Abstract

Summary: Treatment options for idiopathic multicentric Castleman disease (iMCD) are currently limited, especially for patients who do not respond or are resistant to interleukin‐6 inhibitors. For the first time, we innovatively designed a protocol using rituximab–bortezomib–dexamethasone (RVD) as first‐line consolidation therapy in patients newly diagnosed with iMCD. Furthermore, we adopted a no‐maintenance treatment strategy to simplify post‐remission care. Five patients with iMCD were enrolled (including one with TAFRO syndrome) and underwent the RVD regimen, all of whom achieved partial response (PR) or better. After four cycles of RVD, three (60%) patients achieved PR, while one (20%) achieved a complete response. These five patients, who achieved PR or better, discontinued treatment but remained stable for a median follow‐up of 11 months, with a duration of response of 7, 7, 10, 12 and 13 months, respectively. None of the patients experienced grade ≥3 adverse events during the observation period. Collectively, these findings demonstrated that the RVD regimen may be a promising treatment option for patients with iMCD. It was a safe and effective approach that resulted in lasting responses without the need for ongoing maintenance therapy. [ABSTRACT FROM AUTHOR]

However, data on the outcomes of treatment for Candida tropicalis fungemia in patients with neutropenia remain limited.Methods: In the present study, 90 neutropenic adult patients with proven Candida tropicalis fungemia, who received initial antifungal therapy, were retrospectively analyzed.Results: These results revealed that the overall 8-day and 30-day mortality among patients in the entire data set were 22.2% and 33.3%, respectively. However, there was no significant difference between the survival and death group, in terms of baseline characteristics. The univariate analysis of risk factors identified the treatment with azole as a predictor of mortality, while treatments that containing amphotericin B were associated with reduced mortality. In addition, the survival rate on day 30 was observed in 60.7% (17/28) of patients who were initially treated with echinocandins, while this was observed in 86.4% (19/22, P= 0.039) and 100% (13/13, P= 0.024) of patients treated with amphotericin B plus echinocandins and amphotericin B, respectively.Conclusion: These data indicate for the first time that the initial therapy with amphotericin B-based agents was associated with a better survival rate and could be assessed as the optimal strategy for the treatment of Candida tropicalis fungemia in patients with neutropenia.Keywords: Candida tropicalis, candidemia, hematological malignancy, neutropenia, amphotericin B, echinocandins

Whole-exome sequencing identifies novel somatic alterations associated with outcomes in idiopathic multicentric Castleman disease In this study, paraffin tissues from 22 patients with iMCD were subjected to whole exome sequencing (WES) and the association of the somatic genomic alterations of patients with their outcomes was analysed. Aiming to identify the genetic characteristics in iMCD patients, we performed WES in these 22 patients. Abstract

Antithyroid drugs (ATDs) induced leukopenia is commonly seen, but life-threatening agranulocytosis is a rare occurrence. Interestingly, agranulocytosis accompanied with plasmacytosis in bone marrow (BM) is rarer. In this study, we admitted a patient with Graves’ disease who had been treated with 15 mg/d methimazole (MMI) for 42 days. She presented with agranulocytosis and plasmacytosis in bone marrow (BM). The patient withdrew taking MMI and was treated with broad-spectrum antibiotics and G-CSF. After two weeks, the patient’s peripheral blood improvement was achieved and BM nearly returned to normal level. The case provides evidence that the elder patients with a high dose of MMI treatment are prone to develop agranulocytosis, especially the duration of treatment is longer than three months. We summary the literatures, and propose our new viewpoint on the mechanism of plasmacytosis in ATDs-induced agranulocytosis patients.

Abstract

B-cell lymphoma is the most common non-Hodgkin lymphoma and is usually treated with the R-CHOP regimen. However, some patients might develop interstitial pneumonitis (IP), which has led to death in some patients. With advances in detection techniques, infections are now considered to be an important etiologic factor in IP, particularly the common opportunistic causative agent Pneumocystis jiroveci. A retrospective cohort analysis at our center has tentatively elucidated that TMP-SMX can achieve effective IP prevention. There are no prospective studies to confirm the preventive effect of TMP-SMX and its mechanism.

ABSTRACTIn this study, we present a rare case of fatal breakthrough Candida tropicalisinfection in a patient with acute lymphoblastic leukemia (ALL) while on posaconazole prophylaxis. Then, we explore the mechanisms underlying azole resistance by focusing on enhanced efflux pumps and changes in the azole target enzyme Erg11p, which was encoded by the ERG11 gene. Our study demonstrates that Y132C substitution of Erg11p combined with MDR1 overexpression may be the pan-azole resistance mechanisms in Candida tropicalis.
