Angiotensin II, the effector peptide of the renin angiotensin system, regulates cellular growth in response to developmental, physiological, and pathological processes. It is well established that Angiotensin II has an important role in regulating cardiovascular hemodynamics and structure. Multiple lines of evidence have suggested the existence of at least two distinct Angiotensin II receptor subtypes, as AT1 and AT2 receptors. Most of the known effects of Angiotensin II in cardiovascular tissues are attributable to the AT1 receptor, but less is known about the AT2 receptor. AT1 and AT2 receptors may exert opposite effects in terms of cell growth and blood pressure regulation. Herein, we will discuss the role of the AT2 receptor in the cardiovascular system. Key words: Renin, Angiotensin II, AT2 Receptor, Cardiovascular System, Anjiotensin II (AII) gelişimsel, fizyolojik ve patolojik süreç içinde hücresel büyümeyi düzenlemekle birlikte, reninanjiotensin sisteminin de etkili bir peptididir. AII'nin kardiyovasküler hemodinamiyi düzenlemesindeki rolü oldukça güçlü şekilde kanıtlanmıştır. Birçok çalışmada AT1 ve AT2 reseptörleri şeklinde tanımlanan, birbirinden farklı en az 2 tane Anjiotensin II reseptör subtipi olduğu ortaya konulmuştur. AII kardiyovasküler sistem (KVS) üzerindeki pek çok etkisini AT1 reseptörü üzerinden gerçekleştirmekte olup, AT2 reseptörünün katkısı ise çok fazla bilinmemektedir. AT1 ve AT2 reseptörlerinin, hücre büyümesi ve kan basıncı düzenlenmesinde birbirine zıt etkileri vardır. Biz bu derlemede AT2 reseptörlerinin KVS üzerindeki etkilerini ele alacağız. Amaç: Total larenjektomi uygulanan hastalarda farengokutanöz fistül gelişme sıklığı ve fistül gelişimi üzerinde etkili olan faktörler değerlendirildi.Hastalar ve Yöntemler: Larenks epidermoid karsinomu nedeniyle total larenjektomi yapılan 53 hasta geriye dönük olarak incelendi. Risk faktörleriyle birlikte farengokutanöz fistül sıklığı, fistül gelişme süresi, tedavi yaklaşımları ve hastanede kalış süreleri incelendi.Bulgular: On dokuz hastada %35.9 cerrahiden ortalama 7.4 gün sonra farengokutanöz fistül gelişti. Yaş, sigara, kronik sistemik hastalık varlığı, tümörün T1-T2 evresi, yerleşimi, diferansiyasyonu, eşzamanlı iki taraflı boyun diseksiyonu yapılması, farenks kapatılmasında T/Y şeklinde dikiş tekniklerinin kullanılması ve ameliyat sonrası ilk 48 saatte ateş görülmesi, fistül gelişimi ile ilişkili bulunmadı; buna karşın, tümörün T4 evresi, alkol kullanımı, ameliyat sonrası anemi, hipoproteinemi fistül gelişimiyle anlamlı ilişki gösterdi. Hastanedeki kalış süresi fistül gelişen hastalarda ortalama 40.5 gün, fistül gelişmeyenlerde 15.3 gün idi. Sonuç: Ameliyat sonrası anemi, hipoproteinemi, alkol kullanımı ve T4 evresi farengokutanöz fistül gelişimini anlamlı derecede artırmaktadır, Objectives: We investigated the incidence and etiologic factors of pharyngocutaneous fistulas occurring after total laryngectomy. Patients and Methods: The study included 53 patients 46 males, 7 females; mean age 59 years; range 34 to 84 years who underwent total laryngectomy for epidermoid carcinoma. Risk factors for pharyngocutaneous fistula, its incidence and time of occurrence, treatment approaches, and length of hospital stay were evaluated. Results: Pharyngocutaneous fistula developed in 19 patients 35.9% within a mean of 7.4 days postoperatively. Age, smoking, the presence of chronic systemic disease; T1-T2 stage, localization, and differentiation of tumor, concomitant bilateral neck dissection, T/Y shaped pharyngeal closure, and high fever in the first postoperative 48 hours did not show any significant relationship with the formation of fistula. Factors significantly associated with fistula formation were T4 stage, alcohol consumption, postoperative anemia, and hypoproteinemia. The mean hospital stay was 40.5 days in patients with pharyngocutaneous fistula compared with 15.3 days in patients without fistula. Conclusion: Postoperative anemia, hypoproteinemia, alcohol consumption, and T4 stage are significant risk factors for pharyngocutaneous fistula formation.
