Introduction Accumulated evidence shows a strong link between intake of saturated fatty acids, hypercholesterolaemia and both cardiovascular disease (CVD) incidence and mortality (Keys, 1980; Hu et al, 2001; Kris-Etherton et [...], Background/Objectives: The role of individual fatty acids in the development of cardiovascular disease (CVD) is well established, but the effects of an overall pattern of fatty acids in CVD risk has yet to be elucidated. Circulating fatty acid levels are related to metabolic disturbances associated with the metabolic syndrome and CVD, due to disturbances in the activity of enzymes that catalyse fatty acid desaturation ([DELTA]-desaturases). Therefore, we determined patterns of fatty acids and estimated desaturase activity in plasma and analysed how these patterns were related to a 10-year CVD risk estimates in a middle- aged male population in Northern Ireland. Subjects/Methods: Principal components analysis (PCA) was performed for defining fatty acid patterns in 379 men aged 30-49 years. Logistic regression analyses were then carried out for analysing the relationship between these fatty acid patterns and the 10-year CVD risk estimates. Results: The PCA generated three high fatty acid patterns: high saturated fatty acid (SFA), high omega 3 fatty acid (omega 3) and high monosaturated fatty acid (MNFA). Results from logistic regression analyses show that a 1 s.d. increase in the SFA pattern score was significantly and positively associated with an increase in the 10-year CVD risk category (odds ratio 1.71, 95% confidence interval 1.33-2.21, P < 0.0001) even after adjustment for lifestyle factors. There were no significant relationships between the other two pattern scores and the 10-year CVD risk. Conclusions: An unhealthy fatty acid pattern representing both dietary intake and in vivo fatty acid metabolism is related to the 10-year CVD risk estimates and provide evidence that, as with dietary patterns, the synergistic effect of multiple fatty acids may be more important in relation to the development of CVD risk. Infection of the gastrointestinal tract with the bacteria Helicobacter pylori (H. pylori) is thought to play a causal role in the development of a number of disorders of the stomach and intestines, including gastritis (inflammation of the stomach), peptic ulcer disease (ulceration of the stomach or intestines) and possibly stomach cancer. Over 95 percent of patients suffering from duodenal ulcer, for example, have evidence of H. pylori infection. Not all infected subjects immediately develop gastrointestinal symptoms. It is of interest to know the prevalence of H. pylori infection in the population at large; however, most studies of infection rates in populations other than gastrointestinal disease patients have used selected groups of subjects such as blood donors or health care staff volunteers. To better ascertain the percentage of the population that suffers from H. pylori infection, a sensitive and specific assay (enzyme-linked immunosorbent assay for immunoglobulin G) to detect the presence of H. pylori was used on frozen blood samples from 749 randomly selected men between the ages of 30 and 75 living in South Wales. The overall prevalence of H. pylori infection was 57 percent. Age differences were seen: 30 percent of the subjects between 30 and 34 had evidence of infection, while 59 percent of the subjects over 45 were infected. There was an inverse relationship between social class and infection rate; subjects in the lowest social class were much more likely to carry H. pylori than subjects in higher social classes. The patterns of H. pylori infection are consistent with the patterns of peptic ulcer disease and stomach cancer seen in the general population, further supporting a link between bacterial infection and these diseases. - Abstract
Observational studies have reported different effects of adiposity on cardiovascular risk factors across age and sex. Since cardiovascular risk factors are enriched in obese individuals, it has not been easy to dissect the effects of adiposity from those of other risk factors. We used a Mendelian randomization approach, applying a set of 32 genetic markers to estimate the causal effect of adiposity on blood pressure, glycemic indices, circulating lipid levels, and markers of inflammation and liver disease in up to 67,553 individuals. All analyses were stratified by age (cutoff 55 years of age) and sex. The genetic score was associated with BMI in both nonstratified analysis (P = 2.8 × 10(-107)) and stratified analyses (all P < 3.3 × 10(-30)). We found evidence of a causal effect of adiposity on blood pressure, fasting levels of insulin, C-reactive protein, interleukin-6, HDL cholesterol, and triglycerides in a nonstratified analysis and in the <55-year stratum. Further, we found evidence of a smaller causal effect on total cholesterol (P for difference = 0.015) in the ≥55-year stratum than in the <55-year stratum, a finding that could be explained by biology, survival bias, or differential medication. In conclusion, this study extends previous knowledge of the effects of adiposity by providing sex- and age-specific causal estimates on cardiovascular risk factors.

