BACKGROUND Although some studies have described the association between serum ferritin levels and specific disorders in child and adolescent psychiatry, few have focused on mental health with low serum ferritin levels in children and adolescents. This study examined the effects of iron administration on psychological state of children and adolescents with reduced serum ferritin concentration. Nocturnal enuresis (NE) is a syndrome associated with abnormal nocturnal urine production, urination mechanism, and sleep arousal. NE is strongly associated with attention-deficit/hyperactivity disorder (ADHD), and it has been reported that NE occurs in approximately 30% of children with ADHD. There have been several reports on the efficacy of atomoxetine as treatment for NE with ADHD, while the efficacy of guanfacine is still limited. We report our experience of treating an 10-year-old girl with NE with ADHD with a single dose of guanfacine. The patient first visited our hospital because of difficulty concentrating, restlessness at home and school, and nocturnal incontinence. She was diagnosed with NE with ADHD based on a review of her personal history from her mother. Her NE symptoms improved with guanfacine monotherapy (1 mg/day. The patient weighed 28 kg).

While the WHO has already published its mental health and psychosocial considerations during the COVID-19 outbreak, there has been almost no discussion of the specific measures that healthcare institutions should take with regard to the mental health of healthcare practitioners. We would like to take this occasion to propose a perspective on this matter from research on social support. Social support can be classified in various ways such as informational, appraisal, instrumental, and emotional. When battling an unknown virus such as this one, having correct informational support is necessary above all else. It is also important that appropriate evaluation for the contribution of practitioners in the institutions as appraisal support. Instrumental support, meanwhile, refers to guaranteeing a system where practitioners can get adequate rest and not excessively strain themselves, and emotional support is when practitioners are shown adequate understanding and appreciation by those around them. There is another view that categorizes social support by how it is given and received: received support and perceived support. It is important for practitioners to receive perceived support, because perceived support is confirmed to have a positive effect on physical and mental health. For the sake of the physical and mental health of healthcare practitioners, it is important for healthcare sites and organizations to take social support into account and for healthcare institutions to adopt the perspective of social support in their day-to-day operations.

Background: Few studies on gender-based diagnostic characteristics of adolescent suicide attempters in emergency departments (EDs) have included individuals with autism spectrum disorder (ASD). Objective: This study aimed to examine the clinical features of adolescent suicide attempts in EDs, focusing on gender differences and considering individuals with ASD. Method: Ninety-four adolescent patients, aged less than 20 years, who had attempted suicide and had been hospitalized in an emergency department, participated in this study. Psychiatric diagnoses according to DSM-IV criteria and clinical features were compared between male and female patients. Results: The number (%) of males was 15 (16.0), and that of females was 79 (84.0). The mean age (SD) of males was 17.1 (1.5), and that of females was 16.9 (1.6). The attempt methods were more serious, length of stay in the emergency room longer, and rate of outpatient treatment lower in males. In addition, suicide attempters with ASD were significantly more frequent in male. Adjusting for age and gender, adjustment disorder was significantly associated with the presence of suicide attempters with ASD using a multivariable logistic regression. Conclusion: Males were less likely to visit psychiatric service previous to attempting suicides, and may be likely to complete suicides. In addition, suicide attempters with ASD are characteristic in male, and likely to have comorbid adjustment disorder. ED visits offer a window of opportunity to provide suicide prevention interventions for adolescents, and therefore, psychiatrists in EDs have a crucial role as gatekeepers of preventing suicide reattempts, especially in adolescent males including individuals with ASD having adjustment disorder.

The condition of periodic psychosis of adolescence based on the clinical features of recurrent depressive symptoms, sub-stupor, and psychotic symptoms whose features return to a normal state within 2 weeks with no residual symptoms has been often seen during adolescence. However, international recognition of periodic psychosis of adolescence is low and the condition is not recognized as an independent disease in ICD-10 or DSM-5. We presented a case report of a depressive episode central to periodic psychosis of adolescence in a 16-year old female. The symptoms presented in the case correspond to the DSM-5 classification of premenstrual dysphoric disorder. However, a diagnosis of periodic psychosis of adolescence was made due to the presence of clinical features of victim mentality, increased irritability, suicidal ideations, and changes in consciousness over short periods of time and sub-stupor. This report was focused on the medical treatment of the episode of periodic psychosis of adolescence with the aim of verifying its current significance.

