Abstract Background Recent studies have shown that deficient mismatch repair (dMMR) rectal cancer may be related to treatment resistance, resulting in a worse prognosis than proficient MMR (pMMR) rectal cancer. The purpose of this study was to explore whether surgery plus other treatments (radiotherapy and chemotherapy) can bring more benefits to these patients than surgery alone. Methods A retrospective study of 168 patients with rectal adenocarcinoma who underwent total mesorectal excision was conducted using immunohistochemical methods to determine MMR status and a propensity score matching model to minimize potential confounding factors between subgroups of patients with different treatment regimens. Kaplan–Meier analysis, log-rank tests, and Cox regression models were used to assess overall survival (OS) and disease-free survival (DFS) in patient subgroups. Results Only 6.9% (n = 168) of patients in the total cohort had dMMR rectal adenocarcinoma, and the most common cause of dMMR was a PMS2 deletion (103, 61.3%). The median DFS of the surgery alone group was 45.7 months (IQR, 40.9 to 77.8), and the median DFS of the surgery plus other treatment group was 43.9 months (IQR, 14.2 to 80.1). The surgery alone group was superior to the surgery plus other treatment group (HR, 0.16; 95% CI, 0.07 to 0.38; p = 0.005). There was no significant difference in OS (45.8 (IQR, 41.0 to 79.8) vs. 45.9 (IQR, 38.5 to 80.3)) between the two groups (HR, 0.57; 95% CI, 0.23 to 1.40; p = 0.263). Conclusions For patients with locally advanced dMMR rectal adenocarcinoma, compared with surgery alone, surgery plus other treatment options (radiotherapy and chemotherapy) do not grant long-term survival benefits but rather shorten DFS.

Abstract We evaluate the prognostic value of chemotherapy and other prognostic factors on overall survival among colon patients with deficient mismatch repair (dMMR), and determine the optimum time to start chemotherapy after surgery. Data of 306 colon cancer patients with dMMR who received radical surgery were collected from three Chinese centers between August 2012 and January 2018. Overall survival (OS) was assessed with the Kaplan–Meier method and log-rank. Cox regression analysis were used to assess influencing prognosis factors. The median follow-up time for all patients was 45.0 months (range, 1.0–100). There was a nonsignificant OS benefit from chemotherapy for patients with stage I and stage II disease, including high-risk stage II disease (log-rank p: 0.386, 0.779, 0.921), and a significant OS benefit for patients with stage III and stage IV disease for receiving post-operation chemotherapy (log-rank p = 0.002, 0.019). Stage III patients benefitted from chemotherapy regimens that contained oxaliplatin (log-rank p = 0.004), and Starting chemotherapy with oxaliplatin treatment earlier resulted in better outcomes (95% CI 0.013–0.857; p = 0.035). Chemotherapy regimens containing oxaliplatin can prolong the survival time of stage III and IV dMMR colon cancer patients. This beneficial manifestation was more pronounced after starting chemotherapy treatment early post operation. High risk stage II dMMR colon patients including T4N0M0 cannot benefit from chemotherapy.

