This study aimed to clarify the effects of gardening on hemodynamic response, rating of perceived exertion (RPE) during exercise, and body weight in patients in whom phase 2 cardiac rehabilitation (CR) was interrupted due to the Coronavirus disease 2019 (COVID-19) pandemic. Among 76 outpatients participating in consecutive phase 2 CR in both periods from March to April and June to July 2020, which were before and after CR interruption, respectively, at Sanda City Hospital were enrolled. The inclusion criterion was outpatients whose CR was interrupted due to COVID-19. Patients under the age of 65 were excluded. We compared the data of hemodynamic response and RPE during exercise on the last day before interruption and the first day after interruption when aerobic exercise was performed at the same exercise intensity in the gardener group and the non-gardener group. Forty-one patients were enrolled in the final analysis. After CR interruption, the gardener group did not show any significant difference in all items, whereas the non-gardener group experienced significant increase in HR (Peak) (p = 0.004) and worsening of the Borg scale scores for both dyspnea and lower extremity fatigue (p = 0.039 and p = 0.009, respectively). Older phase 2 CR patients engaged in gardening did not show any deterioration in hemodynamic response or RPE during exercise, despite CR interruption and refraining from going outside. Gardening may be recommended as one of the activities that can maintain or improve physical function in older phase 2 CR patients during the COVID-19 pandemic.

Context Adrenocorticotropin (ACTH) loading is used to increase the success rate of adrenal vein sampling (AVS). Objective We aimed to determine the effect of intraprocedural cortisol measurement (ICM) on ACTH-stimulated AVS (AS-AVS) owing to a lack of reliable data on this topic. Methods This multicenter, retrospective, observational study took place in 28 tertiary centers in Japan. Among 4057 patients enrolled, 2396 received both basal AVS (B-AVS) and AS-AVS and were divided into 2 groups according to whether ICM was used. The effect of ICM on AS-AVS was measured. Results In patients who underwent both AVS procedures, the ICM group had significantly higher success rates for both B-AVS and AS-AVS than the non-ICM group did. However, the probability of failure of AS-AVS after a successful B-AVS and the probability of success of AS-AVS after a failed B-AVS were not significantly different in the 2 groups. For subtype diagnosis, propensity-score matching revealed no significant difference between the 2 groups, and the discrepancy rate between B-AVS and AS-AVS for subtype diagnosis was also not significantly different. Conclusion ICM significantly increased the success rate of B-AVS and AS-AVS in protocols in which both AVS procedures were performed and had no effect on subtype diagnosis. However, in protocols in which both AVS procedures were performed, the results suggest ICM may not be necessary when performing AS-AVS if ICM is used only when B-AVS is performed. Our study suggests that ICM during AVS plays an important role and should be recommended.

Left ventricular hypertrophy (LVH) is often seen in patients with primary aldosteronism (PA), and the prevalence of LVH is reportedly higher among patients with PA than patients with essential hypertension. However, the correlation between aldosterone levels and LVH is undefined, and how aldosterone affects LVH in patients with PA remains unclear. We, therefore, retrospectively assessed a large PA database established by the multicenter JPAS (Japan Primary Aldosteronism Study) to reveal the factors associated with LVH in patients with PA without suspected autonomous cortisol secretion. In the 1186 patients with PA studied, the basal plasma aldosterone concentration, plasma renin activity, and the aldosterone-to-renin ratio did not significantly correlate with left ventricular LV mass index (LVMI) in single or multiple regression analyses. However, the plasma aldosterone concentration after the captopril challenge test or saline-infusion test, which are associated with autonomous aldosterone secretion, correlated significantly with LVMI, even after adjusting for patients’ backgrounds, including age and blood pressure. In addition, hypokalemia and the unilateral subtype also correlated with LVMI. We report on the detection of the [CII] 157.7 $\mu$m emission from the Lyman break galaxy (LBG) MACS0416_Y1 at z = 8.3113, by using the Atacama Large Millimeter/submillimeter Array (ALMA). The luminosity ratio of [OIII] 88 $\mu$m (from previous campaigns) to [CII] is 9.31 $\pm$ 2.6, indicative of hard interstellar radiation fields and/or a low covering fraction of photo-dissociation regions. The emission of [CII] is cospatial to the 850 $\mu$m dust emission (90 $\mu$m rest-frame, from previous campaigns), however the peak [CII] emission does not agree with the peak [OIII] emission, suggesting that the lines originate from different conditions in the interstellar medium. We fail to detect continuum emission at 1.5 mm (160 $\mu$m rest-frame) down to 18 $\mu$Jy (3$\sigma$). This nondetection places a strong limit on the dust spectrum, considering the 137 $\pm$ 26 $\mu$Jy continuum emission at 850 $\mu$m. This suggests an unusually warm dust component (T $>$ 80 K, 90% confidence limit), and/or a steep dust-emissivity index ($\beta_{\rm dust}$ $>$ 2), compared to galaxy-wide dust emission found at lower redshifts (typically T $\sim$ 30 - 50 K, $\beta_{\rm dust}$ $\sim$ 1 - 2). If such temperatures are common, this would reduce the required dust mass and relax the dust production problem at the highest redshifts. We present observations of [NII] 205 $\mu$m, [OIII] 88 $\mu$m and dust emission in a strongly-lensed, submillimeter galaxy (SMG) at $z=6.0$, G09.83808, with the Atacama Large Millimeter/submillimeter Array (ALMA). Both [NII] and [OIII] line emissions are detected at $>12\sigma$ in the 0.8$"$-resolution maps. Lens modeling indicates that the spatial distribution of the dust continuum emission is well characterized by a compact disk with an effective radius of 0.64$\pm$0.02 kpc and a high infrared surface brightness of $\Sigma_\mathrm{IR}=(1.8\pm0.3)\times10^{12}~L_\odot$ kpc$^{-2}$. This result supports that G09.83808 is the progenitors of compact quiescent galaxies at $z\sim4$, where the majority of its stars are expected to be formed through a strong and short burst of star formation. G09.83808 and other lensed SMGs show a decreasing trend of the [NII] line to infrared luminosity ratio with increasing continuum flux density ratio between 63 $\mu$m and 158 $\mu$m, as seen in local luminous infrared galaxies (LIRGs). The decreasing trend can be reproduced by photoionization models with increasing ionization parameters. Furthermore, by combining the [NII]/[OIII] luminosity ratio with far-infrared continuum flux density ratio in G09.83808, we infer that the gas phase metallicity is already $Z\approx 0.5-0.7~Z_\odot$. Objectives We investigated the clinical significance of ACTH stimulation during adrenal venous sampling (AVS) by surgical outcome of primary aldosteronism (PA). Design Multicenter retrospective study by Japan PA study. Method We allocated 314 patients with both basal and ACTH-stimulated AVS data who underwent adrenalectomy to three groups: basal lateralization index (LI) ≥2 with ACTH-stimulated LI ≥4 on the ipsilateral side (Unilateral (U) to U group, n = 245); basal LI n = 15); and basal LI ≥2 with ACTH-stimulated LI n = 54). We compared surgical outcomes among the groups using the Primary Aldosteronism Surgical Outcome (PASO) criteria. Results Compared with U to U group, U to B group had poor clinical and biochemical outcomes and low rates of adrenal adenoma as pathological findings (P = 0.044, 0.006, and 0.048, respectively), although there were no significant differences between U to U and B to U groups. All patients in U to B group with clinical and biochemical benefits, however, had adrenal adenoma as pathological findings and could be well differentiated from those with poor surgical outcomes via basal LI (>8.3), but not ACTH-stimulated LI. These results were similar even when we defined each group based on a cut-off value of 4 for basal LI. Conclusions Although PA patients in U to B group had worse surgical outcomes than did those in U to U group, basal LI could discriminate among patients with better surgical outcomes in U to B group.

