The first coil should be as thick, long, and large as possible. A retrospective study at the Toranomon Hospital revealed that if the volume of the first coil is 1/3rd or more of the total coil volume, there are less chances of retreatment. Thus, a thick and long first coil contributes to stable long-term anatomical results. The length of the first coil should be approximately 8% of the volume of the aneurysm. After the placement of the first coil, a smaller and shorter coil should be placed sufficiently close to it to complete the procedure. The goal is to achieve prevention of acute re-rupture in ruptured acute aneurysms and life-long prevention of rupture in unruptured asymptomatic aneurysms. If the coil does not detach, check the position of the detachment point, and replace the power supply. In addition, refer to the direction for use of the device for prevention of coil disconnection failure and countermeasures. Although it is not a recommended method, a method of rotating the delivery wire at the detached position to thread the coil and a method of reconstructing the electric circuit with a crocodile clip has been reported.

A 71-year-old woman was receiving dialysis for microscopic polyangiitis(MPA). She was transported to the emergency room after developing a headache and losing consciousness. She was in a coma and had dense left paralysis. Head computed tomography(CT)showed a subarachnoid hemorrhage. Head three-dimensional CT angiography showed an aneurysm proximal to the right M2(inferior trunk), which was considered as the source of bleeding. We performed an emergency aneurysm clipping surgery. However, we placed a permanent clip on the arterial wall defect when the clip detached from the parent artery. Pathological examination of the aneurysm revealed that the layered structure of the artery was damaged because of fibrin degeneration, suggesting that the aneurysm was caused by vasculitis. Coma persisted after surgery, and the patient was transferred to another hospital.We found only one report demonstrating an association between a cerebral aneurysm and MPA. Aneurysms resulting from vasculitis are common at the periphery, rather than at vessel bifurcations. Cyclic inflammatory activity causes progressive destruction of the blood vessel wall, further expanding the lumen and forming an aneurysm. Therefore, such aneurysms are more fragile than true aneurysms, and are more likely to be damaged or detached during surgery, as in the present case. Trapping of the parent artery should be considered when the aneurysm is damaged or detached. Accordingly, preoperative planning could anticipate the need for superficial temporal artery to middle cerebral artery bypass.

Objective Gamma knife radiosurgery (GKRS) is occasionally a useful tool for maintaining good brain status in patients with brain metastases (METs). Conversely, we recently experienced patients with delayed cyst formation (DCF) several years after GKRS, a complication not previously reported. Herein we assessed the frequency and characteristics of DCF after GKRS for METs. Methods Eighty of 1209 patients with METs treated with GKRS maintained good brain status for more than 3 years without regrowth of tumorous lesions in the brain. In this study, DCF was defined as secondary cyst formation more than 3 years after the first GKRS in patients with METs who did not have cysts at the start of MET treatment. The 80 patients were divided into 2 groups (DCF group and non-DCF group) for assessment of the frequency and characteristics of DCF. Of the patients with cystic METs at the start of MET treatment, 16 were included in the latter group. Results Among these 80 patients, 8 had DCF after GKRS (DCF group), detected by magnetic resonance imaging from 37 to 121 months after the first GKRS (median interval of 53 months). Of these 8 patients, 7 were symptomatic, and surgical treatments including Ommaya reservoir placement were needed in 5. A comparison of the non-DCF and DCF groups revealed that a higher number of GKRS treatments was a risk factor for DCF. Moreover, patients surviving more than 5 years after the initial GKRS are at risk for DCF. Conclusion Although DCF is not a widely recognized complication of GKRS for METs, we advocate careful follow-up, with surgical intervention for DCF if necessary, for frequently irradiated and long-surviving patients with METs treated with GKRS.

Study design A case report of aneurysmal bone cyst of the mobile spine. Objective To present a pediatric case of aneurysmal bone cyst (ABC) of the mobile spine treated successfully by selective arterial embolization (SAE) with n-butyl cyanoacrylate (NBCA). Summary of background data The endovascular treatment is an important treatment option for the ABC of the mobile spine. However, there are few reported case treated by SAE with NBCA. Methods A 12-year-old healthy boy was introduced with a complaint of back pain. Computed tomography showed an osteolytic expanding mass in the pedicle, lamina, and spinous process on the level of T10. Magnetic resonance imaging revealed a multicystic mass with fluid-fluid levels that extended into the spinal canal and compressed the spinal cord from the dorsal side. Digital subtraction angiography showed an abnormal blood supply from the posterior spinal branch of the left T9 artery. An abnormal mass was diagnosed as an ABC based on these neuroradiologic examinations. Result The patient underwent SAE with NBCA (Histoacryl, B. Braun, Melsungen, Germany) diluted with Lipiodol (Laboratorie Guerbet, France) in a rate of 1:4. Computed tomography performed 1 month after treatment revealed partial ossification of the bone cortex. MR imaging showed shrinkage of the cyst and decompression of the spinal cord. Back pain of the patient disappeared and the shrunken cyst was stable without recurrence during the 3-year follow-up period. Conclusion SAE in a pediatric case with ABC of the mobile spine was effective in improving of clinical symptom and shrinking of the cyst.