BackgroundNeck-shoulder pain is one of the most common discomfort symptoms among nursing staff, mostly caused by a heavy workload, restricted workstation, and prolonged poor posture.ObjectiveTo investigate the prevalence of neck-shoulder pain among nursing staff in tertiary general hospitals in Hunan Province, and to analyze workload-related risk factors.MethodsFrom October to December 2018, a multi-stage stratified randomized cluster sampling method was adopted to select a total of 1 200 nursing staff who met the inclusion andexclusion criteria in six tertiary general hospitals from five geographic subdivisions of Hunan Province were selected. A self-designed questionnaire was used to collect demographic characteristics, and past 1-month prevalence of neck-shoulder pain, a neck disability index (NDI) was used to assess the impact of neck pain on daily life, and functional disability was graded by the index of impaired neck function; a self-designed questionnaire was adopted to evaluate potential workload-related risk factors (working time, postural load, and force load) of neck-shoulder pain after a Delphi expert consultation. A total of 1 161 valid questionnaires were returned, and χ２test and logistic regression model were used to screen the potential risk factors for neck-shoulder pain.ResultsThe prevalence rate of neck-shoulder pain in the past 1 month was 87.5% (1 016/1 161) and varied by different characteristics of the nursing staff, ranging from 76.9% to 91.2%. The scores of the 10 items of the NDI ranged from 0 to 5, but most of them were 0, 1, and 2; of the 10 items, neck pain had the greatest impact on sleep (1.30±1.21) and the least impact on self-care behaviors (0.35±0.65). The index of impaired neck function showed that the majority (65.4%) of nurses had mild cervical spine dysfunction and 30.1% had moderate cervical spine dysfunction. The univariate analysis results revealed that 24 out of the 31 workload-related risk factors had significant differences, and further multiple analysis results showed that three variables entered the logistic regression model. Under the same conditions, nurses with ≥5 h of cumulative head-down tasks per shift had 3.03 times higher neck-shoulder pain risks compared with those with ＜1 h. Compared with “occasionally or never”, nurses who “sometimes” and “often” tilted their necks back and maintained this posture for a long time showed 2.36 and 2.94 times higher risks for neck-shoulder pain respectively. The nurses who reported “sometimes” and “often” having difficulty using their force at work due to unnatural posture had 2.78 times and 7.08 times higher neck-shoulder pain risks than those who “occasionally or never” respectively.ConclusionThe reported rate of neck-shoulder pain among nurses in tertiary general hospitals in Hunan Province is high, but most of them are mild dysfunction. Working hours and posture load may affect the risk of neck-shoulder pain.

The hot cracking behaviors of Mg-5Zn-xEr (x = 0.83, 1.25, 2.5, 5 wt.%) alloys are investigated by optimized hot cracking experimental apparatus, optical microscope, and scanning electron microscope, such as contraction behaviors, feeding behaviors, and permeability characteristics. It is found that the solid phase fraction at hot crack initiation and within the freezing range both increased with increasing Er contents up to 2.5 wt.% and then decreased at 5 wt.% Er content. The Mg-5Zn-5Er alloy exhibits the lowest solid phase fraction (87.4%) and a reduced freezing range (74.2 °C), which leads to more effective liquid feeding in the latter stages of solidification. Combined with the grain size, the permeability of the mushy zone, and fracture morphology, the overall permeability is optimal in the Mg-5Zn-5Er alloy, which is beneficial for feeding the cavities and micro-pores. Meanwhile, a large amount of W phase precipitated by the eutectic reaction (L→α-Mg + W phase), which facilitates healing of the incurred cracking. Conversely, the Mg-5Zn-2.5Er alloy shows inferior feeding ability due to the lowest solid phase fraction (98.3%), wide freezing range (199.5 °C), and lowest permeability. Therefore, the Mg-5Zn-2.5Er alloy exhibits maximal hot cracking susceptibility, and the Mg-5Zn-5Er alloy exhibits minimal hot cracking susceptibility. This work provides guidance for improving the hot cracking resistance of cast Mg-Zn-Er alloy and enables an understanding of the hot cracking behaviors of Mg-Zn-RE alloys.

ObjectiveParkinson's disease (PD) is a neurodegenerative syndrome, and deep-brain stimulation (DBS) is an effective therapy for carefully screened patients with PD. However, delayed recovery after anesthesia, which occurs after taking prolonged general anesthesia for such patients, has been reported less frequently in literature. This report explores the possible causes of postoperative awakening delay in patients undergoing DBS surgery due to general anesthesia and provides a reference for anesthesia management of similar operations in the future.Case PresentationThree patients with PD elective underwent DBS surgery. The first patients demonstrated walking disability, gait deficits, unstable posture, limb stiffness, and imbalance. The second demonstrated left limb static tremor, stiffness, and bradykinesia. The third demonstrated bradykinesia, rigidity, walking deficits, and decreased facial expression. These included two males and one female with a mean patient age of 60.7 ± 6.7year, weight of 63.7 ± 11 kg, the height of 163.3 ± 7.6 cm, and preoperative American Society of Anesthesiology rating of 2.3 ± 0.6. The preoperative Glasgow Coma Scale mean score was 15. All patients completed the operation under general anesthesia (the mean anesthesia time was 5.3 ± 1.1 h). The mean operation time was 252 ± 60 min. The mean bleeding volume was 50 ml, and the urine volume was 867 ± 569 ml. However, all the patients showed unconsciousness after 95 ± 22 min after stopping the anesthetic, and the respiratory function was in good condition, but they could not cooperate with anesthesiologists and had no response to the anesthesiologist's instructions. The mean hospital stay was 17 ± 7 days. All patients were discharged uneventfully. The average number of days patients followed up postoperatively was 171 ± 28.5 days. Motor and speech were improved significantly postoperatively in three patients compared with preoperatively. Taking anti-Parkinson medication was markedly reduced. There were no complications during postoperative follow-up.ConclusionsTo prevent delayed recovery occurring after DBS surgery in Parkinson's disease, it is recommended to take scalp nerve block + general anesthesia to complete the procedure while avoiding general anesthesia.