We present ALMA detection of the [O iii] 88 μm line and 850 μm dust continuum emission in a Y-dropout Lyman break galaxy, MACS0416_Y1. The [O iii] detection confirms the object with a spectroscopic redshift to be z = 8.3118±0.0003. The 850 μm continuum intensity (0.14 mJy) implies a large dust mass on the order of 4×106M⊙. The ultraviolet-to-far infrared spectral energy distribution modeling, where the [O iii] emissivity model is incorporated, suggests the presence of a young (τage ≍ 4 Myr), star-forming (SFR ≍ 60M⊙yr−1), and moderately metal-polluted (Z ≍ 0.2Z⊙) stellar component with a stellar mass of 3 × 108M⊙. An analytic dust mass evolution model with a single episode of star formation does not reproduce the metallicity and dust mass in ≍ 4 Myr, suggesting an underlying evolved stellar component as the origin of the dust mass.

A fundamental quest of modern astronomy is to locate the earliest galaxies and study how they influenced the intergalactic medium a few hundred million years after the Big Bang. The abundance of star-forming galaxies is known to decline from redshifts of about 6 to 10, but a key question is the extent of star formation at even earlier times, corresponding to the period when the first galaxies might have emerged. Here we present spectroscopic observations of MACS1149-JD1, a gravitationally lensed galaxy observed when the Universe was less than four per cent of its present age. We detect an emission line of doubly ionized oxygen at a redshift of $9.1096\pm0.0006$, with an uncertainty of one standard deviation. This precisely determined redshift indicates that the red rest-frame optical colour arises from a dominant stellar component that formed about 250 million years after the Big Bang, corresponding to a redshift of about 15. Primary aldosteronism causes renal structural damage after glomerular hyperfiltration, and primary aldosteronism-specific treatment leads to an acute fall in estimated glomerular filtration rate (eGFR). We investigated whether this change affected the long-term eGFR slope in a retrospective cohort from the multicenter Japan Primary Aldosteronism Study. We allocated patients with primary aldosteronism to the adrenalectomy (n=202) and MR (mineralocorticoid receptor) antagonist (n=303) groups based on their treatment history and analyzed the association between the initial eGFR fall and long-term eGFR slope. The increased age, low serum potassium levels, high eGFR, and high plasma aldosterone levels were independent predictors for a large initial eGFR fall in both groups. Our analysis of tertiles based on the initial eGFR fall revealed that in the MR antagonist group, patients with a small initial eGFR fall had a significantly steeper long-term eGFR slope than those with a large initial fall (tertile 1 versus 2, P =0.025; tertile 1 versus 3, P =0.017). These associations were not identified in the adrenalectomy group. Thus, the smaller the acute fall in eGFR by initiation of MR antagonists, the greater was the rate of long-term eGFR decline. While the acute fall in eGFR induced by primary aldosteronism-specific treatment is occasionally a clinical concern, our findings highlight the favorable implications of the acute fall with respect to long-term renal outcomes.

Context Aldosterone-producing adenomas are a curable subtype of primary aldosteronism (PA); however, hypertension persists in some patients after adrenalectomy. Objective To identify factors associated with, and develop prediction models for, blood pressure (BP) normalization or improvement after adrenalectomy. Design Retrospective analysis of patients treated between 2006 and 2018, with a 6-month follow-up. Setting A nationwide, 29-center Japanese registry encompassing 15 university hospitals and 14 city hospitals. Patients We categorized 574 participants in the Japan Primary Aldosteronism Study, who were diagnosed with PA and underwent adrenalectomy, as BP normalized or improved, on the basis of their presentations at 6 months postsurgery. Main Outcome Measure The rate of complete, partial, and absent clinical success. Predictive factors related to BP outcomes after PA surgery were also evaluated. Results Complete clinical success was achieved in 32.6% and partial clinical success was achieved in 53.0% of the patients at 6 months postsurgery. The following five variables were independent predictors for BP normalization: ≤7 years of hypertension, body mass index ≤25 kg/m2, no more than one antihypertensive medication, absence of medical history of diabetes, and female sex. The area under the receiver operator characteristic curve was 0.797 in the BP normalization model. Conclusion We established models that predicted postoperative BP normalization in patients with PA. These should be useful for shared decision-making regarding adrenalectomy for PA.

