Abstract

BackgroundCurrent standard-of-care technologies, such as imaging and cyst fluid analysis, are unable to consistently distinguish intraductal papillary mucinous neoplasms (IPMNs) of the pancreas at high risk of pancreatic cancer from low-risk IPMNs. The objective was to create a single-platform assay to identify IPMNs that are at high risk for malignant progression.Study designBuilding on the Verona International Consensus Conference branch duct IPMN biomarker review, additional protein, cytokine, mucin, DNA, and microRNA cyst fluid targets were identified for creation of a quantitative polymerase chain reaction-based assay. This included messenger RNA markers: ERBB2, GNAS, interleukin 1β, KRAS, MUCs1, 2, 4, 5AC, 7, prostaglandin E2R, PTGER2, prostaglandin E synthase 2, prostaglandin E synthase 1, TP63; microRNA targets: miRs 101, 106b, 10a, 142, 155, 17, 18a, 21, 217, 24, 30a, 342, 532, 92a, and 99b; and GNAS and KRAS mutational analysis. A multi-institutional international collaborative contributed IPMN cyst fluid samples to validate this platform. Cyst fluid gene expression levels were normalized, z-transformed, and used in classification and regression analysis by a support vector machine training algorithm.ResultsFrom cyst fluids of 59 IPMN patients, principal component analysis confirmed no institutional bias/clustering. Lasso (least absolute shrinkage and selection operator)-penalized logistic regression with binary classification and 5-fold cross-validation used area under the curve as the evaluation criterion to create the optimal signature to discriminate IPMNs as low risk (low/moderate dysplasia) or high risk (high-grade dysplasia/invasive cancer). The most predictive signature was achieved with interleukin 1β, MUC4, and prostaglandin E synthase 2 to accurately discriminate high-risk cysts from low-risk cysts with an area under the curve of up to 0.86 (p = 0.002).ConclusionsWe have identified a single-platform polymerase chain reaction-based assay of cyst fluid to accurately predict IPMNs with high malignant potential for additional studies.

16. Assessing Influence of Mismatch Repair Mutations on Survival in Patients After Resection of Pancreatic Ductal and Periampullary Adenocarcinoma.

Abstract

Background: Pancreatic ductal adenocarcinoma (PDAC) is the third leading cause of cancer-related deaths in the United States. Previous studies have indicated that microsatellite instability and deficient mismatch repair (MMR) may be associated with improved survival in patients with pancreatic cancer. Here, we aim to investigate the impact of deficient MMR (dMMR) status on oncologic outcomes in patients after resection of PDAC and periampullary adenocarcinoma. Methods: This is a single-institution, retrospective study based on a prospectively maintained database. Pancreatic ductal adenocarcinoma (N = 342) and periampullary adenocarcinoma patients (N = 76) who underwent pancreatic resection surgery between 2016 and 2021 were included. Immunohistochemistry staining results of MMR proteins and next-generation sequencing data were recorded. Cancer-type dependent Cox regression analyses were performed to assess overall and disease-free survival, which was complemented with a 1:2 propensity-score matching for each of the cancer types in order to compare oncologic outcomes. Results: A total of 418 pancreatic cancer patients were included in the analysis. Fifteen patients (3.5%) were diagnosed as dMMR (PDAC N = 7 and periampullary adenocarcinoma N = 8). Cox regression modeling of dMMR status interaction with TNM staging and cancer type revealed that dMMR status strongly improves overall survival (p < 0.05). After propensity-score matching, Cox regression identified dMMR status as a significant marker of improved overall survival (HR = 0.27, 95%CI 0.09–0.88, p = 0.029). Conclusions: Overall, our findings suggest that dMMR status is associated with markedly improved survival outcomes in patients after resection of pancreatic and periampullary cancer. 24. Our Jefferson Bicentennial: Thank You Dr. George McClellan

