Mechanical ventilation through endotracheal intubation is an important measure for ICU treatment of critically ill patients, which can effectively improve tissue hypoxia and enhance patients' oxygenation function. Objective To compare the hemodynamics and stress reaction of opioid free anesthesia (OFA) and opioid anesthesia (OA) in patients undergoing video-assisted thoracoscopic surgery. Methods A total of 110 patients who underwent thoracoscopic bullectomy or lobectomy under general anesthesia at Fuyang People’s Hospital from January to October 2023 were randomly divided into OA group (n=55) and OFA group (n=55). Patients in OA group received OA combined with thoracic paravertebral nerve block during surgery, while the patients in OFA group used esketamine instead of opioids. Hemodynamic indicators [mean arterial pressure (MAP) and heart rate (HR)] and stress response indicators [blood glucose (Glu), cortisol (Cor), 8-isoprostaglandin F2α (8-iso)] at different time points and the dosage of propofol used during surgery were observed. Postoperative recovery time, Visual Analogue Scale (VAS) score and adverse reactions (nausea, vomiting, skin itching and mental symptoms) within 48 h after surgery were recorded. Results At the beginning of the study, 110 patients were included, of which 4 patients were excluded (1 patient was converted to open chest surgery, 1 patient experienced allergic reactions during surgery, and 2 patients were lost to follow-up). Finally, 53 cases were included in each of OA group and OFA group. Compared with the OA group, MAP and HR were significantly higher during induction, intubation and skin incision, while MAP and HR were significantly lower during extubation in OFA group (P＜0.05). And the dosage of propofol used during surgery was lower in the OFA group than that in the OA group [(428.4±147.5)mg vs (499.5±171.1) mg, t=2.291, P=0.024]. Besides, the incidence of postoperative nausea and vomiting was lower in the OFA group (1.89% vs 15.09%, χ2=4.371, P=0.037). There was no statistically significant difference in the stress response indicators (Glu, Cor, 8-iso), postoperative recovery time, incidence of skin itching and mental symptoms, and VAS score between two groups (P＞0.05). Conclusion Multimodal OFA using esketamine combined with thoracic paravertebral nerve block can effectively control surgical stress response, reduce intraoperative propofol dosage, maintain the circulatory system steady during anesthesia.

Background The incidence of adverse pregnancy outcomes has remained high in recent years, which poses a serious threat to maternal and neonatal life and health. Chronic stress is known to be a risk factor for adverse pregnancy outcomes, while the relationship between allostatic load (AL) as a composite physiological index of chronic stress, and adverse pregnancy outcomes has not been clarified. Objective To explore the effect of AL on adverse pregnancy outcomes in women in late pregnancy. Methods Women in late pregnancy who met the study requirements were recruited as study subjects by using the convenience sampling method from November 2021 to November 2022 in the obstetrics outpatient clinics of the 901 Hospital, Joint Logistic Support Force of the Chinese People's Liberation Army, Jin'an Maternal and Child Health Care Hospital. Basic information such as general and obstetric data were collected through questionnairs, biological indicators were collected through physical examination and laboratory tests, and AL scores of the study subjects were calculated by referring to AL-related literature; pregnancy outcome information was obtained by reviewing the hospital electronic medical record system. Multivariate Logistic regression analysis was used to explore the effect of AL on adverse pregnancy outcomes in women in late pregnancy. Background As an objective index to evaluate chronic stress, allostatic load (AL) is associated with adverse pregnancy outcomes. At present, there are few longitudinal studies to analyze the influencing factors of AL among pregnant women. Objective To investigate the status of AL and its influencing factors in pregnant women at different trimesters. Methods This study was designed as a prospective study. Questionnaire survey, physical examination and laboratory examination were administered to 152 women in the first (≤14 weeks), second (23-27 weeks) and third (30-34 weeks) trimesters of pregnancy collected in the obstetrics outpatient clinics of the 901 Hospital, Joint Logistic Support Force of the Chinese People's Liberation Army, Jin'an Maternal and Child Health Care Hospital from November 2021 to November 2022 by using convenience sampling method. The Edinburgh Postpartum Depression Scale (EPDS) was used to assess the depression during pregnancy. Referring to previous studies, the relevant assessment indexes of metabolic system, cardiovascular system, and immune system were used to calculate the total score of AL, and AL≥3 score was used as a criterion for determining high AL level at different trimesters of pregnancy. Multivariate Logistic regression analysis was used to explore the influencing factors of AL in the first, second and third trimesters. Results Among 152 pregnant women, the mean total AL score was (2.06±1.68), (2.07±1.84) and (2.07±1.68) in the first, second and third trimesters; 52 (34.2%), 54 (35.5%) and 50 (32.9%) women were in high level of AL (total score of AL≥3) in the first, second and third trimesters. Background Chronic stress is known to be associated with the onset of depression, allostatic load (AL) may play a significant role in the pathogenesis of depression as a comprehensive physiological index to evaluate response to the chronic stress. Objective To identify the association between AL level and depression among women in early pregnancy.