Objectives Because of the influence on the renin-angiotensin-aldosterone system, it is recommended to avoid, if possible, the use of angiotensin-converting-enzyme inhibitors, angiotensin II type 1 receptor blockers, diuretics, β-blockers, and mineralocorticoid receptor antagonists during the diagnostic period of primary aldosteronism. A laterality index more than 4 in adrenocorticotropic hormone (ACTH)-stimulated adrenal venous sampling (ACTH-AVS) is a widely used classification of the unilateral subtype that can benefit from adrenalectomy. Here, we revealed clinical features of patients taking drugs that could affect the primary aldosteronism diagnosis (DAPD) and investigated whether the classification with laterality index more than 4 in ACTH-AVS is applicable to these patients. Patients and methods Using a large database of primary aldosteronism patients in Japan, we analyzed 2122 patients with successful ACTH-AVS. Results Patients who received any DAPD (n = 209) showed higher prevalence of comorbidity burdens and took more antihypertensive drugs compared with patients without DAPD. In patients taking DAPD, those with laterality index more than 4 had a higher prevalence of hypokalemia, a higher aldosterone-to-renin ratio and a higher prevalence of adrenal mass than those with laterality index of 4 or less. Adrenalectomy was performed in 76% patients with laterality index more than 4 and 20% with laterality index of 4 or less. Patients who underwent adrenalectomy showed biochemical cure in 89% with laterality index more than 4 and 50% with laterality index of 4 or less (P = 0.001). Multivariate regression analysis showed that laterality index more than 4 was an independent predictor of a biochemical cure. Biochemical cure rate in patients with laterality index more than 4 was consistently high, irrespective of the potential effect of individual DAPD on laterality index. Conclusion Our findings suggest that in primary aldosteronism patients to whom DAPD were administrated due to severe clinical features, laterality index more than 4 in ACTH-AVS could accurately predict a biochemical cure after adrenalectomy.

We present observations of a giant Lyman-alpha blob in the SSA22 proto-cluster at z=3.1, SSA22-LAB1, taken with the Atacama Large Millimeter/submillimeter Array (ALMA). Dust continuum, along with [C II]158um, and CO(4-3) line emission have been detected in LAB1, showing complex morphology and kinematics across a ~100 kpc central region. Seven galaxies at z=3.0987-3.1016 in the surroundings are identified in [C II] and dust continuum emission, with two of them potential companions or tidal structures associated with the most massive galaxies. Spatially resolved [C II] and infrared luminosity ratios for the widely distributed media (L[C II]/LIR~0.01-0.001) suggest that the observed extended interstellar media are likely to have originated from star-formation activity and the contribution from shocked gas is probably not dominant. LAB1 is found to harbour a total molecular gas mass Mmol=(8.7+/-2.0)e+10 Msun, concentrated in the core region of the Ly-alpha-emitting area. While (primarily obscured) star-formation activity in the LAB1 core is one of the most plausible power sources for the Ly-alpha emission, multiple major-mergers found in the core may also play a role in making LAB1 exceptionally bright and extended in Ly-alpha as a result of cooling radiation induced by gravitational interactions., 23 pages, 11 figures, accepted for publication in ApJ

We present new spectroscopic observations of Lyman-$\alpha$ (Ly$\alpha$) Blob 2 ($z \sim$ 3.1). We observed extended Ly$\alpha$ emission in three distinct regions, where the highest Ly$\alpha$ surface brightness (SB) center is far away from the known continuum sources. We searched through the MOSFIRE slits that cover the high Ly$\alpha$ SB regions, but were unable to detect any significant nebular emission near the highest SB center. We further mapped the flux ratio of the blue peak to the red peak and found it is anti-correlated with Ly$\alpha$ SB with a power-law index of $\sim$ -0.4. We used radiative transfer models with both multiphase, clumpy and shell geometries and successfully reproduced the diverse Ly$\alpha$ morphologies. We found that most spectra suggest outflow-dominated kinematics, while 4/15 spectra imply inflows. A significant correlation exists between parameter pairs, and the multiphase, clumpy model may alleviate previously reported discrepancies. We also modeled Ly$\alpha$ spectra at different positions simultaneously and found that the variation of the inferred clump outflow velocities can be approximately explained by line-of-sight projection effects. Our results support the `central powering + scattering' scenario, i.e. the Ly$\alpha$ photons are generated by a central powering source and then scatter with outflowing, multiphase HI gas while propagating outwards. The infalling of cool gas near the blob outskirts shapes the observed blue-dominated Ly$\alpha$ profiles, but its energy contribution to the total Ly$\alpha$ luminosity is less than 10%, i.e. minor compared to the photo-ionization by star-forming galaxies and/or AGNs., Comment: v1: 13 pages, 6 figures + Appendix. The anaerobic threshold (AT), obtained during cardiopulmonary exercise testing (CPET), is an important prognostic measure and can be used to develop an exercise prescription in patients after a myocardial infarction (post-MI). The purpose of this study was to examine the central and peripheral determinants of AT in post-MI patients end-tidal oxygen partial pressure (PETO2) measures. We performed cardiopulmonary exercise testing (CPET) on 148 consecutively enrolled post-MI patients to determine PETO2 measured at the AT (AT PETO2) and ΔPETO2 (difference between resting PETO2 and AT PETO2). We subsequently investigated the relationship between these measures of PETO2 and the individual AT for each patient. We present a detailed investigation of millimeter-wave line emitters ALMA J010748.3-173028 (ALMA-J0107a) and ALMA J010747.0-173010 (ALMA-J0107b), which were serendipitously uncovered in the background of the nearby galaxy VV114 with spectral scan observations at $\lambda$ = 2 - 3 mm. Via Atacama Large Millimeter/submillimeter Array (ALMA) detection of CO(4-3), CO(3-2), and [CI](1-0) lines for both sources, their spectroscopic redshifts are unambiguously determined to be $z= 2.4666\pm0.0002$ and $z=2.3100\pm0.0002$, respectively. We obtain the apparent molecular gas masses $M_{\rm gas}$ of these two line emitters from [CI] line fluxes as $(11.2 \pm 3.1) \times 10^{10} M_\odot$ and $(4.2 \pm 1.2) \times 10^{10} M_\odot$, respectively. The observed CO(4-3) velocity field of ALMA-J0107a exhibits a clear velocity gradient across the CO disk, and we find that ALMA-J0107a is characterized by an inclined rotating disk with a significant turbulence, that is, a deprojected maximum rotation velocity to velocity dispersion ratio $v_{\rm max}/\sigma_{v}$ of $1.3 \pm 0.3$. We find that the dynamical mass of ALMA-J0107a within the CO-emitting disk computed from the derived kinetic parameters, $(1.1 \pm 0.2) \times 10^{10}\ M_\odot$, is an order of magnitude smaller than the molecular gas mass derived from dust continuum emission, $(3.2\pm1.6)\times10^{11}\ M_{\odot}$. We suggest this source is magnified by a gravitational lens with a magnification of $\mu \gtrsim10$, which is consistent with the measured offset from the empirical correlation between CO-line luminosity and width., Comment: 19 pages, 11 figures, accepted for publication in ApJ

