Background: Diagnostic dilemma between clinical Meniere's disease and radiological endolymphatic hydrops (EH) has emerged since the introduction of hydrops magnetic resonance imaging (MRI). The aim of this study is to explore the potential application of hydrops MRI on diagnosing the EH. Methods: This review was developed from peer-reviewed articles published in those journals listed on journal of citation reports. The MEDLINE database of the US National Library of Medicine, Scopus, and Google Scholar were used to collect articles based on the guidelines (PRISMA 2020 statement) for reporting reviews. Results: Initially, 470 articles were retrieved from 1983 to 2023, and 80 relevant articles were ultimately selected. The sensitivity (69%-92%) and specificity (78%-96%) values varied from each laboratory for detecting EH via hydrops MRI, probably due to candidate selection and the grading system employed. Conclusion: The application of hydrops MRI allows (1) differentiation between EH and sudden sensorineural hearing loss; (2) determination of the affected side of EH; and (3) confirmation of the diagnosis of EH concomitant with other disorders. Notably, not all differentials for EH can be visualized on MR images. One of the existing gaps to be filled is that updated hydrops MRI fails to identify distortion, that is, rupture, collapse, fistula, or fibrosis of the inner ear compartments, akin to what histopathological evidence can demonstrate. Objective/Hypothesis: This study correlated stage of Meniere's disease (MD) with MR imaging of endolymphatic hydrops (EH) to assess the role of MD staging in modern era. Study Design: Retrospective study. Methods: Fifty‐four MD patients (60 ears) underwent an inner ear test battery and were further confirmed by MR imaging. Sixty MD ears were divided into stages I–IV, and hydrops MR images at each stage were compared. Results: Hydrops MRI demonstrated that EH at the cochlea with respective Grades 0/I/II were 3/7/1 ears for stage I, 0/5/3 ears for stage II, 1/6/26 ears for stage III and 0/2/6 ears for stage IV. Significant relationship was not identified between MD stage and grades of cochlear hydrops. Similarly, no significant relationship was shown between MD stage and grades of vestibular (saccular/utricular) hydrops. The optimal cutoff value of four‐tone average for predicting severe type (Grade II–III) cochlear/vestibular EH was 48 dB, which was within the stage III. Hence, prevalence of severe type (Grade II) cochlear EH in stages III (79%) and IV (75%) was significantly higher than stages I (9%) and II (38%). Similarly, severe type saccular/utricular EH in stages III (64%) and IV (75%) also showed significantly higher than stages I (18%) and II (25%). Conclusion: Although conventional MD staging fails to correlate with the grades of EH on hydrops MRI, late‐stage MD may indicate heightened EH severity in the cochlea and vestibule. There is no comprehensive and up-to-date overview of audiovestibular approach to the posterior fossa tumors in the literature. This paper reviewed the literature relating to tumors at the posterior cranial fossa to find red flags alerting a posterior fossa lesion from audiovestibular perspectives. This review was developed from articles published in those journals listed on the journal citation reports. Through the PubMed database, Embase, Google Scholar, and Cochrane library, 60 articles were finally obtained based on the PRISMA guidelines for reporting reviews. The presence of one red flag indicates a positive predictive value of 33% for detecting a posterior fossa lesion. Clinical features, namely, 1) mid-frequency sudden sensorineural hearing loss (SNHL), 2) bilateral sudden SNHL, and 3) rebound nystagmus may indicate a posterior fossa lesion, representing one, two, and three red flags, respectively. Those with 1) mid-frequency sudden SNHL, 2) bilateral sudden SNHL, and 3) rebound nystagmus trigger one, two, and three red flags, respectively, alerting clinicians the possibility of a posterior fossa lesion, which warrant MR imaging to exclude life-threatening or treatable conditions. This article explores the use of hydropic MR imaging as a potential diagnostic tool for delayed endolymphatic hydrops (DEH), a condition that resembles Meniere's disease. The study reveals that DEH patients had the highest occurrence of hydrops in the cochlea, followed by the saccule and utricle. Most cases of cochlear and vestibular hydrops in DEH patients were classified as severe. The authors propose that hydropic MR imaging has the potential to be used in the future as a diagnostic tool for DEH. The article also discusses the clinical evaluation and diagnostic imaging findings in patients with DEH, highlighting the similarity in symptoms between patients with and without hydrops. This study investigated the role of serum osmolality in Meniere's disease (MD) patients with acute sensorineural hearing loss (SNHL). Retrospective study. Twenty definite MD patients with acute unilateral SNHL were treated with an osmotic diuretic (Isosorbide, 100 mL daily) and assigned to Group A. Another 20 age- and sex-matched definite MD patients with acute SNHL were not given Isosorbide and assigned to Group B. Both groups underwent audiometry and blood examination for serum osmolality before and after treatment. Group A revealed a significant increase in serum osmolality after treatment. The optimal cut-off values for increased serum osmolality in Group A were +1.5 mOSM/L for predicting hearing improvement at frequencies of 250–1000 Hz, and +2.5 mOSM/L at 2000–4000 Hz. Comparing increased levels of serum osmolality (> +2.0 vs. ≤ +2.0 mOSM/L), Isosorbide dosing at 3.0 L vs. 1.0 L, significantly differed in the odds ratio (OR). Isosorbide at a total dosage of 3.0 L thus improves the hearing threshold by >10 dB at frequencies of 250–2000 Hz. The Isosorbide at a total dosage of 3.0 L may increase serum osmolality by > +2.0 mOSM/L, and improve the hearing threshold for hydropic ears at least >10 dB at low- and mid-frequencies. Increasing life expectancy and declining birth rate are two key drivers of population aging globally, resulting in a falling population. This study investigated the evolution of pediatric patients with vertigo/dizziness during the past two decades. From 2001 to 2010, a total of 17,123 new patients with vertigo/dizziness visited our neurotological clinic. Of them, 472 (2.8%) were children (Group A). In contrast, 260 children (1.3%) out of 20,404 new patients with vertigo/dizziness were experienced during the period 2011–2020 (Group B). All patients underwent an inner ear test battery before diagnosis. The incidence of pediatric vertigo/dizziness in relation to overall neurotological cases significantly decreased from the first decade (2.8%) to the second decade (1.3%). Approximately 80% prevalence of pediatric vertigo/dizziness cases were referred to as vestibular migraine and benign paroxysmal vertigo of childhood regardless of Group A or B. Correlation between annual birth rate (x) and incidence of pediatric vertigo/dizziness cases in relation to overall neurotological cases (y) represented as y = 6.488x − 0.037 via linear regression analysis. The evolution of pediatric vertigo/dizziness revealed decreased incidence from 2.8% (2001–2010) to 1.3% (2011–2020), which may be related to the declined annual birth rate from 11.7‰ (2001) to 7.0‰ (2020). Purpose: Despite sporadic case reports describing hearing problems in patients with coronavirus disease 2019 (COVID-19), whether COVID-19 affects the audiovestibular system remains