Remifentanil is useful in balanced anesthesia; however, there is concern regarding opioid-induced hyperalgesia. The effect of remifentanil on rebound pain, characterized by hyperalgesia after peripheral nerve block has rarely been studied. This study evaluated whether intraoperative remifentanil infusion may increase postoperative analgesic requirement in patients receiving preoperative interscalene brachial plexus block (IBP).Sixty-eight patients undergoing arthroscopic shoulder surgery under general anesthesia were randomly allocated to remifentanil (R) or control (C) group. Preoperative IBP with 0.5% ropivacaine 15 mL was performed in all patients. Intraoperative remifentanil was administered only in the R group. Postoperative pain was controlled using intravenous patient-controlled analgesia (IV-PCA) and rescue analgesics. The primary outcome was the dosage of fentanyl-nefopam IV-PCA infused over 24 h postoperatively. The secondary outcomes included the numeric rating scale (NRS) score recorded at 4-h intervals over 24 h, amount of rescue analgesics and total postoperative analgesics used over 24 h, occurrence of intraoperative hypotension, postoperative nausea and vomiting (PONV) and delirium.The dosage of fentanyl-nefopam IV-PCA was significantly less in C group than R group for postoperative 24 h. Fentanyl 101 [63-158] (median [interquartile range]) µg was used in the C group, while fentanyl 161 [103-285] µg was used in the R group (median difference 64 µg, 95% CI 10-121 µg, P = 0.02). Nefopam 8.1 [5.0-12.6] mg was used in the C group, while nefopam 12.9 [8.2-22.8] mg was used in the R group (median difference 5.1 mg, 95% CI 0.8-9.7 mg, P = 0.02). The total analgesic consumption: the sum of PCA consumption and administered rescue analgesic dose, converted to morphine milligram equivalents, was higher in the R group than C group (median difference 10.9 mg, 95% CI 3.0-19.0 mg, P = 0.01). The average NRS score, the incidence of PONV and delirium, were similar in both groups. The incidence of intraoperative hypotension was higher in R group than C group (47.1% vs. 20.6%, P = 0.005).Remifentanil administration during arthroscopic shoulder surgery in patients undergoing preoperative IBP increased postoperative analgesic consumption.

BACKGROUND: The precise underlying mechanism of antioxidant effects of dexmedetomidine-induced neuroprotection against cerebral ischemia has not yet been fully elucidated. Activation of Nuclear factor erythroid 2-related factor (Nrf2) and Heme Oxygenase-1 (HO-1) represents a major antioxidant-defense mechanism. Therefore, we determined whether dexmedetomidine increases Nrf2/HO-1 expression after global transient cerebral ischemia and assessed the involvement of Protein Kinase C (PKC) in the dexmedetomidine-related antioxidant mechanism. Objective Social Network Site Use Motives Scale (SUMS) was developed under the assumption that it consists of six factors, but only four factors were extracted as a result of Exploratory Factor Analysis. The goal of the present study was to investigate whether SUMS consists of four or six factors using Confirmatory Factor Analysis (CFA) and Exploratory Structural Equation Modeling (ESEM) approach.Methods A Korean college student sample (n=600; mean age, 21 years; 58% female) filled out the SUMS and the Social Network Site Addiction Proneness Scale. CFA and ESEM were used to assess the factor structure of the SUMS.Results Results indicated that a four-factor solution to the SUMS had inadequate fit in the sample examined using both CFA and ESEM and a six-factor solution to the SUMS had insufficient fit using CFA, whereas fit was optimal using ESEM for the six-factor model. In addition, the scale showed adequate convergent validity and reliability.Conclusion These findings support the six-factor model of SNS use motives and suggest that ESEM is a more appropriate method than CFA for examining the factor structure of the SUMS. The results displayed the usefulness of the ESEM framework in the investigation of use motives.

