Adaptation of existing guidelines can be an efficient way to develop contextualized recommendations. Transparent reporting of the adaptation approach can support the transparency and usability of the adapted guidelines.To develop an extension of the RIGHT (Reporting Items for practice Guidelines in HealThcare) statement for the reporting of adapted guidelines (including recommendations that have been adopted, adapted, or developed de novo), the RIGHT-Ad@pt checklist.A multistep process was followed to develop the checklist: establishing a working group, generating an initial checklist, optimizing the checklist (through an initial assessment of adapted guidelines, semistructured interviews, a Delphi consensus survey, an external review, and a final assessment of adapted guidelines), and approval of the final checklist by the working group.International collaboration.A total of 119 professionals participated in the development process.Participants' consensus on items in the checklist.The RIGHT-Ad@pt checklist contains 34 items grouped in 7 sections: basic information (7 items); scope (6 items); rigor of development (10 items); recommendations (4 items); external review and quality assurance (2 items); funding, declaration, and management of interest (2 items); and other information (3 items). A user guide with explanations and real-world examples for each item was developed to provide a better user experience.The RIGHT-Ad@pt checklist requires further validation in real-life use.The RIGHT-Ad@pt checklist has been developed to improve the reporting of adapted guidelines, focusing on the standardization, rigor, and transparency of the process and the clarity and explicitness of adapted recommendations.None.

Objectives: The objectives of this scoping review are to estimate the prevalence of obesity and overweight in the Saudi community and in different age groups, genders, and geographical location, in addition to the change in prevalence over time. Methods: This scoping review of evidence was conducted in accordance with the Joanna Briggs Institute methodology for scoping reviews and was reported following the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) guidelines. The population of this review was categorized into four age groups: young adults (18–25 years), adults (26–45), (mid-life adults) (46–60) and old people (60+). Each group was then categorized by gender into males and females. We included studies of adults aged 18 years and above. The pooled prevalence of obesity and overweight of the population, based on BMI, was estimated after stratification based on the age, gender, and geographical area. In addition, the change in the prevalence of obesity/overweight over time from 2011 to 2021 was investigated from the pooled data. The Metaprop program in Stata was used for statistical analysis. Results: A total of 39 studies with 640,952 participants were included in this review. The pooled prevalence of obesity and overweight in the age group of ≤25 years old, including both genders, was 30%. However, it was higher in young males (40%) compared to young females (25%). The prevalence of obesity and overweight among young adults has dropped by over 40% between 2012 and 2021. The overall pooled prevalence rate of obesity and overweight in the age groups >25 years old (adults, mid-life, and old people), including both genders, was 66%, with similar prevalence among males (68%) and females (71%). In addition, a similar prevalence was observed among both adult and old people (62% and 65%, respectively), but was higher in the mid-life group (76%). Furthermore, mid-life women had the highest prevalence among all groups (87%), compared to 77% among males in the same age group. The same difference in prevalence between the gender persisted in older females compared to older males (79% vs. 65%, respectively). There is a noticeable drop in the pooled prevalence of overweight and obesity among adults > 25 years old of over 28% between 2011 and 2021. There was no difference in the prevalence of obesity/overweight by geographical region. Conclusions: Despite the noticeable drop in the prevalence of obesity in the Saudi community, the prevalence of high BMI is high in Saudi Arabia irrespective of age, gender, or geographical location. Mid-life women have the highest prevalence of high BMI, which makes them the focus of a tailored strategy for intervention. Further research is needed to investigate which are the most effective interventions to address obesity in the country.

Background It is estimated that one in four persons in the UK will be > 65 years old by 2050. Endoscopic retrograde cholangiopancreatography (ERCP) is the recommended therapy for removals of common bile duct (CBD) stones. There is a lack of UK data on the outcome of ERCP in the very elderly. We aimed to investigate the safety and efficacy of ERCP in the management of CBD stones in the elderly >90 years old compared with those aged 65-89 years old. Methods We retrospectively analysed patients undergone ERCP between 2016-2020. Those with conditions other than suspected CBD stones were excluded. The following data was collected: general demographics, comorbidities, American Association of Anaesthesiology (ASA) grading, presence of dementia, anticoagulation therapy, endoscopic report on completed intention of treatment which is defined by either removal of stone and/or stent insertion. Also, post-procedure complications were collected including post-ERCP pancreatitis (PEP), bleeding, perforation, cholangitis, respiratory, and cardiovascular (CVS) events. Readmission within 7 days and death within 30 days were collected. Outcomes were reported as mean ± (SD) or n (%). Statistical significance was considered when P Results Total, 126 patients ≥ 90 years(group A) compared to 262 patients aged 65-89 years(group B). Higher rate of dementia, and anticoagulation therapy in Group A (17.4%) vs (0.4%),p Conclusions ERCP for removal of CBD stones in > 90 years old is effective. Risk of post-ERCP pancreatitis and overall adverse events were not higher in the > 90 years compared to 65-89 years old. We report a 30-day mortality rate of 3.2%. The most common cause of death was pneumonia including COVID-19 peumonia.