unclear. This study assessed the evolution of incidence of audiovestibular disorders during the pandemic COVID-19 period. Method: Three audiovestibular disorders namely, sudden sensorineural hearing loss (SSHL), autonomic dysfunction, and Meniere's disease (MD) were analyzed and compared from 2016 to 2020. Results: The annual new cases at our clinic comprised overall 2107, 1997, 1984, 2068, and 1829 from 2016 to 2020, respectively, and the respectively annual cases of SSHL were 54, 46, 42, 45 and 38. Accordingly, annual incidences of SSHL in relation to overall cases of audiovestibular disorders were 2.6%, 2.3%, 2.1%, 2.2% and 2.1% from 2016 to 2020, respectively, exhibiting a non-significant difference (p > 0.05). In contrast, incidence of autonomic dysfunction in the year 2020 was 15.3%, which revealed significantly higher than 8.5–13.1% from 2016 to 2019 (p < 0.001). Restated, the incidence of autonomic dysfunction in 2020 displayed a significantly higher percentage than the other 4 years. Conversely, the incidence of MD in 2020 was 9.8%, showing a significant decline compared with the other 4 years (12.6–15.6% from 2016 to 2019, p < 0.001), Conclusion: Evolution of incidence of audiovestibular disorders during the pandemic COVID-19 period revealed increase in the incidence of autonomic dysfunction and decrease in that of MD, while incidence of SSHL remained unchanged from 2016 to 2020. The aim of this study was to devise a novel test for assessing the cervico-ocular reflex (COR) system via head vibration (termed h-COR) or neck vibration (termed n-COR) method. Thirteen patients with complete loss of bilateral vestibulo-ocular reflex (VOR) showing oscillopsia were assigned to Group A, while 13 patients with bilateral VOR loss but no oscillopsia were Group B. Another 13 healthy elderly served as a control. The COR test was performed via modifying the ocular vestibular-evoked myogenic potential (oVEMP) test by tapping at the forehead with head rotation (h-COR) or at mid-dorsal neck with head straight (n-COR). Both h-COR and n-COR tests displayed similar cI–cII waveforms. None of the Group A or healthy elderly showed present h-COR, while 8% of Group A and 31% of the healthy elderly revealed present n-COR. In contrast, present h-COR and n-COR were elicited in 85 and 77% of Group B, respectively. Restated, significantly higher response rate of COR in Group B (without oscillopsia) than Group A (with oscillopsia) indicates that present COR is related to the alleviation of oscillopsia. Long-term noise exposure may damage the cochlea and endolymph resorption system, which induces episodic vertigo and/or fluctuating hearing loss in later years. This study adopted clinical symptoms, inner ear test battery, and/or magnetic resonance (MR) imaging to evaluate development of secondary endolymphatic hydrops (EH) in patients with noise-induced hearing loss (NIHL). Forty NIHL patients with secondary EH were assigned to Group A. Another 40 age-and sex-matched NIHL patients without EH were assigned to Group B. All patients underwent an inner ear test battery. MR imaging was performed when diagnosis of EH was equivocal via above testing. Group A had significantly higher mean hearing levels (MHLs) than Group B at 1000, 2000, 4000, and 8000 Hz. Both groups displayed a significantly declining sequence of abnormality rates of the inner ear test battery. Under receiver operating characteristic (ROC) curve analysis, the cutoff threshold at 4 kHz for predicting the presence of secondary EH in NIHL patients was 52 dBHL, with a sensitivity of 62% and a specificity of 69%. NIHL patients revealing a typical 4 kHz dip-type audiogram with dip threshold >52 dBHL may predict development of secondary EH. Background: Video gaming (VG) has since the 1980s become increasingly ubiquitous entertainment among the adolescents and young adults. Many young people expe-rienced dizzy spells, but not vertiginous episodes, after playing VG. Objectives: This study performed ocular vestibular-evoked myogenic potential (oVEMP) and cervical VEMP (cVEMP) tests in subjects before and after engaging on VG to investigate the effect of VG on the otolithic reflex system. Methods: Twenty subjects who frequently played VG (>3 days per week) for more than 10 years were assigned to the long-term group. Another 20 subjects with engaging on VG <3 days per week or <10 years were assigned to the short-term group. Each subject underwent baseline oVEMP and cVEMP tests first, followed by playing VG for 1 h. Then, all subjects underwent the same paradigm. The "VG-year" is defined as frequency of VG playing within 1 week (day/week) multiplied by total length of VG engagement (year). Results: Engagement on VG rarely affected the oVEMP responses for either short- or long-term players. In contrast, the response rates of cVEMP significantly declined from pre-VG period (80%) to post-VG period (58%) in the short-term group, but not in the long-term group. The cutoff value for predicting absent cVEMP in VG engagement is 21 VG-year. Conclusion: Short-term engagement on VG may result in temporary cVEMP loss, while permanent cVEMP loss could be identified in long-term VG players. The cutoff value for predicting absent cVEMP in VG engagement is 21 VG-year, indicating that damage to the sacculo-collic reflex system could be anticipated in a subject who has played VG at least 1 h per session, 7 days weekly for 3 years. The declining order of abnormality rates in inner ear test battery of patients with Meniere's disease (MD) is consistent with the decreasing sequence of prevalence of hydrops formation in temporal bone donors of MD. This study investigated inner ear deficits in MD patients with falls, without falls, and after sac operation to compare their inner ear deficits. Twenty MD patients with falls (Group A), 20 MD patients without falls (Group B), and 20 MD patients after sac operation (Group C) were enrolled. All patients underwent an inner ear test battery. Group A (with falls) revealed a declining sequence of abnormality rates running from audiometry (92%), cervical vestibular-evoked myogenic potential (cVEMP) test (71%), caloric test (42%) to ocular VEMP (oVEMP) test (38%). In contrast, Group B (without falls) displayed a different declining sequence of abnormality rates running from audiometry, cVEMP test, and oVEMP test to caloric test. Similarly, Group C also displayed the same declining sequence of inner ear deficits to Group A, indicating that Groups A and C may share a similar mechanism. Objectives: This article provides an overview of the causes and differential diagnosis of sudden deafness (SD) and sudden sensorineural hearing loss (SSHL). Design: Contemporary review. Study sample: This review is based on peer-reviewed articles published in those journals listed on journal of citation reports. Through the PubMed database of the US National Library of Medicine, Scopus, and Google Scholar using the keywords of "sudden deafness", "acute hearing loss", and "sudden sensorineural hearing loss", totally 1493 papers were considered and 166 relevant papers were selected. Results: Sensorineural hearing loss of sudden onset may be classified as primary SD and secondary SSHL. Proposed aetiologies of primary SD comprised viral infection, vascular insufficiency, autoimmune disorder and stress theory, while causes of secondary SSHL include neoplasm, stroke and irradiation. Conclusion: SD/SSHL is a syndrome that comprises various entities, and results from a variety of aetiologies. An inner ear test battery in SD/SSHL patients helps determine its aetiology, and