Dissoluble magnesium alloys for fabrication of fracturing tools have received increasing attention in recent years. However, most of the existing research is focused on the small-sized samples prepared in the laboratory, and there is almost no report on the industrial dissoluble magnesium alloys. In this study, large-scale Mg-Al-Zn-Mn-Ca-Ni alloys with a diameter of 110 mm were prepared by a semi-continuous casting and two-step extrusion method, and the corresponding microstructure and mechanical and corrosion properties were also investigated. It was found that after two-step extrusion, the mainly precipitate phases in the Mg-Al-Zn-Mn-Ca-Ni alloy are bulk-like AlMnNi, strip-like Al3Ni, and granular-like and lamellar-like Mg17Al12 phases. Due to the combined effects of grain refinement and precipitation strengthening, the Mg-Al-Zn-Mn-Ca-Ni alloy obtained excellent mechanical properties after two-step extrusion, and its ultimate tensile strength, yield strength, and elongation were 314.6 MPa, 191.2 MPa, and 13.1%, respectively. Moreover, the corrosion rate of the alloy in 3 wt.% KCl at 93 °C was as high as 97.61 mg·cm−2·h−1. This work provides a high-performance, low-cost, and large-scale alloy product for the fabrication of dissoluble fracturing tools.

To explore whether lncRNA deleted in lymphocytic leukemia 2 (DLEU2) could accelerate the migratory, invasive and proliferative abilities, thus influencing the progression of HCC. DLEU2 level in HCC tissues and adjacent normal tissues was firstly determined. Its level in HCC tissues with different tumor sizes (≤ 5 cm or > 5 cm), different tumor stages (stage I-II or III-IV) and either with vascular invasion or not was determined. Potential influences of DLEU2 on proliferative, migratory and invasive abilities of SMMC7721 and HCLM3 cells were assessed. The interaction between DLUE2 and enhancer of zeste 2 polycomb repressive complex 2 subunit (EZH2) was evaluated by RNA Binding Protein Immunoprecipitation (RIP) assay. Finally, the effect of DLEU2/EZH2 regulatory loop on proliferative ability of HCC cells was detected. DLEU2 was upregulated in HCC tissues, especially in those larger than 5 cm in tumor size, accompanied with vascular invasion and in worse tumor stage. Knockdown of DLEU2 attenuated proliferative, migratory and invasive abilities of SMMC7721 and HCLM3 cells. RIP assay proved that DLEU2 could interact with EZH2. Knockdown of EZH2 attenuated the inhibited proliferation in HCC cells with DLEU2 knockdown. DLEU2 accelerates the proliferative, migratory and invasive abilities of HCC cells via binding to EZH2, thus aggravating the progression of HCC.