We report a massive quiescent galaxy at $z_{\rm spec}=3.0922^{+0.008}_{-0.004}$ spectroscopically confirmed at a protocluster in the SSA22 field by detecting the Balmer and Ca {\footnotesize II} absorption features with multi-object spectrometer for infrared exploration (MOSFIRE) on the Keck I telescope. This is the most distant quiescent galaxy confirmed in a protocluster to date. We fit the optical to mid-infrared photometry and spectrum simultaneously with spectral energy distribution (SED) models of parametric and nonparametric star formation histories (SFH). Both models fit the observed SED well and confirm that this object is a massive quiescent galaxy with the stellar mass of $\log(\rm M_{\star}/M_{\odot}) = 11.26^{+0.03}_{-0.04}$ and $11.54^{+0.03}_{-0.00}$, and star formation rate of $\rm SFR/M_{\odot}~yr^{-1}, Comment: 13 pages, 7 figures (in main text. 21 pages including appendices), accepted for publication in ApJ

Kondo insulators have recently aroused great interest because they are promising materials that host a topological insulator state caused by the strong electron interactions. Moreover, recent observations of the quantum oscillations in the insulating state of Kondo insulators have come as a great surprise. Here, to investigate the surface electronic state of a prototype Kondo insulator YbB$_{12}$, we measured transport properties of single crystals and microstructures. In all samples, the temperature dependence of the electrical resistivity is insulating at high temperatures and the resistivity exhibits a plateau at low temperatures. The magnitude of the plateau value decreases with reducing sample thickness, which is quantitatively consistent with the surface electronic conduction in the bulk insulating YbB$_{12}$. Moreover, the magnetoresistance of the microstructures exhibits a weak-antilocalization effect at low field. These results are consistent with the presence of topologically protected surface state, suggesting that YbB$_{12}$ is a candidate material of the topological Kondo insulator. Non-Fermi liquids are strange metals whose physical properties deviate qualitatively from those of conventional metals due to strong quantum fluctuations. In this paper, we report transport measurements on theFeSe1−xSx superconductor, which has a quantum critical point of a nematic order without accompanying antiferromagnetism. We find that in addition to a linear-in-temperature resistivity ρ_xx ∝ T , which is close to the Planckian limit, the Hall angle varies as cot(θ_H) ∝ T^2 and the low-field magnetoresistance is well scaled as Dρ_xx/ρ_xx ∝ tan^2(θ_H) in the vicinity of the nematic quantum critical point. This set of anomalous charge transport properties show striking resemblance with those reported in cuprate, iron-pnictide, and heavy fermion superconductors, demonstrating that the critical fluctuations of a nematic order with q ≈ 0 can also lead to a breakdown of the Fermi liquid description.

We present our new ALMA observations targeting [OIII]88um, [CII]158um, [NII]122um, and dust continuum emission for three Lyman break galaxies at z=6.0293-6.2037 identified in the Subaru/Hyper Suprime-Cam survey. We clearly detect [OIII] and [CII] lines from all of the galaxies at 4.3-11.8sigma levels, and identify multi-band dust continuum emission in two of the three galaxies, allowing us to estimate infrared luminosities and dust temperatures simultaneously. In conjunction with previous ALMA observations for six galaxies at z>6, we confirm that all the nine z=6-9 galaxies have high [OIII]/[CII] ratios of L[OIII]/L[CII]~3-20, ~10 times higher than z~0 galaxies. We also find a positive correlation between the [OIII]/[CII] ratio and the Lya equivalent width (EW) at the ~90% confidence level. We carefully investigate physical origins of the high [OIII]/[CII] ratios at z=6-9 using Cloudy, and find that high density of the interstellar medium, low C/O abundance ratio, and the cosmic microwave background attenuation are responsible to only a part of the z=6-9 galaxies. Instead, the observed high [OIII]/[CII] ratios are explained by 10-100 times higher ionization parameters or low photodissociation region (PDR) covering fractions of 0-10%, both of which are consistent with our [NII] observations. The presence or absence of itinerant gapless excitations in the organic quantum spin liquid (QSL) candidate EtMe${}_{3}$Sb[Pd(dmit)${}_{2}$]${}_{2}$, probed by a finite residual linear term in the thermal conductivity ${\ensuremath{\kappa}}_{0}/T$, has been a controversial hot topic. Here, the authors find that ${\ensuremath{\kappa}}_{0}/T$ strongly depends on the cooling process. A finite ${\ensuremath{\kappa}}_{0}/T$ is observed in a slowly cooled sample, but not in a rapidly cooled sample. These results provide evidence that the true ground state of EtMe${}_{3}$Sb[Pd(dmit)${}_{2}$]${}_{2}$ is a QSL with itinerant gapless excitations.