provides comprehensive information on the affected territory. Identification of the cause and differential diagnosis of the different types of SD/SSHL may provide substantial benefits such as determining the prognosis, identifying associated risk factors and preventing further hearing loss. Aim: This study investigated the evolution of vestibular disorders in dizzy older adults from young‐old to middle‐old to oldest‐old individuals, which has been less examined in detail so far. Methods: A total of 3714 older patients with vertigo/dizziness in a university hospital were retrospectively reviewed and divided into three groups; namely, the young‐old group aged 65–74 years (n = 2307), the middle‐old group aged 75–84 years (n = 1176) and the oldest‐old group aged ≥85 years (n = 231). All patients underwent an inner ear test battery comprising audiometry, caloric test, cervical and ocular vestibular‐evoked myogenic potential tests, and foam posturography. Results: The ratios between peripheral and central vestibular disorders ranged from the young‐old (60:40%) and middle‐old (36:64%) to the oldest‐old (25:75%) groups. These results show a decreasing sequence in the prevalence of peripheral vestibular disorders (i.e. benign paroxysmal positional vertigo or Meniereʼs disease), and an increasing sequence in that of central vestibular disorders (i.e. vertebrobasilar artery insufficiency) from young‐old to middle‐old to oldest‐old patients. Furthermore, the mean Romberg quotients (value from eyes closed divided by that from eyes open) of the sway area on the foam pad were 1.99 ± 0.93, 2.10 ± 1.06 and 2.62 ± 1.78 in the young‐old, middle‐old and oldest‐old groups, respectively, showing a significant difference among them. Conclusions: Older patients with central vestibular disorders might be more prone to imbalance and falls than those with peripheral vestibular disorders, partly because patients in the latter group retain other reflex systems; that is, the visuomotor reflex to stabilize the gaze and maintain balance. Background: Elder Meniere's disease (MD) patients ultimately lose their vestibular function. Objective: This study utilized an inner ear test battery to investigate evolution of MD. Methods: Total 278 elder MD patients aged >65 years were divided into three groups. Ninety-four patients with bilateral MD (188 ears) were assigned to Group A. The remaining 184 patients with unilateral MD were further divided into two groups. Group B consisted of 20 affected ears with normal vestibular function on the opposite ears, while Group C indicated 184 unaffected ears. All patients underwent an inner ear test battery. Results: Inner ear deficits in Group B declined from the cochlea via the saccule, utricle to semicircular canals. In contrast, Groups A and C did not significantly differ in the abnormality rates of cervical vestibular-evoked myogenic potential (cVEMP), ocular VEMP and caloric tests, indicating that Group C (unaffected ears) may partly share the same mechanism like Group A (affected ears), namely aging and hydropic effects. Conclusion and significance: Evolution of MD may progress from unilateral MD (MD 1.0), via unilateral MD coupled with asymptomatic hydrops on opposite ear (MD 1.5), toward bilateral MD (MD 2.0), where the number 1.0-2.0 means the number of clinically affected ears. Background: Otomycosis still remains intractable in clinical practice, likely because topical antifungal agents lack efficacy or are potentially toxic to the inner ear end organs.Objectives: The aim of this study was to investigate whether terbinafine solution is a potential candidate for treating intractable otomycosis in humans. In addition, the toxic effect on the inner ear was also assessed by animal models treated with terbinafine.Methods: Guinea pigs were instilled with 0.1 mL terbinafine (10 and 25 mg/mL) in the left round window membrane. At 2 weeks after treatment, all animals underwent an inner ear test battery and were then sacrificed for morphological study. Clinically, 20 patients with otomycosis were treated with terbinafine solution at a dosage of 0.4 mg.Results: All terbinafine-treated animals showed intact inner ear function when total dosage of terbinafine was <2.5 mg, which was further confirmed by morphological study. Subsidence of otomycosis was achieved in all 20 patients 1 week after treatment with terbinafine (0.4 mg) without untoward effect. No evidence of recurrence was noted 1 year after treatment.Conclusion: The paucity of inner ear toxicity of terbinafine even at a dosage of 2.5 mg was identified in guinea pig models morphologically and physiologically. Abstract Objective This study compared the prevalence of sudden deafness (SD) in children to investigate the evolution of pediatric SD during the past two decades. Methods From 1996 to 2005, totaling 358 SD patients were experienced. Of them, 25 patients (7%) aged <15 years were assigned to Group A. In contrast, 5 patients (2%) aged <15 years of 242 SD patients encountered during the period 2006–2015 were assigned to Group B. All patients underwent audiovestibular function testing. Results The measles-mumps-rubella (MMR) vaccination reached to 95% vaccination rate in Taiwan after 1994. As 1994 (MMR vaccination years) + 11 (mean age of pediatric SD) equals 2005, this study found that declining prevalence of pediatric SD was from 7% (1996–2005) to 2% (2006–2015) at our hospital, consistent with declining annual cases of SD in Taiwan during the past decades. A significantly higher abnormality rate of mean hearing level (93%) than abnormal caloric responses (20%) was identified indicating that pediatric SD predominately affected the cochlear partition. Conclusion The prevalence of pediatric SD cases has significantly declined during the past 20 years, probably due to global vaccination policy. The article presents the recording of bilateral cervical and ocular vestibular, that evoked myogenic potentials from the stimulation by forehead bone-conducted vibration. It offers information on vestibular stimuli, including air-conducted sound (ACS), bone-conducted vibration (BCV), and galvanic vestibular stimulation (GVS), that show vestibular evoked myogenic potential (VEMP).

Objective: This study revisited seven patients with Lermoyez syndrome over the past 25 years using an inner ear test battery to elucidate its mechanism. Methods: From 1992 to 2017, we have experienced 4096 patients with Meniere's disease (MD) and seven patients (5 males and 2 females, 8 ears) with Lermoyez syndrome. Two of the Lermoyez patients were elderly, aged 71 and 85 years. An inner ear test battery comprising audiometry, ocular and cervical vestibular-evoked myogenic potential (oVEMP and cVEMP) tests, and caloric test were performed. Results: Significant improvement of mean hearing levels (MHLs) was identified at low and middle frequencies after vertiginous attack, but not at high frequencies. Inner ear deficits in Lermoyez patients ran from abnormal hearing (100%) to abnormal cVEMP (43%), caloric (38%) and oVEMP (0) tests, exhibiting a significantly declining sequence. This declining sequence differed from that in MD. Conclusions: Lermoyez syndrome is extremely rare, with prevalence relative to MD of 0.2%. The mechanism is considered as blockage in the ductus reuniens caused by dislodged saccular otoconia. Aging and trauma are two precipitating factors for the dislodged saccular otoconia, which may explain why Lermoyez syndrome occurs most frequently in males and some elderly. [ABSTRACT FROM AUTHOR]