Background/Aim: A recent preclinical study reported that renal cell carcinoma was more susceptible to sevoflurane-mediated metastatic potentiation, compared to non-small cell lung cancer, suggesting that the effect of anesthetic agents on the metastatic potential varies according to cancer type. Based on this report, we conducted a retrospective cohort study to compare recurrence-free survival after nephrectomy, between renal cell carcinoma patients receiving volatile anesthesia and those receiving intravenous anesthesia. Patients and Methods: We reviewed the electronic medical records of patients who underwent partial or radical nephrectomy for renal cell carcinoma at the Seoul National University Hospital. Patients were divided into two groups according to whether volatile or intravenous anesthesia was used for nephrectomy. A total of 651 patients (582 in the volatile and 69 in the intravenous group) were enrolled in the study. Recurrence-free survival after nephrectomy was compared using Cox proportional hazards regression analysis with inverse probability of treatment weighting. Results: Cox regression analysis with inverse probability of treatment weighting revealed that volatile anesthesia had no impact on recurrence-free survival [hazard ratio (HR)=0.45; 95% confidence interval (CI)=0.07-2.85; p=0.398] or overall survival (HR=1.41; 95% CI=0.31-6.44; p=0.661). Conclusion: We found no significant association between volatile anesthesia and poor outcomes after nephrectomy for renal cell carcinoma. Volatile anesthetic-promoted metastatic potentiation of renal cell carcinoma, shown in a preclinical study, does not seem to be translated in the clinical setting.

Measurement of adolescent life satisfaction across cultures has not received much attention in previous empirical research. The present study evaluated measurement invariance of the Satisfaction with Life Scale (SWLS) among adolescents in 24 countries and regions (N = 22,710; age range = 13–19 years; 53% female). A single-factor model with residual covariance between a pair of items tapping past life satisfaction fitted well in 19 countries and regions and showed a partial metric invariance. In a subset of nine countries and regions, partial scalar invariance was supported. Partial metric invariance across all 24 countries and regions was achieved when custom model modifications in five countries and regions were included. Three SWLS items showed evidence of noninvariance across cultures. The measurement model was found to operate similarly across gender and age. Our findings suggest that caution is needed when using the SWLS for measuring life satisfaction among adolescents from different cultures. Local dimming technology enables high dynamic range liquid crystal displays (LCDs). However, when displaying a bright image in a dark background, so-called halo mura (halation caused by local light leakage in oblique viewing directions) appears and degrades image quality owing to reduction in contrast ratio (CR). Here light-absorbing vertically aligned dichroic dye film cells are demonstrated to enhance the CR in a homogenously aligned LC mode. The light leakage in normal and oblique viewing directions is reduced greatly in both simulated and experimental results. Positioning such a functional film above the conventional polarizer can greatly improve the CR of all LCDs while reducing surface reflection.

Objective The Mental Health Continuum-Short Form (MHC-SF) is widely used to measure positive mental health. This study is the first to examine the psychometric properties of the MHC-SF in persons with schizophrenia living in the community.Methods Two hundred thirty-one individuals with schizophrenia living in the community in South Korea filled out the MHC-SF. Confirmatory factor analysis (CFA), bifactor CFA, exploratory structural equation modeling (ESEM), and bifactor ESEM were undertaken to examine the factor structure of the MHC-SF.Results Results showed that a three-factor bifactor ESEM model yielded better fit than the other alternative models. Indices of internal consistency reliability were acceptable.Conclusion The results suggest that bifactor ESEM is an appropriate analysis for examining the factor structure of the MHC-SF in individuals with schizophrenia. The findings support the use of the overall MHC-SF scale rather than the use of the subscales in people with schizophrenia.

