implemented at Nagasaki University Hospital (NUH) for the first time in the Nagasaki medical region of Japan in April 2010 and October 2011, respectively, and they have cooperated with each other in treating trauma patients. The purpose of this study was to investigate the effects on the early mortality at population level of a TC working in cooperation with an EMC. Methods: This is a retrospective study using standardized regional data (ambulance service record) in Nagasaki medical region from April 2007 through March 2017. We included 19,045 trauma patients directly transported from the scene. The outcome measures were prognosis for one week. To examine the association between the implementation of the EMC and TC and mortality at a region, we fit adjusted logistic regression models. Results: The number of patients of each fiscal year increased from 1492 in 2007 to 2101 in 2016.The number of all patients transported to NUH decreased until 2009 to 70, but increased after implementation of the EMC and TC. Overall mortality of all patients in the region improved from 2.3% in 2007 to 1.0% in 2016.In multivariate logistic regression model, odds ratio of death was significantly smaller at 2013 and thereafter if the data from 2007 to 2011 was taken as reference. Conclusions: Implementation of the EMC and TC was associated with early mortality in trauma patients directly transported from the scene by ambulance. Our analysis suggested that the implementation of EMC and TC contributed to the improvement of the early mortality at a regional city with 500000 populations. Abstract

STUDY OBJECTIVE: To determine the relationship between bispectral index (BIS) and sedation. DESIGN: Prospective, observational clinical study. SETTING: Intensive care unit of a public hospital in Japan. PATIENTS: 22 ASA physical status I, II, and III middle-aged (18-65 yrs) and elderly (>65 yrs) patients receiving postoperative sedation with midazolam. INTERVENTIONS: Patients were allocated to two groups: Group M was composed of middle-aged patients (<65 yrs) and Group H elderly patients (>65 yrs). Midazolam was administered at a bolus dose of 0.1 mg/kg, followed by a continuous dose of 0.04 mg/kg per hour, which was adjusted every two hours to achieve a target level of sedation at 3-6 on the Ramsay Sedation Scale (RSS); buprenorphine was administered at a constant rate (0.625 mug kg(-1) hr(-1)). MEASUREMENTS: BIS value, RSS, midazolam dose, body temperature (BT), heart rate, dopamine dose, and mean arterial pressure were recorded every two hours by an independent nurse. Data were analyzed using Spearman rank correlation and the Mann-Whitney U test. MAIN RESULTS: BIS values decreased depending on depth of sedation; a significant correlation was noted between groups in RSS and BIS. The BIS values at levels of RSS 5 and 6 were significantly lower in Group H than Group M. Abstract

To investigate whether or not protoporphyria can be induced in hairless mice, feed containing 0.5% griseofulvin (GF) was given to hairless Hos^ HR/De strain mice for two to four months. Administration of GF caused protoporphyria in the hairless mice, that is, protoporphyrin levels in the erythrocytes and the liver were extremely elevated. Acute skin changes such as erythema, edema, blister and crust formation were produced by exposing the hairless mice with protoporphyria to 400 nm of light using the metal halide lamp. From these results, we confirmed that hairless mice of HR/De strain can be utilized as a model for protoporphyria. These mice are useful for macroscopic and histopathological observations of skin changes and are also convenient for photosensitivity experiments., 熱帯医学 Tropical medicine 30(3). p219-224, 1988

45. Five Cases of Porphyria Cutanea Tarda with Mild Cutaneous Changes: Evaluation of the Efficacy of Phlebotomy by the Pattern Analysis of Urinary Porphyrins

Abstract

Five cases of porphyria cutanea tarda (PCT) with mild cutaneous changes are reported. Acute episodes of photosensitivity were not seen in all patients. Laboratory examinations showed a predominance of uroporphyrin (UP) excretion in the urine in 4 out of 5 cases. One case showed a relatively low urinary porphyrin level even though abnormality of the porphyrin pattern was present. Four cases showing a predominance of UP excretion in the urine revealed a high level of serum coproporphyrin (CP) as well. Histopathologically, all cases showed a deposition of PAS positive materials surrounding the small blood vessels in the upper dermis, in addition to the deposition of IgG materials visualized by the immunofluorescent technique.

Abstract

We performed a histopathological investigation of the bullous lesions in 3 cases of porphyria cutanea tarda. All cases showed subepidermal bullae by light microscopy. PAS positive materials were present on the roof of the bullae and partially present on their bases. Electron‐microscopically, the basal lamina was clearly recognized on the base. From these results, we suggest that the blister in porphyria cutanea tarda occurs initially within the junctional zone; this initial bulla may quickly change into a dermolytic bulla with additional stimulation.

Abstract

Fecal, urinary and erythrocyte porphyrin analyses using the solvent extraction method were performed in 26 patients with porphyria cutanea tarda (PCT) and 144 normal controls. The levels of fecal uroporphyrin (UP) and coproporphyrin (CP) were markedly increased in the PCT group, especially in comparison with the protoporphyrin (PP) level. The values of the UP/PP and CP/PP ratios in the feces were also elevated over those in the control group. It appears that fecal porphyrin excretion is basically similar to urinary porphyrin excretion in PCT. The analysis of fecal porphyrins and the observation of fecal UP/PP and CP/PP ratios may be helpful in the biochemical diagnosis of PCT. In particular, the elevation of CP/PP ratio is characteristic of PCT.

48. PCB and PCQ Concentrations in Subcutaneous Tissue from Patients with PCB Poisoning (Yusho)

Abstract

Polychlorinated biphenyl (PCB) and polychlorinated quaterphenyl (PCQ) concentrations in the blood and subcutaneous fat tissue from 11 patients with PCB poisoning (Yusho) and 10 controls were analyzed by electron captured gaschromatography. The mean concentration levels of blood PCB and PCQ and the CB% ratio were elevated in patients with Yusho. The PCB pattern was the A type in 5 cases and the B type in 6 others. The mean concentration level of PCB in subcutaneous fat tissue was 1579.45 ppb in the Yusho group, 3.9 times higher than that in the control group. The mean concentration level of PCQ was 206.65 ppb, 119 times higher than that in the control group. The mean value of CB% was 3.22 in the Yusho group and 0.66 in the control group. These results show that PCB and PCQ accumulate in the subcutaneous fat tissue of patients with PCB poisoning (Yusho), as late as 17 years after diagnosis of the disease. The PCB and PCQ concentration levels in the subcutaneous fat tissue appear to be useful in diagnosis and evaluation of prognosis in this disease. Investigations of the elimination of PCB and PCQ in subcutaneous tissues should be done.