- Abstract
Background Meta-analysis of individual participant time-to-event data from multiple prospective epidemiological studies enables detailed investigation of exposure-risk relationships, but involves a number of analytical challenges. Methods This article describes statistical approaches adopted in the Emerging Risk Factors Collaboration, in which primary data from more than 1 million participants in more than 100 prospective studies have been collated to enable detailed analyses of various risk markers in relation to incident cardiovascular disease outcomes. Results Analyses have been principally based on Cox proportional hazards regression models stratified by sex, undertaken in each study separately. Estimates of exposure-risk relationships, initially unadjusted and then adjusted for several confounders, have been combined over studies using meta-analysis. Methods for assessing the shape of exposure-risk associations and the proportional hazards assumption have been developed. Estimates of interactions have also been combined using meta-analysis, keeping separate within-and between-study information. Regression dilution bias caused by measurement error and within-person variation in exposures and confounders has been addressed through the analysis of repeat measurements to estimate corrected regression coefficients. These methods are exemplified by analysis of plasma fibrinogen and risk of coronary heart disease, and Stata code is made available. Conclusion Increasing numbers of meta-analyses of individual participant data from observational data are being conducted to enhance the statistical power and detail of epidemiological studies. Global assays, such as resonance-thrombography (RTG), which measure the interaction between platelets, coagulation and fibrinolysis have been used as summary measures of risk for over two decades but have not been evaluated in epidemiological studies. We examined whether RTG indices are risk indicators for incident coronary heart disease (CHD). RTG indices, related haematological variables and other risk factors were measured between 1984 and 1988 in a cohort of 2398 British men. Reaction time (r) and amplitude of fibrin leg (AF) were associated with lifestyle risk factors. During 9 years of follow-up, 282 (12%) men developed a major new CHD event, as classified by World Health Organization criteria. On adjustment for age, only r and AF measured at baseline were related to risk of incident CHD. On multivariate adjustment in a multiple logistic regression model that included age, diastolic blood pressure, body mass index, total and high-density lipoprotein cholesterol, lifestyle risk factors and use of prescribed medicine, these associations weakened but remained significant. Additional adjustment for fibrinogen, viscosity, white cell count and fibrin D-dimer either reduced these associations to non- significance (AF) or to borderline significance(r).

- Abstract
Background Meta-analysis of individual participant time-to-event data from multiple prospective epidemiological studies enables detailed investigation of exposure-risk relationships, but involves a number of analytical challenges. Methods This article describes statistical approaches adopted in the Emerging Risk Factors Collaboration, in which primary data from more than 1 million participants in more than 100 prospective studies have been collated to enable detailed analyses of various risk markers in relation to incident cardiovascular disease outcomes. Results Analyses have been principally based on Cox proportional hazards regression models stratified by sex, undertaken in each study separately. Estimates of exposure-risk relationships, initially unadjusted and then adjusted for several confounders, have been combined over studies using meta-analysis. Methods for assessing the shape of exposure-risk associations and the proportional hazards assumption have been developed. Estimates of interactions have also been combined using meta-analysis, keeping separate within-and between-study information. Regression dilution bias caused by measurement error and within-person variation in exposures and confounders has been addressed through the analysis of repeat measurements to estimate corrected regression coefficients. These methods are exemplified by analysis of plasma fibrinogen and risk of coronary heart disease, and Stata code is made available. Conclusion Increasing numbers of meta-analyses of individual participant data from observational data are being conducted to enhance the statistical power and detail of epidemiological studies. The statistical methods developed here can be used to address the needs of such analyses. © The Author 2010; all rights reserved.

- Abstract
ObjectiveThe prevalence of obesity is increasing in many European countries and in the United States. This report examines the mortality and morbidity associated with being overweight and obese in the Caerphilly Prospective Study and the relative effects of weight in middle age and self reported weight at 18 years.DesignAll men aged 45 to 59 years from the town of Caerphilly, South Wales and outlying villages were identified and 2512 men were examined for the first time between 1979 and 1983. Men were asked to recall their weight at 18 years of age (when the majority had been examined for National Service) so that weight then, weight at screening, and the difference could be related to their 14 year follow up from screening. A total of 2335 men could recall their weight at 18 years. By 14 years of follow up from screening 465 men had died and 382 had had coronary events.ResultsMean body mass index in men who reported their weight at 18 years was 22.3 (SD 2.8) kg/m2 and only 41 of these men (1.8%) were classified as obese (index ≥ 30 kg/m2). The index did not predict all cause mortality when examined by quintile. For major ischaemic heart disease (non-fatal or fatal ischaemic heart disease) the relative odds was 1.73 (95% CI 1.21, 2.48) in the top fifth of the distribution (body mass index ≥ 24.2 kg/m2) compared with the bottom fifth (body mass index <20.1 kg/m2). In men with an index ≥ 30 kg/m2 however, the relative odds were 2.03 (95% CI, 1.03, 4.01) for all cause mortality and 2.17 (95% CI, 1.08, 4.34) for major ischaemic heart disease, adjusted for age, smoking habit and social class. When men were recruited to the study, from 1979 to 1983; the mean body mass index had increased to 26.2 (SD 3.6), a mean increase of 3.9 kg/m2 or 11.2 kg; 299 men (12.1%) were classified as obese and showed significantly increased relative odds of both all cause mortality (1.53 (95% CI 1.14, 2.06) and major ischaemic heart disease (1.55 (95% CI 1.13, 2.11)), adjusted for age, smoking habit and social class relative to the non-obese men. The effect of gain in weight from 18 years to recruitment was also examined; all cause mortality showed highest mortality in the fifth of the distribution who experienced weight loss or minimal weight gain. For major ischaemic heart disease an inconsistent, weak trend was shown, the relative odds rising to a maximum of 1.26 (0.89, 1.80) in the top fifth of weight gain compared with the bottom fifth. Weight gain showed strong associations with potential cardiovascular risk factors measured at recruitment; insulin, triglyceride, glucose, diastolic and systolic blood pressure and high density lipoprotein-cholesterol.ConclusionsBody mass at 18 years of age of 30 kg/m2 or more conferred increased risk for all cause mortality and major ischaemic heart disease during 14 years of follow up of men aged 45 to 59 years. By the baseline examination the prevalence of obesity (body mass index ≥30) had increased from 1.8% to 12.1%; obese men also showed an excess risk of major ischaemic heart disease and overall mortality, but these risks were lower than those predicted from 18 years of age. Weight gain was strongly associated with smoking habit, the greatest weight gain being among ex-smokers and the least among light smokers. Weight gain from 18 years of age to baseline examination showed little relation with subsequent mortality and risk of major ischaemic heart disease when adjusted for age, smoking habit and social class. The lowest mortality rate occurred in the "fifth" of men who gained a mean weight of 16.1 kg. Weight gain is closely associated with some adverse cardiovascular risk factors; in particular with insulin, triglyceride, glucose and diastolic blood pressure.