Firecrackers are still popular among the general public of various populations worldwide. This study investigated inner ear damage in patients with firecracker trauma and analyzed noise levels in 6 kinds of commercially available firecrackers. During the past 20 years, we have experienced 30 patients with firecracker trauma. An inner ear test battery comprising audiometry, cervical and ocular vestibular-evoked myogenic potential (cVEMP and oVEMP) tests and a caloric test was performed. The real-time noise levels were measured outdoors at a distance of 2, 4 and 6 m away from a lighting firecracker to mimic a noise event. Mean hearing levels at high frequencies (4,000 and 8,000 Hz) were significantly higher than those at the low and middle frequencies, indicating that firecrackers mostly cause high-tone hearing loss. For the vestibular damage, abnormality percentages were higher in the results of cVEMP (80%) and oVEMP (60%) tests, but not in the caloric test (8%). In conclusion, most firecrackers exhibited noise levels > 110 dB SPL even at a distance of 6 m. This blast injury simultaneously damaged the cochlea, saccule and utricle, but spared the semicircular canals, indicating that blast exposure potentiates the adverse effect of noise exposure on both cochlear and vestibular partitions. Objective: This study reviewed the development of the inner ear test battery comprising auditory brainstem response (ABR), and caloric, ocular vestibular-evoked myogenic potential (oVEMP), and cervical vestibular-evoked myogenic potential (cVEMP) tests in guinea pig models at our laboratory over the last 20 years. Detailed description of the methodology for testing the small animals is also included. Methods: Inner ear disorders, i.e. ototoxicity, noise exposure, or perilymph fistula were established in guinea pig models first. One to four weeks after operation, each animal underwent ABR, oVEMP, cVEMP, and caloric tests. Then, animals were sacrificed for morphological study in the temporal bones. Results: Inner ear endorgans can be comprehensively evaluated in guinea pig models via an inner ear test battery, which provides thorough information on the cochlea, saccule, utricle, and semicircular canal function of guinea pigs. Coupled with morphological study in the temporal bones of the animals may help elucidate the mechanism of inner ear disorders in humans. Objective: This study reviewed our experience in treating sudden deafness in antepartum (pregnant) and postpartum women during the past 2 decades.Methods: From 1997 to 2016, we have recorded sudden deafness in 16 antepartum (mean age, 32 years) and 3 postpartum (mean age, 31 years) women. Sudden deafness occurred during the 1st, 2nd, and 3rd trimesters in 5, 4, and 7 antepartum women, respectively. In contrast, the mean interval between giving birth and symptom onset in the 3 postpartum women was 18 days. Each patient underwent an inner ear test battery.Results: In 8 antepartum women treated by dextran infusion, the outcome as regards hearing was improved in 7 patients (88%) and unchanged in 1 patient. In contrast, the other 8 antepartum women selected no treatment, and only 1 patient (12%) achieved hearing improvement, exhibiting a significantly better outcome when receiving dextran treatment. For the postpartum women, 2 patients had hearing improvement when treated by antioxidants, while 1 patient retained unchanged hearing without treatment.Conclusion: Medication is needed in sudden deafness in antepartum or postpartum women rather than waiting for a natural course. Objective The staging system of Meniere’s disease utilizes audiograms to probe cochlear dysfunction. We explored the addition of galvanic vestibular-evoked myogenic potentials (VEMP) to further explore vestibular function. Methods Seventy patients with unilateral Meniere’s disease were enrolled in this study. Within 2 weeks of diagnosis, all subjects underwent pure tone audiometry, cervical and ocular VEMP, and caloric test. The prevalence of abnormal tests and the VEMP characteristic parameters such as latencies and amplitudes were analyzed. Results In affected ears, the abnormal rate of acoustic cVEMPs, galvanic cVEMPs, vibratory oVEMPs and galvanic oVEMPs was 37%, 17%, 20%, and 9%, respectively. No significant differences existed in VEMP latencies and amplitudes between affected ears and unaffected ears. Conclusions The impairment of otolithic organs was found to be more than that of vestibular afferents. The deterioration of the saccule was more than that of the utricle, whereas retrolabyrinthine degeneration of sacculo-collic reflex and vestibulo-ocular reflex was similar. Objective This study investigated the effect of betel nut chewing on the otolithic reflex system. Methods Seventeen healthy volunteers without any experience of chewing betel nut (fresh chewers) and 17 habitual chewers underwent vital sign measurements, ocular vestibular-evoked myogenic potential (oVEMP), and cervical VEMP (cVEMP) tests prior to the study. Each subject then chewed two pieces of betel nut for 2 min (dosing). The same paradigm was repeated immediately, 10 min, and 20 min after chewing. On a different day, 10 fresh chewers masticated chewing gum as control. Results Fresh chewers exhibited significantly decreased response rates of oVEMP (53%) and cVEMP (71%) after dosing compared with those from the predosing period. These abnormal VEMPs returned to normal 20 min after dosing. In contrast, 100% response rates of oVEMP and cVEMP were observed before and after masticating chewing gum. In habitual chewers, the response rates of oVEMP and cVEMP were 32% and 29%, respectively, 20 min after dosing. Conclusion Chewing betel nuts induced a transient loss of the otolithic reflexes in fresh chewers but may cause permanent loss in habitual chewers. Objective:This study assesses the effect ofN-acetyl-L-cysteine (NAC) as a single therapy for sudden deafness. Methods:Thirty-five sudden deafness patients with neither systemic disorders nor central signs in electronystagmography were treated with NAC alone and assigned to Group A. For comparison, another 35 sudden deafness patients treated by corticosteroids and plasma expander were assigned to Group B. There were no significant differences between the two groups in terms of age, sex, laterality, and pre-treatment mean hearing level. This study adopted an inner ear test battery in adults with congenital hearing loss to assess residual vestibular function. Thirty patients with non-inherited non-syndromatic congenital hearing loss were assigned to two groups based on their pure tone average (PTA). Subjects in Group A ( n = 10) had PTA >90 dB, and those with PTA ≤90 dB were assigned to Group B ( n = 20). All patients underwent audiometry, and ocular vestibular-evoked myogenic potential (oVEMP), cervical VEMP (cVEMP) and caloric tests. Percentages of abnormal PTA, and cVEMP, oVEMP, and caloric test results showed a significantly sequential decline in inner ear deficits from the cochlea to the saccule, utricle, and semicircular canals. Group A had significantly higher percentages of absent oVEMP and caloric areflexia than Group B. Via receiver operating characteristic curve analysis, the cutoff value of PTA was 65 dB for discriminating between present and absent oVEMP/caloric responses, with a sensitivity of 71 % and a specificity of 88 %. In conclusion, congenitally deaf patients with PTA ≥65 dB may retain less vestibular function than those with PTA <65 dB, as evidenced by higher percentages of absent oVEMP and caloric areflexia. Purpose: This study adopted the cervical and ocular vestibular-evoked