Abstract

1824 was a leap year. Lord Byron died in Missolonghi, Greece. Beethoven’s Ninth Symphony premiered in Vienna. And a surgeon, Dr. George McClellan, five years out of medical school himself, had the vision and aspiration to make Philadelphia the only U.S. city with two medical schools. McClellan was a Yale graduate (chemistry, 1815) attended Penn Med (’19) and due to the politics of the time and his ties to the Presbyterian church, chose to affiliate this new Philadelphia medical school via a charter with Jefferson College – a prior log cabin school located 312 miles away – in the sleepy hamlet of Canonsburg, PA. McClellan, along with Drs. John Eberle and Joseph Klapp and Jacob Green, Esq., were granted “Articles of Union” on October 30, 1824 – this is considered the founding date of Jefferson Medical College (JMC). A skeleton faculty gave lectures. The first faculty meeting occurred on December 20, 1824. The old Tivoli Theater was leased at an annual rate of $550, remodeled and became the “Hall of the Jefferson Medical College”. An infirmary (clinic) was started on May 16, 1825, for free medical and surgical care of outpatients – the first clinic in any American college. Tuition for the first-year session was $78, paid directly to the professors. Trouble ensued. Shortly prior to the planned graduation of the first class (1826) an official protest was read in Harrisburg by the Chair of the Board of Trustees at Penn, challenging the right and power of JMC to grant the M.D. degree. McClellan made a legendary dash to Harrisburg, averted the challenge, and one week later on April 14, 1826, the first JMC commencement was held at the Medical Hall – with 20 graduates receiving their M.D. diplomas (Eight U.S. states represented and one Irishman). Philadelphia now had two M.D. granting institutions. We all owe a debt of gratitude to the vision of George McClellan, the founder of our medical college. 25. Jefferson Health Names Enterprise Surgeon Leaders

Abstract

The past nine years have brought a series of mergers and acquisitions that dramatically expanded the physical footprint and clinical resources of Jefferson Health. What started as an academic medical center in Center City Philadelphia has grown to a robust network of 17 hospitals and physician practices serving patients in five Delaware Valley counties, and beyond. Jefferson Health’s growth has fueled new opportunities to improve patient care, clinical research and medical education. Achieving those goals requires thoughtful integration of people, processes and technology—with strong leadership to guide the journey. In January, Jefferson Health formally named an Enterprise Surgeon Leader for each surgical specialty. These leaders are now conducting assessments and building plans to strengthen their respective domains across the Jefferson enterprise. A top priority is identifying and mapping relevant clinicians and quantifying case volumes with a focus on demand/capacity issues and recruitment goals. Enterprise Surgeon Leaders are also reviewing current clinical protocols and making updates to reflect the latest evidence. Vice Chair for Quality and Safety Scott Cowan, MD, is collaborating with the leaders to identify and track domain-specific quality metrics, as well as to gather their input on a cross-domain “surgeon scorecard” now in development. We’ve asked these enterprise leaders to find ways to teach students, residents and fellows beyond the bound of the current educational structure. To that end, they are conducting enterprise-wide assessments of current undergraduate and graduate medical educational initiatives in their respective domains. On the research front, the Enterprise Surgeon Leaders will be exploring clinical and translational research opportunities to further expand Jefferson’s footprint. Abstract

Background: Communication between caregivers and clinical team members is critical for transitional care, but its quality and potential impact on outcomes are not well understood. This study reports on caregiver‐reported quality of communication with clinical team members in the postpancreatectomy period and examines associations of these reports with patient and caregiver outcomes. Methods: Caregivers of patients with pancreatic and periampullary malignancies who had undergone pancreatectomy were surveyed. Instrument measures assessed care experiences using the Caregiver Perceptions About Communication with Clinical Team Members (CAPACITY) instrument. The instrument has two main subscales: communication, assessing the extent to which providers helped caregivers comprehend details of clinical visits, and capacity, defined as the extent to which providers assessed whether caregivers were able to care for patients. Results: Of 265 caregivers who were approached, 240 (90.6%) enrolled in the study. The mean communication and capacity subscale scores were 2.7 ± 0.6 and 1.5 ± 0.6, respectively (range, 0–4 [higher = better]). Communication subscale scores were lower among caregivers of patients who experienced (vs. those who did not experience) a 30‐day readmission (2.6 ± 0.5 vs. 2.8 ± 0.6, respectively; p =.047). Capacity subscale scores were inversely associated with restriction in patient daily activities (a 0.04 decrement in the capacity score for every 1 point in daily activity restriction; p =.008). Conclusions: After pancreatectomy, patients with pancreatic and periampullary cancer whose caregivers reported worse communication with care providers were more likely to experience readmission. Caregivers of patients with greater daily activity restrictions were less likely to report being asked about the caregiver's skill and capacity by clinicians. Plain Language Summary: This prospective study used a validated survey instrument and reports on the quality of communication between health care providers and caregivers as reported by caregivers of patients with pancreatic and periampullary cancer after pancreatectomy.In an analysis of 240 caregivers enrolled in the study, lower communication scores (the extent to which providers helped caregivers understand clinical details) were associated with higher odds of 30‐day patient readmission to the hospital.In addition, lower capacity scores (the extent to which providers assessed caregivers' ability to care for patients) were associated with greater impairment in caregivers.The strikingly low communication quality and capacity assessment scores suggest substantial room for improvement, with the potential to improve both caregiver and patient outcomes.
