Background : This study investigates the relationship between changes in physical activity levels and risk of metabolic syndrome. Methods : This study examined 1,686 adults aged 40 to 69 years from a community-based cohort study with complete 1st to 4th follow-up data between 2011 and 2020. Changes in physical activity were evaluated through baseline and follow-up surveys using physical activity questionnaires. Metabolic syndrome was diagnosed according to the International Diabetes Federation criteria. A survival analysis was conducted using a multivariate extended Cox regression model with a significance level set at P420 minutes per week at first follow-up) had a 25% decrease in the HR for metabolic syndrome compared with the consistently inactive group (walking ≤420 minutes per week at both baseline and first follow-up) (HR, 0.75; 95% CI, 0.57 to 0.98). Conclusion : Changes in physical activity levels are associated with risk of metabolic syndrome. These results provide important insights for future investigations into the link between physical activity changes and disease occurrence.

Invasion risk assessments are essential for making informed decisions, allocating resources, and implementing targeted strategies to prevent or minimize the harmful effects of invasive species on native biodiversity, agricultural productivity, and natural ecosystems. In this study, the random forest algorithm was used to assess the spatial invasion risk of Lantana camara, one of the world’s top 100 worst invasive weeds, across all continents under current and future environmental conditions. The current invasion risk was relatively high on four continents (i.e., Africa, Australia, Oceania, and South America) within approximately 35°N and 35°S latitude, estimated to cover at least 68.98 % of the total land surface. Furthermore, projections for future environmental changes suggested a substantial increase in invasion risk across all continents, with the most significant changes (251.52 %) observed in Europe compared with current invasion levels. Additionally, invasion risk was predicted to extend beyond 35°N latitude. Categorizing 200 countries and territories into distinct risk levels, 27 countries had current invasion potential, and introduction and establishment was predicted in 114 countries. Moreover, at least 45 countries, including Canada, India, Italy, and United States, were projected to transition from no or low invasion risk to high invasion risk and 28 countries had a risk increase of over 50 %. Current study provides valuable insights into the global invasion risk posed by L. camara. These results are expected to be of great utility for invasive weed management, facilitating the development of control and sustainable management strategies for this notorious weed at both global and local scales.

Abstract Bioactive glass–ceramic (BGC) cage is a substitute for polyether ether ketone (PEEK) cages in anterior cervical discectomy and fusion (ACDF). Only a few comparative studies exist using PEEK and non-window-type BGC cages (CaO–SiO2–P2O5–B2O3) in single-level ACDF. This study compared PEEK cages filled with autologous iliac bone grafts and BGC cages regarding clinical safety and effectiveness. A retrospective case series was performed on 40 patients who underwent single-level ACDF between October 2020 and July 2021 by a single orthopedic spine surgeon. The spacers used in each ACDF were a PEEK cage with a void filled with an autologous iliac bone graft and a non-window-type BGC cage in 20 cases. The grafts were compared pre-operatively and post-operatively at 6 weeks and 3, 6, and 12 months. Post-operative complications were investigated in each group. Clinical outcome was measured, including Visual Analog Scale (VAS) scores of neck and arm pains, Japanese Orthopedic Association score (JOA), and Neck Disability Index (NDI). Dynamic lateral radiographs were used to assess the inter-spinous motion (ISM) between the fusion segment and subsidence. The fusion status was evaluated using a computed tomography (CT) scan. Overall, 39 patients (19 and 20 patients in the PEEK and BGC groups, respectively) were recruited. Eighteen (94.7%) and 19 (95.0%) patients in the PEEK and BGC groups, respectively, were fused 12 months post-operatively, as assessed by ISM in dynamic lateral radiograph and bone bridging formation proven in CT scan. The PEEK and BGC groups showed substantial improvement in neck and arm VAS, JOA, and NDI scores. No substantial difference was found in clinical and radiological outcomes between the PEEK and BGC groups. However, the operation time was considerably shorter in the BGC group than in the PEEK group. In conclusion, a non-window-type BCG cage is a feasible substitute for a PEEK cage with an autologous iliac bone graft in single-level ACDF.