Binaural audio induces sedation and reduces pain and anxiety in surgical patients. This study tested the hypothesis that dexmedetomidine requirement for adequate sedation during spinal anesthesia would be lower in patients listening to music with binaural sound than that in patients listening to plain or no music.A triple-arm, assessor-blind, randomized controlled study.Operating room.One hundred and eighty-nine patients undergoing orthopedic surgery under spinal anesthesia.Patients were randomly assigned to music with binaural sound, plain music, or no music groups. Dexmedetomidine was infused for sedation during surgery. The loading infusion rate was 6 μg/kg predicted body weight (PBW)/h, followed by continuous infusion at 0.6 μg/kg PBW/hr. Loading was stopped after achieving adequate sedation, defined as the Observer's Assessment of Alertness/Sedation (OAA/S) scale score of 3. Infusion rate was adjusted every 30 min per the OAA/S scale.Primary outcomes were the difference in the dexmedetomidine loading dose adjusted for the patient's PBW between (1) the binaural and plain music groups and (2) the binaural and control groups. Secondary outcomes were the total dose and total loading time of dexmedetomidine; Patient State Index; relative powers of the alpha, theta, and delta bands; recovery from sedation; and patient satisfaction score.The final analyses included 184 patients. The PBW-adjusted dexmedetomidine loading dose was significantly lower in patients listening to music with binaural sound (1.15 ± 0.30 μg/kg PBW) than that in patients without music (1.33 ± 0.33 μg/kg PBW; mean difference, 0.18 μg/kg PBW; 95% confidence interval [CI], 0.06 to 0.29; P = 0.002). However, the difference was not statistically significant when compared with the plain music group (1.26 ± 0.36 μg/kg PBW; mean difference, 0.11 μg/kg PBW; 95% CI, -0.01 to 0.23; P = 0.070). Dexmedetomidine total dose, recovery from sedation, and patient satisfaction score showed no difference among the three groups.Compared with no music, music with binaural sound reduced the dexmedetomidine loading dose; however, this sedative-sparing effect of binaural sound was not found when compared to plain music.

The present research was carried out to clarify relationships between peer pressure, social networking site (SNS) addiction, and SNS-use motives in Korean adolescents. Adolescent SNS users ( N = 300, 52.70% female) completed self-report measures of peer pressure, SNS addiction, and SNS-use motives. Findings indicated that adolescents reporting more peer pressure had higher levels of SNS addiction. Moreover, the relationship between peer pressure and SNS addiction was mediated by coping and social-conformity use motives. Results are discussed in terms of implications for prevention and intervention, for adolescents facing peer pressure.

BACKGROUND Spinal anesthesia using a surface landmark-guided technique can be challenging in patients with anatomical alterations of the lumbar spine; however, it is unclear whether using ultrasonography can decrease the technical difficulties in these populations. We assessed whether an ultrasound-assisted technique could reduce the number of needle passes required for block success compared with the landmark-guided technique in patients with abnormal spinal anatomy. METHODS Forty-four patients with abnormal spinal anatomy including documented lumbar scoliosis and previous spinal surgery were randomized to receive either surface landmark-guided or preprocedural ultrasound-assisted spinal anesthesia. All spinal procedures were performed by 1 of 3 experienced anesthesiologists. The primary outcome was the number of needle passes required for successful dural puncture. Secondary outcomes included the success rate on the first pass, total procedure time, periprocedural pain scores, and the incidences of radicular pain, paresthesia, and bloody tap during the neuraxial procedure. Intergroup difference in the primary outcome was assessed for significance using Mann-Whitney U test. RESULTS The median (interquartile range [IQR; range]) number of needle passes was significantly lower in the ultrasound group than in the landmark group (ultrasound 1.5 [1-3 {1-5}]; landmark 6 [2-9.3 {1-15}]; P < .001). First-pass success was achieved in 11 (50.0%) and 2 (9.1%) patients in the ultrasound and landmark groups, respectively (P = .007). The total procedure time, defined as the sum of the time for identifying landmarks and performing spinal anesthesia, did not differ significantly between the 2 groups (ultrasound 141 seconds [115-181 seconds {101-336 seconds}]; landmark 146 seconds [90-295 seconds {53-404 seconds}]; P = .888). The ultrasound group showed lower periprocedural pain scores compared with the landmark group (ultrasound 3.5 [1-5 {0-7}]; landmark 5.5 [3-8 {0-9}]; P = .012). The incidences of complications during the procedure showed no significant differences between the 2 groups. CONCLUSIONS For anesthesiologists with experience in neuraxial ultrasonography, the use of ultrasound significantly reduces the technical difficulties of spinal anesthesia in patients with abnormal spinal anatomy compared with the landmark-guided technique. Our results can lead to practical suggestions that encourage the use of neuraxial ultrasonography for spinal anesthesia in such patients.