A boy with shaken baby syndrome first presented at age 3 months with acute subdural hematoma (SDH) and was treated by subdural tapping at a local hospital. Chronic SDH was identified at a rehabilitation center at age 19 months. The chronic SDH appeared to have developed within the preceding 16 months. His physical and mental development was already delayed. Magnetic resonance (MR) imaging revealed a 20-mm thick right chronic SDH with midline shift and small bilateral subdural effusions. The chronic SDH had compressed the right cerebral hemisphere. MR imaging also disclosed bilateral cerebral atrophy. 99mTc-ethylcysteinate dimer single photon emission computed tomography (SPECT) revealed decreased cerebral blood flow (CBF) in the non-hematoma hemisphere, although CBF is said to decrease on the chronic SDH side, especially if midline shift is present. Burr hole craniotomy with external drainage was performed, but the patient showed no change in CBF postoperatively, although the volume of hematoma decreased. The patient was clinically unchanged immediately after the operation. In this case, SPECT measurement of CBF was important in evaluating the pathophysiology of the delays in physical and mental growth. Atrophy of the bilateral hemispheres was the major mechanism in the decreased CBF, not the compression by chronic SDH. MR imaging and SPECT can determine the surgical indications for chronic SDH in patients with cortical atrophy.

Aneurysms can produce large defects in the parent vessel if the aneurysm tears at the neck of the vessel. We present techniques used to repair a tear at the base of an aneurysm encountered during microsurgical clipping of a ruptured aneurysm. The repair technique involved suturing and covering the aneurysm with an encircling aneurysm clip. A large tear had destroyed the vessel's tubular structure and therefore an encircling clip alone was insufficient for repair. Two microsuture stitches were placed on the tear, so that a split artery reformed a tubular structure. The temporary clip on the distal internal carotid artery was removed for a moment, allowing the retrograde blood flow to provide the counterforce necessary to maintain the vessel's tubular structure. An encircling clip was then applied to cover the entire circumference of the lesion. Another technique involved placing needles over a tear and applying clips while avoiding parent artery stenosis. Placing a clip to cover an entire tear resulted in an arterial stenosis but just applying a clip was insufficient for repair. With this guiding needle method, a clip can adequately be placed without parent artery stenosis. These methods required only a short occlusion time for arterial repair, thus helping avoid ischemic complications. These techniques are useful for repairing an aneurysmal tear at its base, especially if the tear is large.

application/pdf, Background Oculomotor palsy is well known to be due to diabetes mellitus, or aneurysmal compression, or cavernous sinus lesion. Only few reports presented that arterial compression was the reason for oculomotor palsy. Case Description We performed a surgical treatment for a 76-year-old man with left oculomotor palsy. During surgical clipping of the left IC-PC aneurysm, it became clear that the left IC-PC aneurysm had not been compressing the third cranial nerve. The left oculomotor nerve was being pinched between the arteriosclerotic PCA and the SCA. Microvascular decompression was carried out, and the oculomotor nerve palsy improved. Conclusion Arteriosclerotic PCA and SCA may compress the oculomotor nerve. Microvascular decompression is effective in this type of oculomotor palsy.

A 54-year-old man was admitted to our hospital with complaint of sudden headache. The patient had suffered two episodes of transient headache before admission. Computed tomography (CT) revealed acute subdural hematoma (ASDH) on the right side of the cerebral convexity with bilateral extension along the tentorium cerebelli without signs of subarachnoid hemorrhage (SAH) or intracerebral hemorrhage (ICH). Three-dimensional CT angiography and conventional cerebral angiography revealed a left A1-A2 junction aneurysm. Neck clipping of the aneurysm was performed. The aneurysm extended inferiorly, with the dome embedded in the chiasmatic cistern and tightly adhered to the arachnoid membrane. There was no evidence of hematoma in the subarachnoid space. The patient was discharged without neurological deficit. Ruptured aneurysms resulting in ASDH without SAH or ICH are very rare. Radiological investigation such as three-dimensional CT angiography should be performed to find the causative aneurysm in a patient with ASDH with a history of repeated headaches and without traumatic signs or episodes, and the appropriate treatment should be planned with expediency.

Progressive stroke is a serious problem due to the associated morbidity and mortality. Aspirin is recommended for acute ischemic stroke, but does not reduce the frequency of stroke progression. No standard treatment has been approved for the prevention of stroke progression. Cilostazol, which reduces platelet aggregation about 3 hours after single administration, does not increase the frequency of bleeding events when compared with aspirin or a placebo. Moreover, the combination of 100 mg aspirin and 200 mg cilostazol does not increase the frequency of bleeding events compared with only 100 mg aspirin, and thus is expected to prevent stroke progression with a high degree of safety. The present study investigated the safety of this combination of two drugs administered at the above concentrations in 54 patients with acute ischemic stroke within 48 hours of stroke onset. Modified National Institutes of Health Stroke Scale (NIHSS) measurements were performed at baseline and again on day 4 to 7. Progressive stroke was defined as an increase greater than or equal to 1 point on NIHSS. Patient scores on the modified Rankin Scale (mRS) were evaluated at baseline and 3 months after enrollment. Stroke progression occurred in 11.1% of the patients. The percentages of patients with mRS score from 0 to 2 were 42.6% and 75% at baseline and 3 months, respectively. No symptomatic intracranial hemorrhage or major extracranial hemorrhage occurred. These results suggest that administration of aspirin and cilostazol is safe for acute ischemic stroke.

An 85-year-old man presented with a rare large aneurysm of the extracranial internal carotid artery (ICA) due to acute otitis media manifesting as Vernet's syndrome 2 weeks after the diagnosis of right acute otitis media. Angiography of the right extracranial ICA demonstrated an irregularly shaped large aneurysm with partial thrombosis. The aneurysm was treated by proximal ICA occlusion using endovascular coils. The ICA mycotic aneurysm was triggered by acute otitis media, and induced Vernet's syndrome as a result of direct compression to the jugular foramen. Extracranial ICA aneurysms due to focal infection should be considered in the differential diagnosis of lower cranial nerve palsy, although the incidence is thought to be very low.