myogenic potential (cVEMP and oVEMP) tests in Meniere's disease (MD) patients to correlate them with vestibular endolymphatic hydrops (EH) on MR images. Methods: A total of 25 patients with unilateral definite MD identified by positive cochlear hydrops on MR images were enrolled. All patients underwent audiometry, cVEMP test and oVEMP test, followed by MR imaging for confirmation. Results: A significantly declining sequence of abnormality rates in MD patients was identified from the audiometry (92%), cVEMP test (52%) to the oVEMP test (40%), which was consistent with a significantly decreasing order of prevalence of EH on MR images running from the cochlea (100%), saccule (56%) to the utricle (52%). The cVEMP test for detecting the saccular hydrops revealed a sensitivity of 62%, while the oVEMP test for assessing the utricular hydrops showed a sensitivity of 70%. However, correlating VEMP results with vestibular hydrops did not show any significant relationship. In addition, mean hearing level (MHL) at four frequencies (500, 1000, 2000, and 3000 Hz) of Grade I cochlear hydrops (51 ± 19 dB) did not significantly differ from Grade II cochlear hydrops (53 ± 19 dB). Conclusion: Limitations of the updated MR imaging for visualizing the hydrops comprised: (1) failure to correlate vestibular hydrops with VEMP results, and (2) failure to correlate grade of cochlear hydrops with MHL. Objective This study adopted an inner ear test battery and MR imaging in patients with bilateral sudden sensorineural hearing loss (SSNHL) to investigate their causes, disease extent, and evaluate hearing outcome. Patients and methods From 1995 to 2014, 16 patients with bilateral SSNHL received audiometry, caloric test and MR imaging. Vestibular-evoked myogenic potential (VEMP) test was added to the test battery after 2000. Results Percentages of abnormal mean hearing level (MHL), cervical VEMP test, ocular VEMP test, and caloric test in patients with bilateral SSNHL were 100% (32/32), 100% (12/12), 100% (4/4), and 81% (26/32), respectively, implying that not only the cochlear part but also the vestibular part was severely affected in both ears. Causes of bilateral SSNHL were neoplasm in 5 patients, stroke in 5, meningitis in 1, and unknown in 5. Post-treatment MHL did not significantly differ from pre-treatment MHL indicating poor hearing outcome. Seven patients (44%) had passed away within 5 years after onset, 2 patients were lost, and 7 patients survived. Conclusion Via MR imaging, causes of bilateral SSNHL were identified for 69% of cases. Both cochlear and vestibular endorgans/afferents were identified to be severely affected bilaterally by the vestibular test battery and resulted in poor hearing outcome. This paper correlated the Meniere attack with meteorological parameters i.e. atmospheric pressure, temperature, relative humidity and rainfall, to investigate which parameters that trigger the Meniere attack. During the past three years, totally 283 (2015), 351 (2016) and 319 (2017) patients with Meniere's disease (MD) were encountered at a clinic of the university hospital, accounting for 13%, 17% and 16% incidence in relation to overall annual neurotological cases, respectively. The onset of Meniere attack was then correlated with meteorological parameters. The mean seasonal incidence of Meniere attack in relation to overall seasonal neurotological cases in 2015-2017 were 14.8 ± 2.8% (spring), 17.5 ± 2.2% (summer), 16.0 ± 1.8 % (autumn) and 12.8 ± 2.0% (winter), indicating that summer season had a higher incidence of Meniere attacks than winter season. Onset of Meniere attacks correlated significantly with the atmospheric pressure (r = -0.4484, p = 0.0061) and temperature (r = 0.4736, p = 0.0035), and the atmospheric pressure was highly negatively correlated with the temperature (r = -0.9421, p < 0.0001). In contrast, no correlation was identified between the onset of Meniere attacks and relative humidity or rainfall. The atmospheric pressure in the same month with typhoon compared with that without typhoon revealed a median reduction of 13.1 hectopascal. Atmospheric pressure and temperature are correlated with the onset of Meniere attack. Summer season has a higher incidence of Meniere attack than winter season, likely because low atmospheric pressure in summer may aggravate endolymphatic hydrops, especially when accompanied by typhoons in the northwest Pacific region. Objectives Although the caloric test, rotational test, and posturography have been used to investigate balance function conventionally, and they are older than tests of otolithic organs, yet it seems that most clinicians are less familiar with the development of otolithic (saccular and utricular) function in children. This study reviewed the electrophysiological testing used to assess the functional development of the otolithic system in growing children. Methods Based on the literature, studies of cervical vestibular-evoked myogenic potential (cVEMP) and ocular VEMP (oVEMP) tests in children ranging from newborns, small children to adolescents were reviewed. Papers concerning foam posturography in children were also included. Results The cVEMPs can be elicited in newborns at day 5, whereas the oVEMPs are absent in neonatal period. When children grow to 2 years old, the oVEMPs can be induced with eyes closed condition, while the oVEMPs with eyes up condition can be elicited in children aged >3 years old, with the characteristic parameters similar to adult levels. In contrast with cVEMPs, it is until the neck length >15.3 cm (aldolesence), one need not account for neck length in evaluating cVEMP latency. Additionally, foam posturography indicated by the Romberg quotient of the sway velocity/area on foam pad is considered to reflect the otolithic function, which reached adult levels when the children at 12 years old. Conclusion: An inner ear test battery may suggest bilateral involvement and aid in 'grading' of potential disease in a patient with actively symptomatic Meniere's disease (MD). Objectives: This study applied an inner ear test battery in patients with bilateral MD to map the inner ear deficits in each ear. Methods: From 2009 to 2012, 100 (20%) of 498 MD patients were diagnosed with bilateral involvement, which was defined as established MD in one ear, and the opposite ear had inner ear symptoms combined with documented hearing loss. Each patient underwent audiometry, and ocular vestibular-evoked myogenic potential (oVEMP), cervical VEMP (cVEMP), and caloric tests. Results: Grading of inner ear deficits was based on the number of abnormal results in the inner ear test battery. Of 100 patients with bilateral MD, 54% had the same grade and 46% had different grades in their 2 ears. On the other hand, based on four-tone average, 79% had the same Meniere stage and 21% had different stages in their two ears. The difference between ears with asymmetric grades (46%) and ears with asymmetric stages (21%) accounts for 25% of bilateral MD patients, which may be caused by the saccular hydrops in the opposite ear. Objective This study adopted an inner ear test battery comprising audiometry, caloric test, ocular vestibular-evoked myogenic potential (oVEMP) test and cervical VEMP (cVEMP) test to find the factors related to the occurrence of vertigo in patients with otosclerosis. Methods Fifty otosclerosis patients comprising 27 patients with vertigo (Group A) and 23 patients without vertigo/dizziness (Group B) were enrolled. Each patient underwent otoscopy, image study, audiometry, caloric test, and oVEMP and cVEMP tests via bone vibration