Objective Vestibular schwannomas (VSs) can either be solid or contain variable amounts of cystic components. It has been established that gamma knife radiosurgery (GKRS) is an effective treatment modality for solid VSs. However, in the case of cystic VS, given the somewhat unpredictable growth of the cystic portion of these tumors, microsurgery has often been preferred to GKRS in the management of cystic VS. However, to date, a relative paucity of data evaluating the outcomes after GKRS for cystic VS has been available. In the present study, we reviewed our experience treating cystic VSs with GKRS and evaluated the results. Methods The data from patients with a diagnosis of cystic VS who had undergone GKRS from 1999 to 2018 were analyzed. Only those patients who had undergone GKRS as the initial treatment of cystic VSs were included in the present study. Patients who had undergone previous surgical treatment were excluded. The cystic VSs were divided into intratumoral and peritumoral cysts. Results A total of 24 patients were enrolled in the present study. The mean age was 60.8 ± 2.4 years (range, 38–82 years). The mean follow-up period was 55.8 ± 8.1 months (range, 8–145 months). Of the 24 patients, 11 were men. The control rate after GKRS for cystic VSs was 75%. The 5-year progression-free rate was 70%. Six patients had presented with an increasing lesion size after GKS. Of these 6 patients, 2 underwent surgical treatment and 1 underwent repeat GKRS. No statistically significant differences were found between the control rate and cystic type (P = 0.093). Conclusion The results from the present study have demonstrated that GKRS is an effective treatment modality for cystic VSs.

Background: This study aimed at examining the relationships between optimism and life satisfaction in persons with schizophrenia living in the community. Method: In total, 108 persons with schizophrenia who live in the community, ranging in age from 20 to 67 years, completed measures of optimism, perceived stress and life satisfaction. Results: Results showed that optimism significantly predicted life satisfaction even after controlling for gender, age, onset age, employment and perceived stress. Conclusion: Findings add further support to the growing body of evidence implicating that the intervention for enhancing life satisfaction in persons with schizophrenia dwelling in the community could focus on increasing optimism.

Background Spondylolisthesis is a common degenerative spinal deformity. At the level of spondylolisthesis, the anatomy of the interlaminar space may differ from normal spine, in which case optimal angle of the needle insertion for spinal anesthesia may change. This study compared the optimal angle of needle insertion during spinal anesthesia in patients with and without lumbar spondylolisthesis using ultrasound. Methods We recruited 40 patients, 20 with and 20 without lumbar spondylolisthesis (group S and N, respectively). Ultrasonography was performed in the transverse midline and parasagittal oblique views at the spondylolisthesis level and the adjacent upper level. We measured the probe application angle with the longest interlaminar height of the ligamentum flavum-dura mater complex (LFD), depth from the skin to the LFD, depth from the skin to the anterior complex, and intrathecal space width. A positive angle represented a cephalad angulation. Results The optimal needle insertion angle in the transverse midline view at the spondylolisthesis level was (-) 2.7 ± 3.4° in group S and 0.8 ± 2.5° in group N (P$$ < 0.001). In the parasagittal oblique view, it was (-) 2.7 ± 4.5° in group S and 1.0 ± 3.2° in group N (P = 0.004). There were no between-group differences in the angles at the upper level, with all cephalad angles in both views. Other ultrasound image data were comparable between groups. Conclusion In patients with spondylolisthesis, caudad angulation of the spinal needle can aid successful spinal puncture at spondylolisthesis level, both in the midline and paramedian approaches. Neuraxial ultrasonography can improve the technical performance of spinal anesthesia. However, there are no data regarding the optimal approach for ultrasound-assisted spinal anesthesia. This study aimed to compare the midline and paramedian approaches for ultrasound-assisted spinal anesthesia in adult orthopedic patients.A single-center, prospective, randomized controlled trial.Operating room.One hundred and twelve patients undergoing orthopedic surgery were included.Patients were randomized to undergo either the midline or paramedian approach for preprocedural ultrasound-assisted spinal anesthesia.The primary outcome was the number of needle passes required for successful dural puncture. Secondary outcomes were the number of needle insertions, first pass/attempt success rate, procedural time, and the incidence of periprocedural complications.The median number of needle passes was significantly lower in the paramedian group (1 [IQR, 1-2]) than in the midline group (3 [2-6]; P 0.001). The paramedian approach showed higher first pass/attempt success rates compared with the midline group (58.9% vs. 21.4%; 91.1% vs. 53.6%; both, P 0.001). Total procedure times were significantly shorter in the paramedian group than in the midline group (113 [72.5-146.5] vs. 196 [138-298.5] seconds; P 0.001). The quality of sonographic images was graded as good in 94.6% of paramedian sagittal oblique views and 54.5% of transverse median views. No significant intergroup differences were found in the incidence of periprocedural complications.Compared with the midline approach, the ultrasound-assisted paramedian approach showed improved efficacy in terms of the number of needle manipulations, first pass success rates, and procedural time. These results suggest that the paramedian approach may be more efficient for preprocedural ultrasound-assisted spinal anesthesia.NCT03491943.

