Abstract

Achieving both high speed and precision in robot operations is a significant challenge for social implementation. While factory robots excel at predefined tasks, they struggle with environment-specific actions like cleaning and cooking. Deep learning research aims to address this by enabling robots to autonomously execute behaviors through end-to-end learning with sensor data. RT-1 and ACT are notable examples that have expanded robots' capabilities. However, issues with model inference speed and hand position accuracy persist. High-quality training data and fast, stable inference mechanisms are essential to overcome these challenges. This paper proposes a motion generation model for high-speed, high-precision tasks, exemplified by the sports stacking task. Abstract

Background Spontaneous coronary artery dissection (SCAD) is an uncommon cause of acute coronary syndrome (ACS). Various factors predispose individuals to SCAD, including coronary vasospasm, which has been identified in some studies as a related factor. Case summary We present a series of three cases of ACS attributed to SCAD. Two cases involved early de novo recurrence potentially related to coronary spasm, and one case involved iatrogenic coronary artery dissection during a spasm provocation test. Discussion High recurrence rates of SCAD within 30 days of the initial event have been observed, although the effectiveness of pharmacologic therapy has not been conclusively established. While the administration of beta-blockers has been associated with a reduced risk of SCAD recurrence, their use may not be desirable in patients with coronary vasospasm. Detailed evaluations may be beneficial for patients where coronary vasospasm is suspected to tailor appropriate treatment strategies. Moreover, given the higher frequency of iatrogenic coronary artery dissection among patients with SCAD, careful consideration is needed to avoid high-risk periods when scheduling spasm provocation tests. 6. Next-to-minimal $R$-symmetric model: Dirac gaugino, Higgs mass and invisible width

Abstract

We study a singlet extension of the minimal $U(1)_R$ symmetric model, which shares nice properties of Dirac gauginos and $R$-symmetric Higgs sector. At the same time, a superpotential coupling of $R$-charged singlet to the Higgs doublets can give a substantial contribution to the Higgs boson mass. We show that the 125 GeV Higgs boson is consistent with perturbative unification, even if the SUSY scale is as low as 1 TeV and if the $D$-term Higgs potential is suppressed as is often the case in Dirac gauginos. The model also contains a light scalar and fermion, pseudo-moduli and pseudo-Goldstino: The former gets a mass mainly from SUSY breaking soft terms, in addition to a small explicit $R$-symmetry breaking for the latter. We examine how the Higgs mass and width are affected by these light degrees of freedom. Specifically we find thatdepending on parameters of $R$-charged Higgses, the pseudo-moduli lighter than a half of the SM-Higgs boson mass is still allowed by the constraints from invisible decays of the $Z$ and Higgs bosons. 7. Bifurcations in a forced Wilson-Cowan neuron pair

Abstract

We investigate bifurcations of periodic solutions observed in the forced Wilson-Cowan neuron pair by both the brute-force computation and the shooting method. By superimposing the results given by both methods, a detailed topological classification of periodic solutions is achieved that includes tori and chaos attractors in the parameter space is achieved. We thoroughly explore the parameter space composed of threshold values, amplitude, and angular velocity of an external forcing term. Many bifurcation curves that are invisible when using brute-force method are solved by the shooting method. We find out a typical bifurcation structure including Arnold tongue in the angular velocity and the amplitude of the external force parameter plane, and confirm its fractal structure. In addition, the emergence of periodic bursting responses depending on these patterns is explained.

Abstract

Aims Heart failure with preserved ejection fraction (HFpEF), which is caused by wide various conditions, has become a major public health problem. Transthyretin amyloid cardiomyopathy (ATTR-CM), which is thought to be an underdiagnosed disease, can cause HFpEF. Non-invasive diagnosis using 99mTechnetium (Tc)-pyrophosphate (PYP) scintigraphy enables accurate diagnosis of ATTR-CM. The aim of this study was to clarify the prevalence and characteristics of ATTR-CM among Japanese patients with HFpEF. Methods and results This study was a multicentre, prospective, observational study conducted in Japan. We enrolled 373 patients with HFpEF [left ventricular (LV) ejection fraction ≥50%] aged ≥65 years who were admitted to the department of cardiology from September 2018 to January 2022. A 99mTc-PYP scintigraphy scan was performed during admission in all eligible patients. Cardiac 99mTc-PYP retention was graded according to a previously reported visual scale ranging from 0 to 3 points. The scan was considered positive when it revealed moderate-to-severe 99mTc-PYP uptake (Grade 2–3) in both ventricles. Patients were divided into ATTR-CM and non-ATTR-CM patients according to positive (Grade 2–3) or negative (Grade 0–1) 99mTc-PYP scintigraphy, respectively. Medical history, blood tests, electrocardiogram, echocardiography, and magnetic resonance imaging in the two groups of patients were compared. Among the 373 patients with HFpEF, 53 patients (14.2%; 95% confidence interval: 10.7–17.7) showed positive uptake on 99mTc-PYP scintigraphy. An endomyocardial biopsy was performed in 32 patients and confirmed amyloidosis in all cases. Abstract

