Background: Various approaches for thyroid surgery became possible with the use of robotic systems. Transoral robotic thyroidectomy (TORT) is one of the newest approaches and draws attention because of its cosmetic excellence. In this study, we compared the surgical outcomes of TORT and conventional open thyroidectomy (OT). Methods: We retrospectively reviewed and compared the medical records of consecutive patients who underwent TORT or OT for thyroid carcinoma from March 2009 to January 2018. Propensity score matching using 10 clinico-pathologic factors was used to generate two matched cohorts, each composed of 186 patients. Results: The study included 372 patients who underwent TORT (n = 186) or OT (n = 186). Mean age, tumor size, and gender were not different between both groups. The two groups showed similar surgical outcomes, except for a longer operative time for TORT. There was one patient with immediate postoperative bleeding in the TORT group. The patient underwent re-operation for hemostasis with endoscopic approach. In the OT group, one patient had wound seroma, which was treated by several rounds of needle aspiration without infection. Vocal cord palsy was present in one patient in the TORT group, which was recovered in 3 months. Conclusions: TORT could be performed safely and had comparable surgical outcomes with OT in the selected patients. Long-term complications are becoming more important as the survival rate of breast cancer improves. Treatment-related myeloid neoplasm is an important long-term complication in breast cancer survivors as it has a poor prognosis. We evaluated the incidence and risk factors for the development of treatment-related acute myeloid leukaemia (AML)/myelodysplastic syndrome (MDS) in patients treated with early breast cancer. We accessed the national Korean database to identify 153,565 patients diagnosed with breast cancer between January 2007 and October 2016 who underwent surgery for breast cancer. We estimated the cumulative incidence of AML/MDS and analysed the risk factors for developing AML/MDS. Of 153,575 patients, 79,321 received anthracycline-based adjuvant therapy, 14,317 received adjuvant therapy without anthracyclines and 46,657 did not receive adjuvant chemotherapy. Overall, 120 developed AML (105 in the anthracycline group, 9 in the non-anthracycline group and 6 in the control group), and 128 developed MDS (96, 9 and 23 in each group). The 10-year cumulative incidence of AML/MDS was the highest in the anthracycline group (0.221% and 0.199%), followed by the non-anthracycline group (0.122% and 0.163%) and the control group (0.024% and 0.089%). The risk of developing AML/MDS was significantly higher in patients treated with anthracyclines (hazard ratio [HR] 9.531; p < 0.0001 for AML and HR 2.559; p < 0.0001 for MDS) compared to patients in the control group. This study found that anthracycline-based adjuvant therapy significantly increased the risk of AML/MDS in Korean breast cancer patients, with the risk persisting for at least 10 years. While the cumulative incidence was low, the long-term risks of AML/MDS should be taken into account considering the poor outcomes associated with these neoplasms. • Therapy-related myeloid neoplasms are increasing as survival rates for breast cancer improve. • Anthracyclines increased the risk of AML/MDS in breast cancer patients. • Chemotherapy without anthracycline also increased the risk of AML. • Despite the low incidence, therapy-related AML/MDS should be kept in mind. In vitro biodegradation of Bombyx mori silk fibroin (SF) was studied using food-grade proteolytic enzymes to replace acid hydrolysis. Based on the residual protein quantity and yield of amino acids (AAs) after enzymatic hydrolysis, we evaluated the proteolytic enzyme process of SF. FoodPro and Alcalase that are classified as alkaline proteases are selected as two of the best candidate enzymes for hydrolysis of SF. The activity of these enzymes exhibits a broad range of pH (6.5 to 9.0) and temperature (50°C to 65°C). The single enzyme treatment of SF using FoodPro exhibited a hydrolytic efficiency of 20–25%, and >2 g/L AAs were released after reaction for 3 h. A 2-stage enzymatic treatment using a combination of FoodPro and Flavourzyme in a sequence for a reaction time of 6 h was developed to enhance the efficiency of the proteolytic process. The yield of AAs and residual protein quantity in the enzymatic hydrolysates obtained from FoodPro-treated regenerated SF in the 1st step was 2,040 ± 23.7 mg/L and 70.6%, respectively. The yield of AAs was > two-fold (4,519 ± 42.1 mg/L), whereas the residual protein quantity decreased to 55.1% after the Flavourzyme treatment (2nd step) compared to those of the single FoodPro treatment. In the mixed treatment by simultaneously using FoodPro and Flavourzyme, approximately 45% of SF was degraded and 4.5 g/L of AAs were released within 3 h of reaction time. The regenerated SF and its enzymatic hydrolysates were characterized by performing UV-visible spectra, gel electrophoresis, and size-exclusion chromatography analyses. In the 2-stage treatment using FoodPro initially and subsequently Flavourzyme, the aggregates and high molecular weight proteins of SF were dissociated and degraded into the low molecular weight proteins/peptides (10–15 kDa and 27 kDa). Background: Oseltamivir is widely used for the treatment of influenza infection, but data on its effectiveness in treating infections with pandemic influenza A H1N1 2009 are scarce. Therefore, we compared the effectiveness of oseltamivir and routine symptomatic treatment in pandemic influenza A H1N1 2009 infection. Methods: Between August and October 2009, among laboratory confirmed influenza cases who were aged over 15 y with no underlying disease, 90 patients with oseltamivir treatment and 72 patients with symptomatic treatment were evaluated by telephone interviews to compare clinical outcomes. No patient was hospitalized. The primary end-point was the duration of illness. We also assessed the time to attain a sense of well-being and the time to return to normal activity. Results: There was no statistically significant difference in the duration of illness between the oseltamivir-treated group and the symptomatic treatment group (mean duration after the start of treatment, 6.50 ± 3.75 days vs 7.04 ± 3.75 days). There was also no statistically significant difference between the 2 groups in the time to a sense of well-being (1.70 ± 1.57 days vs 2.00 ± 2.12 days) and to return to normal activity (7.13 ± 2.61 days vs 7.58 ± 2.71 days). The complication rate was very low (only 2 cases of mild pneumonia in oseltamivir recipients) and no significant difference was found between the 2 groups ( p = 0.50). Conclusions: Oral oseltamivir treatment does not significantly reduce the duration of illness or influenza-related complications in mild pandemic influenza A H1N1 2009 virus infection in previously healthy adults. Abstract: Glutathione-S-transferases are essential proteins involved in cellular detoxification. The expression of GSTs has been studied extensively under various environmental stressors including xenobiotics. Here, we have isolated AtGST10, one of the phi classes of AtGSTs on the basis of its interaction with BAK1 in a yeast two-hybrid screen. BAK1 is an LRR-RLK, acting in both brassinosteroid signaling and plant defense responses. We found that AtGSTF10 binds to BAK1 through its N-terminal domain. AtGSTF10 is expressed ubiquitously in plant tissues, and the endogenous transcript level of AtGSTF10 was not induced by plant growth regulators or abiotic stressors, except drought, unlike other GSTs. Overexpression of AtGSTF10 conferred higher tolerance to salt and disturbed redox status of transgenic plants. The down-regulation of AtGSTF10 produced by RNA interference caused reduced tolerance to abiotic stress and an accelerated senescence of transformants, indicating that AtGSTF10 is involved in stress tolerance and the BAK1-mediated spontaneous cell death signaling pathway in Arabidopsis.