Acacia mearnsii, among the 100 worst invasive weeds worldwide, negatively impacts native biodiversity, agriculture, and natural ecosystems. Global climate change, characterized by rising temperatures and altered precipitation patterns, enhances the risk of A. mearnsii invasion in Asia, making it crucial to identify high-risk areas for effective management. This study performed species distribution modeling using the maximum entropy (MaxEnt) algorithm to predict the potential introduction and spread of A. mearnsii under various climate scenarios based on shared socio-economic pathways (SSP2-4.5 and SSP5-8.5). Currently, only 4.35% of Asia is invaded, with a high invasion risk identified in six countries, including Bhutan, Lebanon, and Taiwan, where more than 75% of their areas are threatened. Under future climate scenarios, 21 countries face invasion risk, among which 14 countries, such as Georgia, Laos, Republic of Korea, and Turkey, are at moderate to very high risk, potentially encompassing up to 87.89% of their territories. Conversely, Northern Asian countries exhibit minimal changes in invasion risk and are considered relatively safe from invasion. These findings underscore that climate change will exacerbate invasion risks across Asia, emphasizing the urgent need for robust management strategies, including stringent quarantine measures and control efforts, to mitigate the threat of A. mearnsii expansion.

The number of alien species introduced into South Korea continues to increase over the years. In particular, several plants have been introduced as ornamentals. Ludwigia peploides, which is native to the Americas and Australia, is believed to have been planted as an ornamental aquatic plant called “water primrose” and “primrose”. It spread to natural ecosystems through rivers, and its distribution is gradually expanding in Suwon, Hwaseong, Busan, and Jeju. However, there has been no specific study on the ecological risk of L. peploides introduced into South Korea. This study, therefore, investigates the distribution status and ecological risks of L. peploides in South Korea through field surveys and allelopathic material analysis, as well as assessing abiotic risk factors. The distribution was confirmed at a total of 19 sites, with high-density mats of a single species forming along the water’s edge and on the water surface. The maximum distribution area was 13,922 m2 in Chilgok Reservoir in Anseong. Stems and plant fragments transported along waterways were continuously forming colonies through vegetative propagation. When evaluating the overall risk, it is determined that L. peploides has a high potential to cause significant damage to the ecosystem if not managed promptly. Therefore, continuous monitoring is necessary to effectively manage and prevent the habitat expansion of L. peploides. The results of this study are expected to aid in the identification of the current distribution and potential ecological risks of L. peploides in South Korea, providing essential data for ecological risk assessment and the development of effective management strategies.

Abstract Background Phosphine (PH3) resistance in stored grain insect pests poses a significant challenge to effective pest control strategies worldwide. This study delved into understanding PH3-resistant mechanism, with the objective of informing robust and sustainable pest management strategies that could mitigate the impacts of PH3 resistance. Results In this regard, the transcriptomic analysis identified 23 genes associated with chitin synthesis and cuticle formation, which showed significant expression in PH3-resistant (R) strains compared to susceptible strains. Micro-computed tomography (Micro-CT) revealed an extended and tighter cuticular structure in the PH3-R Tribolium castaneum than PH3-susceptible strains but with no changes in the cuticle thickness. This altered cuticle structure may reduce PH3 penetration through cuticles rather than completely closing spiracles during fumigation. It is also hypothesized to prevent water loss from the insect body, as water production decreased in PH3-R T. castaneum due to the down-regulation of the electron transport chain function. Validation of several chitin synthesis gene expression levels revealed consistent results with those of transcriptomic analysis. Conclusion Overall, integrating physical treatments using synthetic amorphous silicates, water absorbents, and cuticle-damaging materials during PH3 fumigation is recommended for its prolonged and controlled usage in the field. Abstract Anthropogenic activities and global climate change increase the risk of Chromolaena odorata invasion and habitat expansion. To predict its global distribution and habitat suitability under climate change, a random forest (RF) model was employed. The RF model, utilizing default parameters, analyzed species presence data and background information. The model revealed that the current spatial distribution of C. odorata covers 7,892,447 km2. Predictions for 2061– 2080 indicate expansion of suitable habitat (42.59 and 46.30%), reduction of suitable habit (12.92 and 12.20%), and preservation of suitable habitat (87.08 and 87.80%) under the SSP (Shared Socio-economic Pathway) 2-4.5 and SSP5-8.5 scenarios, respectively, in comparison to the present distribution. Currently, C. odorata is predominantly found in South America, with limited presence in other continents. However, the data suggest that climate change will elevate the global invasion risk of C. odorata worldwide, particularly in Oceania, Africa, and Australia. Countries such as Gambia, Guinea-Bissau, and Lesotho, which currently have unsuitable habitats, are predicted to have highly suitable habitats with climate change, supporting the idea that global habitat expansion for C. odorata will occur due to climate change. This study indicates that proper management of C. odorata is crucial during the early invasion phase.