Background: Identifying the causes of low peak oxygen uptake (peak VO2) in heart disease patients with renal dysfunction is necessary for prognostic improvement strategies. The purpose of this study was to verify the determinants of peak VO2 for each stage of renal function in heart disease patients, focusing on end-tidal oxygen partial pressure (PETO2). A subtype prediction score for primary aldosteronism has not yet been developed and validated using a large dataset. This study aimed to develop and validate a new subtype prediction score and to compare it with existing scores using a large multicenter database.In total, 1936 patients with primary aldosteronism were randomly assigned to the development and validation datasets, constituting 1290 and 646 patients, respectively. Three prediction scores were generated with or without confirmatory tests, using logistic regression analysis. In the validation dataset, new and existing prediction scores were compared using receiver operating characteristic curve, net reclassification improvement, and integrated discrimination improvement analyses.The new prediction score is simply calculated using serum potassium levels [3.9 mmol/l (four points); 3.5-3.9 mmol/l (three points)], the absence of adrenal nodules during computed tomography (three points), a baseline plasma aldosterone concentration of210.0 pg/ml (two points), a baseline aldosterone/renin ratio of less than 620 (two points), and female sex (one point). Using the validation dataset, we found that a new subtype prediction score of at least 8 had a positive predictive value of 93.5% for bilateral hyperaldosteronism. The new prediction score for bilateral hyperaldosteronism was better than the existing prediction scores in the receiver operating characteristic curve and net reclassification improvement analyses.The new prediction score has clear advantages over the existing prediction scores in terms of diagnostic accuracy, feasibility, and the potential for generalization in a large population. These data will help healthcare professionals to better select patients who require adrenal venous sampling.

Context Recently, the relationship between primary aldosteronism (PA) and various metabolic disorders, including obesity, diabetes mellitus, and dyslipidemia, has been discussed. However, in PA, aldosterone-producing adenoma (APA) and idiopathic hyperaldosteronism (IHA) have different etiologies. Objective Our objectives were to clarify differences in obesity and metabolic disorders between APA and IHA and to gain insight in the pathogenesis of IHA. Design, setting, and participants This is a retrospective cross-sectional study. We assessed the PA database established by the multicenter Japan Primary Aldosteronism Study. For comparative analysis, data were also collected from 274 patients with essential hypertension (EHT). Main outcome measures We compared prevalences of obesity and metabolic disorders between patients with APA and patients with IHA. Comparisons with sex-, age-, and blood pressure-matched patients with EHT were also performed. Correlations between metabolic parameters and plasma aldosterone concentrations (PACs) in each subtype were analyzed. Results Analysis of 516 patients with APA and 1015 patients with IHA revealed PACs were significantly higher in patients with APA than patients with IHA. By contrast, after we adjusted for clinical backgrounds, the prevalence of obesity was significantly higher in patients with IHA than in patients with APA or EHT. Although the prevalences of diabetes mellitus and dyslipidemia did not significantly differ between patients with IHA and patients with APA, triglyceride and HbA1c were significantly higher in patients with IHA than in patients with APA. There was no significant correlation between metabolic parameters and PACs in either subtype. Conclusions Patients with IHA tend to be obese despite lower PACs than in patients with APA. The present results suggest that obesity-related factors contribute to the pathogenesis of IHA.

Submillimeter bright galaxies in the early Universe are vigorously forming stars at ~1000 times higher rate than the Milky Way. A large fraction of stars is formed in the central 1 kiloparsec region, that is comparable in size to massive, quiescent galaxies found at the peak of the cosmic star formation history, and eventually the core of giant elliptical galaxies in the present-day Universe. However, the physical and kinematic properties inside a compact starburst core are poorly understood because dissecting it requires angular resolution even higher than the Hubble Space Telescope can offer. Here we report 550 parsec-resolution observations of gas and dust in the brightest unlensed submillimeter galaxy at z=4.3. We map out for the first time the spatial and kinematic structure of molecular gas inside the heavily dust-obscured core. The gas distribution is clumpy while the underlying disk is rotation-supported. Exploiting the high-quality map of molecular gas mass surface density, we find a strong evidence that the starburst disk is gravitationally unstable, implying that the self-gravity of gas overcomes the differential rotation and the internal pressure by stellar radiation feedback. The observed molecular gas would be consumed by star formation in a timescale of 100 million years, that is comparable to those in merging starburst galaxies. Our results suggest that the most extreme starburst in the early Universe originates from efficient star formation due to a gravitational instability in the central 2 kpc region., Published in Nature on August 30 2018 (submitted version)