Rapidly progressive in-stent restenosis (ISR) after stent deployment from the left main trunk (LMT) to the left anterior descending artery (LAD) without plaque at the LMT ostium has not been reported. A 60-year-old Japanese man with a history of scleroderma, pulmonary fibrosis, and type 2 diabetes developed acute myocardial infarction of the right coronary artery (RCA) and was treated by emergency percutaneous coronary intervention (PCI) for RCA. Nine days later he underwent PCI from the LMT to the LAD. Follow-up coronary angiography (CAG) at 9 and 21 months post-PCI did not reveal ISR in any lesion, but the patient experienced cardiac arrest at 25 months post-PCI. Emergency CAG after resuscitation revealed ISR of the LMT ostium; emergency PCI was conducted. The development of ISR at the ostium of the LMT although the patient was free of plaque 4 months before is extremely unusual. This rare ISR of the LMT ostium progressed rapidly after follow-up CAG revealed no ISR at 21 months post-stent implantation.

11. Effects of luseogliflozin and voglibose on high-risk lipid profiles and inflammatory markers in diabetes patients with heart failure

Abstract

Sodium-glucose cotransporter 2 inhibitors could reduce cardiovascular events in patients with heart failure irrespective of diabetes status. In this prespecified sub-analysis of randomised-controlled trial, we investigated the efficacy of luseogliflozin (2.5 mg daily), a sodium-glucose cotransporter 2 inhibitor, with that of voglibose (0.6 mg daily), an alpha-glucosidase inhibitor, on high-risk lipid profile and inflammatory markers in patients with type-2 diabetes and heart failure. Among the 157 patients studied, there were no significant differences in the mean malondialdehyde LDL or small-dense LDL cholesterol levels between the luseogliflozin and voglibose groups (percent change: 0.2% vs. - 0.6%, p = 0.93; - 1.7% vs. - 8.6%, p = 0.21) after 12 weeks in comparison to levels at the baseline. No significant difference was observed between the two groups in the adiponectin and high-sensitivity C-reactive protein levels after 12 weeks compared to the baseline levels (percent change, - 1.6% vs. - 4.0% and 22.5% vs. 10.0%; p = 0.52 and p = 0.55, respectively). In conclusion, in patients with type-2 diabetes and heart failure, compared to voglibose, luseogliflozin did not significantly improve the high-risk lipoprotein profile including malondialdehyde LDL and small-dense LDL cholesterol or the levels of inflammatory markers, including adiponectin and high-sensitivity C-reactive protein.Trial registration: Trial number: UMIN-CTR, UMIN000018395; Registered 23 July 2015; URL: https://www.umin.ac.jp/ctr/index.htm .