stimuli. Results The sequence of inner ear deficits in Group A was in the order from oVEMP test (84%), cVEMP test (51%), caloric test (38%) and mean bone-conducted (BC) hearing threshold (14%), exhibiting a significantly declining trend ( p < 0.001). The rate of inner ear dysfunction in Group B also declined significantly but in a different order – cVEMP test (55%), oVEMP test (52%), mean BC hearing threshold (33%), and caloric test (18%). Comparison between the two groups revealed a significant difference in the oVEMP test results ( p < 0.01), but not in the results of the BC hearing threshold, caloric test or cVEMP test ( p > 0.05). Further, no significant differences existed between the BC hearing threshold and vestibular function test results. Conclusion Otosclerosis patients with vertigo have more frequent abnormalities of oVEMPs to impulsive stimulation than do those without, consistent with more frequent abnormalities of the utricle. Increasing numbers of acute sensorineural hearing loss (SNHL) are recently experienced in irradiated nasopharyngeal carcinoma (NPC) survivors. This study adopted MR imaging to differentiate between post-irradiation sudden deafness (PISD) and post-irradiation endolymphatic hydrops (PIEH) in long-term NPC survivors with acute SNHL. From 2012 to 2021, consecutive 10 irradiated NPC survivors with acute SNHL were enrolled. All patients underwent an inner ear test battery and MR imaging using HYDROPS-Mi2 technique. Six patients (11 ears) with positive cochlear hydrops on MR images were diagnosed as PIEH, while another 4 patients (4 ears) without cochlear hydrops on MR images were referred to PISD. The interval from the onset of NPC to acute SNHL did not significantly differ between the PIEH (10 ± 6 years) and PISD (8 ± 2 years). No significant difference was found between the two disorders from any of the symptomatic, radiotherapeutic, audiological, or vestibular perspective. Interestingly, most (5/6) patients with PIEH had bilateral involvement, while all (4/4) patients with PISD showed unilateral affliction. A significantly declining sequence of abnormality rates in the inner ear test battery was noted in the PIEH patients, running from the audiometry (100%), cervical vestibular-evoked myogenic potential (VEMP) test (100%), ocular VEMP test (73%), to the caloric test (36%). However, such declining trend was not observed in patients with PISD. Conclusion: The ocular vestibular-evoked myogenic potential (oVEMP) test yields the same information as the rotational test in sleep-deprived subjects as evidenced by increased vestibulo-ocular reflex (VOR) asymmetry. However, the duration of sleep deprivation (12 h) and testing time required for the oVEMP test are shorter than for the rotational test. Objective: This study utilized the oVEMP test in sleep-deprived subjects to investigate the effect of short-duration sleep deprivation on the VOR system. Methods: Twenty healthy resident physicians underwent oVEMP and cervical VEMP (cVEMP) tests twice in a randomized order; one test battery was performed after a normal sleep, and the other was performed after a night duty at the emergency service, which meant a state of 12 h sleep deprivation. Results: All 20 subjects had clear oVEMPs regardless of whether testing was performed after a normal sleep or 12 h sleep deprivation condition. Significant differences were not identified between the two sleep conditions in terms of characteristic parameters, i.e. latencies and amplitude of oVEMPs. However, the mean asymmetry ratio after sleep deprivation (20 ± 13%) was significantly larger than that after normal sleep (8 ± 12%). Conclusion: Although the affected territory in the posterior/anterior inferior cerebellar artery (PICA/AICA) infarction could not be meticulously demonstrated by magnetic resonance imaging (MRI), it could be picked up by the results of a vestibular test battery comprising caloric, ocular vestibular evoked myogenic potential (oVEMP), and cervical VEMP (cVEMP) tests. Objectives: This study applied audiometry and caloric, oVEMP, and cVEMP tests to map affected territory in patients with PICA/AICA infarction. Methods: Fourteen patients, including 11 with PICA infarction and 3 with AICA infarction, were enrolled in this study during the last 8 years. Each patient underwent audiometry, caloric test, oVEMP test, and cVEMP test. Results: In the PICA group, 8 (36%) of 22 ears had a mean hearing level >25 dB. All six ears (100%) in the AICA group had abnormal hearing, and thus both groups revealed a significant difference. Conversely, significant differences were not observed in the vestibular test battery between the PICA and AICA groups. MRI demonstrated infarction at the brainstem for six patients, while one patient also had cerebellar involvement, indicated by loss of visual suppression on caloric nystagmus. Conclusion: Simultaneous galvanic vestibular stimulation (GVS)-cervical vestibular-evoked myogenic potential (cVEMP) and GVS-ocular (oVEMP) tests yielded similar information to that obtained in individual tests. Objective: This study compared the characteristic parameters of cVEMPs and oVEMPs via GVS between individual and simultaneous recording patterns in healthy and elderly subjects. Conclusion: Close observation of pathological eye movements such as disconjugate eye movements, multi-directional gaze nystagmus, and persistent unilateral gaze nystagmus may facilitate the effort of clinicians to arrange magnetic resonance imaging (MRI) study, because physical examinations may overlook the posterior fossa lesions. Objective: This paper reviews our experience of patients with posterior fossa stroke via observation of pathological eye movements over the past 10 years. Methods: Seventy patients with posterior fossa stroke manifested as acute vertiginous attack were admitted. All patients underwent examination of eye movements, MRI, and a battery of audiovestibular function tests. Results: Of the 70 patients, 22 (31%) demonstrated pathological eye movements including persistent (>24 h) unilateral gaze nystagmus in 12 patients, and multi-directional gaze nystagmus in 10 patients. Conjugate eyes movements were identified in 18 patients, and disconjugate eye movements were shown in 4 patients including medial longitudinal fasciculus syndrome in 1, paramedian pontine reticular formation syndrome in 1, and one and a half syndrome in 2. The vestibular test battery revealed abnormal responses for >85% of the patients in each test. This study utilized audiometry, and cervical vestibular-evoked myogenic potential (cVEMP), ocular VEMP (oVEMP) and caloric tests to investigate the sequence of vestibular deficits in patients with noise-induced hearing loss (NIHL). Thirty patients with NIHL underwent an inner ear test battery. Another 30 normal controls with age- and sex-matched were included for comparison. The abnormal percentages of the audiometry, and cVEMP, oVEMP and caloric tests were 100, 70, 57 and 33 % in NIHL patients, which showed significant differences from 13, 13, 7 and 3 % in normal controls, respectively. Conclusion: The ocular vestibular-evoked myogenic potential (oVEMP) test with eyes up condition may help differentiate facial paresis with and without vestibular nerve deficit, while the oVEMP test with the eyes closed condition is not reliable for testing patients with facial paresis. Objective: This study investigated the influence of facial paresis on oVEMPs via the eyes up versus eyes closed conditions. Methods: Twenty patients with unilateral facial paresis underwent a battery of audiovestibular