Objectives The aim of this study was to investigate the impact of adrenal venous sampling (AVS) lateralization cutoffs on surgical outcomes. Patients and Methods Cosyntropin-stimulated AVS was used to guide surgical management of 377 patients with primary aldosteronism (PA) who were evaluated 6 months after surgery. Main Outcome Measures The proportion of patients that achieved clinical benefit and complete biochemical success based on the AVS aldosterone lateralization index (LI) was determined. Results Clinical benefit was achieved in 29 of 47 patients with an LI between 2 and 4, in 66 of 101 with an LI between 4 and 10, and in 158 of 203 with an LI > 10 (P < 0.01 for trend). Complete biochemical success was achieved in 27 of 42 with an LI between 2 and 4, in 60 of 76 with an LI between 4 and 10, and in 127 of 155 with an LI > 10 (P = 0.024 for trend). After adjustment for confounders and using those patients with an LI between 2 and 4 as a reference, a clinical benefit was associated only with those with an LI > 10 (OR, 2.30; 95% CI, 1.03 to 5.16), whereas complete biochemical success was associated with those with an LI between 4 and 10 (OR, 2.83; 95% CI, 1.14 to 7.01) or LI > 10 (OR, 3.55; 95% CI, 1.47 to 8.55). Conclusions Difference of clinical outcome was relatively small when strict LI diagnostic threshold was used; biochemical cure was sufficiently achieved when an LI > 4 was used. Our study by standardized outcome measures validated that an LI > 4 may be appropriate for determining unilateral disease in PA., We investigated the impact of AVS lateralization cutoffs on surgical outcome by standardized measures and validated that an LI > 4 is appropriate to determine unilateral disease in PA.

There have been several clinical studies examining the factors associated with cardiovascular disease (CVD) in patients with primary aldosteronism (PA); however, their results have left it unclear whether CVD is affected by the plasma aldosterone concentration or hypokalemia. We assessed the PA database established by the multicenter JPAS (Japan Primary Aldosteronism Study) and compared the prevalence of CVD among patients with PA with that among age-, sex-, and blood pressure-matched essential hypertension patients and participants with hypertension in a general population cohort. We also performed binary logistic regression analysis to determine which parameters significantly increased the odds ratio for CVD. Of the 2582 patients with PA studied, the prevalence of CVD, including stroke (cerebral infarction, cerebral hemorrhage, or subarachnoid hemorrhage), ischemic heart disease (myocardial infarction or angina pectoris), and heart failure, was 9.4% (stroke, 7.4%; ischemic heart disease, 2.1%; and heart failure, 0.6%). The prevalence of CVD, especially stroke, was higher among the patients with PA than those with essential hypertension/hypertension. Hypokalemia (K + ≤3.5 mEq/L) and the unilateral subtype significantly increased adjusted odds ratios for CVD. Although aldosterone levels were not linearly related to the adjusted odds ratio for CVD, patients with plasma aldosterone concentrations ≥125 pg/mL had significantly higher adjusted odds ratios for CVD than those with plasma aldosterone concentrations

Subtype diagnosis of primary aldosteronism (PA) by adrenal vein sampling (AVS) is recommended as a mandatory step for indicating adrenal surgery. It is a technically demanding procedure, especially in the right adrenal vein. The aim of the study was to predict the subtype diagnosis in the absence of values from the right AVS. From the databases of nine centers (WAVES-J), 308 patients with PA who underwent successful AVS were studied. Based on the ipsilateral ratio (IR) (aldosterone/cortisol ratio of the left adrenal vein [A/Cleft AV] / aldosterone/cortisol ratio of the inferior vena cava [A/CIVC]), the patients were divided into two groups: the patients with IR ≥ 1.0 (n = 262) and those with IR 1.0, the A/Cleft AV was significantly higher in patients with the left unilateral subtype than in patients with the bilateral subtype. Receiver operating characteristic (ROC) curve analysis revealed that an A/Cleft AV cutoff >68 showed 70.8% sensitivity and 93.5% specificity for the left unilateral subtype. Context The number of centers with established adrenal venous sampling (AVS) programs for the subtype diagnosis of primary aldosteronism (PA) is limited. Objective Aim was to develop an algorithm for AVS based on subtype prediction by computed tomography (CT) and serum potassium. Design A multi-institutional retrospective cohort study in Japan. Patients A total of 1591 patients with PA were classified into four groups according to CT findings and potassium status. Subtype diagnosis of PA was determined by AVS. Main Outcome Measure Prediction value of the combination of CT findings and potassium status for subtype diagnosis. Results The percentages of unilateral hyperaldosteronism on AVS were higher in patients with unilateral disease on CT than those with bilateral normal results on CT (50.8% vs 14.6%, P < 0.01), and these percentages were higher in those with hypokalemia than those with normokalemia (58.4% vs 11.5%, P < 0.01). The prevalence and odds ratio for unilateral hyperaldosteronism on AVS were as follows: bilateral normal on CT with normokalemia, 6.2% (reference); unilateral disease on CT with normokalemia, 23.8% and 4.8 [95% confidence interval (CI), 3.1 to 7.2]; bilateral normal on CT with hypokalemia, 38.1% and 9.4 (95% CI, 6.2 to 14.1), and unilateral disease on CT with hypokalemia, 70.6% and 36.4 (95% CI, 24.7 to 53.5). Conclusions Patients with PA with bilateral normal results on CT and normokalemia likely have a low prior probability of a lateralized form of AVS and could be treated medically, whereas those with unilateral disease on CT and hypokalemia have a high probability of a lateralized form of AVS.