Abstract

Aims Sodium glucose co-transporter 2 inhibitors have diuretic effects in both patients with glycosuria and with natriuresis. We sought to assess the effect of luseogliflozin on estimated plasma volume (ePV) in patients with type 2 diabetes and heart failure with preserved ejection fraction (HFpEF). Methods and results This study was a post-hoc analysis of the MUSCAT-HF trial (UMIN000018395), a multicentre, prospective, open-label, randomized controlled trial that assessed the effect of 12 weeks of luseogliflozin (2.5 mg, once daily, n = 83) as compared with voglibose (0.2 mg, three times daily, n = 82) on the reduction in brain natriuretic peptide (BNP) in patients with type 2 diabetes and HFpEF. The analysis compared the change in ePV calculated by the Straus formula from baseline to Weeks 4, 12, and 24, using a mixed-effects model for repeated measures. We also estimated the association between changes in ePV and changes in other clinical parameters, including BNP levels. Luseogliflozin significantly reduced ePV as compared to voglibose at Week 4 {adjusted mean group-difference -6.43% [95% confidence interval (CI): -9.11 to -3.74]}, at Week 12 [-8.73% (95%CI: -11.40 to -6.05)], and at Week 24 [-11.02% (95%CI: -13.71 to -8.33)]. The effect of luseogliflozin on these parameters was mostly consistent across various patient clinical characteristics. The change in ePV at Week 12 was significantly associated with log-transformed BNP (r = 0.197, P = 0.015) and left atrial volume index (r = 0.283, P = 0.019). Conclusions Luseogliflozin significantly reduced ePV in patients with type 2 diabetes and HFpEF, as compared with voglibose. The reduction of intravascular volume by luseogliflozin may provide clinical benefits to patients with type 2 diabetes and HFpEF.

Abstract

Aims:\ud The safety and efficacy of the novel selective cardiac myosin activator, omecamtiv mecarbil, in patients with heart failure with reduced ejection fraction (HFrEF) is tested in the Global Approach to Lowering Adverse Cardiac outcomes Through Improving Contractility in Heart Failure (GALACTIC‐HF) trial. Here we describe the baseline characteristics of participants in GALACTIC‐HF and how these compare with other contemporary trials.\ud \ud Methods and Results:\ud Adults with established HFrEF, New York Heart Association functional class (NYHA) ≥ II, EF ≤35%, elevated natriuretic peptides and either current hospitalization for HF or history of hospitalization/ emergency department visit for HF within a year were randomized to either placebo or omecamtiv mecarbil (pharmacokinetic‐guided dosing: 25, 37.5 or 50 mg bid). 8256 patients [male (79%), non‐white (22%), mean age 65 years] were enrolled with a mean EF 27%, ischemic etiology in 54%, NYHA II 53% and III/IV 47%, and median NT‐proBNP 1971 pg/mL. HF therapies at baseline were among the most effectively employed in contemporary HF trials. GALACTIC‐HF randomized patients representative of recent HF registries and trials with substantial numbers of patients also having characteristics understudied in previous trials including more from North America (n = 1386), enrolled as inpatients (n = 2084), systolic blood pressure

Abstract

Background Effects of sodium‐glucose cotransporter 2 inhibitors on reducing hospitalization for heart failure have been reported in randomized controlled trials, but their effects on patients with heart failure with preserved ejection fraction (HFpEF) are unknown. This study aimed to evaluate the drug efficacy of luseogliflozin, a sodium‐glucose cotransporter 2 inhibitor, in patients with type 2 diabetes mellitus and HFpEF. Methods and Results We performed a multicenter, open‐label, randomized, controlled trial for comparing luseogliflozin 2.5 mg once daily with voglibose 0.2 mg 3 times daily in patients with type 2 diabetes mellitus suffering from HFpEF (left ventricular ejection fraction >45% and BNP [B‐type natriuretic peptide] concentrations ≥35 pg/mL) in a 1:1 randomization fashion. The primary outcome was the difference from baseline in BNP levels after 12 weeks of treatment between the 2 drugs. A total of 173 patients with diabetes mellitus and HFpEF were included. Of these, 83 patients were assigned to receive luseogliflozin and 82 to receive voglibose. There was no significant difference in the reduction in BNP concentrations after 12 weeks from baseline between the 2 groups. The ratio of the mean BNP value at week 12 to the baseline value was 0.79 in the luseogliflozin group and 0.87 in the voglibose group (percent change, −9.0% versus −1.9%; ratio of change with luseogliflozin versus voglibose, 0.93; 95% CI, 0.78–1.10; P=0.26). Conclusion In patients with type 2 diabetes mellitus and HFpEF, there is no significant difference in the degree of reduction in BNP concentrations after 12 weeks between luseogliflozin and voglibose.