function tests. The oVEMP test was performed via bone-conducted vibration stimuli with the eyes up and eyes closed conditions used in a randomized order. Results: Under the eyes up condition, the rate of abnormal oVEMP tests in the lesion ear was 30%, significantly higher than the 0% in the opposite healthy ear, indicating that the lesion ear not the recorded eye is responsible for abnormal oVEMPs. However, abnormal oVEMP test results of lesion ears under the eyes closed condition did not significantly differ from those of opposite healthy ears. Conclusion: The age-related changes in ocular vestibular-evoked myogenic potentials (oVEMPs) elicited by galvanic vestibular stimulation (GVS) and bone-conducted vibration (BCV) might be attributed to the morphological degeneration of the vestibular system. Objective: This study employed GVS and BCV modes for eliciting oVEMPs in healthy subjects to explore the effect of aging on the vestibulo-ocular reflex (VOR) pathway. Methods: Sixty-nine healthy subjects (aged 22-69 years) were divided into 5 groups of 12-19 subjects by decades of age. All subjects underwent oVEMPs using GVS and BCV modes. The prevalence and parameters of oVEMPs, including nI latency, pI latency, nI-pI interval, and nI-pI amplitude were measured and compared. Results: The prevalences of GVS-oVEMPs had nonsignificant differences among all age groups, whereas that of BCV-oVEMPs in the over-60 group was significantly lower than those in the under-60 groups. In GVS-oVEMPs, the group over 60 years had significantly longer nI, pI latencies, and smaller amplitudes when compared with those under 60 years. In BCV-oVEMPs, the nI and pI latencies in the over-60 group were significantly longer than those of the under-60 groups, while the nI-pI amplitudes of groups over 50 years were significantly smaller than those of groups under 50 years. Another 20 definite MD patients with negative cochlear EH on MR images were assigned to Group B. All patients underwent an inner ear test battery followed by MR imaging using HRDROPS‐Mi2 technique. Results: The mean hearing levels (MHLs) at frequencies of 125, 250, 500 and 1000 Hz revealed significantly worse in Group A than Group B. Significantly deteriorated MHLs were noted from Grades 0 to II at low frequency (125, 250, and 500 Hz), but not at mid‐frequency and high frequency. The respective cutoff hearing thresholds at frequencies of 125, 250, and 500 Hz were 27.5, 32.5, and 40 dBHL, which help predict positive cochlear EH on MR images. By using the sum (27.5 + 32.5 + 40 = 100 dBHL) of cutoff thresholds from three low frequencies as a cutoff value, Group A (80%) showed significantly more ears with sum of low‐frequency hearing threshold >100 dBHL than Group B (30%). Conclusion: When sum of three low‐frequency (125, 250, and 500 Hz) hearing levels is >100 dBHL, positive cochlear EH may be shown on MR images in definite MD patients. In contrast, those MD patients with sum of three low‐frequency hearing levels <100 dBHL, MR imaging should be postponed because resolution of EH may cause negative MR images. Conclusion: Patients with recurrent sudden deafness demonstrating normal vestibular-evoked myogenic potentials (VEMPs) in the lesion ear of the second episode may indicate a good hearing outcome. Objective: This study retrospectively reviewed our experience of patients with recurrent sudden deafness during the past 20 years. Methods: Sixteen (1.4 %) of 1156 patients with sudden deafness were diagnosed as having a recurrent episode, including ipsilateral type in 7 and contralateral type in 9. All patients underwent an audiovestibular test battery and blood and MRI examinations. After 2000, nine patients underwent the VEMP test. Results: In the ipsilateral type, the mean interval between two episodes was 2 ± 2 years, which did not differ significantly from 3 ± 3 years in the contralateral type. There were no significant differences in relation to age at onset of the second episode, inter-episode interval, gender, presence of vertigo, and abnormal caloric results. However, abnormal rate of VEMP test in the contralateral type (five of five patients) was significantly higher than that in the ipsilateral type (none of four patients). At the second episode, all four patients with normal VEMPs on the lesion ear had improved hearing, while four of five patients with absent or delayed VEMPs showed unchanged hearing. Conclusion. The inner ear monitoring system is useful for identifying the affected branches of the vestibular nerve in cases of vestibular neuritis, providing insight about the interval for the relief of vertigo. Objective: This study conducted an inner ear monitoring system including audiometry, and caloric, ocular vestibular evoked myogenic potential (oVEMP), and cervical VEMP (cVEMP) tests in cases of vestibular neuritis for assessing the affected branches of the vestibular nerve. Methods: Twenty patients with vestibular neuritis underwent caloric, oVEMP, and cVEMP tests. Type I indicates that one of the three tests is abnormal; type II indicates that two test results are abnormal; and type III indicates that no test result is normal. Results: All patients had normal hearing, bilaterally. Nineteen (95%) of 20 patients had abnormal caloric responses, 11 patients (55%) had abnormal oVEMPs, and 5 patients (25%) had abnormal cVEMPs. Restated, of all 20 patients, 8 patients were classified as type I, 9 were type II, and 3 were type III. The mean intervals between presentation and relief of vertigo were significantly different among the three types. Abstract: Gentamicin is an effective and powerful antibiotic. Extended use or excessive dosages of which can result in irreversible damage to the inner ear. The development of otoprotective strategies is a primary and urgent goal in research of gentamicin ototoxicity. Ginkgo biloba leaves and their extracts are among the most widely used herbal products and/or dietary supplements in the world. We investigated the protection of EGb 761 (a standardized preparation of EGb) on gentamicin ototoxicity and the involvement of reactive oxygen species (ROS) and nitric oxide (NO)-related mechanisms using in vitro organ cultures and an in vivo animal model. Gentamicin induced hair cell damage in cochlear cultures that could be prevented by EGb 761. EGb 761 also significantly reduced gentamicin-induced ROS and NO production. Furthermore, EGb 761 inhibited cellular apoptosis in cultured cochleae treated with gentamicin. In guinea pigs with gentamicin application onto the round window membrane, the mean auditory brain stem response threshold, ratio of cochlear hair cell damage and apoptosis were significantly elevated compared with those in the control group, and this could be prevented by oral administration of EGb 761. Individual EGb 761 components quercetin, bilobalide, ginkgolide A and ginkgolide B, but not kaempferol, significantly prevented gentamicin-induced hair cell damage. These results indicate that EGb 761 has a protective effect against gentamicin ototoxicity through a reduction in the formation of ROS and NO and subsequent inhibition of hair cell apoptosis in the cochlea. Conclusion: The static subjective visual horizontal (SVH) test correlates with the dynamic ocular vestibular-evoked myogenic potential (oVEMP) test in healthy and pathological ears, and further confirms that both tests may, at least in part, share the same utricular reflex pathway. Objective: This study correlated the SVH test results with those of the oVEMP and cervical VEMP (cVEMP) tests to investigate their relationships. Methods: Twenty healthy subjects underwent the SVH test at a view pattern angle of 30° or 70° horizontal tilt under various background distractions to establish the optimal stimulation mode for SVH test. Thereafter, 20 patients with unilateral Meniere's disease underwent the SVH test using the optimal mode. In addition, oVEMP and cVEMP tests were performed in all subjects. Results: The preliminary study in 20 healthy subjects at a view pattern angle of 70° under counterclockwise square background distraction revealed that the mean deviation degree of the SVH test was -0.61 ± 1.17°. Based on the criteria, abnormal percentages of SVH in 20 Meniere's patients were 40%. All healthy subjects had normal oVEMPs and cVEMPs. In contrast, eight patients (40%) showed abnormal oVEMPs and nine (45%) revealed abnormal cVEMPs. The SVH test results correlated significantly with oVEMP results, but not with cVEMP results. Abstract: Previous studies have demonstrated that following intratympanic gentamicin application in the guinea pigs, vestibular evoked myogenic potentials (VEMPs) were absent regardless of stimulation mode using either air-conducted sound (ACS) stimuli or galvanic vestibular stimulation (GVS). Ultrastructurally, both type I hair cells and their calyx terminals were distorted in the saccular macula. However, little is known about the toxic effects of gentamicin on the vestibular ganglion (VG). In this study, absent ACS- and GVS-VEMPs were noted in all the gentamicin-treated ears (100%), which were confirmed by the substantial loss of sensory hair cells in the saccular macula. Moreover, dramatic up-regulation of growth associated protein-43 (GAP-43) expression was detected in the ipsilateral VG neurons. The mean percentage of substance P-like immunoreactive (SP-LI) neurons in the treated VG (81.8±1.9%) was significantly higher than that in the control VG (68.6±3.3%). Conversely, the mean percentage of neuropeptide Y-like immunoreactive (NPY-LI) neurons in the treated VG (13.7±3.8%) was dramatically lower than that in the control VG (49.0±3.8%). Double labeling results shown 82% of SP-LI and 16% of NPY-LI neurons coexpressed with GAP-43, suggested that SP accumulating coincided with NPY decreasing in regenerating VG neurons after gentamicin treatment. This longitudinal study investigated how chronic gunshot noise exposure affects cochlear and saccular function in police officers who engaged in regular target shooting practice using dual protection (ear plugs plus earmuffs) for >10 years. In 1997, 20 male police officers underwent audiometry before and two weeks after shooting. Twelve of the original subjects were re-examined by audiometry coupled with vestibular evoked myogenic potential (VEMP) test in 2007. Significant deterioration of mean hearing thresholds at frequencies of 500 Hz through 4000 Hz was noted ten years later, affecting both ears. However, only the frequencies of 4000 and 6000 Hz on the left ear revealed significant difference in mean hearing thresholds compared with healthy controls. Abnormal VEMP responses were evident in nine police officers (75%), including absent VEMPs 7 and delayed VEMPs 2. In conclusion, deterioration to hearing may occur after long term exposure to gunshots, even when double hearing protection is used. Summary: Objective: This study applied caloric and vestibular evoked myogenic potential (VEMP) tests to evaluate whether children with benign paroxysmal vertigo (BPV) shares similar vestibular features with basilar type migraine. Methods: Twenty subjects of each group including healthy and BPV children were enrolled in this study. Each subject underwent audiometry, caloric and VEMP tests. Diagnosis of BPV was based on the International Classification of Headache Disorder, second edition. Results: All BPV children had normal hearing, bilaterally. Caloric test revealed normal responses in 13 patients and abnormal responses in 7 patients (35%). VEMP test showed 50% abnormality in BPV children, including absent and delayed responses. However, when results of caloric and VEMP testing were considered together, 70% of BPV children had abnormality, exhibiting non-significant difference compared with 75% abnormality in basilar type migraine. Conclusions: A combination of caloric and VEMP results reveals higher abnormality (70%) in BPV children, sharing similar features with basilar type migraine in adults, given the similarity of the two disorders. By easily stimulating the ear with loud sound and recording on tonically contracted neck muscles, vestibular evoked myogenic potential (VEMP) test can reflect inner ear function other than the cochlea and semicircular canal. This expands the test battery for clinicians to explore saccular disease, adding a potential usefulness to the sacculo-collic reflex. The ideal stimulation mode for VEMPs is as follows: 95 dB tone bursts, frequency 500 Hz, stimulation repetition rate 5 Hz, rise/fall time 1 ms, plateau 2 ms, binaural stimulation with bilateral recordings. Animal model using guinea pigs has been established, which sets the stage for useful future studies investigating VEMPs in guinea pigs that would appear to resemble human VEMP responses. Conclusion . Excluding the possibility of acoustic neuroma, therapeutic benefits from dextran 40 can be anticipated in a pregnant woman with sudden deafness, leading to significant hearing improvement, without detrimental effect on the mother or fetus. Objective . The aim of this study was to present our experience in the treatment of pregnant women with sudden deafness during the past decade. Methods . From January 1994 to December 2003, 12 pregnant women with sudden deafness were consecutively encountered at the authors’ clinic. With the exception of one patient with acoustic neuroma who underwent craniotomy, six patients were treated with dextran 40 only, while the other five patients preferred no medication, and served as a control group. Each patient underwent audiometry before treatment and at 1 year postpartum. Results . In the dextran group, five (83%) of six patients had hearing improvement 1 year postpartum, compared with one (20%) of five patients in the control group showing hearing improvement; both groups revealed a significant difference. Those receiving intravenous dextran treatment had significant hearing improvement at the frequencies of 1000, 2000, and 4000 Hz, at 1 year postpartum. Conclusions . Most patients with Ménière's disease (MD) reveal abnormal vestibular-evoked myogenic potentials (VEMPs) and the recruitment phenomenon, whereas most sudden deafness patients display normal VEMPs without the recruitment phenomenon. We therefore recommend using both the recruitment phenomenon and VEMP testing as a diagnostic algorithm to differentiate between MD and sudden deafness as the cause of acute hearing loss. Objective . Between January 2002 and December 2003, 14 consecutive patients with “probable” MD who also had acute sensorineural hearing loss were enrolled in the study, together with another 14 age- and sex-matched patients with idiopathic sudden deafness. Each patient underwent a battery of audiovestibular function tests, including pure-tone audiometry, a stapedial reflex test, distortion-product otoacoustic emissions (DPOAEs), electronystagmography and a VEMP test. All patients were followed for at least 12 months after presentation. Results . Significant differences were found between MD and sudden deafness in terms of the recruitment phenomenon (86% and 21% of cases, respectively) and abnormal VEMP responses (71% and 21% of cases, respectively).