A fundamental issue concerning iron-based superconductivity is the roles of electronic nematicity and magnetism in realising high transition temperature ($T_{\rm c}$). To address this issue, FeSe is a key material, as it exhibits a unique pressure phase diagram involving nonmagnetic nematic and pressure-induced antiferromagnetic ordered phases. However, as these two phases in FeSe overlap with each other, the effects of two orders on superconductivity remain perplexing. Here we construct the three-dimensional electronic phase diagram, temperature ($T$) against pressure ($P$) and isovalent S-substitution ($x$), for FeSe$_{1-x}$S$_{x}$, in which we achieve a complete separation of nematic and antiferromagnetic phases. In between, an extended nonmagnetic tetragonal phase emerges, where we find a striking enhancement of $T_{\rm c}$. The completed phase diagram uncovers two superconducting domes with similarly high $T_{\rm c}$ on both ends of the dome-shaped antiferromagnetic phase. The $T_{\rm c}(P,x)$ variation implies that nematic fluctuations unless accompanying magnetism are not relevant for high-$T_{\rm c}$ superconductivity in this system., 8 pages, 9 figures. We present the near-infrared high resolution imaging of an extremely dense group of galaxies at the core of the protocluster at $z=3.09$ in the SSA22 field by using the adaptive optics AO188 and the Infrared Camera and Spectrograph (IRCS) on Subaru Telescope. Wide morphological variety of them suggests their on-going dramatic evolutions. One of the two quiescent galaxies (QGs), the most massive one in the group, is a compact elliptical with an effective radius $r_{e} = 1.37\pm0.75$ kpc. It supports the two-phase formation scenario of giant ellipticals today that a massive compact elliptical is formed at once and evolves in the size and stellar mass by series of mergers. Since this object is a plausible progenitor of a brightest cluster galaxy (BCG) of one of the most massive clusters today, it requires strong size ($\ga10$) and stellar mass ($\sim$ four times by $z=0$) growths. Another QG hosts an AGN(s) and is fitted with a model composed from an nuclear component and S\'ersic model. It shows spatially extended [O{\footnotesize III}]$\lambda$5007 emission line compared to the continuum emission, a plausible evidence of outflows. Massive star forming galaxies (SFGs) in the group are two to three times larger than the field SFGs at similar redshift. Although we obtained the $K$-band image deeper than the previous one, we found no candidate new members. This implies a physical deficiency of low mass galaxies with stellar mass $M_{\star}\la4\times10^{10}~M_{\odot}$ and/or poor detection completeness of them owing to their diffuse morphologies., Comment: 18 pages, 14 figures, MNRAS accepted

Using galaxies as background light sources to map intervening Lya absorption is a novel approach to study the interplay among galaxies, the circum-galactic medium (CGM), and the intergalactic medium (IGM). Introducing a new measure of z = 3.1$ HI Lya absorption relative to the cosmic mean, Delta_NB497, estimated from photometric data of star-forming galaxies at 3.3 < z < 3.5, we have made two-dimensional Delta_NB497 maps in the z = 3.1 SSA22 proto-cluster region and two control fields (SXDS and GOODS-N fields) with a spatial resolution of ~ 5 comoving Mpc. The Delta_NB497 measurements in the SSA22 field are systematically larger than those in the control fields, and this HI absorption enhancement extends more than 50 comoving Mpc. The field-averaged (i.e., ~50 comoving Mpc scale) Delta_NB497 and the overdensity of Lya emitters (LAEs) seem to be correlated, while there is no clear dependency of the Delta_NB497 on the local LAE overdensity in a few comoving Mpc scale. These results suggest that diffuse HI gas spreads out in/around the SSA22 proto-cluster. We have also found an enhancement of Delta_NB497 at a projected distance < 100 physical kpc from the nearest z = 3.1 galaxies at least in the SSA22 field, which is probably due to HI gas associated with the CGM of individual galaxies. The HI absorption enhancement in the CGM-scale tends to be weaker around galaxies with stronger Lya emission, which suggests that the Lya escape fraction from galaxies depends on hydrogen neutrality in the CGM., 13 pages, 11 figures, 1 table, accepted to MNRAS

Hyperkalemia is an important complication of adrenalectomy for patients with primary aldosteronism (PA). The frequency of hyperkalemia after medication using mineralocorticoid receptor antagonists (MRAs) for PA is unclear. The aim of this study is to investigate the frequency and the risk factors of hyperkalemia after surgery and medication for PA. The data of 376 patients with PA registered in a multicentre-collaborative study in Japan, including surgically treated patients (group A; n=142) and medically treated patients with MRAs (group B; n=234) were studied. We present our ALMA Band 8 observations of a damped Ly${\alpha}$ absorption (DLA) system at $z$=3.150 observed in the spectrum of the quasar Q2233+131 at $z$=3.295. The optical counterpart of this DLA has been identified and it shows a double-peaked Ly${\alpha}$ emission line. Since one possible origin of DLAs at high redshift is an outflowing gas from star-forming galaxies, DLA2233+131 provides a good laboratory to investigate the nature of high-$z$ DLAs. Motivated by this, we have carried out ALMA band 8 observations to study the [C II] line in this system. However, we do not detect any significant emission line in the observed pass bands. Instead, we have serendipitously found three submm continuum sources in the observed sky area. One appears to be the quasar Q2233+131 itself while the other two sources are newly identified submm galaxies (SMGs), called SMG1 and SMG2 in this paper. They are located at a separation of 400.7 and 800.1 from Q2233+131, respectively. Their 646 ${\mu}$m fluxes are 6.35 mJy and 6.43 mJy, respectively, being higher than that of Q2233+131, 3.62 mJy. Since these two SMGs are not detected in the optical images obtained with the Hubble Space Telescope and the Subaru Telescope, they have a very red spectral energy distribution. It is, therefore,suggested that they are high-redshift galaxies or very dusty galaxies at intermediate redshift although we cannot rule out the possibility that they are optically very faint SMG analogs at low redshift. This paper reports the effect of randomness on a quantum spin liquid state of 1T-TaS${}_{2}$ with a two-dimensional perfect triangular lattice. Systematic measurements of heat capacity and thermal conductivity in pure, Se-substituted, and electron-irradiated crystals reveal the microscopic coexistence of localized orphan spins that form random valence bonds and itinerant spinons that appear to form a Fermi surface.