Oxalis latifolia, a perennial herbaceous weed, is a highly invasive species that poses a threat to agricultural lands worldwide. East Asia is under a high risk of invasion of O. latifolia under global climate change. To evaluate this risk, we employed maximum entropy modeling considering two shared socio-economic pathways (SSP2-4.5 and SSP5-8.5). Currently, a small portion (8.02%) of East Asia is within the O. latifolia distribution, with the highest coverages in Chinese Taipei, China, and Japan (95.09%, 9.8%, and 0.24%, respectively). However, our projections indicated that this invasive weed will likely be introduced to South Korea and North Korea between 2041 and 2060 and 2081 and 2100, respectively. The species is expected to cover approximately 9.79% and 23.68% (SSP2-4.5) and 11.60% and 27.41% (SSP5-8.5) of the total land surface in East Asia by these time points, respectively. South Korea and Japan will be particularly susceptible, with O. latifolia potentially invading up to 80.73% of their territory by 2081–2100. Mongolia is projected to remain unaffected. This study underscores the urgent need for effective management strategies and careful planning to prevent the introduction and limit the expansion of O. latifolia in East Asian countries.

Background Spontaneous intracranial hypotension occurs due to cerebrospinal fluid leakage from the spinal column, and orthostatic headache is the most common clinical presentation. Recent studies showed that bilateral greater occipital nerve blockade demonstrated clinical efficacy in relieving post-dural puncture headache after caesarean section. Case A 40-year-old male who presented severe orthostatic headache was consulted to our pain clinic from neurology department. He initially felt a dull nature pain over the whole occipital area which then spread over the frontal and parietal areas. His headache was combined with nausea and vomiting. An epidural blood patch was delayed until final cisternography, and bilateral greater occipital nerve blockade using ultrasound guidance was performed instead. After the blockade, the previously existing headache around the occipital and parietal areas disappeared completely, but mild headache persisted around the frontal area. Conclusions Greater occipital nerve blockade could be a good therapeutic alternative to improve headache resulting from spontaneous intracranial hypotension.

Background Gasserian ganglion radiofrequency thermoablation is a good treatment option for the management of pain in trigeminal neuralgia. We report a case in which the patient of trigeminal neuralgia combined with anterior cerebral artery aneurysm was treated successfully by gasserian ganglion thermoablation without any complication. Case An 85-year-old female presenting with electric shock like sensation in the gum and molar teeth was diagnosed as trigeminal neuralgia. Carbamazepine medication and trigeminal nerve blockade relieved her pain partially, but severe side effects of carbamazepine occurred. Magnetic resonance angiography of the brain showed saccular aneurysm in inferior aspect of the anterior communicating artery. Gasserian ganglion thermoablation under sedation anesthesia using nicardipine was performed carefully without any rupture of the cerebral aneurysm. Conclusions Gasserian ganglion thermoablation could be performed safely in a patient with cerebral aneurysm without any inadvertent event.

Species distribution modeling is widely used for evaluating invasion risk, and for prioritizing areas for the control and management of invasive species. However, selecting a modeling tool that accurately predicts species invasion risk requires a systematic approach. In this study, five species distribution models (SDMs), namely, artificial neural network (ANN), generalized linear model (GLM), multivariate adaptive regression splines (MARS), maximum entropy (MaxEnt), and random forest (RF), were performed and evaluated their model performance using the mean value of area under the curve (AUC), true skill statistics (TSS), and Kappa scores of 12 ecosystem disturbing alien plant species (EDAPS). The mean evaluation metric scores were highest in RF (AUC = 0.924 ± 0.058, TSS = 0.789 ± 0.109, Kappa = 0.671 ± 0.096, n = 12) and lowest in ANN. The ANOVA of AUC, TSS, and Kappa metrics revealed the RF model was significantly different from other SDMs and was therefore selected as the relatively best model. The potential distribution area and invasion risk for each EDAPS were quantified. Under the current climate conditions of South Korea, the average potential distribution area of EDAPS was estimated to be 13,062 km2. However, in future climate change scenarios, the average percentage change of EDAPS distribution relative to the current climate was predicted to be increased over 219.93%. Furthermore, under the current climate, 0.16% of the area of the country was estimated to be under a very high risk of invasion, but this would increase to 60.43% by 2070. Invasion risk under the current climate conditions was highest in the northwestern, southern, and southeastern regions, and in densely populated cities, such as Seoul, Busan, and Daegu. By 2070, invasion risk was predicted to expand across the whole country except in the northeastern region. These results suggested that climate change induced the risk of EDAPS invasiveness, and SDMs could be valuable tools for alien and invasive plant species risk assessment.