15. A failure assessment method for a pipe bend subjected to both a bending moment and internal pressure

Abstract

This paper proposes a new failure assessment method for a steel pipe bend subjected to both a bending moment and internal pressure. Consistent with previous studies, it was shown that the maximum bending moment of a pipe bend subjected to a bending moment increases with the addition of internal pressure. However, it was experimentally confirmed that the addition of this internal pressure has the detrimental effect of significantly reducing the critical deformation (maximum bending angle) of the pipe bend. In addition, it was found that, subsequent to the application of a large deflection, cracks initiate at the most deformed part of the pipe bend during the process of unloading the internal pressure and then the applied load. Herein, the authors propose a practical failure assessment method which uses small-scale tests and nonlinear finite element (FE) analyses to predict the critical deformation and crack initiation position for a full-scale pipe bend. The failure criterion, which uses principal stress, mean stress, and equivalent plastic strain, was developed using small-scale tests. A failure assessment was conducted by comparing the predictions of this criterion with stress and strain histories obtained from FE analyses. Methods and results: This study was a post‐hoc analysis of the MUSCAT‐HF trial (UMIN000018395), a multicentre, prospective, open‐label, randomized controlled trial that assessed the effect of 12 weeks of luseogliflozin (2.5 mg, once daily, n = 83) as compared with voglibose (0.2 mg, three times daily, n = 82) on the reduction in brain natriuretic peptide (BNP) in patients with type 2 diabetes and HFpEF. The analysis compared the change in ePV calculated by the Straus formula from baseline to Weeks 4, 12, and 24, using a mixed‐effects model for repeated measures. We also estimated the association between changes in ePV and changes in other clinical parameters, including BNP levels. Luseogliflozin significantly reduced ePV as compared to voglibose at Week 4 {adjusted mean group‐difference −6.43% [95% confidence interval (CI): −9.11 to −3.74]}, at Week 12 [−8.73% (95%CI: −11.40 to −6.05)], and at Week 24 [−11.02% (95%CI: −13.71 to −8.33)]. The effect of luseogliflozin on these parameters was mostly consistent across various patient clinical characteristics. The change in ePV at Week 12 was significantly associated with log‐transformed BNP (r = 0.197, P = 0.015) and left atrial volume index (r = 0.283, P = 0.019). Conclusions: Luseogliflozin significantly reduced ePV in patients with type 2 diabetes and HFpEF, as compared with voglibose. The reduction of intravascular volume by luseogliflozin may provide clinical benefits to patients with type 2 diabetes and HFpEF. [ABSTRACT FROM AUTHOR]

Abstract

Low-temperature superconducting MRI magnets are usually cooled by a large amount of liquid helium (LHe); however, LHe is getting scarcer and its price has been rising. Therefore, the development of helium free high-temperature superconducting (HTS)-MRI magnet is expected. HTS-MRI magnet fabricated by REBCO tape superconductors has mainly two problems for producing a highly stabilized magnetic field for MRI imaging. One problem is current fluctuation of a power supply, which is necessary for excitation because the permanent current operation of HTS-MRI magnet is difficult. Another is a screening current in REBCO tapes, which affect the magnetic field stability. The MRI magnetic field feedback control test was carried out, that is, the overshooting and the current feedback control, using 32 H HTS-MRI test magnet and the highly stabilized power supply. The magnetic field was evaluated under the current feedback control at 1.5 T (66 A). By the overshooting and the current feedback control, the magnetic field stability was improved to 0.7 ppm/ h which is enough for MRI imaging.

Abstract

The article describes the synthesis of soymorphins, µ opioid peptides derived from soy β-conglycinin β-subunit. The peptides exhibited opioid activity more potently than did human β-casoorphins on guinea pig ileum (GPI) assay. On mouse vas deferens assay, their opioid activities were less potent than on GPI assay, suggesting that they are selective for the µ opioid receptor.

Abstract

Influence of age on parathyroid hormone secretion and release was studied in vitro using rat parathyroid glands in culture and in vivo by EDTA injection. Parathyroid hormone was measured by radioimmunoassay using guinea pig antibody for bovine parathyroid hormone and I-labeled synthetic N-terminal peptide of bovine parathyroid hormone. Significantly less PTH was released from parathyroid glands of 18-mo-old rats than from those of 1 mo of age at various calcium concentrations of the medium in . Secretion of parathyroid hormone in response to EDTA-induced hypocalcemia in vivo was also less in older rats than in younger ones, giving lower ΔPTH/ΔCA values in the older animal.