Objectives] This study was conducted to explore the occurrence levels of endocrine disruptors ( EDCs) in rural areas around a county landfill in Tongren City. [Methods] The water around the landfill was sampled and analyzed. A solid-phase extraction and high performance liquid chromatography-tandem mass spectrometry ( SPE-UPLC-MS/MS) method was established for the determination of 27 ED Cs. After the HLB solid-phase extraction column was activated, a water sample, which was adjusted with phosphoric acid to a pH of 2 ( ± 0. 5) and added with 500 mg of disodium EDTA, was loaded, and 5 ml of water and 20% methanol water was added for washing. Next, 10 ml of elution solution was added for elution, and the collected eluate was evaporated under reduced pressure at 40 oC to near dryness, and 10 ml of reconstitution solution was added to a constant volume. An ACQUITY UPLC BEH C18 (100 x2.1 mm, 2.6μm) chromatographic column was adopted for LC separation by gradient elution with pure water solution-acetonitrile as the mobile phase. For MS detection, the MRM mode was adopted for collection, and the positive and negative ion modes were switched for simultaneous determination, and the internal standard method was used for quantification. [Results] The correlation coefficient R2 was greater than 0. 99 in the linear range of each target substance. The limits of quantitation in the method were between 0.05 and 2.00 ng/L, and the recoveries ranged from 75.3% to 105. 7%. [Objectives] This study was conducted to establish a liquid chromatography-tandem mass spectrometry method for the determination of coumoxystrobin residue in milk. [Methods] A sample was ultrasonically extracted with acetonitrile, purified by a solid phase extraction column, concentrated by nitrogen blowing, and tested on a machine. [Results] In the range of 0. 01 - 10. 00 mg/kg, the linear relationship of coumoxystrobin was good. The linear equation was y -5 045. 96« +935. 332, r2 =0. 999. The detection limit was 0.002 mg/kg, and the quantitation limit was 0. 006 mg/kg. The method has good stability, high precision, and the detection limit can meet the limit requirement of coumoxystrobin in GB 2763 - 2019. This study aimed to investigate the efficacy and safety of drug-eluting beads transarterial chemoembolization (DEB-TACE) treatment in Barcelona Clinic Liver Cancer (BCLC) stage C liver cancer patients. In 39 patients with BCLC stage C liver cancer, after the first cycle of DEB-TACE, 2 (5.1%) and 24 (61.5%) patients achieved complete response (CR) and partial response (PR) to give an overall objective response rate (ORR) of 66.7%. With respect to the second cycle of therapy, the ORR was higher in patients receiving DEB-TACE compared with those receiving cTACE (57.1% vs. 11.1%). After the first cycle of DEB-TACE treatment, the percentages of abnormal albumin (ALB), total protein (TP), total bilirubin (TBIL), and alanine aminotransferase (ALT) worsened at 1 week and recovered at 1 month. The number of patients with abnormal aspartate aminotransferase (AST) did not increase at 1 week but elevated at 1 month. After the second cycle of DEB-TACE or cTACE treatment, no difference was observed between cTACE and DEB-TACE in terms of all adverse events (AEs) at all visits, and most of the AEs did not change after the second cycle in both groups. The most common AEs after the first and second treatment cycles were pain, fever, and nausea/vomiting. These results demonstrate that DEB-TACE offers patients with BCLC stage C liver cancer a clinically active short-term treatment that is safe and relatively well tolerated. To provide a significance for the future integration of the concept of "curriculum ideological and political" in physical education institutions across the country. The author conducted a research on the moral alienation of contemporary college students and the implementation of curriculum ideology and politics in basketball teaching in physical education institutions. As well as the dominant and recessive elements that affect the development of basketball teaching in sports colleges across the country, the main and second classes of basketball are analyzed and the results are obtained. The survey was conducted among 450 students on integrating ideological and moral education into basketball teaching. SPSS using software results indicates that 83 students believe that the proportion of rule awareness cultivated in basketball teaching is high, 142 students believe that the proportion of rule awareness cultivated is high, 72 students believe that the proportion of rule awareness cultivated is average, 55 students believe that the proportion of rule awareness cultivated is low, and 53 students believe that the proportion of rule awareness cultivated is low. In the process of basketball teaching, teachers have insufficient guidance concepts for students' mental health, which is detrimental to the reasonable penetration of mental health education in the basketball teaching process, therefore, the development ratio of students' mental health has not been effectively improved. Therefore, it is recommend that teachers can use basketball PE classes to cultivate students' self-confidence. Eliminate students' psychological barriers, improve their interpersonal relationships, and cultivate their team spirit. Promote the all-round development of students' mental health. Moreover, improving college students' physical education and mental health under curriculum ideological and political education can enhance overall well-being, resilience, and civic engagement. To discover novel nucleosides as potential antiviral agents, 4'-spirocyclic phosphono-nucleosides were designed to mimic the monophosphate of R-1479, a known nucleoside inhibitor of HCV NS5B. Bypassing the first kinase step to nucleoside monophosphate is viewed as advantageous since this phosphorylation is often observed as the rate-limiting transformation to the active NTP for many nucleosides. Efficient synthetic routes were developed with a triphenylphosphine-iodine cyclization reaction as the key step to form the tetrahydrofuran 4'-spirocycle.