We constructed seven intensity-modulated radiation therapy (IMRT) treatment plans for prostate cancer (49 irradiation fields which contained seven randomly-sampled patients and seven fields) and evaluated the dose distributions by using a radiochromic film (EBT3 film) and a 2D detector. We superposed the calculated dose distribution of the IMRT treatment plan on EBT3 film and the 2D detector results and then compared those with the γ-analysis pass rate. The relative positions of the beam and the detector were varied; the results of the analysis of the superior-inferior (SI) direction potentially differed, depending on the detector position, under an irradiation beam with the same fluence map. The detector was moved over a range of’ 8 mm in the SI direction in 1-mm step increments, measurement were made at each position, and the results were analyzed. The γ-analysis compared the dose distributions from EBT3 film and the radiation treatment planning system (RTPS) for each patient and field; the pass rate with the γ-analysis from 98 to 100% was 2.04%. When we compared the dose distributions of the 2D detector and the RTPS, the pass rate from 98 to 100% was 63.2%. The mean values for the ?-analysis pass rates for EBT3 film and the 2D detector were 94.2 and 97.6%, respectively. Volume averaging of the data indicated a mean pass rate and standard deviation of 98.6 and 0.91%, respectively, and a pass rate of more than 96% for all positions. A 2D detector can, therefore, be used as an alternative apparatus for IMRT dose verification.

Tungsten functional paper (TFP), which contains 80% tungsten by weight, has radiation‐shielding properties. We investigated the use of TFP for the protection of operators during interventional or therapeutic angiography. The air kerma rate of scattered radiation from a simulated patient was measured, with and without TFP, using a water‐equivalent phantom and fixed C‐arm fluoroscopy. Measurements were taken at the level of the operator's eye, chest, waist, and knee, with a variable number of TFP sheets used for shielding. A Monte Carlo simulation was also utilized to analyze the dose rate delivered with and without the TFP shielding. In cine mode, when the number of TFP sheets was varied through 1, 2, 3, 5, and 10, the respective reduction in the air kerma rate relative to no TFP shielding was as follows: at eye level, 24.9%, 29.9%, 41.6%, 50.4%, and 56.2%; at chest level, 25.3%, 33.1%, 34.9%, 46.1%, and 44.3%; at waist level, 45.1%, 57.0%, 64.4%, 70.7%, and 75.2%; and at knee level, 2.1%, 2.2%, 2.1%, 2.1%, and 2.1%. In fluoroscopy mode, the respective reduction in the air kerma rate relative to no TFP shielding was as follows: at eye level, 24.8%, 30.3%, 34.8%, 51.1%, and 58.5%; at chest level, 25.8%, 33.4%, 35.5%, 45.2%, and 44.4%; at waist level, 44.6%, 56.8%, 64.7%, 71.7%, and 77.2%; and at knee level, 2.2%, 0.0%, 2.2%, 2.8%, and 2.5%. The TFP paper exhibited good radiation‐shielding properties against the scattered radiation encountered in clinical settings, and was shown to have potential application in decreasing the radiation exposure to the operator during interventional radiology.

Background To date, the clinical features between patients in Japan who have attempted suicide by charcoal burning and those who have attempted suicide by other methods in the context of a mental disorder diagnosis as assessed by structured interviews have not been reported. Methods We enrolled 647 consecutive patients who attempted suicide and were hospitalized for inpatient treatment. Psychiatric diagnoses, frequency of suicide attempts, and clinical features were compared between charcoal burning and other suicide methods. Results Twenty of the 647 patients (3.1%) had attempted suicide by charcoal burning. The ratio of men to women was significantly higher by this method compared with that of other methods. The proportion of patients with mood disorders was significantly higher in the charcoal burning group than that in the other methods group. The occurrence of a psychiatric history in patients in the charcoal burning group was significantly lower than that in the other methods group. Limitations The study sample was limited to a single hospital. Conclusions The results demonstrate the clinical characteristics of patients who attempted suicide by charcoal burning. Therefore, it is necessary to identify the clinical features of patients who have attempted suicide by charcoal burning in Japan.

