A 70-year-old man underwent right upper lobectomy for lung adenocarcinoma. During the operation, hemostatic matrix (as known Floseal®) was used to prevent pulmonary laceration-associated bleeding. When 18F-fluorodeoxyglucose (FDG) positron emission tomography/computed tomography was performed for staging after surgery, intense 18F-FDG uptake was observed in the cicatricial fibrotic tissue in the operation area, and no significant change was observed in that area during the 4-year follow-up. Because it remained stable for several years without treatment, this finding was thought to be due to a foreign body reaction caused by the homeostatic material. [ABSTRACT FROM AUTHOR]

Renal transplantation, which is the best treatment for end-stage renal failure, increases the life expectancy and provides a better quality of life when compared to dialysis. Scintigraphy is a non-invasive, frequently repeated, and easily applicable study for evaluating vascular complications, urinary leakage, and obstructions as well as graft function after transplantation and during follow-up. It is also a guide for the evaluation of medical complications such as acute tubular necrosis and acute rejection. Basal transplant kidney scintigraphy performed within 2-3 days after transplantation also has prognostic value. Although its application is not very different from normal dynamic renal scans, it has some differences in terms of both quantitative and qualitative evaluations of kidney perfusion compared to other renal scans. A sixty-two-year-old male patient underwent orchiectomy and was diagnosed with diffuse large B-cell lymphoma in the testicle and spermatic cord. 18F-FDG positron emission tomography/computed tomography (PET/CT) scanning was performed for initial staging. 18F-FDG PET/CT scan revealed multiple hyper-metabolic lymphadenopathies, lung lesions and mass lesions in the adrenal glands and kidneys. In addition, diffuse increased 18F-FDG uptake suggesting lymphomatous infiltration on the right testicle, prostate and left testicular veins were detected. The genitourinary system involvement is extremely rare in extranodal lymphomas and to the best of our knowledge this is the first case in the literature having 18F-FDG accumulating lesions in all genitourinary system structures. Purpose: To review the surgical results in patients who underwent an eyelid reconstruction with rhomboid flap after periorbital and medial canthal tumor excision. Ma te ri al and Method: This study included 18 patients who underwent eyelid reconstruction with rhomboid flap after periorbital and medial canthal tumor excision. The patient charts were reviewed for topographic data, tumor features, surgical technique, and postoperative results. Results: The mean age of the patients (10 women, 8 men) was 58 years (age range: 22-78 years). The lesion was located in the inferior periorbital-malar area in 8 patients, in the medial canthal area in 8 patients and in the lateral periorbital area in 2 patients. Mean tumor diameter was 8.9 mm (range: 5-13.5 mm). Rhomboid flap was used alone in 14 patients (78%) and in combination with nasojugal advanced flap and semicircular flap in 2 patients (11%) each. The skin defect could be closed primarily in all patients, except one. A secondary surgical intervention was required for ectropion and pyogenic granuloma excision in 1 patient each. The aesthetic outcome was satisfactory in all patients. Mean follow-up time was 23 months (range: 2-64 months). Dis cus si on: Rhomboid flap can be used for medial canthal and periorbital eyelid reconstruction after tumor excision. This flap technique can be combined with other reconstruction methods for large defects. Purpose: To present 3 patients who had a dural carotid-cavernous sinus fistula (CCF) and underwent cavernous sinus embolization through superior ophthalmic vein (SOV) approach. Ma te ri al and Met hod: Medical records of 3 patients with dural CCF who were treated through SOV approach were reviewed. Patient's age, gender, ophthalmic and radiological findings, treatment results, postoperative complications and follow-up time were recorded. Surgical procedure was performed in the interventional radiology unit and using general anesthesia. Through an eyelid crease incision, the SOV was reached and cannulated. The cavernous sinus was embolized with metalic coils advanced through a microcatheter placed into this cannula. Re sults: All 3 patients (3 female, patient ages: 41, 68 and 71 years) had Barrow type D CCF. One patient had a bilateral CCF. All patients had a history of unsuccessful treatment with interventional transvenous routes. The embolization procedure via the SOV was successfully completed in 2 patients. In a patient with bilateral CCF, the SOV was thin and tortuous, and could not be cannulated. All ophthalmic symptoms were improved in 2 patients with a successful embolization, and did not recur during the follow-up periods of 24 and 22 months. In one of these, an ischemic cerebrovascular event and orbital hematoma developed after the embolization. These complications improved without leaving a permanent impairment. Dis cus si on: Cavernous sinus embolization can be performed through SOV approach, when it can not be done via the interventional transvenous routes. This procedure may not be successfully performed in some cases because of the anatomical features of the SOV. Purpose: To review the examination findings and treatment