Heat transport mediated by Majorana edge modes in a magnetic insulator leads to a half-integer thermal quantum Hall conductance, which has recently been reported for the two-dimensional honeycomb material $\alpha$-RuCl$_3$. While the conventional electronic Hall effect requires a perpendicular magnetic field, we find that this is not the case in $\alpha$-RuCl$_3$. Strikingly, the thermal Hall plateau appears even for a magnetic field with no out-of-plane components. The field-angular variation of the quantized thermal Hall conductance has the same sign structure of the topological Chern number, which is either $\pm$1, as the Majorana band structure of the pure Kitaev spin liquid. This observation of a half-integer anomalous thermal Hall effect firmly establishes that the Kitaev interaction is primarily responsible and that the non-Abelian topological order associated with fractionalization of the local magnetic moments persists even in the presence of non-Kitaev interactions in $\alpha$-RuCl$_3$., Comment: 14 pages, 4 figures

To determine the dominant sources for cosmic reionization, the evolution history of the global ionizing fraction, and the topology of the ionized regions, we have conducted a deep imaging survey using four narrow-band (NB) and one intermediate-band (IB) filters on the Subaru/Hyper Suprime-Cam (HSC), called Cosmic HydrOgen Reionization Unveiled with Subaru (CHORUS). The central wavelengths and full-widths-at-half-maximum of the CHORUS filters are, respectively, 386.2 nm and 5.5 nm for NB387, 526.0 nm and 7.9 nm for NB527, 717.1 nm and 11.1 nm for NB718, 946.2 nm and 33.0 nm for IB945, and 971.2 nm and 11.2 nm for NB973. This combination, including NB921 (921.5 nm and 13.5 nm) from the Subaru Strategic Program with HSC (HSC SSP), are carefully designed, as if they were playing a chorus, to observe multiple spectral features simultaneously, such as Lyman continuum, Ly$\alpha$, C~{\sc iv}, and He~{\sc ii} for $z=2$--$7$. The observing field is the same as that of the deepest footprint of the HSC SSP in the COSMOS field and its effective area is about 1.6 deg$^2$. Here, we present an overview of the CHORUS project, which includes descriptions of the filter design philosophy, observations and data reduction, multiband photometric catalogs, assessments of the imaging quality, measurements of the number counts, and example use cases of the data. Using an automated image analysis routine on 34,476 known Sloan Digital Sky Survey quasars, we identify those with two (or more) distinct optical point sources in HSC images covering 796 deg2. We find 421 candidates out to a redshift of 4.5 of which one hundred or so are more likely after filtering out contaminating stars. Angular separations of 0.″6-4.″0 correspond to projected separations of 3-30 kpc, a range relatively unexplored for population studies of luminous dual quasars. Using Keck I/Low Resolution Imaging Spectrometer and Gemini-N/Near-Infrared Integral Field Spectrometer, we spectroscopically confirm three dual quasar systems at z < 1, two of which are previously unknown out of eight observed, based on the presence of characteristic broad emission lines in each component, while highlighting that the continuum of one object in one of the pairs is reddened. In all cases, the [O iii]λ5007 emission lines have mild velocity offsets, thus the joint [O iii] line profile is not double-peaked. We find a dual quasar fraction of 0.26 ± 0.18% and no evidence for evolution. A comparison with the Horizon-AGN simulation seems to support the case of no evolution in the dual quasar fraction when broadly matching the quasar selection. These results may indicate a scenario in which the frequency of the simultaneous triggering of luminous quasars is not as sensitive as expected to the cosmic evolution of the merger rate or gas content of galaxies., R. We present results from a new cosmological hydrodynamics simulation campaign of protocluster (PC) regions, FOREVER22: FORmation and EVolution of galaxies in Extremely-overdense Regions motivated by SSA22. The simulations cover a wide range of cosmological scales using three different zoom set-ups in a parent volume of $(714.2~\rm cMpc)^{3}$: PCR (Proto-Cluster Region; $V= (28.6~{\rm cMpc})^{3} $, SPH particle mass, $m_{\rm{SPH}} = 4.1 \times 10^{6}~\rm M_{\odot}$ and final redshift, $z_{\rm end}=2.0$), BCG (Brightest proto-Cluster Galaxy; $V \sim (10~{\rm cMpc})^{3} $, $m_{\rm SPH} = 5.0\times10^{5}~\rm M_{\odot}$ and $z_{\rm end}=4.0$ ), and First ( $V \sim (3~{\rm cMpc})^{3} $, $m_{\rm SPH} = 7.9 \times 10^{3}~\rm M_{\odot}$ and $z_{\rm end}=9.5$) runs, that allow to focus on different aspects of galaxy formation. In the PCR runs, we follow 10 PCs, each harbouring 1 - 4 SMBHs with $M_{\rm BH} \ge 10^{9}~\rm M_{\odot}$. One of the PC cores shows a spatially close arrangement of seven starburst galaxies with ${\rm SFR} \gtrsim 100~\rm M_{\odot}~yr^{-1}$ each, that are dust-obscured and would appear as submillimeter galaxies with flux $\gtrsim 1~$ mJy at $1.1~ \rm mm$ in observations. The BCG runs show that the total SFRs of haloes hosting BCGs are affected by AGN feedback, but exceed $1000~\rm M_{\odot}~yr^{-1}$ at $z \lesssim 6$. The First runs resolve mini-haloes hosting population (Pop) III stars and we show that, in PC regions, the dominant stellar population changes from Pop III to Pop II at $z \gtrsim 20$, and the first galaxies with ${\rm SFR} \gtrsim 18~\rm M_{\odot}~yr^{-1}$ form at $z \sim 10$. These can be prime targets for future observations with the James Webb Space Telescope. Our simulations successfully reproduce the global star formation activities in observed PCs and suggest that PCs can kickstart cosmic reionization., Comment: 24 pages, 19 figures, accepted for publication in MNRAS