Although the suicide risk of autism spectrum disorder (ASD) has been suggested to be higher than previously recognized, there are few case reports focusing on the process for preventing suicide reattempts. We reported that a 17-year-old male who had attempted suicide by jumping was admitted to our emergency department and hospitalized for lumbar spine fracture. In addition to the diagnosis of adjustment disorder, he was diagnosed as ASD according to his life history. This article presents the characteristics of the suicidal behaviors and the process for preventing a suicide reattempt associated with an adolescent with ASD who attempted suicide.

Orthognathic surgery was performed on 362 patients from 1987 to 1997 at the Second Department of Oral and Maxillofacial Surgery, Osaka Dental University.The results were as follows: 1. The male-female ratio was 1: 2.4.2. The mean age was 22.4 years old.3. The diagnosis of mandibular prognathism was made for most of the patients (89.2%).4. Sagittal splitting ramus osteotomy (SSRO) was performed on 274 cases (75.7%).5. The mean time of operation for SSRO was 3hr. 7 min±57min, and for mandibular body osteotomy was 3hr. 15min±60.7min.6. The mean blood loss during operation for SSRO was 275.8±221.4ml and for mandibular body osteotomy was 171.8±147.5ml.7. Paresthesia of the lip and mental area was observed in 404 sides after opration. 80.7% of patients treated for SSRO recovered within a year.8. TMJ were examined in 202 cases for 1 year after SSRO. The symptom of TMJ was recognized in 5 cases.

We isolated a cDNA clone, Elk-3, that encodes a novel Ets transcription factor from 16-day mouse embryos. The deduced amino acid sequence of the protein was homologous to human ELK-1 and SAP-1. This protein, ELK-1, and SAP-1 shared some unique structural properties such as an Ets DNA-binding site in the amino-terminal region, a serum response factor interacting domain and phosphorylation sites of serine or threonine residues in the carboxy-terminal region. Northern blotting weakly revealed that two transcripts of 4 and 2.1 kb are expressed in the adult ovary and lung and a 2.1-kb transcript predominated in 8- to 14-day embryos. We assayed the transcriptional activities of Elk-3 protein on the cytokeratin EndoA enhancer containing Ets binding sites in endodermal cells. Elk-3 protein strongly repressed enhancer activity but did not affect the activity of the basal promoter in the absence of the enhancer. Furthermore, Elk-3 can suppress the activity of Ets-2 as the transcriptional activator on the EndoA enhancer. These data suggested that the Elk-3 gene product plays a role in transcriptional regulation during embryogenesis.

To study the general physiological role of the Mo25 gene, which has been cloned from mouse cleavage-stage embryos, we isolated a Drosophila equivalent, dMo25, cDNA from an embryo cDNA library. The 2,222 nucleotides contained a single open reading frame encoding a polypeptide of 339 amino acid residues with a calculated molecular mass of 39,278 daltons. The deduced amino acid sequence of the dMo25 cDNA had 69.3% identity with mouse Mo25. A homology search revealed that these were similar to a protein encoded in an open reading frame near the calcineurin B subunit gene on chromosome XI in Saccharomyces cerevisiae. In particular, the carboxy-terminal region was highly conserved in Drosophila, mouse, and yeast. The dMo25 gene was mapped to the left arm of the third chromosome at 73AB, and 2.3- and 1.8-kb mRNA bands were detected during development and in adult Drosophila. Conservation of the gene structure and the wide expression profile indicated that the function of the gene is likely to be fundamental in many cell types as well as during development.