Nano-size distribution of birefringent liquid crystal droplets embedded in the polymer matrix gives rise to an optically isotropic liquid crystal (OILC) phase and the device utilizing OILC composite opens the possibility of alignment layer-free, flexible liquid crystal displays with fast response time. However, few critical issues such as high operating voltages and feeble light scattering are remained to be overcome before their widespread applications. To rectify the drawbacks, a small amount of conductive and anisotropic functionalized carbon nanotubes (f-CNTs) is doped into conventional OILCs. Consequently, switchable electro-optic properties of the proposed approach reveal that driving voltage is decreased by18.7%, response time becomes fast by 27%, and feeble light leakage is reduced to around 3.8 times, compared to conventional one, which helps the proposed approach to be competitive for display and photonic applications of film type of OILCs.

Liquid crystal displays (LCDs) for vehicle displays should exhibit a fast response time in wide temperature range and wide-viewing angle in horizontal and downward directions without grey-scale inversion but limited brightness in the upward direction because the display images can be reproduced in the front window glass of a vehicle, affecting driver’s front visibility. Currently, fringe-field switching (FFS) liquid crystal device is widely commercialised for high resolution and wide-viewing-angle LCD; however, it needs to improve response times and limit the display brightness in the upward direction. As a solution, we propose a homogeneously aligned liquid crystal device in which liquid crystal director does tilt as well as twist deformation in a confined area by both vertical- and fringe-electric fields, exhibiting about two times faster decay response time than that of conventional FFS mode with suppressed luminance in the upward direction. Although the use of fibreoptic guidance is recommended for tracheal intubation through supraglottic airway devices, it can also be performed in a blind manner. Based on the previous finding that a fibreoptic view of the vocal cords was better in the extended neck position than in the neutral position, we hypothesized that neck extension can better facilitate blind intubation through the Ambu® AuraGain™ laryngeal mask than the neutral position. Patients undergoing general anesthesia were randomly assigned to the extension group or the neutral group. After induction of anesthesia, the AuraGain™ was placed in the oropharynx, followed by blind intubation through the AuraGain™ in the assigned neck position within a maximum of two attempts. The primary outcome was successful blind intubation through the AuraGain™ in the first attempt. Of 168 adult patients screened, 124 patients were enrolled and 121 patients were included in the final analysis (extension group, n = 59; neutral group, n = 62). The incidence of successful blind intubation on the first attempt was significantly higher in the extension group than in the neutral group (68% vs. 47%, respectively; relative risk [RR], 1.45; 95% confidence interval [CI], 1.05 to 1.99; P = 0.02). The overall incidence of successful blind intubation was also significantly higher in the extension group than in the neutral group (71% vs 50%, respectively; RR, 1.42; 95% CI, 1.06 to 1.92; P = 0.02). The time required for successful blind intubation and the incidence of hoarseness, cough, or sore throat at 24 hr after extubation did not differ between groups. Neck extension can be used to facilitate blind intubation through the Ambu® AuraGain™ laryngeal mask. Considering the relatively high failure rate, blind intubation via the AuraGain™ should be used as an alternative, not as a first-line choice. www.ClinicalTrials.gov (NCT03408431); registered 24 January 2018.