Methyl bromide (MB) has been used in a wide range of applications, but since it was determined to be an ozone-depleting compound, it has only been used for pre-shipment and quarantine purposes in trade. Phosphine (PH3) is currently the ideal fumigant as an MB alternative worldwide. However, the development of PH3 resistance in the target insect pest and longer PH3 fumigation treatment times raise questions about the continued use of PH3. This study attempted to shorten treatment time via combination treatment with ethyl formate (EF). Planococcus citri was used as the main quarantine pest in Korea, and the acute toxicity of EF, PH3, and EF + PH3 was determined at every developmental stage. EF treatment at 4 h showed LCT99 values of 45.85~65.43 mg∙h/L, and PH3 treatment at 20 h showed that of 0.13~0.83 mg∙h/L depending on the developmental stage. The efficacy of PH3 decreased after reducing the treatment time, but synergistic effects were observed at all stages of development of P. citri when both fumigants were used simultaneously for 4 h. After combined treatment, dihydrolipoyl dehydrogenase expression and the production of two phospholipids, PI(O-16:0) and PC(18:2), were significantly reduced in treated P. citri adults compared with the control. Therefore, combined treatments might be key to reducing the treatment time and resistance of PH3 in the field.

Predicting the distribution of invasive weeds under climate change is important for the early identification of areas that are susceptible to invasion and for the adoption of the best preventive measures. Here, we predicted the habitat suitability of 16 invasive weeds in response to climate change and land cover changes in South Korea using a maximum entropy modeling approach. Based on the predictions of the model, climate change is likely to increase habitat suitability. Currently, the area of moderately suitable and highly suitable habitats is estimated to be 8877.46 km2, and 990.29 km2, respectively, and these areas are expected to increase up to 496.52% by 2050 and 1439.65% by 2070 under the representative concentration pathways 4.5 scenario across the country. Abstract Methylglyoxal (MG) is a reactive dicarbonyl intermediate and a precursor of advanced glycation end products (AGEs). The authors investigated the role played by AGEs in muscle myopathy and the amelioration of its effects by curcumin and gingerol. In addition to producing phenotypical changes, MG increased oxidative stress and reduced myotube formation in C2C12 cells. RAGE (receptor for AGEs) expression was up-regulated and MYOD and myogenin (MYOG) expressions were concomitantly down-regulated in MG-treated cells. Interestingly, AGE levels were higher in plasma (~32 fold) and muscle (~26 fold) of diabetic mice than in control mice. RAGE knock-down (RAGEkd) reduced the expressions of MYOD and MYOG and myotube formation in C2C12 cells. In silico studies of interactions between curcumin or gingerol and myostatin (MSTN; an inhibitor of myogenesis) and their observed affinities for activin receptor type IIB (ACVRIIB) suggested curcumin and gingerol reduce the interaction between MSTN and ACVRIIB. The findings of this study suggest enhanced AGE production and subsequent RAGE-AGE interaction obstruct the muscle development program, and that curcumin and gingerol attenuate the effect of AGEs on myoblasts.

BackgroundInfrequent but serious complications of transforaminal epidural steroid injection (TFESI) occur due to inadvertent intravascular injections. A few studies reported that the different needle types can influence on the occurrences of intravascular incidence in TFESI. This study prospectively evaluated whether short-bevel needle can reduce the incidences of intravascular injection of TFESI compared to long-bevel needles.MethodsFrom March 2013 to December 2013, 239 consecutive patients were enrolled and received 249 fluoroscopically guided TFESI using the classic technique. Confirmation of intravascular spread was done initially with real time fluoroscopy and then with digital subtraction angiography method in a same patient. Injection technique for TFESI was the same for both short-bevel and long-bevel needle types.ResultsThe incidences of intravascular injections with the long-bevel and short-bevel needles were 15.0% (21/140) and 9.2% (4/140), respectively. More than half of intravascular injections occurred simultaneously with epidural injections (8.0%, 20/249). There were no statistically significant differences between the long-bevel and the short-bevel needles in the rates of intravascular injections (P = 0.17).ConclusionsShort-bevel needles did not demonstrate any benefits in reducing the incidence of intravascular injection.