results in patients with orbital subperiosteal abscess who underwent orbitotomy and abscess drainage through skin incision. Material and Method: The records of 17 patients who had undergone surgical intervention for subperiosteal abscess from January 1998 to December 2009 were reviewed. The collected data included age, gender, radiological and clinical examination signs, microbiology results, treatment modalities, and follow-up time. Results: Eleven patients were males, and 6 were females; mean patient age was 22 years (range: 5-74 years). Thirteen patients (76%) were older than 9 years of age. Subperiosteal abscess was secondary to paranasal sinusitis in 14 patients (82%), and secondary to orbital trauma in 3 patients (18%). Location of abscess formation within the orbit were superior in 10 (59%), medial in 3 (18%) and superomedial in 4 patients (23%). Relative afferent pupillary defect was positive in 15 patients (88%). Radiologic scanning showed intracranial abscess formation in 2 patients (13%). Surgical treatment include orbitotomy and drainage of abscess in 17 patients (100%), and anterior ethmoidectomy in 4 patients (23%). One patient (6%) underwent drainage of intracranial abscess through craniotomy. Intraoperative cultures were positive in only 6 (35%) of 17 samples. Recurrence of the abscess and permanent loss of vision developed in 1 patient (6%). The follow-up period ranged from 3 to 14 months (mean, 5.5 months). Discussion: Subperiosteal orbital abscesses may lead to optic neuropathy and intracranial infection and may require surgical drainage for treatment. Abscess drainage through external orbitotomy may provide a rapid and complete resolution in orbital symptoms and visual functions. [ABSTRACT FROM AUTHOR]

Traumatic or iatrogenic rupture of the extraocular muscles is a rare condition. In this report, we describe 3 patients treated because of strabismus secondary to extraocular muscle rupture. The subjects were one female and two males aged 60, 65 and 11 years, respectively. All patients had severe restriction of eye movements and strabismus. The medial rectus muscle was detached during a strabismus surgery in 1 patient, and the medial or lateral rectus muscle was cut because of penetrating orbital trauma in 2 patients. The muscle injury had occurred 41 and 42 years, and 10 days ago. For treatment, the ruptured rectus muscle was found through orbitotomy and reattached in its original insertion after cleaning the surrounding scar tissue. We also performed recession surgery alone or combined with intraoperative botulinum toxin A injection to the opposite rectus muscle in 2 cases. Purpose: TTo evaluate the results of suborbicularis oculi fat (SOOF) lift procedure in patients with facial paralysis. Materials and Methods: The medical records of 12 patients (7 men, 5 women; age range: 49-67 years), who underwent one-sided SOOF lift surgery for chronic facial paralysis between January 2000 and April 2007, were reviewed. Preoperative examination findings, amount of lagophthalmos, surgical technique, complications, surgery results, secondary surgical requirements and follow-up period were recorded. Results: The patients had a lagophthalmos ranged between 6 and 13 mm (mean value, 9 mm) before surgery. SOOF lift procedure was performed via transconjunctival approach in 8 eyes and subciliary approach in 4 eyes. This procedure was routinely performed with lateral canthal suspension procedure. In the early postoperative period, the lower eyelid position and the cheek ptosis improved in all patients. The amount of lagophthalmos was reduced between 3 and 9 mm (mean value, 5 mm). The improvement in the cheek position was sustained until the last examination in all patients, except one. The major surgical complications were periorbital hematoma (1 patient) and subcutaneous suture granuloma (1 patient). Infraorbital nerve hypoesthesia or recurrence of the lower eyelid ptosis did not develop in any patients. The mean follow-up time was 13 months (range, 1-46 months). Discussion: The SOOF lift procedure provides a significant contribution to the correction of cheek ptosis, lower eyelid retraction and lagophthalmos in patients with chronic facial paralysis. Background: Melanocytic nevus is a rare occurrence in the tarsal conjunctiva and only 7 well-described cases have been reported previously in the English literature. Case Report: The medical records of 4 patients with tarsal conjunctival melanocytic nevus were reviewed, together with the relevant literature. All patients (3 women and 1 man; age range: 17-40 years) had been referred with a suspicion of melanoma. There was one tarsal nevus in the lower eyelid in 3 patients and 2 nevi in the upper eyelid in 1 patient. All lesions were darkly pigmented with irregular borders and were associated with a history of a recent growth in size. An intralesional cyst was present in 1 nevus only. After surgical excision, no recurrence or complication occurred during the follow-up period (range: 7-48 months). Conclusion: Tarsal melanocytic nevus has been described in detail in 11 cases, including these 4 cases, in the English literature. The lesion arose from the lower eyelid in all cases except one. Tarsal melanocytic nevi may frequently display clinical features suggesting melanoma, such as advanced patient age, recent growth, dark and irregular pigmentation, nodu-larity, hypervascularity, and the absence of an intralesional cyst.