The COVID-19 pandemic has had a significant impact on public health, including healthcare workers and healthcare systems, worldwide.To investigate COVID-19-related psychological impact on healthcare workers in 12 Arab countries.This was a cross-sectional, hospital-based online survey conducted between 4 May and 8 June 2020. We evaluated stress, depression, anxiety, and insomnia using the Depression Anxiety Stress Scale and Insomnia Severity Index.A total of 2879 respondents from 12 Arab countries completed the survey. Anxiety, depression, stress, and insomnia were reported by 48.9%, 50.6%, 41.4% and 72.1% of respondents, respectively. Lower-middle- and lower-income countries had a significantly higher prevalence of all the psychological outcomes than high-income countries. The prevalence of mental health symptoms was higher among healthcare workers aged 30-39 years, those who worked44 hours per week, and those in contact with COVID-19 cases, as well as healthcare workers who were not satisfied with the preventive measures. The prevalence of mental health symptoms was lower among male healthcare workers.COVID-19 had a considerable impact on the mental and psychological health of healthcare workers in Arab countries. This was aggravated by the geopolitical location of some Arab countries and social norms usually observed during the month of Ramadan. Nous avons évalué le stress, la dépression, l'anxiété et l'insomnie à l'aide de l'échelle Dépression Anxiété Stress et de l'Index de Sévérité de l'Insomnie.Au total, 2879 participants de 12 pays arabes ont répondu à l'enquête. L'anxiété, la dépression, le stress et l'insomnie ont été respectivement signalés par 48,9 %, 50,6 %, 41,4 % et 72,1 % des répondants. La prévalence de tous les résultats psychologiques était significativement plus forte dans les pays à revenu intermédiaire de la tranche inférieure et les pays à revenu faible que dans les pays à revenu élevé. La prévalence des symptômes de santé mentale était plus élevée chez les agents de santé âgés de 30 à 39 ans, ceux qui travaillaient plus de 44 heures par semaine et ceux qui étaient en contact avec des cas de COVID-19, ainsi que chez les agents de santé qui n'étaient pas satisfaits des mesures de prévention. La prévalence des symptômes de santé mentale était plus faible chez les agents de santé de sexe masculin.La COVID-19 a eu un impact considérable sur la santé mentale et psychologique des agents de santé dans les pays arabes. Cette situation a été aggravée par la situation géopolitique de certains pays arabes et les normes sociales habituellement observées pendant le mois de Ramadan. Le fait d'être médecins, jeunes agents de santé et les longues heures de travail étaient des facteurs de risque d'un plus grand impact psychologique de la pandémie.

Fog computing was introduced to complement the cloud computing paradigm in the era of Internet of Things. The main objective of fog computing is to push the processing and storage of the data closer to the edge of the network. This computation paradigm is proposed to reduce the latency of data processing, decrease the data flow from the different sensors to the cloud by local processing, and to prevent single points of failure. Although the fog computing paradigm benefits are clear, there is no clear adoption model that will help in spreading this computation paradigm through the networks. In this paper, we propose a new personal device, namely the “Droplet”. The adoption model for the Droplet make it suitable to be an enabler to the fog computing paradigm. The Droplet is a fixed, always connected, edge device that is owned and managed by the user while allowing other players to use it to realize the benefits of the fog computing. The Droplet is inspired by the wide spread and decrease in cost of single board computers. We discuss some potential applications of the Droplet that makes it appealing to be spread by the users. A prototype for the Droplet was built, and some of the potential applications were developed to run on the Droplet to prove the concept of the new personal device.

The Joint Industry Project (JIP) is a worldwide project that concerns the development of guidance in the form of a draft API Recommended Practice (API RP2 SIM) that addresses SIM (Structures Integrity Management) issues for existing fixed steel offshore structures around the world. It also concerns the Preparation of a methodology that can be used to provide a consistent minimum reliability (against environmental overload) for offshore platforms located in different regions around the world and which is referred to as Regional Acceptance Criteria (RAC). As a member participant in the JIP, ADMA-OPCO took the initiative to develop the Arabian Gulf Regional Acceptance Criteria (RAC). A number of structures were selected for the study representative for the range of structural types and robustness across the fleet of structures composed of about 277 platforms. It involved performing structural pushover analyses for the selected structures and reliability analysis, the work involved determination of the target structural reliability, which equates to an acceptable probability of failure (PF) which is typically a function of the consequence of failure of the platform. The maximum acceptable annual failure probabilities for HIGH, MEDIUM and LOW consequences of failure categories of the whole fleet were determined. The above target PF’s were converted into equivalent RSRs (reserve Strength Ratios) and dead load factors as a RAC.