Background: Atherogenic dyslipidemia, which is frequently associated with type 2 diabetes (T2D) and insulin resistance, contributes to the development of vascular complications. Statin therapy is the primary approach to dyslipidemia management in T2D, however, the role of non-statin therapy remains unclear. Ezetimibe reduces cholesterol burden by inhibiting intestinal cholesterol absorption. Fibrates lower triglyceride levels and increase high-density lipoprotein cholesterol (HDL-C) levels via peroxisome proliferator- activated receptor alpha agonism. Therefore, when combined, these drugs effectively lower non-HDL-C levels. Despite this, few clinical trials have specifically targeted non-HDL-C, and the efficacy of triple combination therapies, including statins, ezetimibe, and fibrates, has yet to be determined. Methods: This is a multicenter, prospective, randomized, open-label, active-comparator controlled trial involving 3,958 eligible participants with T2D, cardiovascular risk factors, and elevated non-HDL-C (≥100 mg/dL). Participants, already on moderate-intensity statins, will be randomly assigned to either Ezefeno (ezetimibe/fenofibrate) addition or statin dose-escalation. The primary end point is the development of a composite of major adverse cardiovascular and diabetic microvascular events over 48 months. Conclusion: This trial aims to assess whether combining statins, ezetimibe, and fenofibrate is as effective as, or possibly superior to, statin monotherapy intensification in lowering cardiovascular and microvascular disease risk for patients with T2D. Background/Aims: Reports on the association between sarcopenic visceral obesity and nonalcoholic fatty liver disease (NAFLD)-associated morbidities remain scarce. We investigated the association between sarcopenia and visceral obesity, and the influence of this association on hepatic and coronary comorbidities. Methods: The appendicular skeletal muscle mass to visceral fat area ratio (SV ratio) was evaluated using bioelectric impedance analysis. NAFLD and significant liver fibrosis were assessed using transient elastography, and high atherosclerotic cardiovascular disease (ASCVD) risk was defined as a 10-year ASCVD risk score >10%. Sarcopenia was defined as appendicular skeletal muscle mass adjusted by body mass index (<0.789 for men and <0.512 for women). Results: In total, 82.0% (n=1,205) of the entire study population had NAFLD, and 14.6% of these individuals (n=176) exhibited significant liver fibrosis. Individuals with the lowest SV ratio had a significantly increased risk of NAFLD, significant liver fibrosis, and high ASCVD risk (all p<0.05). Individuals with both the lowest SV ratio and sarcopenia had the highest risk of developing NAFLD (odds ratio [OR]=3.11), significant liver fibrosis (OR=2.03), and high ASCVD risk (OR=4.15), compared with those with a higher SV ratio and without sarcopenia (all p<0.05). Conclusions: Low SV ratio combined with sarcopenia was significantly associated with an increased risk of NAFLD, significant liver fibrosis, and high ASCVD risk among individuals with a high risk of NAFLD [ABSTRACT FROM AUTHOR]

Aim: Hepatic ketogenesis is a key metabolic pathway that regulates energy homeostasis. Some related controversies exist regarding the pathogenesis of metabolic-associated fatty liver disease (MAFLD). We aimed to investigate whether intact ketogenic capacity could reduce the risk of MAFLD based on transient electrography (TE) in patients with newly diagnosed type 2 diabetes (T2D). Methods: A total of 361 subjects with newly diagnosed T2D were recruited and classified into two groups based on the median serum bhydroxybutyrate (bHB) level, referred to as the intact and impaired ketogenesis groups. The glucometabolic relevance of ketogenic capacity and associations of the baseline serum b-HB and MAFLD assessed with TE were investigated. Results: Compared to the impaired ketogenesis group, the intact ketogenesis group showed better insulin sensitivity, lower serum triglyceride levels, and higher glycated hemoglobin levels. The controlled attenuation parameter (CAP) was lower in the intact ketogenesis group without statistical significance (289.7 ± 52.1 vs. 294.5 ± 43.6; p=0.342) but the prevalence of moderate-severe steatosis defined by CAP =260 dB/m was significantly lower in the intact group. Moreover, intact ketogenesis was significantly associated with a lower risk of moderate-severe MAFLD after adjusting for potential confounders (adjusted odds ratio 0.55, 95% confidence interval 0.30-0.98; p=0.044).