Objective To examine the clinical and epidemiological utility of the concepts of metabolic syndrome and insulin resistance syndrome in two prospective cohort studies of white men. Methods Men aged 45-63 years were screened for evidence of ischaemic heart disease (IHD) between 1979 and 1982 and followed up at regular intervals thereafter. Non-fatal coronary events were validated from hospital records and fatal coronary events from death certificates. Results Analysis of serum insulin concentrations in non-diabetic individuals measured at entry to the study showed no independent contribution to the prediction of subsequent IHD at 10 year follow up. Blood glucose concentrations, however, showed a small independent contribution in the combined cohort in the upper fifth of the distribution. Three different models of metabolic syndrome among non-diabetic individuals were defined based on tertiles, medians, and clusters. The predictive value of each model was assessed using logistic regression before and after adjustment for conventional and metabolic risk factors. After adjustment the odds were non-significant and close to unity. Conclusions This study did not detect any complex relation among the five variables defining metabolic syndrome; the excess risk seems to be no greater than can be explained by individual effects of the defining variables in a multiple logistic model.

BACKGROUND: Behavioral factors are important in disease incidence and mortality and may explain associations between mortality and various psychological traits. PURPOSE: These analyses investigated the impact of behavioral factors on the associations between depression, hostility and cardiovascular disease(CVD) incidence, CVD mortality, and all-cause mortality. METHODS: Data from the PRIME Study (N = 6953 men) were analyzed using Cox proportional hazards models, following adjustment for demographic and biological CVD risk factors, and other psychological traits, including social support. RESULTS: Following initial adjustment, both depression and hostility were significantly associated with both mortality outcomes (smallest SHR = 1.24, p < 0.001). Following adjustment for behavioral factors, all relationships were attenuated both when accounting for and not accounting for other psychological variables. Associations with all-cause mortality remained significant (smallest SHR = 1.14, p = 0.04). Of the behaviors included, the most significant contribution to outcomes was found for smoking, but a role was also found for fruit and vegetable intakes and high alcohol consumption. CONCLUSIONS: These findings demonstrate well-known associations between depression, hostility, and mortality and suggest the potential importance of behaviors in explaining these relationships.

Background Previous research demonstrates various associations between depression, cardiovascular disease (CVD) incidence and mortality, possibly as a result of the different methodologies used to measure depression and analyse relationships. This analysis investigated the association between depression, CVD incidence (CVDI) and mortality from CVD (MCVD), smoking related conditions (MSRC), and all causes (MALL), in a sample data set, where depression was measured using items from a validated questionnaire and using items derived from the factor analysis of a larger questionnaire, and analyses were conducted based on continuous data and grouped data. Methods Data from the PRIME Study (N=9798 men) on depression and 10-year CVD incidence and mortality were analysed using Cox proportional hazards models. Results Using continuous data, both measures of depression resulted in the emergence of positive associations between depression and mortality (MCVD, MSRC, MALL). Using grouped data, however, associations between a validated measure of depression and MCVD, and between a measure of depression derived from factor analysis and all measures of mortality were lost. Limitations Low levels of depression, low numbers of individuals with high depression and low numbers of outcome events may limit these analyses, but levels are usual for the population studied. Conclusions These data demonstrate a possible association between depression and mortality but detecting this association is dependent on the measurement used and method of analysis. Different findings based on methodology present clear problems for the elucidation and determination of relationships. The differences here argue for the use of validated scales where possible and suggest against over-reduction via factor analysis and grouping.
